Treatment of complications due to peritoneal dialysis for chronic renal failure with traditional Chinese medicine.

In this paper, the experience in the treatment of complications due to continuous ambulatory peritoneal dialysis for chronic renal failure with traditional Chinese medicine (TCM) is reported. Modified Renshen Yangrong Tang (Ginseng Nutrition Decoction) was used for anorexia and hypoproteinemia; modified Xiangsha Liujunzi Tang (Decoction of Cyperus and Amomum with Six Noble Ingredients) for abdominal pain and distension; modified Da Chaihu Tang (Major Bupleurum Decoction) for peritonitis; modified Shenling Baizhu San (Powder of Ginseng, Poria and Atractylodes) for diarrhea due to insufficiency of the spleen with abundance of dampness; Lizhong Tang (Decoction for Regulating the Function of Middle-jiao) and modified Sishen Wan (Pills of Four Miraculous Drugs) for insufficiency of both the spleen and the kidney; Siwu Tang (Decoction of Four Ingredients) added with other drugs for cutaneous pruritus, and Guishao Sijunzi Tang (Decoction of Four Noble Drugs added with Chinese Angelica Root and white Peony Root) for renal anemia. The therapeutic principles of invigorating the liver and kidney, strengthening the bones and muscles, and promoting blood circulation to eliminate blood stasis were adopted in the treatment of renal osteopathy, and the therapeutic principles of invigorating the liver and kidney, expelling phlegm and resolving dampness, and promoting blood circulation to eliminate blood stasis in the treatment of hyperlipemia. Shen Tekang capsules (capsules for improving the renal function) was administered to patients for strengthening the viability and improving the nutrition state, and the recipe for treating renal function failure (both formulated by the authors) for improving the renal function so as to decrease the frequency and duration of dialysis.

[Relationships of angiotensinogen gene M235T variant with diabetic nephropathy in Chinese type 2 diabetes mellitus].

OBJECTIVE: To investigate whether angiotensinogen(AGT) gene M235T variant is associated with type 2 diabetes mellitus without nephropathy (DM) and/or diabetic nephropathy (DN) in Chinese type 2 diabetes. METHODS: AGT genotypes of 84 cases with DM, 96 with DN and 98 controls were determined by polymerase chain reaction (PCR) amplification and restriction fragment length polymorphism analysis of the region of the variant. RESULTS: Increased frequencies of T allele (0.82) and TT genotype (0.70) were observed in 96 subjects with DN, compared with those observed in 98 control subjects (0.63 and 0. 43 respectively, P = 0.003, P = 0.0004). The odds ratio associated with TT genotype was 3.47 (95% CI 1.51-7.94, P = 0.0033) for DN in analysis adjusted for several DN risk factors. There was no difference in allele distribution between 84 DM patients and the controls. CONCLUSION: TT genotype of the AGT gene might be an independent risk factor of diabetic nephropathy in Chinese patients with type 2 diabetes mellitus.

[A new explanation for shadow bands on denaturing PAGE for products of STRS--the difference between sense and antisense strands in electrophoresis].

OBJECTIVE: To explore the mechanism of shadow bands on denaturing PAGE for PCR products of STR sequences and try to eliminate them. METHODS: PCR and asymmetric PCR were employed. The amplified fragments were separated on denaturing PAGE and visualized by silver stain. RESULTS: By asymmetric PCR, half of shadow bands of dinucleotide repeats were eliminated and there was only one band for each allele for tetranucleotide sequence. A different migration between the sense strand and antisense strand was identified. CONCLUSION: The difference between sense and antisense strands in electrophoresis is one of the causes in shadow bands, and these shadow bands can be eliminated by asymmetric PCR partly or totally.

Treatment of squamous cell carcinoma of the tongue using arterial embolism with cisplatin-loaded albumin microspheres: a microstructural and ultrastructural investigation.

OBJECTIVE: To study the mechanism of cisplatin-loaded albumin microsphere infusion through the lingual artery to treat carcinoma of the tongue and the in vivo process of microspheres. METHODS: Lingual arterial infusion was performed in seven patients with advanced carcinoma of the tongue using cisplatin-loaded albumin microspheres 56.3 microns in diameter. The specimens were taken immediately after infusion and at 1, 2, 3, 4, 5, and 6 weeks after infusion. They were examined by microscopy and transmission electron microscopy. RESULTS: Histologic and ultrastructural changes after infusion included programmed cell death, ischemic necrosis, and degeneration. Almost all carcinoma cells were killed by the fourth week. Microspheres were gradually hydrolyzed in the body and vanished completely within 6 weeks. CONCLUSION: The mechanism of arterial embolism with cisplatin-loaded albumin microspheres to treat squamous cell carcinoma of the tongue involves the combined effects of cytotoxicity and ischemic necrosis of the tumor after microembolism.

