Intestinal alkaline phosphatase linked to immunoglobulin G of the kappa type.

An electrophoretically slow-moving alkaline phosphatase(orthophosphoric-monoester phosphohydrolase, EC 3.1.3.1, alkaline optimum, ALP) isoenzyme was found in the serum of a patient with osteomalacia. Molecular mass of this ALP was estimated to be 500 000 daltons. Inhibitor studies revealed that the abnormal ALP, which formed a macromolecular complex, was an intestinal isoenzyme. Immunofixation following electrophoresis demonstrated that the macromolecular ALP was a complex composed of intestinal ALP and immunoglobulin G of the kappa type. This isotype specificity of immunoglobulin suggested that the ALP immunoglobulin complex is a specific antigen-antibody complex in circulation.

Intracellular forms of transferrin oligosaccharide chains in rat liver.

Glycosylation of transferrin was investigated in vivo by using antibody monospecific for rat serum transferrin. Pulse-chase experiments indicated that, after intravenous injection of [35S]methionine, labeled transferrin appeared in the rough and smooth microsomes and Golgi subfractions in rapid succession in 10 min and that an additional 10 min was required for it to be secreted. Most of the intracellular transferrin (95%) immunoprecipitated from the total microsome fraction was sensitive to endo-beta-N-acetylglucosaminidase H (endo H), whereas serum transferrin was completely resistant to it. Further fractionation of the total microsomes has revealed that the intracellular transferrin immunoprecipitated from the rough and smooth microsomes and GF3 are all endo-H-sensitive and most of the endo-H-sensitive oligosaccharides were eluted at the position corresponding to Man8GlcNAc on high-resolution Bio-Gel chromatography. This finding suggests that the major form of intracellular transferrin oligosaccharide in the course of intracellular transport from the endoplasmic reticulum to the Golgi apparatus is Man8GlcNAc2. Endo-H-resistant forms were first detected in the GF2 but more in GF1, most of which were sensitive to neuraminidase. Since the heavy Golgi subfraction contains mainly cis-Golgi elements, such as cisternae, and the light subfraction mainly trans-Golgi elements, such as secretory granules, it is strongly suggested that the processing of these large mannosyloligosaccharide chains and the subsequent addition of terminal sugars to them are performed successively in the trans-Golgi region just before secretion.

Concanavalin A-induced and spontaneous suppressor cell activities in peripheral blood lymphocytes and spleen cells from gastric cancer patients.

In 173 gastric cancer patients, activities of Concanavalin-A-induced suppressor cells (Con-AS) and spontaneous suppressor cells (SpS) in peripheral blood lymphocytes (PBL), splenic vein lymphocytes (SVL), and spleen cells (SCs) were investigated. Suppressions by Con-AS in PBL were significantly effective in patients of Stages III and IV, while suppressions by SpS were effective in patients with recurrent tumors. Thus, in PBLs of cancer patients, suppressor precursors, which are considered to be activated in vitro by Concanavalin-A, seemed to appear with the advances of the disease, and SpS activities, which could be already activated in vivo, seemed to increase in the terminal stage. In SCs, increased activities of Con-AS, but normal activities of SpS, were observed, and these suppressor-cell populations consisted of glass nonadherent cells. Suppressor activities of SCs would be due to suppressor T-cells, not to other types of cells. Furthermore, Con-AS existed in the medium-sized lymphocytes, which were fractionated on the basis of cell size, while SpS in the large-sized lymphocytes. A higher proportion of T-cells, bearing Fc receptors for IgG, was observed in the larger-sized lymphocyte fractions. Cell numbers in the large-sized lymphocyte fraction tended to increase with the advances of tumors. From these results, it is suggested that higher presence of suppressor precursors and the increase of SpS activities may occur in cancer patients, depending on the tumor advancing.

Association of HLA-B40 and DRW9 with Japanese alcoholic liver cirrhosis.

