[The influence of socio-economic factors on the results of a prematury-Dysmaturity prevention programme (author's transl)].

All high-risk gravidae with regard to prematurity and dysmaturity (PDP programme) were collected over a time-limited period. More than two thirds (n = 72) of these women were submitted to intensive care (PDP group); one third (n = 33) (control group) refused intensive care. Furthermore, socio-economic factors were taken into consideration in this study and appropriate classification into 4 groups was undertaken. Gravidae of a higher social class were more often willing to undergo intensive care than gravidae of a lower class. In the PDP group 75% of the gravidae were delivered after the end of the 36th gestational week and 51% of the gravidae in the control group. A similar relationship was found in regard to the birth weight of the newborn infants: in the PDP group 74.4% of the babies weighed over 2500 g at birth in contrast to the respective figure of 42.9% in the control group. However, this socio-economic study shows that the results of intensive care are much more successful in women from a lower social stata than in women from a higher social class.

[Therapeutic results in sarcoma of the uterus (author's transl)].

60 cases of sarcoma of the uterine body were treated at the Ist Department of Obstetrics and Gynaecology, Vienna University, during the period 1960 to 1971. Treatment consisted of combined surgery and irradiation (37 cases), surgery alone (8 cases) and irradiation only (14 cases). The 5-year survival rate was 53.3%.

[Radium treatment of primary vaginal carcinoma (author's transl)].

Between 1951 and 1970, 263 cases with primary carcinoma of the vagina were observed at the Irradiation-Department of the Ist University Clinic of Obstetrics and Gynecology in Vienna. As method of choice the individual, fractioned and protracted Radium-Röntgen-Telekoalt-therapy is recommended. Of the 263 primary vaginal carcinomas 112 (42,5%) were cured. During the time mentioned in this report, we did not have any primary mortality; 24 women, however, could only be subjected to a palliative therapy, 20 patients died from an intercurrent disease and 20 women succumbed to a second carcinoma. Conclusively, the authors present a survey about the results in vaginal carcinoma.

[Efficiency of primary radiotherapy in generally or locally inoperable carcinoma of the endometrium].

The usual method of irradiation in the therapy of the carcinoma corporis uteri is described. It is stressed, that none but inoperable carcinomas were irradiated: this means, of course, a negative selection in disfavor of the radiotherapy. A report is given of 802 cases of carcinoma of the corpus uteri, treated by primary radiotherapy in the 1st University Clinic of Obstetrics and Gynecology in Vienna in the years 1950-1970. The 5-years-survival rate in this period was 50.4%. In the last half decade of this period the 5-years-survival rate of 300 patients went down to 48.6%. The reduction of this survival rate may be caused by the increasing age of the patients, who underwent therapy and is corresponding to world-wide experiences. Additive therapies like the "second-look abrasio" for early diagnosis of a local recidivation and the gestagen therapy for the prevention of metastatic carcinoma of the corpus uteri are discussed.

[Intraepithelial carcinoma of the cervix uteri in young women (author's transl)].

78 cases of cervical carcinoma in situ in women aged thirty years or less treated at the 1st Universitäts-Frauenklinik in Vienna in the period 1960-1974 are being reported. This number represents 10,7% of all 729 cases of cervical carcinoma in situ in this period. It was surprising to notice a distinct increase in the incidence of this disease in the last years. The amount of young women with cervical carcinoma in situ went up to 15.8% in the period 1972-1974.

[Primary radiation therapy of sarcoma of the uterus (author's transl)].

We report 24 cases of sarcoma of the corpus uteri treated by sole irradiation. The 5-year-survival-rate was 20,8%. The prognosis seems to be poor in this group treated by sole irradiation. Only inoperable cases of sarcoma of the corpus uteri ought to be treated by radiotherapy alone.

[The importance of immune deficiency for the prognosis of purulent meningitis (author's transl)].

Morbidity and mortality of suppurative bacterial meningitis show a very constant behavior over many decades. The introduction of antibiotics certainly lowered the lethality considerably, but since then it has remained fairly unchanged at about 15-20%. However, in the principal groups of pathogens--meningococci, pneumococci, Haemophilus influenzae--antibiotic resistence plays no special role. The significance of disorders of defense against infections for the course of the disease has not yet been investigated. Of 92 children examined with purulent meningitis, 49 showed a selective antibody deficiency syndrome against bacterial antigens, 38 of 48 children investigated showed further disorders of bactericidal activity. The proportion of these disorders is particularly high in children with defective healing of a meningitis.

[The place of plate thermography in the diagnosis of breast cancer (author's transl)].