Vessel-bearing stems of ASOVINEA TIANIIgen. et sp. nov. (Gigantopteridales) from theUpper Permian of Guizhou Province, China.

Permineralized gigantopterid stems of Vasovinea tianii Li et Taylor gen. et sp. nov. were collected from the Upper Permian of Guizhou Province, China. They are slender and bear prickles, trichomes, and compound hooks. Internally, the stems have a sparganum cortex, eustele, and secondary xylem. The mesarch protoxylem tracheids have annular to helical thickenings, and metaxylem tracheary elements have scalariform and/or transversely elongated, bordered pits, while those of the secondary xylem have scalariform to circular bordered pits. Importantly, the inner part of the secondary xylem has large vessel elements with foraminate-like perforation plates. The hooks and other morphological and anatomical characteristics are similar to those found in gigantopterids, suggesting that Vasovinea is a member of the Gigantopteridales. The vegetative plant is reconstructed from permineralized stems and Gigantopteris-type leaves based on the anatomical similarities and intimate association. The eustele, secondary xylem, and other features support the placement of the order among the seed plants. Ecologically, Vasovinea is suggested to have been a vine or liana that used compound hooks to climb among the trees in a Permian tropical rain forest. The occurrence of vessels could have been an efficient adaptation to allow the slender stems to conduct sufficient water to the large Gigantopteris-type leaves.

[Influence factors in hepatic artery infusion chemotherapy and embolization of liver metastases from alimentary tract cancer].

OBJECTIVE: To evaluate the efficacy and the value of hepatic artery infusion chemotherapy and embolization in the treatment of liver metastases from alimentary tract cancer with Cox's proportional hazard model. METHODS: A total of 92 patients with unresectable liver metastases from alimentary tract cancer were treated by hepatic artery infusion chemotherapy and embolization in 316 times. Of 92 patients, 29 were given infusion chemotherapy only, 63 were given infusion chemotherapy together with embolization. Anticancer agents were the combination of epirubicin and/or cisplatin, mitomycin-C and fiuorouracil plus calcium folinate, added to embolization with lipiodol ultrafluid or lipiodol ultra-fluid and gelatin sponge. Cox's proportional hazard model was used in multivariate analysis. RESULTS: The results showed that the therapeutic efficacy of 9 cases with single liver metastasis was the best. The mean survival period > 30 months (P < 0.05). Comprehensive treatment significantly improves therapeutic efficacy (P < 0.01). Response rates were assessed by CT scan. An overall response rate (CR + PR) of the cases was 45.65%. The mean survival period of 92 patients were 19.6 months. The 0.5, 1, 2, 3, and 5-year survival rates were 95.7%, 73.8%, 36.3%, 20.6% and 11.6% respectively. These was no severe side effect or complication. CONCLUSION: The hepatic artery infusion chemotherapy and embolization were of benefit to patients with liver metastases from alimentary tract cancer. It contributes not only to improving therapeutic efficacy, but also to an improved quality of life for the patients. Single liver metastasis and comprehensive treatment were the most important influence factors in hepatic artery infusion chemotherapy and embolization of the liver metasetases from alimentary tract cancer.

Sheep-associated malignant catarrhal fever in a petting zoo.

In a privately owned petting zoo in Arizona, 17 deer from five different species, white-tailed deer (Odocoileus virginianus), Reeve's muntjac (Muntiacus reevesi), mule deer (Odocoileus hemionus), reindeer (Rangifer tarandus), and axis deer (Axis axis), died of suspected malignant catarrhal fever (MCF) over a period from late 1992 to early 1995. A PCR assay specific for ovine herpesvirus 2, the putative causative agent of sheep-associated MCF, and a competitive-inhibition enzyme-linked immunosorbent assay based on a monoclonal antibody specific to an epitope conserved among all known MCF viral isolates were used to investigate the outbreak. Ovine herpesvirus 2 DNA sequences were detected by PCR from fresh-frozen and/or formalin-fixed, paraffin-embedded tissue samples in seven deer out of eight available animals previously suspected as cases by histopathology. A high seroprevalence to the virus was found among mouflon (Ovis musimon, 80%) and pygmy goats (Capra hircus, 61%), both of which were present on the farm during the outbreak. Sixteen percent of fallow deer (Dama dama) were also seropositive to the virus. After removal of the mouflon and positive pygmy goats, no further MCF cases occurred on the farm, confirming the importance of careful management to avoid mixing clinically susceptible species with carrier species. Until better control measures are available, adherence to this practice is necessary if MCF is to be prevented in intense exposure environments such as zoos and densely populated animal parks.

