[Late side-effects of antineoplastic treatment in childhood (author's transl)].

Every child has a 1:600 chance of developing cancer. Therapeutic progress was impressive throughout the past years. The number of late side-effects of therapy, however, is increasing, too. Among these, problems of fertility, therapy-induced genetic defects, impaired skeletal growth, endocrinologic defects (esp. hypopituitarism), anomalies of metabolism, chronic deficient bone marrow and reduced pulmonary function have to be discussed. One of the most severe consequences is the development of secondary tumors, which may occur in 4% of the long-term survivors. Tumor-treatment has to be performed at a center by a qualified team of oncologists; the same is valid for prevention and/or early detection and treatment of late side-effects of tumor therapy.

[Experimental infection of man using viral strains of bovine papular stomatitis, orf, pseudocowpox and milker's nodule].

An account is given of the close correlations that exist between virus strains of bovine papular stomatitis, orf, pseudocowpox, and milker's nodule. Reference is made to literature data on natural infection of man with the above virus strains. A report then is presented on experimental infection of human volunteers, using paravaccine birus. While fairly tough and elevated nodules, 4 mm to 5 mm in diameter, were produced on the probends' skin, no re-isolation of virus was achieved.

Induction of I region-restricted hapten-specific cytotoxic T lymphocytes.

Murine cytotoxic T lymphocytes (CTL) reactive to TNP-conjugated syngeneic target cells do lyse to a moderate but significant extent TNP-conjugated, I region compatible but H-2K or H-2D region incompatible target cells. Antibody inhibition experiments and "cold inhibition" experiments indicate that some CTL clones recognize TNP-conjugated targets in association with syngeneic I region determinants independently of H-2K or H-2D gene products. The data suggest that besides TNP-conjugated H-2K or H-2D gene products, in principle, also TNP-conjugated I region determinants do stimulate TNP-specific CTL precursor cells and act as target antigens of TNP-specific CTL.

[Intensive supervision of labor with the help of the computor (author's transl)].

A system of supervision which we developed was described. With this system it is possible to make on-line evaluations of the CTG and postpartal blood analyses from the cord blood via computors (AEG 60-10 and AEG 60-50). The computed parameters together with all obstetrical data collected during the pregnancy (results from ultrasonic tests, estrogen and HPL analyses, anamnestic data and additional "clinical findings") as well as off-line evaluated CTG findings made during the course of the pregnancy were scored. The CTG is evaluated on-line via the "cross correlation function" and can be produced in a wide variety of parameters at any time during labor. The complete system is demonstrated on the basis of an individual case analysis. The first statistical individual case analyses of deceleration in CTG were compared with the classical Hon and Caldeyro-Barcia method of calculation.

[Results with a new method for objectivation of rheobase measurements in normal pregnancy and risk of miscarriage or premature birth (author's transl)].

The rheobase (fundamental current strength), which can be used in obstetrics as a parameter of neuromuscular excitability, alters at the onset of labor, as Jung et al. first reported in 1969. It can be determined objectively to a considerable degree using the single-channel myograph manufactured by the Thönnies company, and an improved measuring method. Results of preliminary investigations can be confirmed which indicate that the rheobase in patients with premature labor and bleeding is lower than in women whose pregnancy is normal. Thus, where there is a risk of miscarriage or premature birth, neuromuscular excitability is increased. The fact that rheobase values in some patients who had undergone cerclage for cervical insufficiency are depressed indicates that the insufficiency is partly due to premature labor. It remains open to question whether etiology of the depressed rheobase is an indicator of increased neuromuscular excitability where there is a risk of miscarriage or premature birth.

The role of T cells in anti-herpes simplex virus immunity. I. Induction of antigen-specific cytotoxic T lymphocytes.

Mice infected with herpes simplex virus develop little or no cytotoxic T lymphocyte (CTL) response. However, in lymph nodes (LN's) draining a local site infected with HSV, antigen-specific CTL precursors are sensitized, which upon transfer to in vitro culture conditions develop within 72 hr into effective CTL. The in vivo blockade of CTL differentiation can be overcome by cyclophosphamide, suggesting that a cyclophosphamide-sensitive mechanism blocks the in vivo generation of HSV-immune CTL. The cytolytic activity of HSV-immune CTL is H-2 restricted and antigen specific. Thus CTL sensitized toward HSV type 1 discriminate between syngeneic targets infected with either the immunologic HSV variant type 1 or type 2 (and vice versa). H-2-matched target cells exposed for 30 min to infectious HSV are lysed within 60 min of contact with CTL. Since HSV replication is believed to require more than 4 to 5 hr, the data suggest that either the expression of HSV-dependent "early proteins" takes place within 30 to 90 min or cell membrane-integrated HSV virion represents the target antigen of CTL.

