Surgical treatment of intracranial hematoma and hydrocephalus in an infant with hemophilia A.

A seven-month0old hemophiliac infant developed intracerebral, subdural and intraventricular hematomas and hydrocephalus. Removal of hematomas and placement of ventriculoperitoneal shunt could be performed quite safely even in an infant under the cover of highly potent human antihemophilic factor concentrate. Computerized tomography is very useful for neurosurgical care of the hemophiliac patients as a noninvasive and atraumatic method of examination. Intracranial bleeding is listed as the most common cause of death among hemophiliacs. The literature of intracranial operations on the patients with hemophilia A is reviewed.

Computerized tomography in Moyamoya syndrome.

Twelve patients with Moyamoya syndrome were examined by computerized tomography (CT); nine children and three adults. In one patient, CT was normal. Significant abnormalities on the precontrast CT in the remaining 11 patients were: cortical atrophy with or without varying degree of ventricular dilatation, and irregularly shaped, various sized, often multiple or bilateral lucent foci in the cortex, subcortical white matter and the central gray matter. After contrast enhancement, no significant increase in the attenuation number was observed in and around the lucent foci except in one patient. In three patients, there was a slight increase in density in the central gray matter, however the increase in density was slight when the dense angiographic vascular networks in the basal ganglia were considered.

Periventricular lucency in hydrocephalus on computerized tomography.

The ventricular margin on computerized tomography (CT) was examined in patients with dilated ventricles. In childhood and adult hydrocephalus of various causes, periventricular lucency of several degrees was often detected in the frontal horn, when hydrocephalus had progressed to an advanced stage. Since the periventricular low density zone disappears in a short time after a shunting operation, together with a reduction of the ventricles to normal size, the lucent zone around the ventricles may represent an increase in water content in periventricular white matter. This is consistent with periventricular edema, rather than loss of lipids and proteins.

[Serum immunoglobulin (IgG, IgM, IgA) levels in patients with primary brain tumors (author's transl)].

The concentration of serum immunoglobulins (IgG, IgM, IgA) in 63 patients of primary brain tumors were determined by the method of single radial immunodiffusion. 1) The mean values and standard deviations of three major serum immunoglobulins in 18 cases of meningioma were as follows; IgG, 1297.0 +/- 387.9 mg/dl, IgM, 165.1 +/- 153.4 mg/dl, IgA, 219.5 +/- 76.7 mg/dl. These values were not significantly different with those in normals. 2) 45 cases of gliomas showed; IgG, 1190.5 +/- 378.3 mg/dl, IgM, 160.0 +/- 75.9 mg/dl, IgA, 189.8 +/- 91.5 mg/dl. These values were in normal levels as compared with normal serum immunoglobulins. 3) Values of serum immunoglobulins in 19 cases of glioblastoma multiforme were compared with those in 18 cases of astrocytoma to know the relationship between histological malignancy and levels of serum immunoglubulins. But no significant difference was observed between these values. The levels of three major serum immunoglobulins either in meningiomas or in gliomas were not significantly different with those in normals. Therefore, the levels of serum immunoglobulins could not be used as a diagnotic procedure in cases of primary brain tumors.

Neurological manifestations of rhabdomyosarcomas in the head and neck in childhood. Neuroradiological study.

Two cases of rhabdomyosarcoma in the head and neck with neurological manifestations are presented. Brain scan demonstrated an apparent area of increased uptake of 99mTc in the tumor. Cerebral angiography revealed a sharply demarcated but rather faint tumor stain. The contributing vessels were hypertrophied branches of the external carotid artery, a dural branch of the internal carotid artery, and branches of the vetebrobasilar systes emphasized because the recent combination of chemotherapy in conjuction with surgery and irradiation seems to be encouraging.

Scalp veins as collateral pathway with parasagittal meningiomas occluding the superior sagittal sinus.

In 3 cases with occlusion of the superior sagittal sinus (SSS) with parasagittal meningiomas, carotid angiography demonstrated scalp veins as an important collateral venous pathway. The authors believe that occlusion of the SSS is best seen in half axial oblique carotid phlebograms under contralateral carotid compression. Direct sinography is time consuming and troublesome. In the presence of scalp veins as important collaterals, strategic consideration for surgical treatment is discussed.

Importance of hypertension and genetic factors for atherogenesis in rats.

1. A new experimental system has been used to analyse factors involved in the initiation of atherosclerosis in rats. 2. Arterial fat deposition in the cerebral arteries was affected by blood pressure, serum cholesterol concentrations, strain difference and age, of which high blood pressure was the most important. 3. A genetic factor independent of hypertension was shown to be involved in acute arterial fat deposition in spontaneously hypertensive rats.
