Developmental expression of the prolactin gene in the chicken.

Steady-state levels of prolactin (PRL) mRNA in the pituitary gland during embryonic development were determined by dot blot analyses to relate the changes with those of pituitary and plasma levels of PRL. Steady-state levels of the 1.38-kb mRNA encoding the PRL prohormone remained low until Day 18 of incubation, increased on Day 19 of incubation, and reached maximum levels on the day of hatch but decreased 1 day after hatch. Changes in both pituitary and plasma concentrations of PRL closely mimicked those changes in PRL mRNA levels. Subsequently, both the levels of the pituitary PRL mRNA and PRL remained unchanged whereas those of plasma PRL increased 7 days after hatch. The results indicate that a progressive expression of PRL gene in the pituitary gland occurs 1-2 days before the hatch and concomitant increases in plasma concentrations of prolactin may be associated with physiologic changes in pulmonary respiration and hatching.

The fungicidal effect of ultraviolet light on impression materials.

The effects of ultraviolet (UV) light on fungi and impression materials were tested. UV light (250 microW/cm2) killed most Candida organisms (10(3) cells/ml) within 5 minutes. UV light (8000 microW/cm2) killed most C. albicans (10(7) cells/ml) within 2 minutes of exposure. The effect of UV light on dimensional change and surface roughness of impression materials (irreversible hydrocolloid, agar, and silicone rubber) was tested. The results showed that neither dimensional change nor surface roughness of the impression materials were affected. The results of this study indicate that UV light disinfects impression materials that are contaminated with Candida organisms.

Evaluation of the reticuloendothelial system function by the vascular clearance of chondroitin sulfate iron colloid.

Vascular clearance of chondroitin sulfate iron colloid (CSFe) was evaluated as a test for the reticuloendothelial system (RES) in rabbits. Injected CS59Fe was taken up by the RES in the liver (49%) and bone marrow (41%) after 60 min, suggesting its application for the RES function test. The clearance rate (K-value) of CSFe from the blood was calculated by measuring serum Fe concentrations after releasing iron from CSFe at certain intervals after injection. Depending upon different injected doses, K-values were varied and the phagocytic velocity, computed by multiplying K-values by corresponding injected doses, reached a plateau at high doses, indicating the phagocytosis of CSFe by the RES takes a saturation process. Double-reciprocal plotting of the dose and the phagocytic velocity showed a linear relationship, which provided the data on the maximum phagocytic velocity (Vmax), 0.122 mg/kg/min, and the CSFe concentration producing 1/2 Vmax (Kp), 0.426 mg/kg. Thus, this CSFe clearance test can be used for the evaluation of the RES function.

Immunohistochemical study of lepromin reaction in healthy adults.

Lepromin reaction was studied with immunoperoxidase techniques using monoclonal antibodies. The skin reactions were induced by injecting Mitsuda antigen into healthy adults without a family history of leprosy. Both early (48 hours) and late (3 weeks and 2 months) reactions were examined. In the late reaction, focal collections of epithelioid cells had formed, and not only T- but also B-lymphocytes were observed around the granuloma. CD1a+ cells were also confirmed to have increased in the late reaction.

Regulation of glycosaminoglycan synthesis by parathyroid hormone and prostaglandin E2 in cultured dental pulp cells.

The effects of parathyroid hormone (PTH) and prostaglandin E2 (PGE2) on glycosaminoglycan (GAG) synthesis in bovine dental pulp cells were studied. Dibutyryl cyclic adenosine 3',5'-monophosphate and isobutyl methylxanthine were used to assess whether their effects were mediated by intracellular cAMP. Glycosaminoglycan synthesis was assayed by measuring [35S]sulfate incorporation into the GAG fraction of dental pulp cells. Glycosaminoglycan synthesis was increased 1.3-fold by PTH (4 units per ml) alone, 1.6-fold by PTH in the presence of isobutyl methylxanthine, 1.2-fold by PGE2 (100 ng per ml) alone, and 1.5-fold by PGE2 in the presence of isobutyl methylxanthine. Dibutyryl cyclic adenosine 3',5'-monophosphate enhanced GAG synthesis in a concentration-dependent manner and mimicked the effects of PTH and PGE2. The effects of these hormones on pulp and gingival cells were compared; addition of PTH, PGE2, and dibutyryl cAMP had no effect on gingival cell GAG synthesis, whereas their addition induced significant increases of GAG in pulp cells. These results indicate that PTH and PGE2 are involved in the differentiation of dental pulp cells and that these effects are mediated by cAMP.

