[Distribution pattern of density in lumbar vertebra studied with computed tomography: a study of histogram plot].

The bone mineral status of the cancellous bone in the lumbar vertebrae was evaluated by analysing density histograms and measuring the mean density by computed tomography. The results obtained were as follows: (a) the distribution pattern of bone density in lumbar vertebrae revealed a normal distribution. (b) high correlation coefficients between peak density (r = -0.79) or mean density (r = -0.77) and age was obtained in males, whereas peak densities in females were maintained well at ages younger than 50 years and peak densities abruptly decreased after 50 years of age. Osteoporotic vertebrae, in which multiple osteosclerotic changes were observed, had several peak densities and did not show normal density distribution pattern. These results indicated that our methods combining analysis of density histograms and measurement of mean density are useful to evaluate the bone mineral status.

[A new computed tomography method for quantitative analysis of abdominal aortic atherosclerosis].

Clinical evaluation of aortic atherosclerosis has been most commonly performed by visual analysis of aortic calcification on conventional radiographs or X-ray CT. However, precise evaluation of the degree of calcification or mild changes with increasing age can be difficult by these methods of visual analysis. A new quantitative method of evaluating the abdominal aortic atherosclerosis using CT is reported. Target CT scans of the abdominal aorta were performed at the level of the 1st, 3rd and 4th lumbar vertebrae. Two circular regions of interest (ROI) were selected along the outer margin and inner margin of the abdominal aortic wall on CT images, since it was difficult to precisely trace the aortic wall. Histograms of CT value for each pixel were made from two ROIs. A histogram at the ROI of the outer margin was depicted using class values over the maximum CT value in histogram at the ROI of inner margin which indicated flowing blood. A sum of products of the value in each class by the number in that class was divided by the aortic diameter. The value thus obtained was defined as the atherosclerotic index (S.I.). Forty-five cases were studied with this method. Six cases were excluded because of artifacts. The remaining 39 cases (16 males, 23 females) were analyzed. S.I. increased with aging and was higher in men than in women. However, it increased more rapidly in women (8.68/year) than in men (2.73/year). Because this new method employed the CT value, it is more objectively quantitative than previous methods of visual analysis in evaluating abdominal aortic atherosclerosis.(ABSTRACT TRUNCATED AT 250 WORDS)

[Evaluation of gallbladder emptying in patients with chronic liver disease by 99mTc-EHIDA hepatobiliary scintigraphy].

Gallbladder emptying after intramuscular injection of cerulein was investigated by 99mTc-EHIDA hepatobiliary scintigraphy in 23 patients with biliary disease, 55 patients with chronic liver disease, and 21 normal controls. The mean gallbladder ejection fraction in patients with gallstones and liver cirrhosis was significantly reduced compared with normal controls. (gallstones: 56.3 +/- 21.3%, LC with gallstones: 50.8 +/- 29.6%, LC without gallstones: 55.9 +/- 26.7%, vs. normal controls: 74.4 +/- 12.9%, p less than 0.01). The mechanism for sluggish gallbladder emptying in liver cirrhosis is unknown, however impaired emptying with bile stasis provides a potential pathophysiologic basis for the high frequency of pigment stones.

Immunochemical quantitation of thymine glycol in oxidized and X-irradiated DNA.

The present study demonstrates the usefulness of immunochemical assays for quantitating modified bases in oxidized and X-irradiated DNA. Escherichia coli, phi X174 RF I, PM2, and M13 DNA containing thymine glycols introduced by OsO4 oxidation were used as antigens in a direct enzyme-linked immunosorbent assay (ELISA). The number of thymine glycols per DNA molecule was determined by reactivity with antithymine glycol antibody standardized either to the acetol fragment assay or to the number of Escherichia coli endonuclease III-sensitive sites. The number of thymine glycols was also determined in phi X174 RF I DNA X-irradiated in either phosphate or Tris buffer under air. Using a direct ELISA with phi X174 RF I DNA irradiated in a phosphate buffer solution, the anti-thymine glycol antibody detected damage at the level of 40 Gy. The immunochemical assay was sensitive, specific, quantitative, and independent of DNA structure.