Seventy-seven chronic alcoholics with liver disease were studied to evaluate the HLA antigen association. There were no significant differences of HLA antigen phenotype frequencies (PF) between the patients and controls regarding A and C loci, (62 healthy Japanese). Prevalences of HLA-B40 complex (B40 . 48 . 13) and DRW9 tended to increase among chronic alcoholics. When chronic alcoholics were divided according to whether they had liver cirrhosis or not, the cirrhosis group (42 cases) revealed a significantly higher frequency of HLA-DRW9 (chi 2 = 10.88, p less than 0.001, corrected p less than 0.05, relative risk (R.R.) = 4.17) as compared to controls. There was also a tendency of B40 complex to increase in frequency (chi 2 = 5.51, p less than 0.05, R.R. = 2.65) in the cirrhosis group. Haplotype frequency and linkage disequilibrium parameters of HLA-B40 . 48-DRW9 were significantly higher than those of controls. Moreover, the increased frequency of DRW9 in the cirrhosis group was similar to that in autoimmune disease like ulcerative colitis or SLE. These data suggest that HLA-DRW9 and/or HLA-B40-DRW9 might be closely associated with susceptibility to developing alcoholic cirrhosis and that autoimmune mechanisms might be involved partly in its etiology.

Anaphylaxis-induced increase in plasma cyclic AMP dependent on endogenous catecholamines in rats.

Plasma cyclic AMP levels during anaphylactic shock in rats was studied in 7 groups of animals: (1) control rats; (2) rats with adrenomedullectomy; (3) rats treated with propranolol; (4) rats with reserpinization; (5) rats with 6-hydroxydopamine-induced chemical sympathectomy; (6) rats treated with hexamethonium, and (7) rats treated with cocaine (catecholamine uptake inhibitor). All experiments were carried out in ovalbumin-sensitized rats. Plasma cyclic AMP showed a rapid increase during anaphylactic shock in control rats. Adrenomedullectomy abolished the anaphylaxis-induced increase in plasma cyclic AMP, while hexamethonium had no effect. Propranolol caused a dose-dependent abolition of the increase. The treatment of rats with reserpine, 6-hydroxydopamine or cocaine partially inhibited the increase in plasma cyclic AMP. The results show that the adrenal medulla is the major source of catecholamines during anaphylactic shock, and that catecholamines in the adrenergic neuronal terminals may be partly responsible for the anaphylaxis-induced increase in plasma cyclic AMP.

Treatment of idiopathic pituitary dwarfism with methionyl human growth hormone.

Ten patients with idiopathic pituitary dwarfism were treated with methionyl human growth hormone (met-hGH) produced by recombinant DNA technology. They were given 0.5 IU/kg/week of met-hGH for three months. There were no significant changes in physical, blood, urine examinations. Their height increased between 1.5 and 2.7 cm during the 3 months of treatment, which was calculated to be equivalent 6.0 and 10.8 cm per year with a mean increase of 8.7 cm. Anti-hGH antibody was observed in all patients between one and two months of treatment with a titer between 10(1)X and 10(4)X. In spite of the formation of antibodies, the growth rate did not decrease in 7 of the 10 cases.

Induction of suppressor cell activities in normal lymphocytes by sera from gastric cancer patients.

Inductions of suppressor cell activities in normal lymphocytes by sera from 154 patients with primary gastric cancer were investigated. Sera from patients with advanced cancer induced suppressor cells which significantly depressed the proliferative responses of autologous responder lymphocytes, while those from early cancer did not. An increase in OKT8 reactive T cell populations in normal lymphocytes occurred rather than a decrease in OKT4 reactive T cells after stimulation with sera as well as concanavalin A in relation to the increase of suppressor cell activities. Furthermore, sera-induced suppressor cell activities apparently correlated with spontaneous suppressor cell activities in cancer donors and both of these activities declined after plasma exchange. These results suggest that sera from gastric cancer patients may contain factors which can induce such suppressor cell activities and that serum factors may activate the suppressor precursors to be spontaneous suppressor cells in vivo in a manner similar to that seen in vitro.

[Studies on the cortisol levels in maternal vein, umbilical artery, and umbilical vein classified according to the mode of delivery].