[Concerning the administration of pethidine (Dolantin during delivery: discussion to "Analgesia during delivery--delayed effects for the child? by Riegel, Messow and Pielsticker (author's transl)].

The obstetrical problematic of intrapartal infant death following administration of Dolantin were discussed with the help of both of the cases described in the article by Riegel et al. At the same time, the question of analgesia during delivery with pethidine in combination with a respiratory depressant antidote was examined. Lorfalgyl, according to current opinion, cannot compensate the antidepressive effect of pethidine. Instead, the application of pure pethidine is recommended for obstetric analgesia in the first stage of labor. Today, naloxone used as antidote is particularly suitable before delivery or can be injected into the umbilical vein of the child after delivery. A plan of treatment for pathidine therapy during the first stage of labor in combination with the antidepressant was suggested.

[The replacement of skim milk in the diet of young pigs with an extremely short suckling period. 3. Report. Use of liquid substitute feed on the basis of skim milk, fish meal and wheat under production conditions].

A trial was performed in which 273 piglets received a liquid milk diet between the 12th and 30th day of age. This was compared with a liquid experimental diet, in which 60% of the milk was replaced by fish meal and wheat. Both diets were given as sole feed under practical feeding conditions. The experimental diet produced a weight increase of 81,9% relative to that of the milk diet; milk consumption was 53.7% the required amount of dry matter 117.4%, milk consumption per kg of weight gain 61.5% and expenditure on feed per kg of weight gain 78%. The general growth development of the piglets was quite good irrespective of the different rates of weight increase. The weight gains after 30 days were 6.6 kg+/-1.5 kg for the experimental diet and 7.0 kg+/-1.2 kg for the milk diet. Losses among the piglets amounted to 0.7% and 2.9%. On the basis of the results of the present trial and of previous experiments the authors believe that under practical conditions of feeding 50% of the milk contained in a liquid milk diet fed to piglets that are 12 to 30 days of age could be replaced by wheat and fish meal.

[The replacement of skim milk in the diet of young pigs with an extremely short suckling period. 1. Report. A comparison of skim milk, fish meal and extracted soy beans as a protein source in conjunction with whole wheat meal in a liquid feed].

40 piglets were reared in separate cages from the 13th to the 28th day of age and received a liquid milk diet (controls) or a substituted liquid diet. Half of the protein requirements of the young pigs were covered from skim milk, the other half was supplied through fish meal, extracted soya bean meal or a combination of both meals. Whole wheat meal was used as source of substitution energy. The ration containing extracted soya bean meal reduced all the digestibility coefficients with the exception of NFE. As a result, the levels of energy and nutrient intake and the rates of weight increase were lower in the experimental groups receiving the soya bean meal diet than in the control group. The same performance level as with the control diet was achieved when 50% of the skim milk in the experimental diet were replaced by wheat and fish meal.

[The replacement of skim milk in the diet of young pigs with an extremely short suckling period. 2. Report. Replacement of skim milk by fish meal and wheat in various ways and use of wheat in crude or boiled form in a liquid feed].

15 trials were carried out with 600 young pigs receiving a type of liquid skim milk diet from the 12th to the 28th day of age. 0% to 75% of the proportion of dietary milk were replaced by wheat and fish meal. The experimental rations used in the trial exhibited a high digestibility of crude nutrients. No transitional feeding period was necessary between the feeding of the milk diet and that of the substituted ration. Whole meal from crude wheat was found to be equivalent in quality to whole meal made from cooked wheat. A liquid feed in which about 50% of the dietary milk is replaced by wheat and fish meal is well suited for being fed to young pigs when these are between 13 and 28 days of age.

[Possibilities and limits of salivary gland scanning with the scintillation camera (author's transl)A1].

Scintigraphy with 99mTc pertechnetate has proved as an enlargement of diagnostic possibilities of salivary gland diseases. 58 patients with tumors, posttherapeutical findings, acute an chronic inflammations and sialosis were examined with the Anger-camera. In our opinion scanning is not suitable for demonstrating tumors, but shows better results in estimating the function of glandular parenchyma after operation or radiation. A high significance of the method is given, when there is suspicion or presentation of acute and chronic inflammations, sialoses or Xerostomia with indefinite glandular swelling.

[Histiocytosis X and its importance in Otorhinolaryngology (author's transl)].

