Quadricuspid aortic valve--report of a case associated with severe aortic regurgitation and review of the literature.

A case of a 59-year-old woman who had a quadricuspid aortic valve associated with severe aortic regurgitation is reported herein. The anomaly was revealed by aortography, after which the incompetent valve was excised and replaced by a St. Jude Medical prosthesis. A quadricuspid aortic valve must be considered as a malformation which leads to severe valve failure in later life.

111In (III) uptake by inflammatory and normal tissues.

Tissue distributions of 111In (III) in the rats bearing granuloma, inflammatory tissue induced by turpentine oil, were compared with those of 67Ga. The results showed that indium-111 resembles 67Ga in the manner of uptake by inflammatory and normal soft tissues. The effect of cold-InCl3 on 111In (III) uptake showed that transferrin is not involved in the uptake of 111In (III) into inflammatory tissues but is involved in the uptake into liver and spleen.

Intraoperative ultrasonography in resection of small hepatocellular carcinoma associated with cirrhosis.

Preoperative detectability rates of hepatocellular carcinoma smaller than 5 cm in 113 cirrhotic patients were 91 percent by ultrasonography, 93 percent by computed tomography, and 92 percent by selective angiography. The combination of two methods produced detectability rates of 97 to 99 percent. One hundred three patients underwent various types of hepatic resection with the aid of intraoperative ultrasonography. Forty-four tumors (43 percent) were embedded in the liver, and these tumors were not detected by conventional surgical exploration. The detectability rates were 38 percent for hepatocellular carcinomas smaller than 2 cm, 57 percent for 2 to 3.5 cm tumors, and 71 percent for 3.5 to 5 cm tumors. All undetectable hepatocellular carcinomas were identified by intraoperative echography. The overall detection rate by this method was 98 percent, which was substantially higher than the preoperative rate. Intraoperative ultrasonography is a useful and indispensable method for performing atypical minor hepatectomy for the treatment of small hepatocellular carcinomas associated with liver cirrhosis.

Involvement of transferrin in the uptake of 67Ga in inflammatory and normal tissues.

The results from gel chromatography and electrophoresis showed that 67Ga is exclusively bound with transferrin both in vitro and in vivo, but high concentrations of sodium citrate strongly inhibited the binding of 67Ga to transferrin. The influence of sodium citrate on the uptake of 67Ga into inflammatory and normal soft tissues was also investigated. Sodium citrate decreased the uptake of 67Ga into the liver and spleen, but had no influence on the uptake of 67Ga into inflammatory tissue. These results suggest that the uptake of 67Ga into normal soft tissues occurs in a transferrin-bound form but into inflammatory tissue in an unbound form.

Androgen receptor in hepatocellular carcinoma as a prognostic factor after hepatic resection.

Androgen receptors (AR) in the cytosol of hepatocellular carcinoma (HCC) were assayed in 45 unselected patients in whom radical hepatic resection was performed. Thirty-one patients had detectable amounts of ARs in tumors, ranging from 2.3 to 82.6 fmol/mg protein with the dissociation constants (Kd) of 4.1 - 30.9 x 10(-10) M. The receptor was not found in the remaining 14 cases. AR negative HCCs were significantly more common among women and nonalcoholic patients. Otherwise, there were no significant difference in the clinicopathologic background between patients with AR positive HCCs and those with AR negative tumors. Three patients died of liver failure in the former group, whereas two died in the latter; one patient died of liver failure and the other died of pneumonia (results were not statistically significant). Excluding those five operative deaths, the recurrence rates were 67.9% in the group of patients with AR positive HCCs and 33.3% in the group of patients with AR negative tumors (0.1 less than p less than 0.05). The 5-year survival rate was significantly better (p less than 0.05) in patients with AR negative HCCs (62.2%) than in those with the positive tumors (17.3%). In light of the current results and previous experimental works by others, it is likely that testosterones enhance the growth and invasiveness of human HCC, which is mediated by AR in the tumor.

Blunted hypoxic ventilatory drive in subjects susceptible to high-altitude pulmonary edema.