[Photodynamic therapy--its application for the treatment of esophageal cancer].

Effect of Photodynamic therapy (PDT) for esophageal cancer was discussed in this study. PDT has been performed on 9 cases of superficial esophageal cancer. All cases were followed periodically by endoscopy for 2 years 2 months to 6 years and no case has any local recurrence. But in 2 cases invaded submucosal layer, lymph node metastasis was recognized. We think PDT is one of the very effective therapies in the treatment of intramucosal cancer.

[Efficacy of extended radical operation based on preoperative staging].

From 1985 to 1988, we have resected 203 cases of the thoracic esophageal cancer with right thoracotomy. Those 203 cases were classified as R3 (bilateral cervical, thoracic and abdominal node dissection), R2 + alpha (left cervical, thoracic and abdominal nodes dissection) and R2 (thoracic and abdominal node dissection) based on preoperative staging. The background of R3 mainly contained Iu cases, advanced cases, positive cases of upper mediastinal metastasis, and that of R2 mostly contained high aged and risk cases, though this group showed low LN metastasis. The incidence of postoperative complications were higher R3 greater than R2 greater than R2 + alpha in order. Pneumonia had no relationship to neck dissection. Recurrent nerve palsy was recognized in R3 group about 16%. Operative mortality mostly belong to high aged group over 70 y.or., noncurative cases and R3 group. The survival rate of C greater than 0 resected cases with right thoracotomy after 1985 showed some improvement compared with the cases of standard R2 dissection by right thoracotomy from 1980-1984. It showed no difference of the curative survival curve among R3, R2 + alpha and R2 groups. Thus, it is effective to improve the survival rate of resected esophageal cancer with our indication based on preoperative staging.

A phase II study of vindesine in the treatment of esophageal carcinoma. Japanese Esophageal Oncology Group.

A phase II study of Vindesine for esophageal carcinoma was carried out cooperatively by 10 Japanese institutions. Fifty patients were enrolled in the study over a year and ten months. Four patients could not be evaluated because less than 4 weeks had elapsed since their preceding therapy. There were one complete remission and five partial remissions among the 46 remaining patients, a response rate of 13.0%. The patient experiencing the complete remission is still alive more than 24 months after treatment. The main side effects were depilation, anorexia, stomatitis and leukopenia. Thrombocytopenia and elevations of blood urea nitrogen and creatinine were not marked. There was no mortality connected with the administration of Vindesine. It was concluded that Vindesine is useful in the treatment of esophageal carcinoma as a single agent.

Granuloma formation by artificial microparticles in vitro. Macrophages and monokines play a critical role in granuloma formation.

To investigate the basic mechanisms of granuloma formation, in vitro granulomas were induced by culturing murine spleen cells in the presence of artificial microparticles. Large granulomas developed around dextran beads. The lesions were inducible by spleen cells from either normal mice or athymic nude mice. Minimal inflammation was produced around latex beads. The histologic features and time kinetics of granulomas in vitro. Culture supernatants of dextran induced granulomas contained high levels of interleukin-1 (IL-1) activity but not interleukin-2 (IL-2) or interleukin-4 (IL-4) activity. IL-1 activity was correlated with granuloma size. Additionally, granulomas were produced by culturing spleen cells in the presence of agarose beads coupled to recombinant IL-1 or recombinant tumor necrosis factor-alpha (TNF-alpha). Granulomatous lesions also were induced by macrophage-enriched populations in the presence of monokine-coupled beads. Adherent macrophages, but not nonadherent cells, were required for induction of granulomas in vitro. In contrast, very small lesions were seen when spleen cells or adherent cells were cultured in the presence of beads coupled to recombinant IL-2 or recombinant interferon-gamma (IFN-gamma). These results suggest that macrophages and monokines such as IL-1 and TNF-alpha play an essential role in granuloma formation in vitro.

[Evaluation of antiplatelet drugs on platelet functions--comparison between ticlopidine and dipyridamole after prosthetic valve replacement].

