[High dose gestagen therapy following primary radiation therapy of endometrial carcinoma (author's transl)].

By a high dose gestagen therapy following primary radiation therapy of endometrial carcinoma it was possible to reduce the recurrence-rate 12 months after therapy in comparison with a control-group without gestagen therapy by more than 50%. Therefore this method is recommended not only for treatment of far advanced carcinoma of the endometrium but also as prophylactic therapy in cases of early endometrium cancer.

[Geriatric gynecology with special reference to malignant tumors].

After the enumeration of every single problem encountered in the geriatric gynecology, it has been proved, that the malignant tumors of such cases are of a special significance. according to the epidemiological facts of the Austrian Cancer-Registration-Statistics between the years 1971--1975 (even if the cancer of cervix uteri is excluded) more than 60% of the cases are still seen in the geriatric group. Increasing risk (by rate of incidence) of malignant tumors of internal genitals or the breast in aged women is also known and is proved to be very specific. Inspite of the progress won in geriatric surgical gynecology, the post-operative complications are still the negative factors, which limitate the success. Therefore today radiotherapy of the malignant tumors especially in geriatric-gynecological patients is gaining more and more importance.

[The significance of primary radiotherapy in cervical carcinoma in older women].

In spite of the increasing interest in geriatric-operative therapy primary irradiation therapy still has its important place in the treatment of cervical cancer in cases of higher age. About every third patient with cervical cancer belongs to the age group over 60 years, which is more or less excluded from operative therapy. The advantage of irradiation therapy is to be seen in the low post-therapeutic mortality and the remarkable survival and healing rates.

[Epidemiology of cervical cancer in Austria].

In cooperation with the Austrian National Institute for Statistics the incidence rates of cancer of the cervix uteri in Austria in the period of 1971--1975 were investigated. Not only the different federal countries but also the political districts were concerned by this study. The main findings were a relative high rate of women less than 40 years of age and severe regional differences. Problems in the registration of incidence rates of cancer are pointed out.

[Diagnosis of breast cancer by contact thermography (author's transl)].

Epidemiological studies performed in cooperation with the Austrian National Institute for Statistic show an increase of breast cancer in the age-group of women older than 50 years. Especially in this age-group screening for breast cancer is very important and successful. Mammography in this age-group is the most useful and accurate method. In the group of women aged less than 50 years and particularly less than 45 years the use of mammography for screening is limited because of its risks for the induction of breast cancer in young women. Therefore the combination of clinical examination and contact-thermography is a very promising alternative for the detection of breast cancer. In 133 cases of histologically proved breast cancer the high accuracy of this combination of clinical examination and contact-thermography in comparison with mammography is shown. Based on this good results a recommendation for a detection-scheme for breast cancer with a reduction of radiation exposure in young women by the use of contact-thermography is presented and discussed.

[Partial "rooming-in"--a model from the I department of gynecology and obstetrics, university of Vienna (author's transl)].

A model of a modified "rooming-in" is presented, which enables a longer and more intensive contact between mother and child. Mobile transport-baskets as used also for example in supermarkets etc. enabled us to perform this system of a partial "rooming-in" without any architectural modifications in our clinic and therefore with very low costs. In spite of all these limitations the effect of this "rooming-in" is sufficient. We proved this statement by an inquiry which showed the result, that partial "rooming-in" is preferred a total "rooming-in" by about 75% of our maternity patients.

[Epidemiology of corpus carcinoma in Austria (author's transl)].

The rates of endometrial carcinoma incidence in Austria were investigated for the period between 1971 and 1975 by the authors in cooperation with the Austrian National Institute of Statistics. The incidence of corpus carcinoma has increased also in Austria over the past decades, as may be seen from the ratio between corpus carcinoma and cervical carcinoma which is 1:1.9 at present. Age related incidence was studied, as well, with continuous growth in incidence having been observed along with growing age. In women aged between 60 and 69, corpus carcinoma was almost as frequent as cervical carcinoma, and it was predominant among women of 70 and above. The findings obtained from age group surveys have also suggested that irradiation therapy rather than primary surgery was required for more than 50 per cent of the patients reviewed.

