[Clearing the doubts of all Wang Bing in the Tang dynasty].

The problem is probed on the clues provided by Si ku ti yao zheng (Analysis on Summary of Books in Four Parts). It is claimed that the Wang bing (*) recommended by Wei Kang and that recommended by Yuwen Rong in Yu Jiaxi's work refer to the same person, both are identical to the time of the author of Su wen. Further, the time of the above 2 Wang Bing(s) and the Wang Bing recorded in lie nu zhuan (Biongraphies of chaste women) as the son of Wang Lin, are also overlapped with the time of the author of Su wen. It is possible that they are the same person. However, the details of the Wang Bing mentioned in Lang guan shi zhu ti ming (inscription for Lang guan Stone Pillar) is unknown. Again, it is unlikely that the Wang Bing (*) is the author of Su wen and can be only a farfetched analogy drawn after Su Fu's poet.

Relationship between pathogenesis of Guillain-Barre syndrome and Penner's serotypes of Campylobacter jejuni.

OBJECTIVE: To investigate the relationship between the pathogenesis of Guillain-Barre syndrome (GBS) and Campylobacter jejuni (C. jejuni) in China. METHODS: C. jejuni strains were isolated from fresh stools of 47 GBS patients in Beijing area from 1995 to 1997 by modified Skirrow's method. Serotyping of C. jejuni was performed with Penner's method in 47 GBS patients and 171 patients with C. jejuni enteritis in our hospital during the same period. The stools from which no C. jejuni strains could be isolated were typed by PCR-RFLP (restriction fragment length polymorphism) method. RESULTS: Six C. jejuni strains were isolated by Skirrow's method, including 3 strains of Penner serotype 2, 1 Penner serotype 4, 1 Penner serotype 19 and 1 Penner serotype 26. Three strains of DNA-I, which was equal to the same type of Penner serotype 19, were found by PCR-RFLP method. CONCLUSION: C. jejuni isolated from Beijing area are similar to those reported in literature, which once more confirms the molecular mimicry pathogenetic theory of GBS caused by C. jejuni infection in China. The similarities of serotypes between C. jejuni strains from GBS patients and those from C. jejuni enteritis patients may explain the high incidence of GBS following C. jejuni infection in China.

Relationship between angiotensinogen gene M235T variant with diabetic nephropathy in Chinese NIDDM.

OBJECTIVE: To investigate whether angiotensinogen (AGT) gene M235T variant is associated with non-insulin-dependent diabetes mellitus without nephropathy (DN-), and diabetic nephropathy (DN+) in Chinese non-insulin-dependent diabetes mellitus (NIDDM). METHODS: The subjects in DN+ group, DN- group and control group were well matched with sex, age and duration of disease, and the two case groups were divided into two subgroups as with and without hypertension respectively. The M235T polymorphism of AGT gene of 84 cases with DN-, 96 patients with DN+ and 98 controls were determined by polymerase chain reaction (PCR) amplification and restriction fragment length polymorphism (RFLP) analysis of the region of the variant, i.e. M235T polymorphism. RESULTS: The increased frequencies of T allele (0.82) and TT genotype (0.70) were observed in 96 subjects with DN+ as compared with 98 control subjects (0.63 and 0.43, respectively, P = 0.003, P = 0.0004). The odds ratio associated with TT genotype was 3.47 (95% CI: 1.51-7.94; P = 0.0033) for diabetic nephropathy in analysis adjusted for several risk factors of diabetic nephropathy, such as body-mass index, systolic and diastolic blood pressure, serum cholesterol, low density lipoprotein and high density lipoprotein. Subgroup analysis of the 67 patients in DN+ group with hypertension revealed similar distributions of M235T genotypes and alleles to those in the DN+ without hypertension subgroup. There was no difference in allele and genotype distribution between 84 DN- patients and the controls. Similarly, frequencies of the AGT M235T genotype and allele were not different between two DN- subgroups. CONCLUSIONS: AGT gene M235T polymorphism is associated with diabetic nephropathy in NIDDM. TT genotype of the AGT gene might be an independent risk factor of diabetic nephropathy in Chinese NIDDM patients.

[Expression of cyclin D1 mRNA in acute leukemia patients and its clinical implication].