We measured maternal venous(MV), umbilical arterial(UA), and umbilical venous(UV) cortisol levels of 180 pregnancies at term. In vacuum extraction cases, the cortisol levels of three individual samples were significantly higher than in other modes of delivery. In cesarean section delivery the cortisol levels of three individual samples were the lowest of all modes of delivery. The cortisol level of MV was higher than that of UA and UV in all deliveries. The cortisol levels of three samples showed no difference between the induced labor group and the spontaneous onset of labor group regardless of the delivery mode. In spontaneous vaginal delivery the cortisol levels of three individual samples were significantly higher in primipara than in multipara. Positive correlation was observed in all modes of delivery between the cortisol levels of MV & UA, MV & UV, and UA & UV. There was a negative correlation between the cortisol levels of MV and umbilical arteriovenous cortisol difference in multipara spontaneous vaginal deliveries. There was also a negative correlation between cortisol levels of MV and the umbilical arterial/venous ratio. These data suggest that the fetus may produce cortisol by itself and maternal cortisol may inflow to the fetus.

[Studies on cortisol levels in the maternal vein, umbilical artery, and umbilical vein in cases of fetal distress and neonatal asphyxia].

We measured maternal venous (MV), umbilical arterial (UA) and umbilical venous (UV) cortisol levels of 180 pregnancies at term, and investigated especially cases of fetal distress and neonatal asphyxia. No fetal distress and no neonatal asphyxia cases followed spontaneous vaginal deliveries (94), cesarean sections (18) or breech extractions (6). In cesarean section delivery, the cortisol levels of three individual samples were lower than in other modes of deliveries. The cortisol level of the umbilical artery was significantly higher than that of the umbilical vein in spontaneous vaginal delivery and cesarean section delivery. When spontaneous vaginal deliveries were classified into the fetal distress group and no fetal distress group, the cortisol levels of the umbilical artery and umbilical vein in the former group were significantly higher than those in the latter group. Furthermore, regardless of fetal distress there were significant positive correlations between the cortisol levels of MV & UA, MV & UV, and UA & UV. In addition, as the cortisol level of the maternal vein increased, the umbilical arterial-venous cortisol ratio (UA/UV) gradually approached 1.0. These data suggest that in fetal distress or neonatal asphyxia, the fetus may produce cortisol for itself independent of the mother.

Prognostic significance of lymphocyte proliferative responses to mitogens in gastric cancer patients.

In 501 Japanese patients with gastric cancer, the relationships between preoperative lymphocyte proliferative (LP) responses to mitogens and prognosis of patients were evaluated. Peripheral blood lymphocytes were cultured in the presence of either autologous or allogeneic serum and their LP responses to phytohemagglutinin (PHA) and pokeweed mitogen were investigated. An apparent inverse relationship between LP responses and stage of the disease was found in LP responses to PHA in the presence of autologous serum. The survival rate of patients with higher responses was significantly greater than those with lower responses, when LP responses to PHA in the presence of autologous serum served as the criterion. Probabilities of staging which were computed on the basis of LP response to PHA affirmed the reciprocal relationship between LP responses and stage of the disease. From these results, it is concluded that evaluation of LP responses may be a valuable tool in the assessment of the clinical stage and in the prediction of prognosis.

Intracranial injection of thyrotropin releasing hormone (TRH) suppresses starvation-induced feeding and drinking in rats.

The sites at which TRH produces suppression on feeding and drinking were examined anatomically in the rat brain. This was accomplished by microinjecting nmol concentration of TRH into 6 different brain sites. Intracerebroventricular injection of TRH (25, 50, 100 nmol/rat) suppressed starvation-induced feeding and drinking in a dose related manner. The microinjection in a small amount of TRH (8 nmol/hemisphere) into the medial and lateral hypothalamus produced relatively severe anorexia and adipsia as compared with the other areas including the nucleus accumbens, the substantia nigra, the globus pallidus and the amygdala. It was concluded that the medial hypothalamus is the most sensitive site of TRH-induced anorexia and adipsia and the action of TRH on the lateral hypothalamus is also a possible mechanism mediating the decrease in water intake.
