[Pregnancy and immune system. I. Immunological characterization of mononuclear cells during late pregnancy (author's transl)].

Lymphocyte preparations obtained from peripheral blood of 36 healthy pregnancy women during the last 4 weeks of gestation were tested for various cell surface markers (E-, EA- EAC-rosettes, surface Ig positive cells). 29 healthy, non-pregnant age matched women served as controls. The cell surface marker analysis did not show significantly different results in the two groups tested except diminished percentages of EA-rosettes in the pregnancy group. EA-rosettes-forming cells (Fc-receptor positive cells) are known to the effectors of spontaneous and antibody dependent lymphocytotoxicity. Therefore, slightly depressed lymphocytotoxicity may be expected during late pregnancy.

Immune complexes in multiple sclerosis: relation to clinical pattern.

Using a C1q binding test, circulating immune complexes (IC) were detected in 33.3% of sera from 138 patients and in 19.4% of 124 spinal fluid samples from patients with multiple sclerosis. Most often they occur in sera alone. As a rule their detectable amount is small in sera as well as in spinal fluids. IC were observed with equal frequency during acute exacerbations and in stable phases of the disease. In patients with early MS of less than 3 months duration, IC were detected only rarely, whereas their frequency increased up to 50% in patients with longer standing disease. Immunosuppressive therapy has no influence on IC formation. Patients with immune complexes exhibited a more rapid clinical deterioration if compared as a group with IC-negativ ones. No correlations were found between immune complex formation and the CSF-IgG index or the rate of pleocytosis in spinal fluids. Neither the complement factors C3, C4, C3A nor total hemolytic complement activities (CH50) in serum were significantly decreased in patients with IC formation in serum as compared with the IC-negative group. The results demonstrate that IC formation probably is of no importance in the pathogenesis of multiple sclerosis.

Idiotype-bearing peripheral blood lymphocytes in human multiple myeloma and Waldenström's macroglobulinaemia.

Antisera raised against idiotypic determinants (ID) of myeloma proteins and macroglobulins have been used to differentiate peripheral blood lymphocyte (PBL) populations from individual patients. ID-positive lymphocytes not resembling plasma cells have been regularly found in peripheral blood in these diseases. For further characterization ID-positive lymphocytes were enriched from the peripheral blood by affinity chromatography using heterologous anti-idiotypic sera. Two patients with IgG myeloma, one patient with Waldenström's macroglobulinaemia and two persons with benign monoclonal hyperglobulinaemia (BMH) were examined by this technique. The ID-positive PBL population was shown to be heterogeneous with respect to non-tumour-specific surface markers, such as sheep erythrocytes (SRBC), Fc and C receptors. Tumour-specific idiotypic determinants will thus allow a more correct recognition of the total tumour cell compartment in these diseases.

Cerebrospinal fluid immune complexes in multiple sclerosis.

Using a C1q binding test, immune complexes have been detected in one half of cerebrospinal fluid samples from patients with multiple sclerosis. These results provide additional evidence for the participation of an immune reaction in the disease process.

Immunological characterization of mononuclear cells in peripheral blood and regional lymph nodes of breast cancer patients.

Mononuclear cells from peripheral blood and draining lymph nodes of 40 patients with invasive locoregional breast cancer were examined for immunological cell surface markers (E, EAhuman, EAox, EAC, SIg pos.). Concomitantly, blood lymphocytes from 36 healthy women and axillary and mesenteric lymph-nodes from patients without malignant diseases were tested as controls. In peripheral blood of tumor patients E rosette-forming cells were slightly diminished as compared to the control group, whereas EAox and EAC rosette-forming cells were increased. These differences may be age-dependent rather than tumor-related. In the draining lymph nodes of breast cancer patients as well as in the control lymph nodes, the percentages of EAC rosette-forming cells and SIg positive lymphocytes were significantly increased compared to peripheral blood, whereas E and EAhuman rosette values remained unchanged. Percentages of EAox rosettes on the other hand were strongly diminished in the draining lymph nodes, suggesting that the EAhuman and EAox rosetting techniques detect 2 types of Fc-receptor bearing cells. No significant differences were found between the cell surface marker analysis of tumor-free and metastatic lymph nodes of breast cancer patients and the control lymph nodes.

Platelet aggregation and aggregation inhibition by different antiglobulins and antiglobulin complexes from sera of patients with rheumatoid arthritis.

Antiglobulin complexes were isolated from rheumatoid arthritis (RA) sera in order to investigate their ability to aggregate human platelets and to influence platelet aggregation induced by heat-aggregated IgG. Eleven of 38 seropositive RA sera showed a significant platelet aggregation (PA) titer as compared to 40 normal control sera. No correlation was observed between sheep T cell agglutination titers and PA titers of individual sera. Ten of 22 RA antiglobulin preparations containing IgG, IgM, and IgA antiglobulins and antiglobulin complexes also showed positive PA tests. Such preparations were able to inhibit PA brought about by heat-aggregated human IgG. When IgG and IgM antiglobulins were tested separately, only IgM antiglobulins showed this inhibitory effect, whereas IgG antiglobulins were inactive. Neither IgG nor IgM antiglobulins induced PA alone. Reassociated IgG antiglobulin complexes consisting of carefully prepared IgG and isolated IgG antiglobulins were able to induce PA. Platelet aggregation by RA sera was thus shown to be due to IgG antiglobulin complexes present in the sera. It can be concluded from these experiments that the composition of antiglobulin complexes in individual sera comprised of antiglobulins of different classes and IgG "antigen" is responsible for PA results in vitro. In pathophysiologic terms, the reaction of IgG antiglobulin complexes with thrombocytes in vivo may be an important step in the chain of events leading to generalized vascular damage and deposition of immune complexes in vessel walls which is thought to be responsible for the development of generalized vasculitis in RA.