We examined the efficacy of antiplatelet drugs after prosthetic valve replacement. As an early postoperative group (group E), 49 patients were divided into four groups. In group A only warfarin was given. In group B warfarin and dipyridamole 300 mg/day, in group C warfarin and ticlopidine 300 mg/day, in group D warfarin and ticlopidine 600 mg/day were administered respectively. As a late postoperative group (group L), 45 patients with prosthetic valves were divided into two groups. In group I dipyridamole 300 mg/day in group II ticlopidine 300 mg/day were administered. Both group I and II received warfarin. In group E platelet counts and platelet aggregability were examined to 22 POD. In group L starting two years after valve replacement, platelet counts were examined eight times by three-month intervals, and platelet aggregability, platelet adhesiveness were investigated 36 months in group I and 27 months in group II postoperatively. Results are the following. 1) Neither ticlopidine nor dipyridamole had any effects on the platelet counts through the postoperative period. 2) In the early postoperative period, the administration of antiplatelet drugs to groups B, C, and D suppressed platelet aggregability more than group A. 3) Ticlopidine (groups C and II) reduced ADP and collagen aggregabilities compared with dipyridamole (group B and I) both in the early and late postoperative period. 4) Ticlopidine 300 mg/day (group C) and ticlopidine 600 mg/day (group D) had no difference in ADP and Collagen aggregabilities. 5) No significant difference was shown between dipyridamole (group I) and ticlopidine (group II) in platelet adhesiveness.(ABSTRACT TRUNCATED AT 250 WORDS)

[A case of mucosal esophageal carcinoma with a large intramural metastasis in the stomach].

The prognosis of mucosal carcinoma of the esophagus is extremely good because of low frequency of lymphatic invasion compared with submucosal carcinoma. A 64-year-old male was admitted presented with dysphagia and weight loss. Roentgenogram and esophagoscopy revealed erosive lesions in the lower portion of the esophagus and a huge submucosal tumor that looked like the myosarcoma in the cardial portion. After total resection of thoracic esophagus and partial gastrectomy, esophagogastrostomy was done through the anterior thoracic route. During the operation, we found a few disseminative foci in the thoracic cavity (pl1) and direct invasion of cardial tumor to the celiac portion. Therefore, the operation was absolute by palliative. Histological examination of the resected specimen showed that the IIc-like lesion of the lower portion of the esophagus was a moderately differentiated squamous cell carcinoma invading the mucosa (mm), which was 2.0 x 1.6 cm in size, with marked lymphatic invasion, and intra-mural metastatic foci (7.0 x 5.0 cm) at the cardial portion. The patient died 3 months after the operation with mediastinal lymph node metastasis. In our 19 cases of resected mucosal carcinoma of the esophagus, lymphatic invasion was positive in 16%, and lymph node metastasis was positive in 16%, and intramural metastasis was in only one case. Frequency of intramural metastasis in esophageal carcinoma is about 10% in sm -a3 lesion, 5% in mm. In our experience, the prognosis of curatively resected mucosal carcinoma of the esophagus is generally good. We get long survivals, when curative operation is undergone, even if the lymphatic invasion is positive.

[The role of interleukin-2 in the impaired lymphocyte function after open heart surgery].

To reveal the mechanism of impaired lymphocyte function shown as the suppression of mitogenic proliferation of lymphocytes after open heart surgery, we evaluated the kinetics of Interleukin 2 (IL-2) production and response of lymphocytes sequentially until 7th day postoperatively in 10 patients. Membrane oxygenators were used exclusively and steroids were not administered during this study. In addition to the assay of mitogenic response (PHA), we measured IL-2 production and response utilizing IL-2 dependent cytotoxic T cell line and mitogenic proliferation to standard IL-2 respectively concomitant with plasma cortisol level. The results are as follows. 1. Mitogenic proliferation was suppressed with significance postoperatively (1POD: p less than 0.01). 2. IL-2 production was suppressed with significance postoperatively (3POD: p less than 0.05). In contrast no significant fluctuation of IL-2 responsiveness was observed postoperatively. 3. No correlation between plasma cortisol levels and these impairment was shown in this study. In conclusion impaired IL-2 production might be responsible for deficient lymphocyte function after open heart surgery.