OBJECTIVE: To determine the expression of cyclin D1 gene in patients with acute leukemia and evaluate the relationship between cyclin D1 expression and treatment outcomes. METHODS: The expression of cyclin D1 mRNA was determined in bone marrow cells from 65 acute leukemia patients and 5 normal subjects by RT-PCR technique. RESULTS: Cyclin D1 mRNA was negative in normal bone marrow cells. Among leukemia patients, the rate of cyclin D1 expression was 26.2%, and the highest(63.6%) rate was in the relapse group and the lowest(8.3%) in the CR group. Relapse rate of the patients with cyclin D1 expression was higher than that of those with negative expression over 1-13 months follow-up. CONCLUSION: Overexpression of cyclin D1 does exist in acute leukemia patients and is correlated with the disease progression, especially with relapse.

[Clinical observation of oral appliances in the treatment of obstructive sleep apnea syndrome].

OBJECTIVE: To explore the effect of treatment with the oral appliances on 36 cases with obstructive sleep apnea syndrome (OSAS). METHODS: The patients are requested to put on the proper oral appliances when sleeping. Polysomnography (PSG) are used to measure the apnea hypoventilation index(AHI), lowest SaO2 and longest apnea time before and after treatment. RESULTS: Having been Compared with the obtained data, the AHI decreased, the lowest SaO2 increased and the longest apnea duration shortened significantly (P < 0.05) in OSAS patients after oral appliance treatment. CONCLUSIONS: The oral appliances helped to widen the upper airway, stabilized the lower jaw bone on a forward position to make the tongue and the soft palate move forward, increased the effective amount of inhaled air while sleeping and improved sleeping quality. There fore, it is an effective method for the treatment of OSAS.

[Diagnosis and treatment of primary small vessel vasculitis with involvement of lungs].

OBJECTIVE: To investigate the clinical characteristics of primary small vessel vasculitis with involvement of lungs. METHODS: 13 cases of primary small vessel vasculitis with involvement of lungs from 1993 to 1998 were analyzed retrospectively. RESULTS: Among 13 cases 7 were microscopic polyangiitis (MPA) and 6 were Wegener granulomatosis (WG). The ages of onset were from 17 to 68 years old with average age 48.8 years old. 69%(9/13) were ANCA positive, among them 100%(7/7) MPA were ANCA positive (6/7 were P-ANCA positive, 1/7 was C-ANCA positive), while 33%(2/6) WG were ANCA positive (one case of P-ANCA and another case of C-ANCA was positive respectively). The major symptoms of respiratory system included hemoptasis 69% (9/13), dyspnea 23%(3/13), dry cough 15%(2/13) and chest pain 15%(2/13). The chest x-rays showed multiple patchy shadows in both lungs were mainly found in MPA (3/7) and single or multiple masses or nodular shadows were mainly found in WG (5/6) with or without cavity formation. The appearance of lungs in MPA 71% (5/7) had been explained as "pulmonary infection" and that of WG had been explained as "primary lung cancer or metastatic carcinoma". Symptoms of respiratory system might occur before (3/5) or after (2/5) occurrence of acute renal failure. The treatments with corticosteroid and CTX were effective in these cases, in particularly, pulmonary lesions improved obviously. CONCLUSIONS: It is very difficult to make diagnosis of primary small vessel vasculitis with involvement of lungs and this should be paid more attention. ANCA detection is very useful in diagnosis of MPA. Corticosteroid and CTX are most effective in treating these diseases.

[Evidence of increased endogenous carbon monoxide production in asthma].

