[The effect of protease inhibitors in shock].

UNLABELLED: Protease inhibitors, such as ulinastatin and gabexate mesilate, are widely used for the patients in shock state. The purpose of this study was to evaluate the effects of these protease inhibitors on renal protection and coagulofibrinolytic disorders. The obtained results were as follows: 1. Ulinastatin studies. 1) Beta 2-microglobulin (BMG) excretion and N-acetyl-beta-D-glucosaminidase (NAG) activity in urine were increased in cases of emergency operation (10 patients) compared with elective operation (38 patients). 2) In canine experimental model of renal ischemia, ulinastatin significantly improved urine volume and urine NAG levels. 3) Administration of ulinastatin suppressed urine NAG level in 12 patients, but BMG level did not significantly change. 2. Gabexate mesilate (FOY) studies. 1) All patients in shock state showed coagulofibrinolytic disorders. Especially remarkable hypercoagulability was observed in 21 patients. 2) FOY suppressed platelet aggregation and the release of beta-thromboglobulin and platelet factor 4. 3) In 24 patients, administration of FOY markedly increased the antithrombin III levels in early postoperative period. 4) Fibrinolytic system was not affected any significant changes with administration of FOY. CONCLUSION: Our results suggested that protease inhibitors are useful for management of the patients in shock state.

[A study of MTT-hybrid assay using human bone and soft tissue tumor cells].

We investigated a new chemosensitivity test, MTT-hybrid assay, which was a hybrid of MTT colorimetric assay and double-layered soft agar colony assay, using human bone and soft tissue tumor cells. MTT formazan crystals produced by viable cells in the soft agar medium were solubilized by SDS at 60 degrees C. The absorbance (560 nm) is directly proportional to the cell number over a wide range. The absorbance increased in proportion to colonial growth of osteosarcoma cells, while it decreased in a human diploid cell strain in a few days. Drug sensitivity of tumor cells is supposed to be assessed without contaminating normal cells by MTT-hybrid assay in primary tumor samples. Good correlation of IC50 was observed between MTT-hybrid assay and colony assay. The MTT-hybrid assay shows potential value as a rapid predictive test for chemotherapeutic agents in an individual patient.

[Oncogene of human papillomavirus].

Although certain types of human papillomavirus (HPV) have been supposed to be associated with carcinogenesis of cervix, a definitive conclusion has not yet been reported. To show that HPV is a causative agent of cervical cancer, we performed several lines of experiments and obtained the results described below. 1) In attempting to identify an activated oncogene(s), we showed that cellular DNAs from 8 cervical carcinomas induced transformation of NIH 3 T 3 cells and, in all cases, HPV sequences were retained in the secondary transformants. 2) Using a retrovirus vector system, we showed that HPV-16 had transforming activity for mouse or rat established cells. The transforming genes are located in the E6 and E7 ORF: the former governs tumorigenicity in nude mice and the latter influences cell growth properties such as saturation density, colony formation in soft ager and immortalization. These results strongly suggest that specific types of HPV are involved in carcinogenesis of the cervix. However, HPV only may not be sufficient to induce carcinomas, because even the E6 and E7 ORFs of HPV-16 could not transform rat primary cells immediately after infection, but could do so after a long latency. Evaluation of the change during the latency is very important to clarify the mechanism of cervical carcinogenesis.

[Long-term survival with Hufnagel ball valve--a case report].

Authors report a 61-year-old female with an aortic aneurysm at the distal end of the Hufnagel ball valve which was implanted surgically for the treatment of aortic insufficiency 23 years previously. The Hufnagel ball valve has been working well for the past 23 years, but the aneurysm was found enlarging in size, therefore the replacement of aneurysm with a 20 mm dacron graft was performed following removal of the well-working Hufnagel ball valve. There were 151 cases with Hufnagel ball valve implantation in the literature and at least 7 cases of them were reported in Japan. Among them, the majority survived only several years after surgery and the cases with long-term survival were very few in number. Our case is unique, because the valve in this case has worked well for 23 years.

[A case of Wyburn-Mason syndrome].