The effects of organic solvents on trimethadione n-demethylation in rats.

Many organic solvents are frequently used as support solvents to dissolve chemicals in the study concerning drug metabolism mediated by cytochrome P450. However, some organic solvents used as the support solvents affect the chemical's metabolism. It has been reported that some organic solvents are metabolized by CYP2E1 or inhibit its enzymatic reaction. In this study we investigated the effects of organic solvents, such as acetonitrile (AN), dimethylsulfoxide (DMSO), ethanol (EtOH), methanol (MeOH), polyethylene glycol (PEG) and propylene glycol (PG) on TMO (trimethadione) metabolism, which is mainly mediated by CYP2E1 in the rat. In the in vivo study, male SD rats were pretreated with an organic solvent intraperitoneally at a dosage of 0.5, 1 or 2 mmol/kg 1 hour before TMO administration orally at the dose of 4 mg/kg. After 2 hours, serum concentrations of TMO and DMO were determined by gas chromatography/flame detection (CG/FTD) and the serum DMO/TMO ratio was employed for assessment of the metabolic capacity of TMO. In the in vitro study, hepatic microsomal fraction was used as an enzyme source of TMO N-demethylase and enzyme activities were determined by the production of DMO. Pretreatment with DMSO and PG decreased the DMO/TMO ratio in a dose-related manner in vivo study. Furthermore, in vitro study TMO N-demethylase activity was inhibited by DMSO, EtOH and PG with different potency in a concentration related manner. However, no remarkable effects were observed by AN or PEG both in vivo and in vitro study. These results indicated that there are variations in the inhibitory effects of these organic solvents on CYP2E1-mediated metabolism and AN and PEG will be useful solvents to dissolve chemicals in the metabolic study mainly mediated by CYP2E1.

Effects of inhibitors of the tyrosine kinase signaling cascade on an in vitro model of allergic airways.

It has been shown that activation of protein tyrosine kinases (PTKs) is the earliest detectable signaling response to FcepsilonRI cross-linking on mast cells. Following tyrosine kinase activation, a family of mitogen-activated protein kinases (MAPKs) was found to be activated as well. Activation of this PTK signaling cascade will lead to mast cell degranulation. This review summarizes our recent studies on the role of PTK signaling cascade in an in vitro guinea pig model of allergic asthma using PTK inhibitors, genistein and tyrphostin 47, and MAPK kinase inhibitor, PD098059. Inhibitors of the PTK and MAPK signaling pathways significantly attenuated the ovalbumin (OVA)-induced bronchial anaphylactic contraction and enhanced relaxation of constricted airways, respectively, and substantially blocked the release of histamine and peptidoleukotrienes from chopped lung preparations induced by OVA. Based upon their substantial inhibitory effects on the Schultz-Dale reaction, further examination on the potential anti-asthmatic effects of PTK cascade inhibitors in an in vivo model of allergic asthma is recommended.

[Inhibited effect of phosphorylated insulin-like growth factor binding protein-1 on hepatoma cells in vitro].

OBJECTIVE: To elucidate the inhibited effect and regulated mechanism of IGFBp-1 on the carcinoma cells. METHODS: The recombinant human IGF II (rhIGF II), phosphorylated and nonphosphorylated binding protein-1(pBp-1 and npBp-1) that were purified from pregnant woman's amnoitic fluid were used to cultivate cancer cells from 15 cases with hepatocellular carcinoma and ascites. The cell counting and MTT methods were used for determining the dynamics change of proliferation on HCC, and the in situ or dot hybridization with Digoxin label used for the expression of IGF II mRNA qualitatively and quantitatively at different times. RESULTS: The cells grew more slowly, in pBp-1 group, than in control group at the same time, which began to decrease at two days(P < 0.05) by MTT. In situ hybridization showed that there are hybridization signal of IGF II mRNA in all groups during the culture, and scanning dot hybridization showed the OPTMD of IGF II mRNA in pBp-1 group was statistically different from control and other groups (P < 0.01), while in npBp-1 group there was no significant effect compared with other groups (P > 0.05). CONCLUSION: The pBp-1 can inhibit the growth of hepatoma cells and the expression of IGF II mRNA, which may be regulated and inhibited by pBp-1.