It has been proposed that subjects susceptible to high-altitude pulmonary edema (HAPE) show exaggerated hypoxemia with relative hypoventilation during the early period of high-altitude exposure. Some previous studies suggest the relationship between the blunted hypoxic ventilatory response (HVR) and HAPE. To examine whether all the HAPE-susceptible subjects consistently show blunted HVR at low altitude, we evaluated the conventional pulmonary function test, hypoxic ventilatory response (HVR), and hypercapnic ventilatory response (HCVR) in ten lowlanders who had a previous history of HAPE and compared these results with those of eight control lowlanders who had no history of HAPE. HVR was measured by the progressive isocapnic hypoxic method and was evaluated by the slope relating minute ventilation to arterial O2 saturation (delta VE/delta SaO2). HCVR was measured by the rebreathing method of Read. All measurements were done at Matsumoto, Japan (610 m). All the HAPE-susceptible subjects showed normal values in the pulmonary function test. In HCVR, HAPE-susceptible subjects showed relatively lower S value, but there was no significant difference between the two groups (1.74 +/- 1.16 vs. 2.19 +/- 0.4, P = NS). On the other hand, HAPE-susceptible subjects showed significantly lower HVR than control subjects (-0.42 +/- 0.23 vs. -0.87 +/- 0.29, P less than 0.01). These results suggest that HAPE-susceptible subjects more frequently show low HVR at low altitude. However, values for HVR were within the normal range in 2 of 10 HAPE-susceptible subjects. It would seem therefore that low HVR alone need not be a critical factor for HAPE. This could be one of several contributing factors.

Passive air-pouch anaphylaxis in rats. I. Induction of anaphylaxis.

Induction of an experimental passive anaphylaxis of the air-pouch type, passive air-pouch anaphylaxis, was carried out in an attempt to induce a reproducible anaphylaxis model suitable for quantitative studies. Rats were injected subcutaneously with 10 ml of air into the dorsal skin to make an air-pouch and with 2 ml of antiserum at an appropriate dilution for passive sensitization, and then 5 ml of air was removed. The challenge with 5 ml of antigen solution into the air-pouch 48 h later provoked mast cell degranulation and increased vascular permeability induced by released histamine. Treatment with monovalent hapten prior to the antigen challenge almost completely inhibited histamine release and plasma exudation to levels similar to those in the nonsensitized group. In this model, mast cell-dependent late-phase allergic reaction, such as leukocyte migration or the increase of plasma exudation following mast cell degeneration, was not observed.

Passive air-pouch anaphylaxis in rats. II. Pharmacological characterization.

The effects of some antiallergic agents on passive air-pouch anaphylaxis (PAPA) in the dorsal skin of rats were investigated by measuring plasma exudation and histamine content in the pouch fluid. Antiallergic agents, disodium cromoglycate (DSCG), tranilast and ketotifen, dose dependently inhibited both plasma exudation and histamine release, except that ketotifen showed a dose-unrelated inhibition of histamine release. An antihistamine, pyrilamine, suppressed plasma exudation without affecting histamine release. A cyclooxygenase inhibitor, indomethacin, and a 5-lipoxygenase inhibitor, nordihydroguaiaretic acid (NDGA), showed no significant effect. A slow-reacting substance of anaphylaxis antagonist, FPL55712, exerted no effect at 0.3 and 3.0 micrograms/ml, but at 50 micrograms/ml it suppressed not only plasma exudation but also histamine release. Dexamethasone suppressed plasma exudation dose dependently without inhibiting histamine release significantly. A beta-stimulant, isoproterenol, and prostaglandin E2 also exerted significant inhibitory effects on plasma exudation but not on histamine release at 1 mg/ml. Theophylline inhibited both plasma exudation and histamine release. These observations indicate that PAPA is useful for studying antiallergic agents.

The hemopexin receptor on the cell surface of human polymorphonuclear leukocytes.