A extremely rare case of unilateral retinocephalic vascular malformation (Wyburn-Mason syndrome) was reported. A 5 year-old girl was seen to a ophthalmologist complaining of exophthalmus of her left eye after trauma on her face for the past 2 months. On ophthalmologic examinations, retinal arteriovenous malformation was recognized in her left eye and she was referred to neurosurgical service for the evaluation on occurrence of traumatic carotid cavernous fistula. Plain CT did not show any abnormality but enhanced CT revealed an abnormally enhanced lesion in the thalamus and basal ganglia and along to the left optic nerve. Left carotid angiography and vertebral angiography revealed a huge arteriovenous malformation in the left thalamus and basal ganglia extending to the left orbita. The arteriovenous malformation was fed from the C3 segment of the carotid artery, anterior, middle and posterior cerebral arteries, and drained to the internal cerebral vein and basal vein. The intraorbital part of the malformation was mainly fed from the external cerebral artery. She was diagnosed as Wyburn-Mason syndrome. We stressed in the paper that enhanced CT was useful for the diagnosis of the Wyburn-Mason syndrome.

[Clinical study of inflammatory diseases, which are difficult to differentiate from tumorous lesions].

Fourteen inflammatory disease cases in which differential diagnoses from tumorous lesions had been difficult and which were confirmed by thoracotomies, were studied. They included 4 plasma cell granulomas, 3 pulmonary abscesses, 2 fungal diseases, and one case of pseudolymphoma, lymphocytic interstitial pneumonia, Wegener's granulomatosis, round atelectasis, organized pneumonia, respectively. In some cases with plasma cell granuloma, lymphoproliferative disease, Wegener's granulomatosis and round atelectasis, it might be possible to correctly diagnose the cases preoperatively, on chest X-ray, clinical symptoms and blood examinations. However in cases with plasma cell granuloma in aged patients, fungal disease, pulmonary abscess and organized pneumonia, there were few characteristic findings for differentiation from benign or malignant tumors. Therefore open thoracotomy will be inevitable for correct diagnosis.

Design of RNA enzymes for sequence-dependent cleavage of RNA.

We have designed synthetic ribozymes which can cleave specific sites of RNA. An undecamer, 5S rRNA and transcripts using T7 RNA polymerase were hydrolyzed at the X position of RNA containing (N)nUX(N)n (N = A, G, C and U; X = A, C and U) by these ribozymes.

[An in vitro drug screening assay, scintillation assay, and its clinical application].

We have developed an in vitro drug screening test by scintillation assay with which we can assess the chemosensitivity of solid human tumors during the 5 days following tumor resection. We are able to evaluate 62% (271/439) of the human tumors' chemosensitivity results. Seventy-seven of these cases were examined for in vitro-in vivo correlations. In 14 of 33 in vitro sensitivity cases (42%) we found clinical responses. And, in 41 of 44 in vitro resistance cases (95%) we found progressive disease. These in vitro-in vivo correlations with the scintillation assay were similar to those in other reports with different types of chemosensitivity assay. The true-positive accuracy of these tests averaged 50%, while the true-negative accuracy averaged 90%. We think that tumor cell heterogeneity is one of the main reasons for low true positive accuracy in the drug screening assays. In some studies we implanted human tumors into nude mice. The resulting xenografts demonstrated that fresh human tumors are composed of heterogeneous subpopulations of cells with varying chemosensitivities and varying DNA indexes. This heterogeneic nature of human tumor cells means that a single biopsy specimen may not comprehensively define the chemosensitivity of a patient disease.

Breathing 30 per cent oxygen attenuates bronchial responsiveness to methacholine in asthmatic patients.

The effect of breathing 30% oxygen on bronchial responsiveness to inhaled methacholine was studied in 30 asthmatic patients using a direct graphical recording method. An aerosol of methacholine solution was inhaled during tidal breathing by measuring respiratory resistance (Rrs) continuously, and dose-response curves were obtained on two separate days during air and 30% oxygen inhalation. Oxygen inhalations for 20 min caused no change in baseline Rrs. The threshold dose of methacholine, the dose which increased Rrs from the baseline value while breathing 30% oxygen, increased significantly compared with that while breathing air (p less than 0.01). No significant difference was found between the mean values of the slopes of the dose-response curves of the two groups. End-expiratory oxygen concentration (FETO2) during air breathing decreased significantly from 13.7% to 12.0% at the point where methacholine dosage reached threshold. This study suggests that hyperoxia attenuates the bronchial responsiveness to methacholine in asthmatic patients.

[Studies on distribution, excretion and subacute toxicity of squalane in dogs].