A report is presented of 319 unselected patients investigated by the method of plate thermography according to Tricoire, which is a comparatively new method in the diagnosis of breast cancer. 281 cases were additionally examined by mammography and those cases in whom malignancy was suspected underwent further cytological and histological investigation. The results of plate thermography and mammography are compared. The high degree of diagnostic accuracy achieved in the present series of cases by the use of plate thermography indicates that this procedure is not only a valable adjunct in the diagnosis of breast cancer but, moreover, seems to be recommendable as a screening method.

Electro-acupuncture's influence on the closure mechanism of the female urethra in incontinence.

The influence of electro-acupuncture on the sphincter of the female urethra was checked on 20 patients with stress incontinence without gross anatomic variation. This was determined by means of simultaneous cysto-urethromethrometry. After 30 minutes of electric stimulation through acupuncture needles which were placed into the skin of the lower legs and lower abdomen, a significant increase in the so-called "closing pressure" was found. Seventeen patients even showed positive pressure. In a control group of another 20 patients who, however, did not undergo acupuncture, there were only two cases of slight increase in closing pressure. In addition, a second control group of 20 patients was given a placebo suppository in order to eliminate any psychic factor as far as possible. Significant change of the closing pressure could not be found in any of these cases. Even though the actual working mechanism of electro-acupuncture is not understood, these experiments seem to confirm the assumption of electro-acupuncture's positive influence on the closing mechanism of the female urethra.

[Improvement of the patch-test reading through contact thermography].

The evaluation by contact-thermography (with heat sensitive cholesterinester crystals) of patch tests for contact dermatitis is an important aid to detect weak positive or false negative reactions. In addition, it facilitates the interpretation of patch tests in dark skin persons. The number of diagnosed cases of contact dermatitis can be increased by about 10% without increasing the expenditure of time on the part of the patient or the physician.

[The value of cytological screening for neoplasms in young women].

The question of the incidence of positive or suspect cervical cytology-tests in young women of 30 years or less of age out of a total of 54,604 cervical cytology-tests is discussed. The results of clinical, cytological, and histological follow-up are reported. According to our experiences that 158 smears of group Pap. III and 99 smears of group of Pap IV and V were found in young women of 30 years or less of age this group of patients should not be excluded from consecutive cervical cytology-tests or national health-screening programs.

[Encephalitis and humoral immunity].

Fourteen children with viral infection of the central nervous system have been examined with respect to their function of humoral immunity. Serum concentrations of immune globulins have been measured and antibody titers determined. Iso- and hetero agglutinins as well as antibodies against diphtheria, pertussis, and tetanus if the children were vaccinated have been measured. Anti viral antibodies were tested if children reported the respective infection in their history. Selective antibody deficiency have been found in three children all of whom had normal serum IgG and two elevated IgM levels. These three children showed complications in the course of infection. Nine children had elevated serum IgM and titers of IgM-antibodies were also in or above the high normal range.

[The importance of the risk factor "obesity" during gravidy (author's transl)].

Between 1. 6. 1972 and 31. 12. 1974 obese gravidae with a body weight of 90 kg or more were observed (7,6% of all patients in this period). This 315 patients were compared with a group of 315 patients with normal body weight chosen by random. The results of both groups were compared and evaluated by statistic (t-test, x2-test). A significant higher rate of Diabetes, latent Diabetes, EPH-Gestosis and cases of over-term deliveries were found in the group of obese patients.

[The importance of the risk factor "obesity" during delivery, postpartal and for the newborn (author's transl)].

The course of delivery and labor, the post partum period and the postpartal condition of the newborn were reviewed in 315 obese patients with a bodyweight of 90 kg or more at delivery. The results were compared with those of a group of 315 patients with a body-weight of less than 90 kg, choosed by random and evaluated by statistic (t-test, x2-test). In the group of obese gravidae we found a significant higher rate of cases with induction of labor, protracted deliveries, breech presentation and operative deliveries. Also the rate of manual removal of the placenta, post partum uterine infections and disorders in the milk secretion was significant higher in this group. Post partum abnormal glucose tolerance tests ocurred abnormal more frequently in the obese patients. The weight of the infants of obese mothers was higher as in the control group. Significant higher were the perinatal mortality and the cases of fetal death in utero in the obese group compared to the non obese group.

[Experiments of electro-acupuncture-analgesia during delivery (author's transl)].

We report about 20 patients who underwent electroacupuncture therapy during delivery with intent to reduce labor pains. Acupuncture steel needles situated in the thoracal, lumbal, sacral region and in the region above the symphysis were stimulated by a rectangular impulse current. A good effect for the reduction of labor pains could be observed in 5 cases, a moderate one in 6 cases. An absolute analgetic effect could not be achieved in any case, and in 9 cases no analgetic effect of acupuncture therapy on labor pains was visible. Acupuncture had no influence on labor or fetal heart rate, monitored by cardiotocography.