OBJECTIVE: To investigate the role of carbon monoxide (CO) in the pathogenesis of asthma. METHODS: We examined CO, nitrite and nitrate (NO2[not readable: see text]/NO3[not readable: see text]) and tumor necrosis factor alpha (TNF-alpha) levels in plasma of 26 asthmatic children and 22 matched normal children. At the same time, we examined the plasma and bronchoalveolar lavage fluid (BALF) CO, NO2[not readable: see text]/NO3[not readable: see text] and TNF-alpha levels in guinea-pigs with allergic asthma. RESULTS: Asthmatic children in acute attack stage had increased levels of CO, NO2[not readable: see text]/NO3[not readable: see text] and TNF-alpha in plasma compared with normal control subjects (all P < 0.01), while in stable stage plasma CO and NO2[not readable: see text]/NO3[not readable: see text] reduced to normal levels (both of the P > 0.05), but TNF-alpha was still higher than the normal controls (P < 0.01). Guinea-pigs with allergic asthma had increased amounts of CO and TNF-alpha in plasma and CO, NO2[not readable: see text]/NO3[not readable: see text] and TNF-alpha in BALF as compared with normal control subjects (P < 0.05-0.01), but no changes in levels of circulating NO2[not readable: see text]/NO3[not readable: see text] (P > 0.05) were found. There was no difference between glucocorticoid treated asthmatic guinea pigs and normal controls in levels of CO, NO2[not readable: see text]/NO3[not readable: see text], and TNF-alpha. The elevation of plasma CO correlated with elevated NO2[not readable: see text]/NO3[not readable: see text] level in asthmatic children (P < 0.05), so did in the BALF in allergic guinea-pigs. CONCLUSIONS: Our present data showed that endogenously produced CO was increased during the course of asthma, which suggested that the role of CO in asthma might worth further study.

[The expression and regulation of heme oxygenase-1 in asthmatic guinea pigs].

OBJECTIVE: To explore the expression and pathogenic mechanism of heme oxygenase-1(HO-1) in asthma. METHODS: Two groups of guinea pigs (10 in each) were treated with the specific stimulator(hemin) and inhibitor(Sn-PP) of HO-1 respectively. Their effects on the levels of HO-1 activity, COHb, cGMP and IgE in serum or blood and lung tissues were compared with each other. Acute asthmatic group (AAG), dexamethasone-prevented group(DPG), stable asthmatic group, ovalbumin-sensitized group and normal control group(NCG) were also included. Lung tissues of tested animals were pathologically observed and immunohistochemically stained. RESULTS: In the AAG and hemin stimulated group, the levels of COHb, cGMP and IgE were significantly increased with higher levels of activity and expression of HO-1 protein compared with the NCG (t = 4.575-10.188, P < 0.01). In DPG and Sn-PP inhibited groups, however, all measured parameters were markedly reduced compared with the AAG(P < 0.01). The other groups were normal. CONCLUSIONS: The increased production and activity of HO-1 resulted in increases of endogenous CO and cGMP and promoted inflammation and immunoreaction in asthma.

[Characteristics of expression of CD44v and receptor for HA-mediated motility (RHAMM) in multi-step gastrocarcinogenesis].

OBJECTIVE: To explore the potential significance of expression of CD44 and RHAMM at different stages of gastric carcinogenesis. METHODS: The expression pattern of CD44 and RHAMM in non-cancerous mucosa, preneoplastic lesions and carcinomas of the stomach was examined by RT-PCR, Southern blot hybridization, immunohistochemical staining and Western blot hybridization. RESULTS: Expression of CD44 v5 could be observed not only in cancers, but also in pre-neoplastic and non-cancerous specimens. CD44 v7 was commonly seen in non-cancerous lesions but its expression tended to decrease with the progression of pre-malignant phenotype. CD44 v8-v10 could only be observed in some intestinal metaplasia, dysplastic proliferation and undifferentiated carcinoma. Nevertheless, CD44 v6 was found exclusively in cancer tissues, with a positive rate of 14%. The frequency of RHAMM detection was about 10% in non-cancerous mucosa but tended to increase with tumor development, reaching an average of 74% in gastric cancers. In 80% of the invading cancer cells, RHAMM and CD44v coexpressed, or complementarily expressed. CONCLUSION: Expression of RHAMM is more associated with progression of gastric malignancy. Irrespective of its low frequency, CD44 v6 is still a reliable marker of malignant transformation of gastric mucosa. Co-expression or complementary expression of CD44v and RHAMM might facilitate invasion and metastasis of gastric cancer.

[Clinical observation of the effect of shuxinsu capsule in treating angina pectoris].