Dihydrothymidine and thymidine glycol triphosphates as substrates for DNA polymerases: differential recognition of thymine C5-C6 bond saturation and sequence specificity of incorporation.

The ability of dihydrothymidine (DHdTTP) and thymidine glycol (dTTP-GLY) 5'-triphosphates to serve as substrates for different DNA polymerases was investigated. DHdTTP but not dTTP-GLY was used as a substrate by E. coli DNA polymerase I (Pol I). Within the detection limit of the assay used, neither T4 DNA polymerase nor avian myeloblastosis virus (AMV) reverse transcriptase used DHdTTP or dTTP-GLY as substrates. The ability of DHdTTP and dTTP-GLY to undergo enzyme-catalyzed turnover to the monophosphate paralleled their ability to serve as substrates for polymerization. These results, along with kinetic parameters for the incorporation of DHdTTP with Pol I, strongly suggest that the saturation of thymine C5-C6 bond and the substituent groups at C5 and C6 differentially exert effects on binding to DNA polymerases. DNA sequencing gel analysis of the polymerization products revealed that most single adenine sites were capable of templating DHdTTP, however, DNA synthesis was partially arrested at multiple adenine sites, suggesting that sequential incorporation of DHdTTP produced significant disorder in the primer terminus.

Ethanolaminephosphotransferase in rat lung: selectivity for endogenous and exogenous diacylglycerol.

Ethanolaminephosphotransferase (EC 2.7.8.1) activity was determined in lung microsomes using diacylglycerols generated endogenously from [14C]glycerol 3-phosphate and different mixtures of fatty acids. Ethanolaminephosphotransferase used endogenously generated dipalmitoylglycerol better than dioleoylglycerol. The apparent Km and the reaction rates for four different endogenously generated mixtures were the same (16 nmol/mg microsomal proteins). The apparent Km values for CDP-ethanolamine were the same (0.26 mm) for endogenously generated dipalmitoylglycerol and dioleoylglycerol. The amount of diacylglycerol generated in microsomes was 2-3-times the apparent Km for diacylglycerol. Dipalmitoylglycerol, supplied exogenously as a Tween 20/phosphatidylglycerol emulsion, was nearly twice as active as dioleoylglycerol. Both dipalmitoylglycerol and dioleoylglycerol were more active as substrates when emulsions were made with phosphatidylglycerol/Tween 20 than with Tween 20 alone. The results suggest that ethanolaminephosphotransferase in lung is relatively nonselective for molecular species of diacylglycerol. In addition, the results suggest that the concentration of diacylglycerol and the physical state in which it is presented to the enzyme can affect the apparent selectivity of ethanolaminephosphotransferase for diacylglycerols.

Antioxidant activity of synovial fluid, hyaluronic acid, and two subcomponents of hyaluronic acid. Synovial fluid scavenging effect is enhanced in rheumatoid arthritis patients.

To test the scavenging of reactive oxygen species (ROS), we added synovial fluids from patients with rheumatoid arthritis (RA) and osteoarthritis, as well as hyaluronic acid (HA) and its 2 subcomponents, D-glucuronic acid and N-acetyl-D-glucosamine, to 2 ROS-generating systems, activated neutrophils and xanthine-xanthine oxidase. Synovial fluid from RA patients, HA, and D-glucuronic acid markedly decreased the O2-, H2O2, OH., and chemiluminescence measured in both systems. HA and synovial fluid, which are known to be susceptible to degradation by excessive ROS in RA patients, also seem to play an active role in protecting articular tissues from oxidative damage.

Evaluation of endoscopic ultrasonography for the diagnosis of submucosal tumors of the esophagus.

