[Abortion (author's transl)].

Today the problem of abortion presents a fundamentally different aspect to the obstetrician in respect of causality, diagnosis and treatment, than 10 to 15 years ago. When giving advice to a pregnant woman at the beginning of treatment of a threatened abortion, or after abortion has taken place, it is important for the treating physician in the hospital and in obstetric practice to point out that according to recent studies conception of pregnancy does not necessary mean carrying the child to full term. With particular view to the mental encouragement and hope which a woman tries to find after an abortion, to enter into a new pregnancy, she must be told that recent studies have shown that only part of the pregnancies result in live births. Loss of a conceived pregnancy is mainly a matter of fate largely governed by chance, which she must share with many other women; that is to say, abortion is only very rarely due to an anatomic defect or to functional failure. The article explains the problems inherent in modern therapy of threatened abortion. By means of control of a fairly large group of patients treated by the author from impending abortion to actual birth, the article shows which chances apply to a woman admitted to hospital because of threatened abortion, of giving birth to a healthy child after appropriate treatment. Today the number of malformations in a group of patients with impending abortion and successful delivery, is in no way different from the internationally known standard of malformations after full-term pregnancy in a group of normal women.

Response of mouse intestine to 14 MeV neutrons.

At the Hamburg-Eppendorf Hospital neutron facilities the relative biological effectiveness (r.b.e.) of d,T-neutrons was determined with respect to survival of mouse intestinal crypts. (CBA/Rij x C57BL/Rij)F1 mice were irradiated to the whole body at different depths inside a tissue-equivalent phantom. Irradiations were carried out with a collimated neutron beam at about 6 rad/min given in single doses ranging from 450 to 1000 rad. For reference, gamma-rays from a 60Co therapy unit were used. The number of surviving intestinal crypts per circumference of the jejunum was determined 3 1/2 days after irradiation according to the method of Withers and Elkind. The number of surviving stem cells was calculated on the basis of Poisson statistics. The doses necessary to reduce survival to ten crypt stem cells per circumference amounted to 689 +/- 19 rad for neutrons and 1449 +/- 29 rad for 60Co gamma-rays. From these figures an r.b.e. of 2 . 1 +/- 0 . 1 is obtained. Measurements at different depths in the phantom did not show any variation of r.b.e. with depth along the axis of the neutron beam.

[Aspects to the malignoma of the parotid gland and extratemporal facial nerve surgery (author's transl)].

Malignomas of the great salivary glands are--after own examinations--localized to ca. 90% in the parotid gland and ca. 10% in the sublingual gland. The frequency of different malignomas of the parotid gland, their age and sex incidence, pareses of the facial nerve before therapy, methods of therapy, relapses, letality and survival time are discussed. On own cases the method of extra-temporal reconstructive plastic surgery of the facial nerve is demonstrated.

[Comments and the diagnosis of vitality of early pregnancy in threatening abortion].

In a series of 79 consecutive patients examined by sonar and assays of human chorionic gonadotrophin in the first half of pregnancy 41 ultimately aborted. On analysis of the sonar and post abortion findings it was found, that the aborted pregnancies fell into 3 clearly defined groups: blighted ova, missed abortions and hydatidiform moles. Strict diagnostic sonar criteria of abnormality were established for these groups. Assays of human chorionic gonadotrophin have shown to be useful guides. But the overlap between their normal and abnormal ranges does not permit their use as sole arbitors of which pregnancies should be terminated.

[Influence of local hyperthermia induced by micro-waves and X-rays on the Walker carcinoma of the rat (author's transl)].

The authors studied the influence on the solid Walker carcinoma in the rat exerted by a slight local hyperthermia induced by micro-waves, which was applied alone and combined with X-ray irradiations. It could be demonstrated that the tumor has the same temperature as the sub-peritoneal region. Thus the final temperature reached by the treatment with micro-waves can be exactly controlled. Heating up to 41 degrees C for 30 minutes produces an increase of the survival rate of animals with tumors of 2 to 6 grams from 17% to 27%, whereas the healing rate is 57% after an X-ray irradiation with 1130 rad and 75% after the combined treatment. Each of the three therapy methods produces a significant prolongation of the survival time of the dying animals. The disadvantages of an anisologic tumor-host system are discussed on the basis of the results achieved.

[An improved bronchoscope (author's transl)].

This bronchoscope differs from those normally used in the following ways: 1. In the endobronchial position the bronchoscope makes possible improvement in ventilation of the contralateral lung by increasing the area of the side-openings above the bifurcation. - 2. An inflatable cuff on the outer tube seals the bronchoscope against the trachea like a breathing tube. - 3. Jet-ventilation van be employed when the bronchoscope is open.

Primary in vitro sensitization of virus specific cytotoxic T lymphocytes.

CBA mouse-derived splenic lymphocytes treated with either beta-priopionlactone-inactivated or u.v. light-inactivated parainfluenza (sendai) virus stimulated in vitro unprimed syngeneic T-lymphocytes to differentiate into cytotoxic T-lymphocytes (CTL). The CTL generated were virus specific and H-2 restricted. For optimal CTL responses to be induced (i) a critical treatment of stimulator cells, (ii) an optimal ratio of responder to stimulator cells and (iii) an in vitro incubation period of 5 days was required. The in vitro system for the induction of primary virus-specific CTL responses may be useful to analyse the sensitization phase of H-2 restricted virus-specific CTL.

[Aspects of labyrinthectomy (author's transl)].

Since the introduction of Antibiotics labyrinth operations have become very seldom. In those days before the Antibiotic Era the indication to function-destructing radical surgery was justified in cases of suppurative labyrinthitis. Only in cases of mild serous (irritative) labyrinthitis were conservative treatments with drugs successful. Nowadays, labyrinthoperations are not necessary in most cases. Endocranial complications and high mortality rates of the disease are no longer feared today. All the same, there are cases were the labyrinth operation is justified despite high dosis of Antibiotica. Spceial problems and indications for labyrinthectomy are discussed with own examples.