Molecular therapy for glioblastoma.

Glioblastoma (GB), the relatively frequent and most malignant form of primary brain tumor, is fatal within 1 to 2 years of onset of symptoms, despite conventional therapy. Molecular therapy promises to be an effective and possibly curative treatment. Several molecular strategies have been tested, either in animal models or clinical trials. These include: prodrug activating systems, introduction of tumor suppressor or cell-cycle-related genes, inhibition of growth factors and/or their receptors, inhibition of neovascularization, immunomodulatory maneuvers, oncolytic viruses and inhibition of matrix metalloproteinases. Of special interest for the development of optimal molecular therapy of GB, is the choice of the most efficient and least toxic gene vectors (adenovirus, retrovirus, herpes simplex virus), the route of administration of the therapeutic agent (intratumoral with or without debulking and intracarotid), avoidance of collateral damage to the perineoplastic neuropil and adequate preclinical studies. The ultimate molecular therapy will probably involve the application of multiple simultaneous (combinatorial) therapeutic modalities. The safety and efficiency of these in humans can only be judged by properly controlled therapeutic trials.

MHC class I molecules, structure and function.

MHC class I molecules (MHC-I) are cell surface recognition elements expressed on virtually all somatic cells. These molecules sample peptides generated within the cell and signal the cell's physiological state to effector cells of the immune system, both T lymphocytes and natural killer (NK) cells. In addition, molecules structurally related to MHC-I, collectively known as MHC-Ib, are more specialized and, in some cases, interact with more limited subsets of lymphoid cells. Using the recently determined structure of the classical MHC-I molecule, H-2Dd, as a paradigm for structure and function, we review other MHC-I and MHC-Ib molecules, with an emphasis on how the same basic structural fold is employed by classical MHC-I molecules to bind specific peptides and T cell receptors, and is exploited by the MHC-Ib molecules in more stringent molecular interactions. It is instructive that structurally related molecules have evolved to perform a number of unique and distinct functions in immune and non-immune recognition.

[The effect of vasectomy on spermatogenesis and proliferation of rat spermatogenic cell].

In order to explore the effect of vasectomy on spermatogenesis and proliferation of rat spermatogenic cell, we detected the DNA content of rat spermatogenic cells by flow cytometry at 4 weeks and 8 weeks after vasectomy. The results showed that the percentages of 1C cell group (spermatid and sperm) in the two experimental groups were apparently lower than that of the two control groups respectively (P < 0.05), but the percentage of 2C cell group (spermatogonium and spermotocyte) in the two experiment groups were much higher than those of the two control groups respectively (P < 0.05); The proliferation index (PI) of experimental groups was significantly lower than that of control groups (P < 0.05) and most of the cells in the experimental groups were on G0/G phase. The percentage of S-phase cell in the 8 weeks experimental group was obviously lower than that of the 4 weeks experimental group. These findings suggest that vasectomy in rat may affect many stages in spermatogenesis and delay the proliferation of spermatogenic cell.

Enhancing effects of silkworm expressed recombinant human macrophage colony-stimulating factor on hematopoietic recovery of irradiation-injured mice.