Promyelocytic leukemia HL-60 cells can be induced to differentiate to granulocytes, under the conditions of cultures in the presence of dimethyl sulfoxide (DMSO). Examination of the binding of 125I-labeled hemopexin to DMSO-induced HL-60 cells showed that the density of hemopexin receptors on the induced-cells was 1.35 times that on the uninduced cells. We proposed that a specific receptor for hemopexin was present on the plasma membranes of polymorphonuclear leukocytes (PMNs). The binding of human [125I]hemopexin to human PMNs at 4 degrees C was saturable with time and with increasing concentrations of [125I]hemopexin. Scatchard analysis of the binding revealed the presence of approximately 5.7 x 10(4) binding sites per cell with an apparent dissociation constant (Kd) of 2.3 x 10(-9) M. [125I]Hemopexin was rapidly bound then dissociated from the cells after the release of heme, when the cells were incubated with radioactive hemopexin at 37 degrees C. Incubation of the cells with the [59Fe]heme-hemopexin complex resulted in an accumulation of [59Fe]heme in the cells, with a temperature of 37 degrees C but not that of 4 degrees C. Ouabain or NaF inhibited not only the binding of [125I]hemopexin to PMNs but also the uptake of [59Fe]heme from [59Fe]heme hemopexin by the cells. Neither NH4 Cl nor chloroquine inhibited the uptake. Detergent extracts of 125I-labeled PMNs were incubated with a hemopexin-coupled Sepharose CL-6B. A polypeptide reacting with hemopexin-Sepharose was estimated to have a molecular weight of 80,000, as determined by polyacrylamide gel electrophoresis, in the presence of sodium dodecylsulfate. We propose that PMNs take up heme from hemopexin, as mediated by the 80,000 dalton receptor for hemopexin.

[Early pathophysiological findings in experimental concussion in rabbits].

Early neurophysiological changes and pathological findings were studied following experimental cerebral concussion in rabbits. The animals were lightly anesthetized and subjected to occipital trauma with a pendulum impactor. Experimental concussion was defined as fulfillment of at least two of the three following criteria: apnea for more than 10 seconds, absence of the light reflex for more than 10 seconds, and bradycardia or tachycardia (less than 50% or more than 10% of the preimpact rate, respectively). There were 13 cases of lethal concussion, 16 of nonlethal concussion, and 15 of nonconcussion. The resultant head acceleration ranged from 250 to 1660 G and its duration was 0.8 to 5.5 msec. During the first 30 seconds after impact, electroencephalographic activity showed little change even in cases of lethal concussion, but slowed over the next several minutes due to respiratory suppression. The duration of apnea in cases of nonconcussion was under 9 seconds and, in cases of nonlethal concussion, averaged 21 seconds. Five of the rabbits with lethal concussion were apneic throughout the observation period, and the remaining eight exhibited apnea for an average of 46 seconds after impact. The light reflex was absent for less than 9 seconds in nonconcussion cases and for an average of 71 seconds in cases of nonlethal concussion. One animal with lethal concussion exhibited absence of the light reflex throughout, and the remaining 12 animals for an average of 39 seconds. Bradycardia was seen in 15 cases of nonlethal and two cases of lethal concussion. Six rabbits with lethal concussion had tachycardia. Two animals without concussion showed bradycardia and the remainder showed no change.(ABSTRACT TRUNCATED AT 250 WORDS)

Effect of gallium nitrate on the in vivo distribution of 67Ga in rats bearing inflammatory lesions.

Effect of gallium nitrate on the pharmacokinetics and tissue accumulation of 67Ga was investigated in rats bearing turpentine oil-induced abscess. Gallium nitrate accelerated the blood disappearance of 67Ga, but this effect was less potent than that of ferric nitrate. The accumulation of 67Ga in liver was significantly lowered by gallium nitrate, but no significant decrease of 67Ga accumulation in abscess was observed. On the other hand, 67Ga accumulation in bone was slightly facilitated by gallium nitrate. Ferric nitrate exerted a similar effect on tissue accumulation of 67Ga as gallium nitrate.

[Study on thermal pulp testing of immature permanent teeth].

