Prediction and evaluation criteria in perazine therapy of acute schizophrenics: psychopathological results.

In a pharmacopsychiatric pilot study 28 patients with an acute schizophrenic illness were treated for 4 weeks with perazine (Taxilan). Research goals in the psychopathological investigation were: (1) to assess the range of efficacy, target symptoms and syndromes of perazine and to compare these results with previous studies; (2) to compare the quality, interrater reliability, and practicability of two instruments of psychopathological assessment (AMDP and BPRS) for measuring change during treatment; (3) to apply new mathematical methods for finding statistically significant changes in symptomatology on the item level as well as on the levels of syndrome and total score; (4) to test whether pre-treatment prediction based on psychopathological evaluation or psychophysiological data is possible. The target symptoms and syndromes of perazine were the "positive" schizophrenic syndromes, above all thought disturbances and delusional phenomena. This result was consistent with a former perazine investigation. The BPRS proved to be a practicable and reliable instrument for a more global evaluation of change in psychopathology. The AMDP seemed to be especially useful for evaluating the homogeneity of a sample and showing differentiated psychopathological profiles. On this scale, changes in symptomatology became statistically apparent more clearly and more quickly on the syndrome level rather than on the level of single items. As to interrater reliability, both scales perform well. Dichotomizing the original scaling may mean a loss of information from both scales (AMDP and BPRS) in their evaluation of change. Hence it seems necessary to introduce new mathematical procedures (e.g., Friedman and Dunn-Rankin tests) into pharmacopsychiatric research.(ABSTRACT TRUNCATED AT 250 WORDS)

[Late results in fallopian tubes studies on the Bleier clip].

We report on 243 patients with sterilisation by the Bleier Clip. 234 patients were operated by coeliotomia posterior. 9 cases were sterilized in combination with Caesarean section. In 4 of 243 women we observed pregnancies during the follow-up period (failure rate = 16%). The mean time of the 4 conceptions after operation was 43 months. This result shows that the primary cause was not the operation technique but the systemic failure of the Bleier Clip. In cases with controls by hysterosalpingography all Fallopian tubes were occluded, 84% together with a sactosalpinx.

[Significance of the albumin content for axillary metastasis in breast cancer].

A very pronounced and significant difference in albumen concentration is seen in 337 breast cancer tissues compared with 94 histologically benign breast tissues. In cancer of the breast the mean albumen concentration is 8.4 mg/g tissue, whereas benign breast tissues show in 94% of the cases an albumen concentration of over 10 mg/g tissue, mean 16.4 mg/g. In breast cancer, 72% of the tissues have an albumen concentration up to 10 mg/g (cancers of low albumen content), whereas in 28% the albumen concentration lies over 10 mg/g (cancers of high albumen content). On analysing the axillary lymph node involvement in the two groups: low albumen and high albumen concentration, we find significantly more axillary metastases in the low-albumen than in the high-albumen cancer group. The difference is most pronounced when more then 3 lymph nodes are involved. This high lymph node involvement is found twice as often in the low-albumen than in the high-albumen group. It can be concluded that low-albumen breast cancers show a noticeably higher tendency to axillary lymph node metastases than do high-albumen cancers.

[Relation of steroid hormone receptors to axillary lymph node metastasis in breast carcinoma].

In 337 patients with primary cancer of the breast, the oestrogen and progesterone receptor concentration was correlated to the metastasis of axillary lymph nodes taking into account the age of the patients and the size of the primary tumour. The frequency of axillary lymph node involvement increases significantly with size, although not quite steadily but rather stepwise within the T classification. Whereas the tumour diameter influences the receptor status only slightly, the age of the patients plays a major rôle in the oestrogen receptor concentration. In the age group up to 50 years the oestrogen receptor concentration is significantly lower than in the age group over 50 years. There is no comparable influence of age on progesterone receptor concentration. With all 337 breast cancers, no difference in axillary lymph node metastasis is found either between oestrogen receptor positive and oestrogen receptor negative cancers, or between progesterone receptor positive and progesterone receptor negative cancers. In the age group up to 50 years, if looked upon separately, the oestrogen receptor positive cancers do show significantly more often an axillary lymph node involvement, than do oestrogen receptor negative cancers. As this difference exists in the age group up to 50 years only, it is assumed that endogenous oestradiol favours the axillary spread in oestrogen receptor positive cancers. With oestrogen receptor negative cancers the axillary lymph node involvement increases from the younger to the older age group, explaining why in unselected series the oestrogen receptor negative cancers show a more unfavourable course.

The application of flow cytometry for the quantification of the response of experimental tumors to irradiation.

R1H rhabdomyosarcomas of the rat were irradiated with 15 Gy of x-rays and the depopulation and repopulation of the tumor was determined by means of flow cytometry, quantitative histology and colony assays. The results show that flow cytometry yields important information about the cell population kinetics of irradiated tumors, but misleading results are obtained if flow cytometry is not combined with the determination of the numerical density of cells.

[Extracranial olfactory neuroblastoma (author's transl)].

It is reported of an 59-year-old woman with an extracranial olfactory neuroblastoma (aesthesioneuroblastoma). Clinical symptoms were unilateral obstructed nasal respiration, bleeding and dysosmia. Important but difficult is the histological diagnosis. The best therapy is radical surgery; the origin of the tumor, the fila olfactoria and lamina cribrosa, have to be resected widely. A "superradical" intervention of the non-involved regio olfactoria of the opposite side is not necessary. Questions of radiation and prognosis are discussed.

Factors contributing to tumour growth after irradiation.

Rhabdomyosarcomas of the rat were irradiated with 15 Gy of X-rays and the changes in several parameters were measured as a function of time. In the course of 10 days after irradiation, the percentage of tumour cells changes by a factor of 10, the fraction of clonogenic cells by a factor of 20, and the volume of tumour cell nuclei by a factor of nearly 3. During the same time interval, tumour volume increases only by 50%, indicating that volume curves yield little information about the drastic changes occurring in tumours during the first 2 weeks after irradiation.