OBJECTIVE: To study the hematopoietic enhancing effects of recombinant human macrophage colony-stimulating factor (rhM-CSF) expressed in silkworm. METHOD: Balb/c mice were irradiated with sublethal dose of 60 Co gamma-rays and then administered intraperitoneally with silkworm expressed rhM-CSF (1000 U per individual for 7 days) in treatment group or with normal saline (for 7 days as well) in control group. The hematopoietic recovery of irradiation mice was observed by comparing peripheral white blood cell (WBC) counts, differential counts of WBC and bone marrow hematopoietic progenitor cell colony forming assay in soft agar at different time after irradiation. RESULTS: The total WBC counts (x 10(9)/L) of treatment group at day 15 and 20 after irradiation(3.42 +/- 1.20, 5.56 +/- 2.50, respectively) were significantly higher than those of control group (2.03 +/- 0.90, 3.72 +/- 2.30; both P < 0.05). On days 10, 15, 20, 25 and 30 after irradiation, the monocyte counts (x 10(9)/L) of treatment group (0.08 +/- 0.06, 0.16 +/- 0.10, 0.48 +/- 0.35, 0.47 +/- 0.21 and 0.33 +/- 0.17, respectively) were all significantly higher than those of control group (0.025 +/- 0.016, 0.05 +/- 0.04, 0.23 +/- 0.16, 0.33 +/- 0.19 and 0.17 +/- 0.13; all P < 0.05). On days 15, 20 and 25, the granulocyte count (X 10(9)/L) of treatment group (1.03 +/- 0.61, 2.18 +/- 1.19 and 3.28 +/- 1.09) were also higher than those of control group (0.62 +/- 0.37, 1.40 +/- 0.99 and 2.20 +/- 0.74; all P < 0.05). On day 9 after irradiation, the bone marrow CFU-GM yield of control group (19 +/- 11/10(6) cells) was significantly lower than that of treatment group (78 +/- 30/10(6) cells, P < 0.05). CONCLUSION: rhM-CSF expressed in silkworm could accelerate hematopoietic recovery in irradiated mice.

Identification of mutation in a candidate gene for hereditary multiple exostoses type II.

OBJECTIVES: To identify possible mutations in our previously cloned candidate gene for hereditary multiple exostoses type II (EXT2) in affected members of EXT families so as to confirm that it is the disease-causing gene. METHODS: The mutation was detected first by single strand conformational polymorphism (SSCP) of all coding exons of the candidate gene and then by sequencing analysis. RESULTS: After analyzing 37 patients from 20 Chinese EXT families by SSCP and DNA sequencing analysis, one 2-bp insertion mutation was identified in this candidate gene in affected members of an EXT family. This mutation resulted in the frameshift and generated a truncated gene product consisting of 105 amino acids. CONCLUSIONS: The identification of the mutation in the candidate gene indicates that this novel gene is responsible for EXT2 (one of the disease-causing gene of EXT).

[Study on electrochemical behavior of HL-60 cells during the etoposide-inducing apoptosis].

OBJECTIVE: To study the electrochemical behavior of apoptotic HL-60 cells induced with etoposide. METHODS: Electrochemical cytosensor was used to monitor HL-60 cells treated with etoposide. RESULTS: The peak current and electron transfer rate decreased as etoposide dosage increased. The decrease appeared at 2 micrograms/ml etoposide treatment and was remarkable at 20 micrograms/ml, which occurred before the typical changes of cell morphology and DNA contents for apoptosis. FCM-DNA analysis showed that the apoptotic cells were of 10.5%-20% while the peak current decreased from 1.8 microA to 0.4 microA after 2.3 and 4 hours treatment with 200 micrograms/ml etoposide, and the peak current after 4 hours treatment was 5 times lower than that before induction. CONCLUSION: The changes of electrochemical behavior during the early stage of apoptosis occurred earlier and could be accumulated.

The role and significance of Luo Zhiyuan's Shu yi hui bian in the history of plague in Lingnan (south of the five ridges).

Being the earliest monograph on plague in China, Luo Zhiyuan's Shu yi hui bian, not included in the National Catalogue of TCM Books, include the following contents: personal idea on the etiology of plague; Luo's friend Wu Xuanchang' unpublished Shu yu zhi fa on the treatment and manifestations of plague; Luo's specific recipe for plague based on medified Wang Qingren's Jie du huo xue decoction based on Wang Qingrens yi lin gai cuo; therapy for critical cases; many therapies applied on Lingnan, including experimental recipes, external therapy, preventive methods, and preventing recurrence methods; Luo's special administrating methods, including persisting day-and-night method, immediate persisting method, single-dose persisting method, and double-dose persisting method. He also gave several cured case records. His book, featuring unique idea with good effect, was repeatedly printed and extensively distributed, exerting influence, more or less, on the plague monographs of later ages, and occupying important position in the history of plague on Lingnan and the whole country as well. His idea of "that poisons and static blood" in pathogenesis and therapeutic principle of antitoxicity and activating blood is coincided with the results of present day clinical and laboratory studies. His administration of medicines is heuristic to the therapy of critical cases with Chinese medicaments and to the recognition of pathogenesis, etiology, and treatment of modern plague as well as other diseases of similar etiology and pathognesis and is worth of further study.