OBJECTIVE: To observe the therapeutic effect of Shuxinsu capsule (SXSC) in treating angina pectoris patients. METHODS: One hundred and fifty angina pectoris patients were divided into two groups. Group A (80 cases), randomly divided into two subgroups, SXSC treated and Yixin oral liquid (YXOL) control group, each group consisted of 40 cases. Group B (70 cases) was an openly treated group (OTG). RESULTS: 17 cases were markedly effective in the SXSC treated group in relieving the symptoms, effective were 16, the total effective rate was 82.50%, while in the control group, the markedly effective cases, the effective cases and the total effective rate were 13, 16 and 72.50% respectively; there was significant difference between these two groups (u = 1.99, P < 0.05). In the treated group (SXSC and OTG) of 110 cases, the markedly effective, effective cases on ECG were 34 and 30 respectively. The total effective rate was 58.18%, higher than that of the control group, which was 55.00%, but the difference was insignificant. The activity of SOD in RBC and the amount of plasma LPO were significantly different after treatment in the SXSC group. Both of the medications had effect to reduce serum total cholesterol (TC) and triglyceride (TG), the TC was lower and high density lipoprotein-cholesterol (HDL-C) higher significantly after treatment in the SXSC group. CONCLUSION: SXSC has significant effect on angina pectoris.

[Study on prevention and treatment of middle and aged women diabetes with kidney deficiency and bone metabolic disturbance].

OBJECTIVE: To Study the therapeutic effect of Chinese herbal medicine (CHM) for supplementing Qi, activating blood circulation and tonifying Kidney on prevention and treatment of middle and aged women diabetes with Kidney Deficiency and bone metabolic disturbance. METHODS: Clinical observation was taken in 52 patients, who were divided into two groups, the control group (treated with hypoglycemic agent alone) and the treated group (treated with hypoglycemic agent and CHM). RESULTS: Before treatment, patients of both groups showed obvious higher blood alkaline phosphatase, beta 2-microglobulin (beta 2-MG) level, urinary beta 2-MG, calcium and phosphorus level, but lower serum estradiol level than those in normal subjects. After 3 months' treatment, no apparent change on serum estradiol level was observed, but other parameters were all lowered obviously in the two groups, the changes revealed more obvious in the treated group. The symptoms of Kidney Deficiency, such as lumbodorsal pain, general fatigue, palpitation and vertigo, were improved after treatment, and the improvement was also more obvious in the treated group. CONCLUSION: CHM for supplementing Qi, activating blood circulation and tonifying Kidney was effective in improving Kidney Deficiency and mineral substance loss of bone in middle and aged women diabetes patients. The CHM and western drugs may acted synergistically.

[The significance of inflammatory markers in sputum of asthmatic and chronic obstructive pulmonary diseases patients before and after glucocorticoid treatment].

OBJECTIVE: To study the change of cytokines and eosinophil cationic protein (ECP) level in the sputum before and after glucocorticoid (GC) inhalation treatment so as to comprehend their effect on asthmatic and chronic obstructive pulmonary diseases (COPD) patients. METHODS: A method to induce sputum with inhaled hypertonic saline was used. The level of interleukin (IL)-5, IL-8 and ECP was measured with enzyme-linked immunosorbent assay method. RESULTS: The concentration of ECP decreased from (500.3 +/- 49.6) microg/L to (59.8 +/- 10.9) microg/L, the percentage of eosinophils (Eos) dropped from (11.6 +/- 1.7) x 10(-2) to (4.1 +/- 0.7) x 10(-2) and there is significant difference in the concentration of IL-5 in the group of asthmatic patients after GC treatment. However, the concentration of IL-5 in the COPD patients did not show significant change after the same therapy. CONCLUSION: Respiratory tract inflammation in asthma is related to Eos activation and increase in ECP and IL-5 excretion, while respiratory tract inflammation in COPD is related to neutrophil increase. These changes can be considered as the indicator of airway inflammation in asthma or COPD. Through regulating the quantity and function of the inflammatory cells and inhibiting the formation of cytokines to control the asthmatic airway inflammation, GC inhalation treatment will have better effect in treating asthmatic patients than COPD patients.

[The relationship between Guillain-Barre syndrome and serotypes of Campylobacter jejuni isolated from the patients].

