It has been suggested that the isoflavone, genistein,, may have some role as a chemopreventive agent against cancer in humans. Levels of genistein and its beta-glucoside conjugate, genistin, ingested in soybeans and related bean products by the Japanese were quantified by HPLC, to estimate daily intake of these compounds. Amounts of genistein and genistin in soybeans, soy nuts and soy powder were in the range of 4.6 to 18.2 and 200.6 to 968.1 micrograms/g food, respectively. The values for soy milk and tofu (bean curd) were 1.9 to 13.9 and 94.8 to 137.7 micrograms/g food, respectively. Levels of isoflavones in fermented soybean products, miso (bean paste) and natto (fermented soybeans), were 38.5 to 229.1 micrograms/g food for genistein and 71.7 to 492.8 micrograms/g food for genistin. Thus, the level of genistein in the fermented soybean products was higher than in soy beans and soybean products such as soy milk and tofu. From these observations, it is suggested that the beta-glycosyl bond of genistin is cleaved to produce genistein by microbes during fermentation to yield miso and natto. Soy sauce was also found to contain both isoflavones, but at levels lower than in miso and natto. On the basis of these data for average annual consumption of soybeans and related products, daily intake of genistein and genistin by the Japanese is calculated to be 1.5-4.1 and 6.3-8.3 mg/person, respectively. These levels are much higher than those for Americans or Western Europeans, whose mortality rates for breast, colon and prostate cancers are greater than the Japanese.
The depth, length, and depth:length ratio of the mandibular fossa, and the angles of the posterior slope of the articular eminence were measured for 91 temporomandibular joints (TMJ) with anterior disk displacement (ADD) in 79 female patients with TMJ dysfunction. As a control, 48 joints in 24 females without TMJ dysfunction were similarly measured. One of the angeles of the posterior slope of the articular eminence was larger for joints with ADD than for the controls (P < 0.01, Student's t-test). There were no other statistically significant differences between the two groups. Among the 91 joints with ADD, there was no statistically significant difference in any variable between joints with ADD with reduction (n = 46) and those with ADD without reduction (n = 45); likewise, there was no association between the magnitude of angle of the posterior slope of the articular eminence and overjet, overbite, canine relationship, or molar relationship.
OBJECTIVE: The purpose of this study was to clarify electromyographic and mandibular kinesiographic properties of the chewing movements in patients with unilaterally painful nonreducing disk displacement of the temporomandibular joint. STUDY DESIGN: Chewing movement in 50 female patients was evaluated by electromyograph and mandibular kinesiograph, and the results were compared with those in 31 normal controls. RESULTS: In the analysis by electromyograph, some differences between patients and controls were found. In the analysis by mandibular kinesiograph, chewing movement showed deviation to the chewing side in the TMJ-affected-side chewing but did not show deviation in the TMJ-unaffected-side chewing in the horizontal plane. The maximal anteroposterior width between opening and closing paths in the sagittal plane was smaller in the experimental subjects. CONCLUSION: These differences between patients and controls may be helpful to diagnosis for painful nonreducing disk displacement of the temporomandibular joint.
We report a 17-year-old girl with short stature, external ophthalmoplegia, atypical retinal pigmentary degeneration, sensorineural hearing loss, and cardiac conduction defect (Kearns-Sayre syndrome). A large-scale deletion (6741 base pairs) in mitochondrial DNA was found in her muscle specimen. She also had insulin-dependent diabetes mellitus (IDDM). On admission, her plasma glucose level was elevated at 31.0mmol/l with mild ketoacidosis, and haemoglobinA1c elevated at 16.5%. After improvement of diabetic ketoacidosis, she was placed on insulin 24-30 units/day despite her small body weight of 25 kg. There was reduced excretion of urinary C-peptide at 3.97 nmol/day. In addition, she had idiopathic hypoparathyroidism with a serum calcium level of 2.15 mmol/l, phosphate 1.7 mmol/l, and intact PTH below 10 ng/l. Human leucocyte associated antigen typing showed A24, A26; B54, B61; CW1, CW3; DR8, DR14; DQ1 and DQ3, suggesting that the presence of HLA-A24 and CW3 antigen contributed to the association of IDDM and hypoparathyroidism, similar to Japanese patients with polyglandular autoimmune syndrome, complicated by hypoparathyroidism and IDDM. We suggest that a genetic linkage, as well as mitochondrial dysfunction, may be responsible for the association of the two disease states. This is an extremely rare case of Kearns-Sayre syndrome, presenting in association with IDDM and idiopathic hypoparathyroidism.
