Temporal and spatial expression of a position specific antigen, AV-1, in chick limb buds.

In a previous study, we demonstrated the presence of a position-specific antigen (AV-1) in chick limb buds at an early developmental stage. Here, we reported the temporal and spatial expressions and the biochemical characterization of the AV-1 antigen. Indirect immunofluorescence staining and immunoblot analysis clearly showed that the AV-1 antigen is a glycoprotein that is localized on the plasma membrane and that it is expressed from stage 19 and highly expressed at stages 22-26 in some middle-distal to anterior-distal region of limb buds. In the wing bud, at stage 28, the AV-1 antigen was faintly detected in the restricted space between the precartilaginous regions of the radius and the ulna, and those of the metacarpals 2 and 3, but not those of the metacarpals 3 and 4. Such stage-specific and "position-specific" expressions of the AV-1 antigen in limb buds strongly suggest that the AV-1 antigen or cells containing it are involved in determination of the limb pattern formation.

Retinoic acid promotes proliferation and chondrogenesis in the distal mesodermal cells of chick limb bud.

Distal and proximal mesoderm of chick limb bud was respectively dissociated and cultured in the medium containing various concentrations of retinoic acid (RA). At low concentrations (5-50 ng/ml), RA promoted proliferation and chondrogenesis in the distal mesodermal cells. The distal cells of stage 20-24 limb buds were responsive to RA, although those of stages 25-27 were unresponsive. Both the cells of anterior and posterior regions of the distal mesoderm were responsive to RA, while the cells of proximal mesoderm were unresponsive. At higher concentrations, the growth-promoting effect of RA was reduced and chondrogenesis in the distal cells was rather inhibited. These results were discussed in relation to the role of RA as the morphogen in normal limb development and experimental duplicate formation.

Follow-up study of lipid peroxides, superoxide dismutase and glutathione peroxidase in the synovial membrane, serum and liver of young and old mice with collagen-induced arthritis.

Because reactive oxygen species (ROS) are generally believed to play an important role in tissue injury in rheumatoid arthritis, we examined the levels of lipid peroxides, superoxide dismutase (SOD), and glutathione peroxidase (GSH-Px) in the synovial membrane, serum and liver of young (8 wk) and old (12 mo) mice with collagen-induced arthritis. In the synovial membrane, serum and liver, lipid peroxide levels of both young and old mice were increased beginning on the 3rd day after the onset of arthritis. SOD activity, which scavenges O2- and inhibits lipid peroxidation, rose markedly in the synovial membrane of young mice in parallel with the increase in lipid peroxide levels, but not so markedly in old mice. Liver GSH-Px activity, which metabolizes already formed lipid peroxides, also rose in young arthritic mice to a greater degree than in old mice. This study suggests that in inflammatory synovial lesions, lipid peroxides are generated due to an increase in ROS concentration, with resultant cytotoxicity, and that younger animals or humans can prevent this unfavorable reaction more effectively than aged ones by enzyme induction. The hypothesis that lipid peroxides formed in the oxidative lesions of the primary organ are released into the serum, trapped by the liver and metabolized there is further supported by the present study.

"Transformation" of human endothelial cells by SV40 virions.

Human endothelial cells derived from the umbilical vein were transformed with SV40 virions. A cell line subcultured for over 60 serial passages was characterized in comparison with its untransformed counterpart which was culturable for less than five passages. The SV40-transformed human endothelial cells, designated SV-HUVEC, were positive not only for tumor (T) antigen specific to the SV40-transformed cell, but also for two markers of endothelial cells, Factor VIII-related antigen and a receptor for Ulex europaeus agglutinin I. By transformation the growth potential of the human endothelial cells was increased and their serum requirement was decreased. The SV40-transformed endothelial cells were, however, unable to form colonies in soft agar or to form tumors in athymic nude mice, although a small nodule was produced at the site of inoculation. Subcultivation of these cells up to the 62nd passage eventually resulted in crisis and loss of further cell division. Thus, the human endothelial cells were transformed by SV40 while retaining certain normal functions but without showing tumorigenicity.

Development of motion analyzing system for implanted tilting disk valves.

A motion-analyzing system was developed to quantitatively evaluate functions of the implanted tilting disk valve, e.g., an open angle and a duration of valve open and/or shut. In this system a picture of the valve on an x-ray cinefilm is displayed on the CRT screen by the TV camera for extraction of valve position. Data extracted are stored in the computer and are used to calculate parameters for motion analysis of the valve. The reliability of this system was confirmed within 3 degrees of error using the valve model prior to the evaluation of cinefilms clinically procured. With the aid of this motion analyzing system, complicated motions of the tilting disk valve were easily quantified and the useful information as given for clinical diagnosis and further valve development.

Preoperative radioactive therapy for esophageal carcinoma. Randomized evaluation trial in eight institutions.

A cooperative, prospective, randomized study to evaluate the effectiveness of preoperative irradiation in curatively resected esophageal carcinoma was performed in 364 cases in eight institutions from August 1982 to November, 1983. Based on the survival curves, postoperative irradiation alone was superior to preoperative plus postoperative irradiation. Because of the progression of the disease and complications after operation, there were many inevaluable cases in this study. However, analysis revealed there was no bias caused by a greater number of unfavorable patients being selectively included in inevaluable cases in the postoperative irradiation only group. Our data question the value of preoperative irradiation being performed in addition to resection plus postoperative irradiation.

An evaluation of serum high density lipoproteins-phospholipids.