Effects of parathyroid hormone, 1,25-dihydroxyvitamin D3, and prostaglandin E2 on alkaline phosphatase activity in cultured dental pulp and gingiva cells of bovine calf.

The effects of parathyroid hormone (PTH), 1,25-dihydroxyvitamin D3, and prostaglandin E2 (PGE2) on alkaline phosphatase activity on cultured dental pulp and gingiva cells of bovine calf were compared. In pulp cells, PTH, 1,25-dihydroxyvitamin D3, and PGE2 significantly increased alkaline phosphatase activity, but no increase in the enzyme activity by these factors was observed in gingiva cells. Dibutyryl cAMP also increased alkaline phosphatase activity in both types of cell, but the increase in pulp cells was greater than that in gingiva cells. Treatment of the cultured pulp cells with PTH or PGE2 significantly increased the intracellular cAMP content. These results suggest that calciotropic factors such as PTH, 1,25-dihydroxyvitamin D3, and PGE2 may be involved in the differentiation of dental pulp cells and that some of these effects (those of PTH and PGE2) are mediated by cAMP.

Study on the bile salt, sodium scymnol sulfate, from Rhizoprionodon acutus. II. The structures of scymnol, anhydroscymnol and sodium scymnol sulfate.

The crystal structures of anhydroscymnol (I) and scymnol (II), which were prepared from sodium scymnol sulfate (III) isolated from the bile of Rhizoprionodon acutus, have been determined by means of X-ray diffraction analyses. The crystals of I are orthorhombic, space group P2(1)2(1)2(1) with Z = 4; unit-cell dimensions: a = 13.562(2), b = 21.636(2), c = 8.735(2) A; II orthorhombic, space group P2(1)2(1)2, with Z = 4; unit-cell dimensions a = 18.553(2), b = 19.887(2), c = 7.986(2) A. Both structures, (24R,25S)-(+)-24,26-epoxy-5 beta-cholestane-3 alpha,7 alpha,12 alpha,27-tetrol (I) and (24R)-(+)-5 beta-cholestane-3 alpha,7 alpha,12 alpha,24,26,27-hexol (II), were solved from diffractometric data by direct methods and refined by least-squares calculations to R = 0.073 (I) and R = 0.062 (II) (2044 (I) and 2250 (II) observed independent significant reflections (I greater than 3 sigma(I)), respectively. All the hydroxyl groups of both compounds are involved in a hydrogen-bonding network. The structure of III was determined to be (24R,25S)-(+)-3 alpha,7 alpha,12 alpha,24,26-pentahydroxy-5 beta-cholestan-27-yl sodium sulfate, based on the chemical data that alkaline degradation of III with aqueous potassium hydroxide gives only I.

Isolation and structural determination of a mutagenic substance in creatine pyrolysate.

The pyrolysis product of creatine showed potent mutagenic activity to Salmonella typhimurium TA98 with metabolic activation by S9 mix. One of the mutagenic substances in creatine pyrolysate was isolated and named Cre-P-1. Its structure was determined by X-ray crystallography to be 4-amino-1,6-dimethyl-2-methylamino-1H,6H-pyrrolo[3,4-f]benzimidazole- 5,7-dione. Cre-P-1 is the first registered mutagenic heterocyclic amine containing oxygen atoms in the molecule, that has been isolated from pyrolysis products.

Expression of prolactin gene in incubating hens.

Steady-state levels of the 1.38-kb mRNA encoding the prolactin prohormone, measured by northern and dot blotting using a radioactive cDNA probe, rose 10-fold with concomitant increases in concentrations of prolactin in anterior pituitary and plasma from non-incubating to incubating stages in Gifujidori bantam hens. Daily injections of a low dose of thyrotrophin-releasing hormone (TRH, 0.25 micrograms/kg) for 4 days caused no changes in the concentrations of prolactin of the mRNA, but a high dose (2.5 micrograms/kg) of TRH did induce a significant increase in prolactin mRNA levels compared to saline controls. The results suggest that gene expression for prolactin increases substantially during the incubation period and TRH may contribute, at least in part, to the increase in transcription of prolactin mRNA for prolactin biosynthesis in Gifujidori bantam hens.

Nifedipine-induced gingival hyperplasia: a clinical and in vitro study.

