Dietary influences on 8-MOP plasma levels in PUVA patients with psoriasis.

To study the effect of diet on 8-MOP plasma levels, these levels were measured three times in twenty PUVA patients with psoriasis; while fasting, after a low-fat breakfast, and after a fat-rich breakfast. No significant differences were found between the maximal 8-MOP plasma levels in the fasting condition and the levels after the low-fat breakfast, both peaking 2 h after ingestion. After the fat-rich breakfast, however, the 8-MOP plasma levels were not only significantly lower at 2 h but also peaked later, reaching their maximum at 3-4 h after intake. The drug should therefore be given in a dietetically standardized way. This raises the question whether some cases of therapy resistance can be dietetically influenced.

Ketoconazole (R 41 400) in the treatment of dermatophyte infections: a clinical, mycological, and morphological study.

Ketoconazole (100 mg, orally, once daily) was investigated in nine patients with extensive dermatophyte infections. After treatment ranging between one week and three months, clinical cures (healing of lesions and negative cultures) were observed in all cases. In vitro growth of Trichophyton rubrum, T. mentagrophytes, and T. verrucosum isolated from infected skin scales were completely inhibited by concentrations of ketoconazole of 10 microgram/ml and above. No evidence for the development of drug resistance was obtained from regular in vitro sensitivity tests.

Thrombocytopenia: first symptom in a patient with dyskeratosis congenita.

A case of dyskeratosis congenita is reported. This rare hereditary disease usually has the following progression: ectodermal dystrophy (reticular skin pigmentation, nail dystrophy, leukokeratosis of mucosal membranes), appearing in the first decade, followed in about 50% of these patients by a hematopoietic disorder resembling Fanconi's anemia, usually developing in the second or third decade. Carcinomas may occur in leukokeratotic areas in the third, fourth, or fifth decade. This patient's clinical course is interesting because the thrombocytopenia developed as an isolated symptom at the age of 5 years and preceded the skin anomalies by three years. The diagnosis of dyskeratosis congenita was made only after an evolution of five years. The diagnosis of dyskeratosis congenita--although it is a rare disease--should be considered in every child first seen with aplastic anemia or thrombocytopenia.

Computer assisted monitoring of contact dermatitis patients.

All contact dermatitis patients are told to avoid their specific allergens. As regards topical pharmaceutical agents, however, it is almost impossible for these patients to identify the products that contain their allergens. In order to provide reliable information for these patients, we have designed a computer assisted data system. The CODEX (COntact DErmatitis indeX) system consists of three computer readable files: a Product File containing the complete composition of the pharmaceutical products on the Belgian market that are applied on the skin and the mucous membranes, a Patient File with the patient's anamnesis, and a Literature File with cross-referenced material on contact dermatitis. Each patient is given a list of the products that contain his/her allergen(s). The data bases are analyzed statistically and updated periodically. Cosmetics in general are excluded.

Follow-up study of patients with contact dermatitis caused by chromates, nickel, and cobalt.

The patch test results from 1,000 patients tested with the ICDRG standard series are analyzed for positive reactions to potassium dichromate, nickel sulphate, and cobalt chloride. Nickel appears to be the most common sensitizer, especially among women, although false-negative reactions may occur if it is tested with a 2.5% concentration in yellow petrolatum. Chromate dermatitis is generally of occupational origin among men. It is also quite common among Belgian women, probably because of the widespread use of Javel (liquid bleach), containing sodium dichromate as a colouring and stabilizing agent. Positive patch test reactions to cobalt are generally accompanied by allergic reactions to nickel or chromate. Its role as a primary allergen, however, must never be overlooked. The relationship between these metals and hand and foot eczema is evaluated. The results of a follow-up questionnaire are given. More than 70% of the patients allergic to chromates or nickel continued to suffer from contact eczema after the diagnosis had been made and therapeutic measures taken.

Skin sensitizing properties of arylalcanoic acids and their analogues.

While participating in a clinical trial of suprofen, a new antiphlogistic agent, a 32-year-old atopic woman developed a severe contact dermatitis reaction of the hands to this substance. To check for cross-reactivity patterns, patch tests were performed with similar arylalcanoic acid derivatives. Positive reactions were obtained for ibuprofen, ketoprofen, fenoprofen, naproxen, alclofenac, and metiazinic acid, which are all antiphlogistic and anti-inflammatory agents for systemic use, and chlorambucil, which is an antineoplastic agent. The antigenic determinant seems to correspond to a very well-defined structure: the carboxyl group (-COOH) has to be separated from the aryl structure by at least one C atom (substituted or not). Furthermore, the aryl group must be substituted with different chemical groups but not in the orthopositions.

Dihydroabietyl alcohol (Abitol): a sensitizer in mascara.

A nickel-sensitized female patient developed a contact dermatitis reaction to her mascara that was maintained by her spectacle frames, which contained nickel. On patch testing, she reacted to dihydroabietyl alcohol (Abitol), which was present in her mascara, and to hypoallergenic adhesive tape containing methyl abietate. There was also cross-reactivity with colophony and abietic acid.

Dimethylthiourea, an unexpected hazard for textile workers.

Our patient was a 24-year-old female textile cutter who had a conjunctivitis and an erythematous itching dermatitis of the eyelids, nasal mucous membranes, and the corners of the mouth. The patch tests with the standard series and a pharmaceutical test battery were negative. Further epicutaneous testing revealed a strong positive reaction to the textile cutting patterns she handled, which were duplicated by diazo processing. Specifically, the contact dermatitis reaction was caused by dimethylthiourea, an additive in diazo-sensitized paper.

A retrospective study of Patch test results from 163 patients with stasis dermatitis or leg ulcers. I. Discussion of the Patch test results and the sensitization indices and determination of the relevancy of positive reactions.

Patch test results from 163 patients with leg ulcers and/or chronic venous insufficiency, tested with a modified ICDRG standard test battery and a pharmaceutical test series, are analyzed and compared with the results obtained from a randomly selected control group of patients with eczematous conditions. A sensitization index for the most common contact allergens present in this test series is calculated, and the relevancy of hypersensitivity to specific topical agents is determined.

A retrospective study of patch test results from 163 patients with stasis dermatitis or leg ulcers. II. Retesting of 50 patients.

Retesting 50 patients with stasis dermatitis after a period of time (3-25 months) revealed significantly less positive patch test results (p less than 0.05). All procedures were, of course, standardized. The main reasons for this were elimination of the sensitization agents in the interim and testing with a less acute skin condition. Allergens that are difficult to avoid, such as the 'para' compounds and balsam of Peru and related substances, show a higher degree of persistence in subsequent patch testing.