[Results of an interdisciplinary study on the value of contact thermography in the diagnosis of breast disease (author's transl)].

An interdisciplinary study was carried out to determine the accuracy of contact thermography in the detection of breast cancer. 200 patients were examined in form of a blind test, the mammographic and clinical findings being listed separately and inaccessible to the thermography team. The findings obtained on contact thermography also were listed separately and afterwards compared with the clinical and mammographic findings. 50 out of these 200 cases required histological clarification, which revealed 24 carcinomas and 26 benign conditions of the breast. Mammography and contact thermography both failed to detect 3 out of the 24 carcinomas, clinical examination gave a false negative result in 2 cases. Therefore, in the present case material, the accuracy of mammography and contact thermography is equal and enables us to state that the combination of clinical examination and contact thermography of the breast is of high accuracy and suited for routine mass screening programmes to enable the detection of breast disease without radiation exposure.

[The effect of transcutaneous nerve stimulation on labor pain (author's transl)].

The analgesic effect of transcutaneous electric nerve stimulation (TNS) during delivery has been studied. 57 (55.9%) of 102 women treated considered TNS had given them good or very good relief from pain; 24 (23.5%) reported that they had received some relief, while 21 patients (20.6%) were of the opinion that they were not helped by the technique. CTG monitoring ocurred in all cases. No side effects on the fetus or the mother have been noted. A positive aspect of TNS was the quick course of delivery and the absence of obstetrical complications in the cases treated with this method. Therefore the designation of TNS as a new, riskless, but effective technique for pain relief during childbirth seems justified.

[Accuracy of triple-diagnosis of the "small" carcinoma of the mamma (author's transl)].

Mammography was carried out in 3,500 patients. Of these 1,800 were examined with plate-thermography and in 450 cytology was done. 12 cases of "small" (10 mm) Ca were found, in 3 cases with thermography. In 4 cases thermography was negative, in one case for technical reasons. Mammography is the method of choice, thermography and cytology are valuable aids.

[The value of scintigraphy for the early detection of osteomyelitis (author's transl)].

For therapy and development of acute haematogenic osteomyelitis early detection of this disease is decisive. In our study 74 children were investigated scintigraphically with Tc-pyrophosphat because of suspicion of an inflammatory bone process. In all 23 cases of osteomyelitis scintigraphy showed an indication in form of increased activity in the respective bones; so the sensivity of this investigation, found in our study, was 1.0. In 10 of 51 cases without osteomyelitis scintigraphy, however, also showed increased activity. Because of this limited specifity of scanning further investigations are needed in the diagnosis of osteomyelitis. The main advantage of scintigraphy as compared with roentgen observation is the high sensitivity in the first days of illness. In addition scanning may supply valuable informations about the inflammatory process as long as it is active.

[Therapy and prognosis of female genital sarcomas].

A report is given about 75 genital sarcomas treated at the 1 st university clinic of obstetrics and gynaecology in vienna during the period 1960--1972. This number consists of 60 sarcomas of the corpus uteri, 3 cervical sarcomas, 8 ovarian sarcomas and 4 sarcomas of the vagina. Their therapy and prognosis is discussed. The importance of radical operative therapy with postoperative radio therapeutic treatment is emphasized and the necessity of a central documentation and a uniform classification of clinical and histological stages is pointed out.

[Multiple malignant tumours in gynaecology (author's transl)].

At the radiotherapy unit of the Ist Department of Gynaecology, University of Vienna, 8500 malignant tumours were observed in the years 1950-1975. Multiple primary neoplasms were recorded in 204 of these cases (2.4%). In 76 women (37.2%) the double neoplasms were situated in the genital region (local multiplicity), whilst in 128 cases the second neoplasm was situated in a distant organ, with cancer of the breast predominating (61 cases = 29.9%). A third of the multiple neoplasms were diagnosed simultaneously, whilst in the remaining two thirds a peak occurrence in the first year before or after the diagnosis of the gynaecological neoplasm is observed. Seven cases of triple neoplasms are presented as a separate group. Our observations are compared with the appropriate literature.