Two cases of gingival hyperplasia associated with long-term administration of nifedipine, a drug that dilates coronary arteries, are reported. The clinical and histopathological features of the gingival hyperplasia induced by nifedipine were similar to those induced by phenytoin, an anticonvulsant drug. In the present cases, gingival inflammation had developed before drug administration. In one case, extensive dental plaque control in addition to surgical removal of the overgrown gingival tissues resulted in satisfactory progress without the need to discontinue drug administration, suggesting that the preexisting gingival inflammation was involved in the development of this periodontal disease. In the other case, change from nifedipine to another drug resulted in spontaneous recovery, strongly suggesting that the drug had induced the gingival hyperplasia. Nifedipine had no direct effects in vitro on proliferation or collagen synthesis of gingival fibroblastic cells from one of the patients. Study of these two cases suggests that both local inflammatory factors and long-term administration of nifedipine were responsible for the gingival hyperplastic changes observed.

Immunoadsorption with PH-350: as a beneficial therapy for acute Guillain-Barre syndrome.

The subjects here were patients (two males and a female) with the severe acute Guillain-Barre Syndrome admitted to our hospital between September 1987 and December 1988. One case was treated with a double filtration plasmapheresis and two cases were with a immunoadsorption. Of the two extracorponeal thrapeutical methods, the clinical efficacies seemed not to be different however biochemical and serological laboratory data showed prominent differences. For instance the large molecular substances in blood; immunoglobulins, coagulating factors and albumin were reduced in plasma levels by the double filtration plasmapheresis, but not or less by the immunoadsorption. Considering the expectable side effects of the two methods, it may say that the immunoadsorption should be applied as a beneficial extracorponeal method for the acute Guillain-Barre Syndrome.

[Clinical experience of extracorporeal shock wave lithotripsy for cystine calculi].

Thirty patients (16 men and 14 women) with cystine urinary stones were treated by extracorporeal shock wave lithotripsy (Dormer HM-3) from December 1984 through October 1989. The average patient age was 35.2 years with a range of 14 to 59 years. Seventy per cent of these subjects had had previous open surgical operations for stones. The cases consisted of 7 ureteral stones and 37 renal stones, including 15 staghorn calculi. An average of 1.3 session of ESWL was carried out to treat ureteral stones. Thirty-seven renal units with renal stone required 96 sessions of lithotripsy (average 2.6 sessions per unit). Seven patients with ureteral stones required auxiliary procedures, i.e., one transurethral lithotripsy (TUL), two percutaneous nephrostomies (PNS) and one open surgery. Thirty-seven renal stones, including staghorn calculi was treated by ESWL and auxiliary treatment of 21 TUL procedures, one PNS, 16 PNL procedures and one chemical chemolysis. Successful fragmentation (residual debris less than or equal to 4 mm) was achieved in 85.7% of ureteral stones, 90.9% of renal stones and 73.3% of staghorn calculi. The stone free rates of patients with ureteral stones, renal stones and staghorn calculi were 71.4%, 50.0% and 53.5%, respectively, at 3 months after ESWL. No serious complications were seen in this series. Fever above 38.5 degrees C was the most common complications (13.5%). Ureteral perforation was encountered once in TUL procedures. Transfusion and selective arterial embolization were needed for one case treated by PNL procedures. Although cystine stone is harder to be fragmented by ESWL than other stone composition, ESWL and endourology may be effective and safe procedures for cystine stone patients.(ABSTRACT TRUNCATED AT 250 WORDS)

[Ultrasonographic study on kidneys in patients with acute renal failure].

Ultrasonotomograms of 22 kidneys were obtained in 11 patients with renal-acute renal failure (renal-ARF). The underlying diseases of renal-ARF were acute tubular necrosis in 8 patients and acute on-set chronic glomerulonephritis in 3 patients. They were treated by hemodialysis in 10 patients and intermittent peritoneal dialysis in 1 patient. Ultrasonic measurement of the size of kidneys revealed that the thickness (anterior-posterior diameter) and the ratio of thickness to length (T/L) were greater in patients with ARF than in those with chronic renal failure and normal renal function. The patients with a low value of T/L (under 0.60) had a significantly greater urine volume than those with high a value of T/L (0.60 or more). The sonographic features of renal-ARF kidneys were marked increase in parenchymal echogenicity and appearance of hypoechoic swollen renal pyramids with sharpness of the corticomedullary border. In the course of ARF, these sonographic changes gradually disappeared when the patients had recovered from ARF. However, the prognosis was poor in patients with severer sonographic findings. We believe that repeated ultrasonic examination of the kidneys in patients with renal-ARF is useful for not only differential diagnosis of post-renal urinary obstruction but evaluating the course of ARF.

[Surgical indications for thoracotomy in cases of small advanced lung cancer by use of DNA content analysis and mediastinoscopy].