Digestion with endo-alpha-(1-->4)-polygalacturonase liberated the enzyme-resistant region (PG-1c) as an active site of the anti-complementary and mitogenic pectic polysaccharide (GR-2IIc) from Glycyrrhiza uralensis. Partial acid hydrolysis of PG-1c resulted in acidic oligosaccharides, and methylation analysis and GC-MS analysis of the acidic oligosaccharides suggested that PG-1c comprised a rhamnogalacturonan core such as -->2)-Rha-(1-->4)-GalA-(1-->2)-Rha-(1-->4)-GalA-(1-->-->4)-GalA-(1-->4) as the acidic moiety. Degradation of uronic acids by lithium decreased the anti-complementary and mitogenic activities of PG-1c. Although the products from PG-1c were still active, the methylglycoside of alpha-L-Rha-(1-->4)-alpha-D-GalA-(1-->2)-alpha-L-Rha-(1-->4)-alpha-D-Gal A did not show both activities. The products obtained by the lithium degradation from PG-1c gave fractions containing various neutral oligosaccharide-alditols. Among these fractions the longest and the short oligosaccharide-alditol fractions had relatively potent anti-complementary activity, whereas all oligosaccharide-alditol fractions expressed weak but significant mitogenic activity. GC-MS analysis indicated that the short oligosaccharide-alditol fraction contained various kinds of di- to tetrasaccharide-alditols. However, malto-oligosaccharide-alditols, and malto-, isomalto-, and laminari-oligosaccharides did not show anti-complementary and/or mitogenic activities, and these results suggested that certain neutral carbohydrate chains in PG-1c were responsible for the expression of mitogenic activity as well as anti-complementary activity of PG-1c.
Severe cytopathic effects, formation of swollen cells and small syncitia, appeared in 19-PFT-F cell cultures at 2 days when a large amount of the OF1 strain of porcine cytomegalovirus (PCMV) was inoculated and cultured in media containing 5, 10, 30 and 60 ng per ml of 12-O-tetradecanoylphorbol 13-acetate (TPA). Characteristic focal clusters of swollen cells appeared on the 19-PFT-F monolayers 3 to 4 days after inoculation with a small amount of the virus. Cells in the foci contained specific fluorescent antigens in the nucleus and cytoplasm. The infectivity could be determined for 4 days after inoculation by means of measuring focus formation in the presence of medium containing TPA. The virus replicated more rapidly in the medium with, rather than without, TPA.
PURPOSE: To show the distribution of an anticoagulant factor, thrombomodulin, in a rabbit eye. METHODS: Immunohistochemical staining using polyclonal anti-rabbit thrombomodulin antibody was performed with the excised eyes of pigmented adult rabbits. Streptavidin-biotin method was used to obtain intense staining. RESULTS: The endothelium of the chorioretinal vessels showed positive linear staining. The ciliary non-pigmented epithelium showed granular staining, while the pigmented epithelium stained negatively. The positive staining was also detected in the trabecular plexus endothelium. The vascular endothelium of the ciliary body stained weakly. CONCLUSIONS: It is suggested that thrombomodulin in the vessel walls and the aqueous drainage apparatus works to maintain normal blood fluidity and aqueous outflow. The role of nonvascular thrombomodulin in the ciliary non-pigmented epithelium is unclear.