Phospholipids in high density lipoproteins (HDL) is being used as a negative risk indicator of atherosclerosis. Phospholipids in HDL may not demonstrate the actual level of HDL-phospholipids when determined by the precipitation or ultracentrifugal methods, because HDL fractions contain very high density lipoproteins (VHDL) and albumin. In the present study, the true level of phospholipids in HDL was estimated using high performance liquid chromatography (HPLC), and it was compared with the level of phospholipids in HDL determined by the precipitation method. Sera from 18 healthy subjects were used as materials. In the HPLC method, the HDL fraction was extracted making sure that it contained no free albumin, which is albumin not bound to phospholipids. The HDL fraction was separated into subfractions. It was found that phospholipids in the VHDL fraction make a 20.2 +/- 7.3% (mean +/- S.D.) part of the total HDL-phospholipids. A large part of the VHDL fraction was constituted of albumin-bound phospholipids. A significant correlation was observed between HDL-phospholipids determined by the precipitation method, which contain albumin, and the actual HDL fraction phospholipids determined by HPLC, which do not contain VHDL (r = 0.903, p less than 0.01). These results suggest that HDL-phospholipids values determined by the precipitation method give useful clinical data.

[Multidisciplinary treatment of esophageal carcinoma].

From 1965 until Aug. 1987, a total of 1,249 cases of primary thoracic esophageal carcinoma were resected. Some 70%-80% of these cases treated before 1980 underwent preoperative irradiation. From 1976-1987, in addition to preoperative irradiation, postoperative irradiation of the neck and upper mediastinum was performed in 20% of resected cases. (1) Evaluation of combined radiotherapy: In C greater than 0 resected cases, the postoperative T-shape irradiated group was significantly better than the nonirradiated and preoperative irradiation--only groups, with a risk factor of p less than 0.05, However in comparison to historical controls, no significant improvement was obtained and in the preoperative irradiated group. (2) Multidisciplinary treatment based on preoperative staging: The effectiveness of CDDP treatment in cases of recurrence or non-resectable cancer of the esophagus was 44.4%. Based on these findings, we have administered adjuvant CDDP + VDS in resected cases from 1985 and have obtained encouraging 1-year survival. Since 1985, extended dissection in 3 regions (cervical, thoracic and abdominal) has been carried out. Compared to C greater than 0 resected since 1981, in which dissection of lymph-nodes in two major regions was performed, dissection of lymph-nodes in the three major regions has resulted in an improved 1-2 year survival, despite the increased numbers of stage IV cases. The findings thus indicate that the presence of lymph-node metastasis significantly affects prognosis. Therefore, in cases that are preoperatively evaluated as sm or more extensive invasion, it is necessary to combine extensive lymph-node dissection, postoperative irradiation or chemotherapy.

Direct evidence for granuloma-inducing activity of interleukin-1. Induction of experimental pulmonary granuloma formation in mice by interleukin-1-coupled beads.

Pulmonary granulomas were induced in BALB/c mice by the intratracheal injection of Sephadex G-50 and latex beads. Very large granulomas developed around Sephadex G-50 beads. Minimal inflammation was produced in mice given latex beads. Aqueous extracts prepared from pulmonary granuloma lesions induced in mice by Sephadex G-50 beads contained high levels of interleukin-1 (IL-1) activity but not interleukin-2 (IL-2) activity. IL-1 activity in the extracts correlated with granuloma size. In a subsequent step, large granulomas were induced by the intratracheal injection of Sepharose 4B beads coupled to fractions of the extracts containing IL-1 activity (ie, granuloma-derived IL-1) prepared from Sephadex G-50-induced granulomatous lungs. In addition, large granulomas were induced by the intratracheal injection of recombinant IL-1-coated Sepharose 4B beads. In contrast, very small granulomas were seen when IL-2-coated or plain Sepharose 4B beads were injected into mice. These results indicate that IL-1 participates in the induction and/or expression of granulomas.

[Multidisciplinary treatment of thoracic esophagus carcinoma based on preoperative staging].

Recently, with developments in CT scanning and endoscopic ultrasound (EUS), in addition to conventional ultrasound (US), significant advances have been made in the evaluation of preoperative staging. Evaluation of the presence of A3 invasion is made on the basis of X-ray, CT and EUS findings. In superficial cases of ep, mm or sm, EUS with particular reference to the X-ray and endoscopic appearance is very effective, and in our department the diagnostic accuracy in cases of mucosal lesions is 88%, while it is 84% in sm cases. Diagnosis of metastasis to cervical and abdominal lymph nodes is performed by US, while diagnosis of mediastinal lymph node metastasis is performed by EUS. The accuracy of US for detecting cervical and abdominal lymph node metastases, including those approximately 0.5 cm in size, is 95%, while that of EUS in the diagnosis of metastasis to mediastinal nodes is 89%. The 5-year survival of patients with lymph node metastasis significantly affects prognosis. Based on this, since 1985, extended dissection in the cervical, thoracic and abdominal regions has been carried out. Since 1985, some resected cases have been treated with pre- or postoperative adjuvant chemotherapy regimens, centered primarily on CDDP (majority of cases with CDDP + VDS). A significant improvement has been obtained in the 1-2-year survival of C greater than O resected cases treated since 1985, compared with cases in 1980-1984. This improvement is thought to be due to dissection of lymph nodes in the three major regions, based on more accurate preoperative evaluation and the introduction of postoperative adjuvant chemotherapy.