Endoscopic ultrasonography was carried out on 55 patients whose X-ray films or endoscopic examinations indicated the presence of a submucosal tumor. Endoscopic ultrasonography revealed 8 cases of extraluminal compression and 48 cases of submucosal tumors. Histological studies were performed on 29 cases with submucosal tumors. In 28 of the 29 cases (97%) the location of the tumor in the esophageal wall was correctly estimated ultrasonographically, and appropriate treatment was selected. Tumors ranging from 3 to 50 mm in diameter could be measured accurately. This method may be helpful in follow-up studies. Endoscopic ultrasonographic findings, such as characteristics of the tumor border and internal echoes, were studied to predict the histological diagnosis of the tumor. Leiomyoma, cyst, granular cell tumor, lipoma, and intraluminal metastasis of esophageal cancer were all found to have specific ultrasonographic findings indicating the histological nature of the tumor.

Establishment and characterization of rat pancreatic beta cell lines transformed by simian virus 40.

Rat pancreatic beta cells were transformed by simian virus 40 (SV40). The established beta cell lines expressed tumor antigen specific to SV40 and retained the ability to produce immunoreactive insulin (IRI) even at a high passage level. One of 12 transformed beta cell lines, SV-PB1205, was further investigated in vitro and in vivo. Firstly, effects of drugs on replication of the pancreatic beta cells were examined in in vitro experiments. Various drugs increased the uptake of 3H-thymidine into the cells. Those included D-glucose, tetragastrin and secretin. However, such effect was not observed for glucagon and growth hormone. Secondly, the SV40-transformed pancreatic beta cells were transplanted into diabetic rats. This produced such improvement of plasma glucose level at least for 2 weeks. The Significance of those experiments was discussed.

A ventrally localized inhibitor of melanization in Xenopus laevis skin.

Melanophores normally differentiate in dorsal but not in ventral skin of Xenopus laevis. We have sought factors which might regulate this differentiation pattern, and we have obtained a putative melanization inhibiting factor (MIF) from ventral but not from dorsal skin. Preliminary studies reveal that MIF is destroyed by heat or trypsin treatment, indicating its protein composition, and has a molecular weight in the range of 300 kDa. The effects of MIF on the differentiation of neural crest derivatives to melanophores were examined in vitro in the presence of tyrosine and fetal calf serum (FCS). Tyrosine enhances melanophore differentiation in vitro at concentrations equivalent to those estimated in adult Xenopus blood plasma (20 microM). FCS also stimulates melanization, by way of materials other than the tyrosine contained in FCS. MIF strongly inhibits outgrowth and melanization of neural crest cells from neural tube explants. MIF also inhibits the differentiation of melanoblasts contained in cultured explants of ventral skin. Inhibition of melanization or melanophore differentiation by MIF occurs even in the presence of L-tyrosine and/or FCS. We suggest that MIF plays an important role in the establishment of dorso-ventral pigment patterns in amphibia.

A gradient of responsiveness to the growth-promoting activity of ZPA (zone of polarizing activity) in the chick limb bud.

Limb bud mesoderm of stage 22-23 embryos was dissected into four pieces along the anteroposterior axis and dissociated cells of each region were separately cultured under various conditions. When the cells were cultured in medium containing 0.1% fetal calf serum (serum-poor medium) only a slight increase in cell number occurred in the cultures of all four regions. However, when the cells were cultured in medium containing 10% FCS, only cells of two central regions proliferated rapidly, and no growth promotion was observed in cells in the most anterior and posterior regions. Using the serum-poor medium, we examined the growth-promoting effects of cocultured limb bud fragments and of some growth factors on the cells of four regions. Anterior, distal, and proximal fragments promoted cell proliferation and their promotive effect on the cells of each region was equal. On the other hand, posterior fragments (containing ZPA) showed stronger promotive effects on preaxial cells than on postaxial cells. For comparison with the growth-promotive effect of the posterior fragment, fibroblast growth factor (FGF), epidermal growth factor (EGF), insulin, and retinoic acid were tested in cell culture. FGF showed position-dependent growth promotion, while EGF and insulin promoted growth in the cells of all four regions to a similar degree. Retinoic acid showed no effect on cell growth at low concentrations, and was rather toxic at high concentrations. These results suggest that the cells of the posterior region secrete an FGF-like growth factor(s), which controls normal limb development and experimental duplicate formation.