A 44-year-old premenopausal woman with bone metastases of breast cancer was initially treated with systemic chemotherapy (CEF) and radiation therapy after standard mastectomy. However, progressive change of bone metastases with elevation of tumor markers (CEA, NCC-ST 439) was detected, so continuous administration of LH-RH agonist and combination chemotherapy (CEF) were conducted. Subsequently, complete objective regression was attained after 40 weeks.

Skeletal immature rabbits were used to study the pathogenesis of Osteochondritis Dissecans (OCD). Both histological studies and a radiographical examination were utilized after sagittal and coronal surgical chondral fractures were made in the femoral condyles cartilage. Serial microangiographies were performed in rabbits between 0 and 84 days after the chondral fractures were made. Analyses of the histology and microradiography findings suggest in either a coronal or sagittal direction, that avascular lesions like an experimental OCD occur as a sequence of chondral injury. A fracture in a wide pedicle of a stable cartilaginous flap with abundant cartilage canals heals in the usual way. However, a fracture in an unstable fragment with a small isthmus devoid of cartilage canals and of nutritious vessels, probably doesn't heal completely and a fragment closely resembling OCD is instead formed. An experimental OCD depends on the slender hinge of the flap and on the lack of stability in a rabbit's non-ossified epiphyseal cartilage. The damage to the cartilage canals and the rupture of vessels in the canals by a chondral fracture and the disturbance in the revascularization in the healing process by abnormal mechanical forces are thus most likely considered to be the main factor for OCD production.
A 23-year-old male with splenius type torticollis underwent selective denervation of C1 and C2 roots and C3-C6 spinal posterior rami, with excellent results. Based on this experience, we stress the importance of dynamic EMG and thorough review of video recording for identifying the involved neck muscles. In the operative procedures, the nerve rami are identified under the semispinalis muscle. To dissect the correct plane under the semispinalis muscle, the key point to keep in mind is to follow the great occipital nerve to the proximal part. An ultrasonic blood flow meter is useful to locate the vertebral artery in the spaces between the occiput and the C1 lamina and between C1-C2 laminae. Because fine nerve branches run in the soft tissues over the laminae, to avoid transient neuroapraxia, dissection around the C1, C2 arches should not be carried out under the periosteum, as is usually done in case of laminectomy. Dissection of C1 and C2 rami as far as the very medial part is rather difficult because of bleeding from the venous plexus and the presence of the vertebral artery. Therefore, the intradural approach might be easier for sectioning the C1 and C2 roots. We would like to add these points to the original description by Bertrand.
It is not rare for a spinal Schwannoma to have cystic formations, but there are various opinions about the mechanism. Since the installation of MRI in our institution in 1989, we have experienced 4 cases of cystic spinal Schwannoma. The diagnosis of such lesions was based on identifying cystic formations histopathologically. Among these cases, tiny hemorrhages were noted microscopically in three of them along with hemosiderin depositions and phagocytes containing hemosiderin. Besides, there are also a lot of sinusoidal vessels with thin endothelium. Perhaps in these three cases, it is the tiny hemorrhages that resulted in degenerative changes which in turn caused the formation of microcysts. But in addition to the above findings, a lot of hyalinized vessels were also demonstrated in the angiomatous components of two cases. Therefore, it is also possible that the microcyst formation is secondary to the degenerative changes caused by ischemia within the tumors. On the other hand, in the only case without hemorrhage, neither phagocytes containing hemosiderin nor abnormal sinusoidal vessels could be found. In stead, a lot of clear foamy cells with positive lipid staining were seen. There may be some factors underlying the xanthomatous change of these tumors that cause their vacuolar formation from degenerated of the foamy cells. It is possible that all three factors mentioned above may act alone or in combination to contribute to the formation of cysts. At its late stage, a spinal Schwannoma may have various findings which can not be classified into either Antoni A or Antoni B type. In conclusion, a spinal Schwannoma can occasionally have cystic formations. Its mechanisms can be a hemorrhagic or an ischemic process occurring within the tumor.