Peripheral blood lymphocyte subpopulations in multiple myeloma.

Antisera raised against idiotypic determinants of myeloma proteins and macroglobulins have been used to differentiate peripheral blood lymphocytes populations from individual patients. I.D.-positive lymphocytes not resembling plasma cells have been regularly found in peripheral blood in these diseases. This lymphocyte population is heterogeneous with respect to non-tumor-specific surface markers, such as SRBC-, Fc- and C-receptors. Tumor specific idiotypic determinants will thus allow a more correct recognition of the total tumor cell compartment in these diseases.

Immunological characterization of mononuclear cells and morphological findings in patients with mammary carcinoma.

Mononuclear cells from peripheral blood and draining lymph nodes of 40 patients with invasive mammary carcinoma were examined for various immunological cell surface markers including surface membrane immunoglobulins and rosetting properties (E, EA, EAC). No significant relationship could be established to anyone of the following criteria for which the literature reports varying prognostic values: Clinical staging of the disease , histological tumor type, grading, nuclear differentiation, round cell infiltration, perivenous infiltration, sinus histiocytosis, and lymph node reaction patterns (lymphocyte predominance, germinal center predominance, lymphocyte depletion, unstimulated nodes). From the reported results it is concluded that the analysis of lymphocyte cell surface markers in mammary carcinoma is not a suitable parameter for supporting the existence of specific or unspecific anti-tumor immune reactions which may be suspected from certain histological reaction patterns.

[Effect of therapy on the lymphoid cell distribution in the venous blood of breast cancer patients].

Rosetting properties (E, EAh, EAox, EAC rosettes) and presence of surface immunoglobulins (SIg) were examined on peripheral blood lymphocytes from 30 breast cancer patients immediately prior to therapy and 4 weeks thereafter. Therapy consisted of limited radical surgery followed by combined X-ray and telecobalt radiotherapy. The results were compared to patients who had received the same treatment 1 year ago (n = 13), 2 years ago (n = 13) and 3 to 10 years ago (n = 20). All irradiated patients exhibited a considerable leuko- and lymphopenia with a particular decrease of E and EAh rosettes, and a concommittant relative increase of EAox and EAC rosettes. SIg positive cells showed no significantly different percentages before and after therapy although in absolute counts they were similarly reduced as the other subpopulations after radiotherapy. The possible prognostic influence of radiation induced lymphopenia is discussed without coming to clear conclusions.

[Imprint cytology of axillary lymph nodes in breast neoplasms for intraoperative rapid diagnosis].

A total of 385 lymph nodes of patients suffering from breast cancer were examined intraoperatively for the presence of metastases. We applied imprint cytology and stained using the "Diff-Quick" set (3 times 30 seconds). The percentage of correct identifications following instant cytological examination amounted to 94.5 if compared with subsequent paraffin histology. The 21 diagnostic failures were composed of 3 false negative and 18 false positive results. These data make the advantages for routine clinical application quite obvious.

The seroepidemiological pattern of acute viral hepatitis. An epidemiological study on viral hepatitis in the Hannover region.

The natural incidence of the etiologically distinct types of viral hepatitis was determined by investigating acute phase sera of symptomatic hepatitis cases occuring in the Hannover area in 1975 for the presence of hepatitis B surface antigen, antibodies to hepatitis A, hepatitis B core and surface antigens, and by measuring the IgM serum levels. Fourteen different seroepidemiologic patterns were recognized. Although there was a high prevalence of hepatitis A antibody in the population, the frequency of hepatitis A was low (n = 56) suggesting that the hepatitis A virus does not play a major role in symptomatic hepatitis in the Hannover area at present. Spread of the hepatitis A virus was mostly associated with person-to-person contact or tourist travel in southern Europe. Hepatitis B was the predominant type of hepatitis (n = 211). Hepatitis non-A, non-B was observed infrequently (n = 62). A high percentage of patients with hepatitis B and hepatitis non-A, non-B reported parenteral exposure to potentially contaminated materials. No other findings, however, suggested an infectious etiology of hepatitis non-A, non-B.

Influence of continuous-flow blood separator leucophoresis on in vitro functions of human lymphocytes.

The influence of continuous-flow blood separator leucophoresis on these in vitro functions of human lymphocytes was studied. In ten separation experiments, lymphocyte functions were examined in the presence and absence of mononuclear phagocytes before, during and at the end of continuous flow leucophoresis. Cell surface markers and effector functions in spontaneous cell-mediated cytotoxicity (SCMC) and antibody-dependent cellular cytotoxicity (ADCC) assays did not change significantly under the conditions of leucophoresis. By contrast, separator lymphocytes exhibited a significantly increased mitogenic response to pokeweed mitogen and PPD. Removal of mononuclear phagocytes corrected for this phenomenon. PHA and Con A responses were not affected by leucophoresis, nor was the number of circulating haemopoietic stem cells (CFU-C) significantly altered during a 4 hr continuous flow blood separator run.

[Quantitative determination of serum proteins using a new laser nephelometer (author's transl)].

Methodological aspects of quantitative serum protein determinations, using a newly developed laser nephelometer (PDQTM laser nephelometer, Hyland), are reported. For within-day precision, determination of the four serum proteins IgG, IgA, IgM and C'3, showed coefficients of variation between 2.5% and 4.6%. For precision from day to day, the average variation coefficient was 8%, the lowest value being 5%. Provisional normal values for the four proteins were ascertained from a small group of 60 healthy individuals. These results, as well as those obtained in states of disease, correlate with values obtained by radial immunodiffusion. It is concluded that the method is reliable, and has a number of important advantages compared to radial immunodiffusion; namely, the much higher precision, better sensitivity, particularly in lower concentration ranges, and availability of results within two hours.