The duration of reaction to cool stimulation was investigated in 185 normal maxillary right and left central and lateral incisors from 55 pediatric patients aged from 6 to 15 years, who were brought to the Department of Pedodontics of Nippon Dental University Hospital, and from 9 students aged from 19 to 20 years in the School of Dental Technology and Hygiene affiliated with Nippon Dental University by using Dichloro-Difluoro-Methane coolants [PULPER (G-C Dental Industrial Corp.) and PULPOFLUORANE (Septodont Co.)] in order to determine the efficacy of thermal pulp testing for mature and immature permanent teeth. The results obtained are as follows: 1) The mean duration of reaction to cool stimulation according to the type of teeth was 0.93 sec for the maxillary left lateral incisors, 0.91 sec for the maxillary left central incisors, 0.88 sec for the maxillary right central incisors and 0.95 sec for the maxillary right lateral incisors, showing no statistically significant difference in the duration between the tooth types. 2) The duration of reaction to cool stimulation according to age was shortened with aging except for the 6-year-old patients. 3) The patients were divided into those 10 years old or younger and those older than 10 years because the tooth roots of the maxillary anterior teeth are developed at 10 years of age. The mean duration of reaction to cool stimulation was 1.02 sec in the group with immature permanent teeth, which consisted of patients aged 10 years or younger, while it was 0.82 sec in the group with mature permanent teeth, which consisted of patients aged 11 years or older.(ABSTRACT TRUNCATED AT 250 WORDS)

[Manipulation of gallium restorative materials. 1. Control of wetting action of mixture by mulling with ethanol].

The wetting action of gallium alloy is a handicap when handling the material. Ethanol mulling after mixing was assayed in an attempt to control the wetting action of such mixtures. A mixture of gallium and Ag-Sn-Cu spherical alloy particles was mulled with a small piece of an ethanol-soaked sponge for 10 seconds following mixing by an amalgamator. The effect of this mulling on the adherence of the mixture to the capsules, physical properties and microstructures was determined. Mulling with ethanol effectively controlled the wetting action of the mixture, and reduced adherence to the capsules without affecting physical properties. A decreased reaction phase around the alloy particles was observed in the microstructure of the ethanol-mulled specimen.

[A new transluminal angioplasty for the treatment of arteriosclerosis obliterans].

A Peripheral Simpson Atherectomy Catheter (PSAC), which permits transluminal excision and removal of diseased intima, has been tested experimentally and clinically. The device consists of a tubular housing and a cutting mechanism to excise the intima protruding into the housing. Experimentally, vascular clay model was used to estimate its efficacy. Multiple passages improved lumen size and inner smoothness, and there was some difficulty to treat curved models. Clinically, 9 lesions in 7 patients of iliofemoral atheromatous disease were treated with PSAC. Recanalization was achieved successfully except for two lesions, one a soft atheroma and the other a curved vessel. No angiographic dissections or acute arterial occlusions occurred. Despite the procedure was complicated, it seemed safe and effective. In order to improve this method, an angioplastic catheter which had a laser probe in its housing was newly designed. Nd-YAG laser was irradiated with this catheter to the atherosclerotic lesions which were induced by a high cholesterol diet in rabbit. It left smoother surface and less thermal injury than usual irradiation with contact laser probe, but it needed more energy. These results showed that PSAC can be an alternative transluminal angioplasty and the new designed laser catheter is useful, but more experiments are required to establish these procedures.

Adverse effects of preoperative hepatic artery chemoembolization for resectable hepatocellular carcinoma: a retrospective comparison of 138 liver resections.

During the past 8 years we have treated 268 patients with primary hepatocellular carcinoma (HCC); total extirpation of the tumor was carried out in 138. Thirty-one of the patients with resectable HCCs had already been treated with transcatheter arterial embolization (TAE) of the liver before they were referred to us. The clinical values of preoperative TAE were retrospectively evaluated for those 31 patients and for the remaining 107 patients without TAE. There were no substantial differences between the two study groups in the clinical and histopathologic backgrounds. No differences were observed in the extent of liver resection, estimated blood loss during surgery, and operation time. During surgery, however, troublesome intra-abdominal complications relevant to TAE were encountered in 15 patients, and detection of tumors was impossible, even with intraoperative ultrasonography, in five patients in the group with TAE. Such findings were not present in any of the patients without TAE. Postoperative morbidity and mortality rates were similar in the two groups. There was no significant difference in the rate of recurrence of tumor in the liver, but the recurrence time was significantly shorter in the group with TAE. TAE did not improve the long-term survival rates in patients either with or without cirrhosis. Results of our study seem to indicate that preoperative TAE is meaningless in the treatment of resectable HCCs and therefore should be avoided, particularly in patients with advanced cirrhosis of the liver.