OBJECTIVE: To investigate the relationship between Campylobacter jejuni (CJ) serotype and the high incidence of Guillain-Barre syndrome (GBS) following CJ enteritis in China. METHODS: CJ was isolated from fresh stool of 47 GBS patients in Beijing area from 1995-1997 with a combination of Skirrow method and micro-filtration method. Serotyping of the CJ strains was performed with passive hemagglutination method with standard serum from CDC. 171 CJ strains from CJ enteritis in our hospital in the same period were serotyped with the same method. The stool from which no CJ could be isolated was typed with PCR-RFLP method. RESULTS: 3 Penner 2, 1 Penner 4, 1 Penner 19 and 1 Penner 26 strains were isolated and serotyped. Three strains which could not be isolated were found by PCR-RFLP method to be DNA-I, which is equivalent to the type of Penner 19. There were some CJ strains which could be typed with serum from CDC. CONCLUSION: CJ from GBS patients in Beijing area are similar to those reported in the world; it is suggested that CJ may cause GBS by molecular mimicry mechanism in China. The most common serotype of CJ from GBS was reported to be Penner 19 in Japan, while it was rare in CJ enteritis. However, the similarity of the serotypes between CJ from GBS patients and CJ from enteritis patients may explain the high incidence of GBS following CJ infection in China. Penner 26 is reported to be isolated from GBS patients for the first time.

[Central nervous system involvement in systemic lupus erythematosus].

OBJECTIVE: To investigate central nervous system (CNS) involvements in systemic lupus erythematosus (SLE) so as to enhance our knowledge in diagnosing and treating CNS involvement of SLE. METHODS: The clinical data of 171 in patients with CNS involvement of SLE in our hospital were retrospectively reviewed. RESULTS: The mean SLE disease duration at onset of CNS involvement was (2.21 +/- 1.87) years and in 163 (95.3%) it was associated with active disease. Cerebral spinal fluid abnormality was seen in 91.4% (138/151) of the patients with CNS involvement of SLE. Among them protein elevation was found in 113, pressure elevation in 69, white cell elevation in 51 and glucose reduction in 6. For the evaluation of CNS involvement of SLE, the sensitivity of cranial CT and MRI was 77.4% and 81.4% respectively (P > 0.50). The positive rate of antiribosomal P in patients with diffuse CNS involvement of SLE was significantly higher than that in patients without CNS involvement (P < 0.01). On the contrary, the positive rate of ACL in focal CNS involvement of SLE was significantly higher than that in diffuse type or in patients without CNS involvement (P < 0.01). The total mortality rate in 171 patients with CNS involvement of SLE was 18.7%. The Mortality rate in the period of 1993 to 1998 (4.0%, 3/75) was significantly lower than that of 1980 to 1992 (30.2%, 29/96), P < 0.01. 24 SLE patients with CNS involvement received intrathecal dexamethasone and methotrexate, 22 cases (91.7%) improved considerably. CONCLUSION: CNS involvement occurs in the early stage of SLE, most of the cases are associated with active disease. Cerebral spinal fluid analysis is the most essential test and cranial imaging serves as a supplementary approach, of which CT is preferred. ACL is associated with focal CNS involvement of SLE while antiribosomal P with diffuse CNS involvement of SLE, suggesting there might be different mechanisms in CNS involvement of SLE. Intrathecal therapy is an useful alternative for patients with CNS involvement of SLE refractory to conventional therapy.

[Clinical application of esophageal manometry and 24-hour esophageal pH monitoring].

OBJECTIVE: To evaluate the clinical application of esophageal manometry and 24-hour esophageal pH monitoring. METHODS: From 1990 to 1996, ambulatory esophageal manometry and 24-hour esophageal pH monitoring were performed on 101 patients with different functional esophageal diseases, including Barrett's esophagus 54 patients non-cardiac chest pain (10) and post-cholecystectomy (37). Esophageal motility studies were made to compare the effects of different anti-reflux therapies, to identify primary esophageal motility disorders, to assess the effect of anti-acid drugs, and to study the relationship between esophageal motility disorder and other non-esophageal origin disease. RESULTS: In this study, anti-reflux surgery was better than medical therapy in controlling gastroesophageal reflux (GER) for Barrett's esophagus. Esophageal manometry was the best method for detecting primary esophageal motility disease. Omeprazole was superior to cimetidine in terms of control of GER and reduction of gastric acidity. Cholecystectomy resulted in GER and many symptoms after surgery appeared to have an esophageal origin. CONCLUSIONS: Esophageal manometry and 24-hour esophageal pH monitoring are the most sensitive and specialized methods for detecting esophageal motility disorders. They are superior to other routine diagnostic methods such as radiology and endoscopy.
