Endoscopic therapy of premalignant and malignant changes in the stomach.

Among a total of 34 patients with epithelial gastric polyps, premalignant or malignant polyps were discovered in ten cases, and removed with the aid of the polypectomy snare. Of six borderline lesions, some of which have been followed up for more than two years, none has developed into a carcinoma. One patient who presented with a malignant hyperplasiogenous polyp - an extremely rare finding - has so far remained free from cancer. In a further case with early carcinoma, Type I, two borderline lesions subsequently developed at different parts of the stomach, finally being followed by a second early carcinoma, Type IIc. One of the adenomas removed revealed a neoplastic transformation, so that immediate surgery was necessary. All in all, the results show that, under certain condition, endoscopic polypectomy can represent a valuable therapeutic procedure in premalignant and malignant changes in the stomach.

[Damage to the epithelium of the pancreatic duct following pancreatography (author's transl)].

Following pancreatography with x-ray contrast-medium used for clinical ERP the epithelium of the pancreatic duct was examined histologically, under the scanning electron and under the transmission electron microscope in 15 cats. Immediately following pancreaticography severe edematous lesions of the epithelium are to be seen, e. g. loss of microvilli, acute apical edema of the cells, single-cell-necrosis and erosive defects. 24 hours after pancreaticography some of the animals still manifest high-grade hydropic damage to the ductal epithelium, including rupture of the apical cell-membrane focal lysis of cytoplasm, irreversible mitochondrial lesions and initial degeneration of the nucleus. In the morphogenesis of such lesions in addition to the mechanical stressing of the pancreatic duct system and the physical and chemical properties of the contrast-medium, in particular an eventual damage to the Papilla Vateri with consecutive lowering of the emptying of the ducts, play an important role. By administering secretin to accelerate the emptying of the duct system, damage to the ductal epithelium can largely be avoided by shortening the duration of contact of the contrast medium with the epithelium.

[Statistical studies on the clinical importance of the "upper calix syndrome"].

Urographical examinations of 290 patients prove a statistically ascertained favouring influence of the "syndrome of the upper calyx" on the development of inflammatory changes of the kidney. Therefore the regular clinical control of patients with dysplasias of the upper group of the calyx, Fraley-syndrome and strictures of the calyx neck is demanded.

[Reducing of unwanted side effects of modified fluid gelatin by promethazine: controlled clinical trial with orthopaedic patints (author's transl)].

In order to determine the undesirable side effects of standard commercially available plasma substitutes, 450 stationary patients covering 5 age groups were randomly allocated to various methods of pre-medication and 3 batches of modified fluid gelatin (Neo-Plasmagel). The incidence of allergoid and anaphylactoid reactions depended on the pre-medication (p less than 0.025). Atosil proved to be an strong histamine blocker: the incidence of reactions between the control and the promethazine group was p less than 0.005. Seen as a whole reactions among females were greater than with males (p less than 0.025). No relationship could be determined statistically between the different batches and the incidence of side-effects. -The prophylactic use of Neo-Plasmagel in orthopaedic patients is therefore dependent on a sufficient blockade of histamine receptors.

[Irrigation treatment of concrements in the common bile duct with octanoate (Capmul 8210) (author's transl)].

An irrigation treatment using the octanoate mixture Capmul 8210 of a maximum duration of 10 days was performed in 11 patients with concrements in the common bile duct using either a T-drain (n = 6) or a permanent naso-biliary tube (n = 5). In 6 out of the 11 cases total or partial dissolution led to voiding of concrements without further operative or invasive procedures. The treatment was tolerated in all but two cases. There was no evidence of systemic toxic side effects. Local inflammatory mucous membrane damage due to the instillation was observed in two cases.

The CHILD syndrome. Congenital hemidysplasia with ichthyosiform erythroderma and limb defects.

The term CHILD syndrome is proposed as an acronym for congenital hemidysplasia with ichthyosiform erythroderma and limb defects. The syndrome is characterized by unilateral erythema and scaling, with a distinct demarcation in the middle of the trunk. The dermatosis is either present at birth or develops during the first weeks of life. Ipsilateral limb defects may vary from hypoplasia of some fingers to complete absence of an extremity. In addition, ipsilateral hypoplasia of other parts of the skeleton, as well as defects of the brain and the viscera are found. In some cases, ipsilateral punctate epiphyseal calcifications have been observed. Two further cases of this syndrome are reported, and a review of 18 previous observations is presented. The ratio of females to males is 19 : 1. Apparently, the CHILD syndrome is genetically determined. Arguments are presented in favor of the hypothesis that the conditions is due to an X-linked dominant gene lethal in hemizygous males.

Endoscopic lithotripsy in the common bile duct.

Electrohydraulic lithotripsy of stones lodged in the common bile ducts of human patients has been successfully accomplished. For this purpose, the lithotripsy probe was integrated with a Dormia basket. Common duct stones not otherwise amenable to release by endoscopic papillotomy can be removed by this procedure. A prerequisite, however, is that the stone to be shattered must be firmly lodged within the Dormia basket.

Orbicularis transposition: concept of a functional correction of uni- or bilateral paralysis of the lower lip.

A concept is described for the correction of the lower lip in cases of uni- or bilateral paralysis following injuries of the mandibular branch of the facial nerve, by transpositioning the affected orbicularis muscle to the innervated parts of the upper lip and the zygomatic muscle respectively. The surgical procedure is demonstrated in detail on the basis of several examples. Positive results are clinically and electromyographically evident. Therefore other techniques have been abandoned by us in favour of the transposition of the orbicularis muscle, which is simple in practice and ensures functionally and aesthetically successful results.

[Pattern of serum and red cell electrolytes in acute haemolytic uraemic syndrome (author's transl)].

Sodium, potassium and magnesium were measured in serum resp. plasma and in erythrocytes of oliguricanuric children with acute haemolytic uraemic syndrome (HUS). Potassium was in serum resp. plasma and in red cells in normal range or decrease compared with other forms of acute renal failure in children. These findings were different to statements in the relevant literature, which describe hyperkaliaemia. We suppose a potassium depletion in the early state of HUS. The cause is still unknown. Furthermore we found a hyponatriaemia according to severity of edema and gastroenteritis. Sodium in red cells and magnesium in plasma and erythrocytes were slightly elevated.

[Successful treatment of renal hypertension in a 2 1/2-year-old girl with the converting enzyme inhibitor captopril (author's transl)].

A girl aged 2 1/2 year on chronic hemodialysis, treated with converting enzyme inhibitor Captopril because of severe uncontrollable renal hypertension, returned for the first time to a satisfactory stable blood pressure. After three months of this therapy the bilateral nephrectomy was performed. Two weeks later the patient showed a stable blood pressure, normal for age. None of the known side effects was seen. Whether the oxalat deposits found with nephrectomy were due to the treatment with Captopril, cannot yet be answered.