Twenty-seven patients with small advanced lung cancer were treated at Kyoto Katsura Hospital. Of 27 patients, 19 patients underwent thoracotomy. The mean survival time of the 19 resected patients was 49 months, while that of the 8 unresected patients was 36 months. There was no statistically significant difference between these mean survival times. Of the 19 resected patients, 6 patients underwent a relative curative operation (RCO), 4 patients underwent a relative noncurative operation, and 9 patients underwent an absolute noncurative operation. The respective mean survival times were 79 months, 30, and 33. We compared the mean survival time of RCO cases with that of the unresected cases, and there was a significant difference in favor of the RCO cases. So we recommend thoracotomy for patients with small advanced lung cancer if RCO is possible, and that requires of a histological classification of less than N2. We performed mediastinoscopy for 18 of the 27 patients to check for mediastinal lymph node metastasis, and 16 of those patients were confirmed to have metastasis pathologically. Mediastinoscopy is useful and reliable for confirming lymph node metastasis, because it gives us a pathological diagnosis. We also performed DNA content analysis on the 19 resected cases. There were 7 DNA diploidy cases, 11 DNA aneuploidy cases, and 1 case couldn't be determined. There was no significant difference between the mean survival times of DNA diploidy cases and the DNA aneuploidy cases.

[A pilot study of etoposide, split-dose cisplatin and THP in small cell lung cancer].

Sixteen untreated patients with small cell lung cancer were treated with a combination of Etoposide (70 mg/m2 i.v., day 3-5), Cisplatin (40 mg/m2 i.v., days 1 and 8) and THP (20 mg/m2 i.v., days 1 and 8). Cycles were repeated every 4 weeks and 15 patients received more than two cycles. The objective response rate was 80% (12 of 15) in complete cases, 91% (10 of 11) in limited disease and 50% (2 of 4) in extensive disease. CR rate was 33% in complete cases, 46% in limited disease. The median duration of response was 22.6 weeks. The median survival was more than 12 months in complete cases. Toxicity was primarily myelosuppression. Three of 15 patients had leukopenia of grade 4, 3 of 15 of grade 3, and 7 of 15 of grade 2. Nausea and vomiting were well tolerated by metoclopromide and methylprednisolone. The renal toxicity was minimal. There were no chemotherapy-related lethal complications. This schedule of Etoposide, split-dose Cisplatin and THP is effective and safe for patients with small cell lung cancer. However, the advantage of THP is still controversial.

[Gene analysis of pulmonary lymphoproliferative disorders].

The authors performed gene analysis of pulmonary lymphoproliferative disorders. Cell suspensions were obtained from tissues of malignant lymphoma or pseudolymphoma in Cases 1 to 3. High-molecular-weight DNA was extracted from these specimens, digested with restriction endonucleases, size-fractionated by agarose-gel electrophoresis and transferred by the Southern procedure to nitrocellulose. Hybridization to nick-translated 32P DNA probes of the immunoglobulin JH, C kappa, C lambda, regions, and T cell receptor beta 1 region. In case 1 and 2, which were diagnosed as B cell lymphoma, cells from tumor had rearranged heavy chain genes, clearly establishing the clonal nature. In Case 3, which was diagnosed as pseudolymphoma, the tumor contained clonal immunoglobulin gene rearrangements as detected with both the JH heavy chain and C kappa light chain gene probes. It was concluded that gene analysis is an effective procedure for establishing a diagnosis of lymphoma in neoplastic disorders of uncertain cell type and for detecting clonal T cell or B cell populations with atypical lymphofollicular hyperplasia.

[The effect of intravenous injection of isosorbide dinitrate on the hypertension during general anesthesia: a multi-center controlled study].

The antihypertensive effect of intravenous injection of isosorbide dinitrate (ISDN) was evaluated in 137 patients undergoing elective surgery during general anesthesia [neuroleptanesthesia (NLA) or enflurane-nitrous oxide-oxygen-anesthesia (GOE)]. ISDN in dose of 20 micrograms.kg-1 or 40 micrograms.kg-1 was given as a bolus injection in 30 sec. ISDN produced a significant decrease in arterial pressure and central venous pressure; the maximum decrease was observed in 7 min after administration of ISDN. The antihypertensive effect of ISDN was dose-dependent, but there was no significant difference between two groups of patients given 20 or 40 micrograms.kg-1, or between those anesthetized with NLA or GOE. ISDN did not significantly alter heart rate, thereby causing a significant decrease in rate pressure product which reflects myocardial oxygen demand. The results suggest that a bolus injection of ISDN is a simple, practical and effective means of controlling hypertension during general anesthesia.