In the previous papers, we demonstrated, by using rats, that squalane (2,6,10,15,19,23-hexamethyltetracosane) could stimulate the fecal excretion of 2,3,4,7,8-pentachlorodibenzofuran, which was regarded as the most important etiologic agent of yusho among PCB and PCDF congeners found in the causal rice oil. We also reported that, in rats, squalane was not essentially absorbed from the gastrointestinal tract, and did not show any appreciable side effects during the 3-month treatment. In the present paper, we have investigated the distribution, excretion and subacute toxicity of squalane in beagle dogs. The fecal excretion of squalane accounted for about 83% of dose during the initial 2 days after administration at a single oral dose of 1,200 mg/kg to male dogs. On day 3, absorbed squalane was mostly distributed to the hair and the skin, and the concentrations in these tissues were decreased on day 6. These results suggested that most of squalane administered orally was not absorbed from the gastrointestinal tract, but a part was absorbed and excreted through the hair. In addition, squalane distributed into the liver was found to be eliminated rather slowly. A long-term (13-week) treatments with squalane orally at doses of 400 mg/kg/day or 1,200 mg/kg/day in male and female dogs, resulted also in accumulation of squalane in the liver at a level of about 3% (400 mg/kg) or about 6% (1,200 mg/kg) of the daily dose. This accumulation of squalane in the liver was highest among all the tissues. Nevertheless, no appreciable toxic signs were observed in the serum biochemical tests and the hepatic functional test for squalane groups. Therefore, squalane accumulating in the liver, did not seem to disturb the hepatic physiological functions. It was suggested also in a long-term treatment that the skin and the hair played the most important role in the elimination of squalane. In conclusion, the present studies on subacute toxicity tests suggested that squalane did not give any significant toxic effects on dogs as well as rats.

[A case of a 10-year survivor with squamous cell carcinoma of the lung, treated with radiation after an exploratory thoracotomy].

We have experienced a rare case of an unresectable lung cancer in a male patient who has survived 10 years after radiation therapy. The patient was aged 58 at the time of diagnosis. In May 1978, he was hospitalized because of an abnormal shadow in the upper left lung filed, and the diagnosis of squamous cell carcinoma was made by TBLB. Since subsequent surgery revealed an extensive tumor invasion to the large blood vessels, an exploratory thoracotomy was performed. Following postoperative 60 Gy radiation therapy, tumor regression was observed on examination of chest x-ray films. In July, 1988, 10 years and 3 months after this surgery, a back pain developed due to the recurrence of the tumor, and radiation therapy was given again. This relieved the pain. As of December 1988, he receives treatment on an outpatient basis and his performance status has been excellent.

[Influences of irradiation on the anastomotic sites of artificial grafts].

To examine the acute influences of irradiation on the anastomotic sites of EPTFE vascular grafts, an experimental study was performed using dogs. After replacement of superior vena cava and bilateral femoral arteries with EPTFE grafts, a total of 20 to 100 Gy doses were given at the anastomotic sites of superior vena cava and right femoral artery. There was no degradation of grafts themselves or rupture in the anastomoses at cumulative radiation doses of 20 to 100 Gy. However, in many dogs, receiving over 50 Gy doses at the anastomotic sites of superior vena cava, stenoses and/or overgrowth of granulation tissues were observed, although at those of right femoral artery, no remarkable differences were observed between dogs which received and those which did not receive irradiation.

[Auditory brainstem response in dead on arrival cases].

The relationship between ABR on the one hand and acid-base balance, EEG findings and prognosis on the other was studied in 47 cases of DOA. As for the acid-base balance, the balance tipped to acidosis in almost all cases. The ABR showed wave I to wave V in 25 cases, wave I to wave III in 2 cases and only wave I or no response in 20 cases on the admission day. The EEG was isoelectric in 27 out of 35 cases. All the patients who showed some activity in EEG had ABR which showed waves I through V. However, 11 of the 27 patients who were isoelectric EEG showed wave I through V potentials in ABR. By the relationship between EEG and ABR, the cases could be classified into 4 types, namely, 1) there are EEG activities with ABR showing up to wave V and the other types EEG are isoelectric with ABR findings that 2) wave V appears, 3) wave V disappears to leave wave I-III and 4) only I wave or becomes no response. The acid-base balance of the cases was inclined to acidosis; therefore, the effect of acidosis on ABR, EEG and disturbance of the brain tissue due to anoxic state wave suspected. Prognosis of DOA cases was poor and 87.2% died. All the patients who exhibited no response in ABR and isoelectric EEG at anytime of their course, died eventually. These findings suggest that combination of EEG and ABR enables one to learn more exactly the central function in those with disturbance of consciousness.