Histiocytosis X is an entity of neoplastic diseases of unknown etiology and pathogenesis. In the own case material, half of the patients presented with clinical symptoms originating in the head and neck areas (7/14). With respect to the clinical course, localized histiocytosis X should be distinguished from acute-disseminated and chronic-disseminated disease, the three corresponding with the classical eosinophilic granuloma, Hand-Schüller-Christian's and Abt-Letterer-Siwe's disease; transitional forms are well-known. Therapy is surgical in cases of localized eosinophilic granuloma and cytostatic in cases of disseminated disease; in the latter Vinca-alkaloids and corticosteroids are preferably used. Any kind of treatment, however, has to be individualized and modifications result from the thorough observation of the clinical course, which is unpredictable for the individual case. Planning an individual concept of therapy for each patient is most important and should be done by surgeons together with the pediatric oncologist.

[Indication for operation in the area of the nasopharynx and the middle of the base of skull (author's transl)].

For the treatment of malignancies of the nasopharynx and the middle of the base of skull the high voltage irradiation is the therapy of first choice. It can also be used in combination with cytostatic drugs or in the form of synchronization. The indication for operative procedure with functional lesions is given only in few cases. Today, operative approach is only indicated for benign pathological changes. The removal of small benign tumors, posterior conchotomy or hyperplastic tubal elevations can be performed perorally directly looking through a nasopharyngeal endoscope. The same is true for the removal of cicatricial stenoses or mucosal scars. With deformations (choanal atresia) the transpalatinal way of approach is preferred to others. This method is also convenient for large tumors of the nasopharynx, the choanas and middle of the base of skull. If this topographic region is exceeded a large combined operation (transmaxillary, paralateronasal, transpterygoidal, transcygomatical, retro-submandibular) will be necessary. With plastic reconstruction a good functional result can be obtained. This is shown with own examples.

[Childhood facial paralysis due to tumours (author's transl)].

Facial nerve palsy due to malignant tumours is uncommon in childhood and adolescence. If present the malignancy may be primary or metastatic, and usually brain tumours, leukaemias, or lymphosarcomas are found. Rhabdomyosarcomas, parotid malignancies, Ewing's sarcomas, Wilms' tumours and neuroblastomas are rare. The nerve lesion may be due to direct tumour infiltration, external pressure on the nerve and secondary ischaemia. In disseminated malignancy the lesion is usually due to extending intraneural infiltration and all parts of the nerve may be affected. If facial nerve palsy coexists the prognosis in childhood malignancies is extremely poor.

[Adequate treatment of micro-invasive carcinoma of the uterine cervix (author's transl)].

At the University Department in Heidelberg 58 micro-invasive carcinomas of the uterine cervix were found between 1965 and 1974. 8 cases were diagnosed in other institutions by directed biopsies. Three cases were detected during the microscopic examination of uteri removed for emergency indications and three cases were detected after hysterectomy for cytological findings indicative of carcinoma in situ of the uterine cervix. In 44 cases an optimal conization of the cervix was carried out. Microscopic examination of these cases showed early stromal invasion in 33 cases. In 6 cases a newtork infiltration was found und in 5 cases a plump infiltration was found. The incidence of radical Wertheim operations of 21 in this group is explained by cases in this group who had inadequate diagnosis prior to admission. In 8 cases the indication for the Wertheim radical hysterectomy was a carcinomatous lymphangiosis in the conization specimen. In none of these cases metastatic tumor was found in the lymph nodes although 13 to 44 lymph nodes were examined per case. In accordance with the newer literature it is recommended to limit treatment of micro-invasive changes of the uterine cervix to routine hysterectomy or to optimal conization of the cervix if certain prerequisites are met. The results of the follow-up examinations of the reported cases appear to justify this recommendation.

[Tumorscanning of the head and neck malignomas (author's transl)].

Tumorscanning of malignomas and their metastatic nodules in head and neck is an enrichment of diagnostic possibilities. For the examination such substances can be used, which are tumoraffine and well to mark with a radio-isotope. Therefore we have marked the cytostatic drug Bleomycine with 57Co. Scans were performed with the Anger-Camera 4 to 6 hrs after intravenous injection of 1 mCi 57Co-Bleomycine. 60 patients with different malignomas have been examined, 46 with tumor-localization in head and neck, the others in thoraxabdomen. 34 (85%) of the head and neck malignomas could be demonstrated; 21 (81%) were squamous cell carcinomas, 13 (69%) sarcomas. The possibilities of this method are shown; its clinical importance lies in the identification of primary tumors and their metastatic nodules, of relapses and tumorrests as well as the localization of distant metastasises.
