Synthesis of 17O isotope labeled Leu-enkephalin and 17O n.m.r.

Oxygen-17 isotope was introduced into the alpha-carboxyl group of glycine, 1-phenylalanine, 1-leucine and 1-tyrosine by acid catalyzed exchange of 17O from H2O(17) or by acid hydrolysis of respective amino acid methyl esters in H2O(17). Quantitative enrichment of glycine was achieved by acid hydrolysis of amino acetonitrile in H2O(17). For alpha-amino protection in amino acids t-butoxycarbonyl (Boc) group was employed for 17O labeled enkephalin synthesis. Five analogues of Leu-enkephalins (I-V) labeled with 17O at different amino acid residues were synthesized by solid phase method. 17O n.m.r. spectra were measured at 24.4 and 67.8 MHz for Leu-enkephalins 17O labeled at Gly2 and Phe4 positions. A downfield shift was observed for 17O labeled Gly2 Leu-enkephalin upon heating. This shift is indicative of the rupture of intramolecular hydrogen bonds. The preliminary results confirm the hypothesis that an intramolecular hydrogen bond exists between the carbonyl group of Gly2 and NH group of Leu5.

Neoplastic meningitis from a pineal tumour treated by antibody-guided irradiation via the intrathecal route.

A monoclonal antibody (UJ181.4) was labelled with 131I and given intrathecally to a patient who was critically ill with neoplastic meningitis due to a disseminated pineoblastoma. The target antigen had first been demonstrated on tumour cells by immunocytological testing of cerebrospinal fluid (CSF). As a preparation for therapy, a test dose of 131I-UJ181.4 antibody was given simultaneously with a control antibody labelled with 125I. Good evidence of in vivo immunolocalization was obtained by external gamma counting and by analysis of the radiolabels on centrifuged CSF cells. Specificity ratios of between 10:1 and 20:1 were obtained by these methods. A single relatively small therapeutic dose of 131-I (870 MBq) given by the intrathecal route, resulted in a marked clinical improvement and sustained remission for 22 months.

[Practice relevant aspects of immunohistochemistry in pulmonology].

Own experiences in practical application of immunohistochemistry are demonstrated in cases of lung fibrosis, sarcoidosis, tuberculosis and allergic vasculitis. Methods for demonstration of immunoglobulins (IgG, IgE) are useful in diagnosis of allergic vasculitis (IgG) and allergic alveolitis (IgE). Lysozyme is an important morphological parameter for the estimation of the activity of sarcoidosis. First results of application of antisera against mycobacteria and Kveim antigen are discussed in respect to more precise differential diagnosis of granulomatous lung diseases (sarcoidosis, tuberculosis).

[Morphologic studies in fibrosing alveolitis. I. Content of alveolar macrophages and lymphocytes in lung parenchyma in correlation with the duration of the disease].

In 50 patients with fibrosing alveolitis the content of lymphocytes and alveolar macrophages was semiquantitatively estimated and correlated to the duration of the disease. Increasing duration of the disease was correlated with a decrease of the content of lymphocytes and macrophages. The findings demonstrate close correlations between the duration of the disease and the infiltration of lung tissue with macrophages and lymphocytes.

[Animal experiment studies of morphologic changes in the trachea and lung in high frequency ventilation].

Prolonged high frequency ventilation (24 hrs) in dogs leads to morphological changes in the trachea as well as in the bronchi. Erosive changes of tracheal epithelium were observed whereas small bronchi and bronchioli exhibited luminal obstructions by mucus plugs, caused by hypersecretion of goblet cells. Using histochemical methods degenerative changes of bronchiolar epithelium were observed.

[Value of bronchoalveolar lavage in assessing the activity of sarcoidosis].

BAL was performed in different courses of sarcoidosis in order to evaluate the importance of this examination. We found a higher degree of lymphocytosis in Löfgren's syndrome and in relapses than in the others. Probably there is a direct connection between high intensity alveolitis and reduction of diffusing capacity. There could not be found any correlation to other markers of activity (lung function, SACE, 67-Gallium-Scan).

[Chemiluminescent measurements of alveolar macrophages in sarcoidosis patients].

The phagocytic activity of alveolar macrophages (AM) in a group of patients with stage I, stage II and stage III pulmonary sarcoidosis (gradation according to Wurm) has been investigated. Additionally a classification in different forms of the course of sarcoidosis was made (acute = Loefgren's syndrome, latent = primary-chronic, and relapses). Patients with other lung diseases and healthy subjects were recruited as control group. The phagocytic activity (stimulation with opsonized yeast cell wall particles) of AM, which were isolated by bronchoalveolar lavage, was determined by means of chemiluminescence (CL)-measuring using lucigenin and luminol, respectively, as amplifiers. The investigations showed that the lucigenin-dependent yeast cell wall-induced CL of AM in patients with sarcoidosis is significantly increased in comparison to the control group. No significant changes of the luminol-dependent CL of AM from sarcoidosis patients could be detected. The lucigenin-dependent CL-response of AM is obviously an indicator of the intensity of the alveolitis and thus of the activity of the pathological process in sarcoidosis. The results suggest that in pulmonary sarcoidosis there is a hyperreactive AM/lymphocytes-system.

Isotopic labeling of tyrosine, followed by 17O n.m.r.

A simple chemical procedure has been developed in order to introduce oxygen-17 labels into the carboxylic and phenolic sites of L-tyrosine. Detailed studies of the 17O n.m.r. chemical shift as a function of pH reveal an unusually large titration shift upon the deprotonation of the phenol group. This result suggests that 17O n.m.r. may contribute useful information about side chain properties in peptides and proteins.