The authors report a case of pleomorphic xanthoastrocytoma (PXA) with neurofibromatosis type 1 (NF-1). A 21-year-old woman, noted to have had a number of café au lait spots since birth, presented with the chief complaint of headache. Neuroradiological studies revealed a cystic tumor of the left parietal lobe. Surgical treatment was performed. The tumor was located on the surface of the parietal lobe with the formation of cysts. Light, electron microscopic, and immunohistochemical examination showed the characteristics of typical PXA. This is the second reported case of NF-1 associated with PXA.
Antihypertensive effects of beni-koji were studied using 29 outpatients with mild hypertension in a placebo-controlled double-blind comparative fashion. After a 4-week vehicle (apple juice) run-in period, 13 patients were assigned to receive beni-koji aqueous extracts containing juice once daily (27 g of beni-koji eq. per day) for 8 weeks and 16 were assigned to vehicle. Two patients assigned to the vehicle group did not complete the study. In addition to casual blood pressure, 24-hr non-invasive ambulatory blood pressure (ABP) was monitored in 6 patients given the beni-koji drink and 5 patients given the vehicle. 1) In the beni-koji group, both casual systolic and diastolic pressure decreased significantly during the treatment period (from 150 +/- 10/96 +/- 6 mmHg to 140 +/- 10/89 +/- 10 mmHg, p < 0.01). The averages of the 24-hr blood pressure recorded in ABP (24-BP) also significantly decreased (from 141 +/- 17/95 +/- 13 mmHg to 132 +/- 21/86 +/- 10 mmHg, p < 0.05) when compared with those of the control period. Casual pressure normalized (less than 140/90 mmHg) in 4 patients who received beni-koji. Circadian variation of the blood pressure by ABP showed a significant decrease during the daytime. 2) In the vehicle group, casual systolic pressure did not change significantly (from 155 +/- 8 mmHg to 151 +/- 12 mmHg), but diastolic pressure decreased significantly (98 +/- 7 mmHg to 93 +/- 6 mmHg). Casual blood pressure did not normalize in any of the patients and 24-BP did not change significantly. 3) Summative evaluation of safety showed that no problems appeared in the beni-koji group. In conclusion, beni-koji appears to be an effective and safe food material for mild essential hypertension. The mechanism of the antihypertensive effect of beni-koji still remains to be investigated.

We investigated the effects of neurons in the rostral and caudal ventrolateral medulla (RVL and CVL) on cerebral and renal microcirculation in rats. Rats were anesthetized with chloralose, paralyzed with tubocurarine, and artificially ventilated. Cerebral and renal blood flows (CBF and RBF) were measured simultaneously using laser-Doppler flowmetry. Chemical stimulation of the RVL neurons by microinjection of the excitatory amino acid L-glutamate increased arterial pressure (AP), whereas that of the CVL neurons decreased AP. Stimulation of the RVL neurons also elicited a stimulus-locked increase in CBF and a decrease in RBF. The percent change in CBF and RBF was dose-dependent as stimulus intensity was increased. Cerebral and renal vascular resistance (CVR and RVR) levels were calculated from changes in CBF or RBF and changes in mean AP. The percent reduction in CVR and percent elevation in RVR were also dose-dependent. Chemical stimulation of the CVL neurons elicited a stimulus-locked decrease in CBF and an increase in RBF. The percent reduction in CBF and percent elevation in CVR were dose-dependent. The percent reduction in RVR was also dose-dependent, while the percent elevation in RBF was not significant. Blood withdrawal reduced AP by a similar degree to CVL stimulation, but did not significantly decrease CBF. The results suggest that RVL and CVL neurons integrate cerebral and systemic microcirculation.
