Thymopentin treatment of herpes simplex infections. An open, monitored, multicenter study.

Twenty-seven patients suffering from long-standing, severe, recurrent herpes simplex (14 labial and 13 genital) who had not responded adequately to prior therapy were recruited for this open, monitored study. They were treated with thymopentin 50 mg subcutaneously three times weekly for a period of 6 weeks. Clinical controls were performed once a week and then again 6 weeks after cessation of therapy; laboratory investigations were done at time points 0, 3, and 6 weeks. Additionally, information was collected with regard to the clinical course during the following year. Thirteen of 14 patients with labial infection and 10 of the 13 with genital herpes improved markedly (p less than 0.05) as shown by decrease in the relapse rate of at least 50%, shorter relapse episodes, and improvement of symptoms such as pain and itching. Fourteen of these 27 patients experienced no relapse for a period longer than 4 months after cessation of therapy. These favorable results were paralleled by a statistically significant increase in the T cell helper/suppressor ratio. This finding indicates that thymopentin acts as an immunodomulator; it is assumed that the activation of T helper cells induces-presumable via interleukin 2-the proliferation of cytotoxic T lymphocytes and natural killer cells which play a major role in the natural immune defense. No serious side effects of thymopentin were recorded.

Multicenter clinical evaluation of a hydrocolloid dressing for leg ulcers.

The need for a moist environment for the normal healing process led to the development of occlusive dressings. Results from this study support the contention that a moist wound environment is favorable to the healing process in humans as well as in animal models. Hydrocolloid dressings are effective in the practical daily management of chronic and even refractive ulcerations of the lower extremities and offer a time-saving treatment alternative with a high degree of patient acceptability.

Mephenesin contact dermatitis with erythema multiforme features.

5 patients, all using a mephenesin -containing ointment, with an acute contact dermatitis also presented an id-like spread with erythema multiforme features. In 4 patients, patch tests were performed and since mephenesin was the common allergen in each of these cases, we may assume that this is also the cause of the erythema multiforme-like lesions.

Bleomycin-induced Raynaud's phenomenon and acral sclerosis.

This study presents a case of two of the more uncommon side effects of bleomycin therapy for a metastasized testicular carcinoma: Raynaud's phenomenon and acral sclerosis. It is suggested that this association indicates a common pathogenic mechanism. These side effects may serve as an experimental model in the study of Raynaud's phenomenon and acral sclerosis.

Morphological study of the reticular erythematous mucinosis syndrome.

Histopathological studies were conducted on skin biopsies of 4 patients with the reticular erythematous mucinosis (REM) syndrome. The diagnosis was made on the basis of the clinical picture in 3 of the cases and from histological characteristics in the 4th. These morphological studies were done parallel with a study of skin biopsies with quite a similar histological picture but in which the clinical picture clearly indicated differing entities. The nosologic place of REM syndrome is questioned and the impression arises that there is a clear-cut histogenetical connection with several inflammatory skin diseases.

The effect of thymopentin treatment on the relapse rate in frequently relapsing herpes simplex virus infections.

Twenty-four patients suffering from longstanding severe recurrent herpes simplex, who had not responded to prior therapy, were treated with s.c. thymopentin injections 50 mg, three times weekly, over a period of six weeks. They were followed up at weekly intervals over this period and then six weeks later. Moreover, the longest relapse-free period observed in the year after the treatment was recorded in the investigator's documentation. Thirteen of the 14 patients with labial herpes simplex and 10 of the 13 patients with genital herpes simplex improved markedly as shown by a decrease in the relapse rate of at least 50%, shorter episodes of relapse and improvement of symptoms such as pain and itching. Fourteen of these 27 patients experienced no relapse for a period longer than four months after cessation of the therapy. No serious side-effects were observed. Laboratory examinations before, during and after thymopentin did not reveal significant alterations except for an increase in the T-helper/T-suppressor ratio. The effect of thymopentin is assumed to be due to T-helper cell activation resulting in enhanced interleukin-2 production with subsequent proliferation of cytotoxic T lymphocytes and natural killer cells which are capable of producing immune interferon.

The relationship between 8-methoxypsoralen skin and blood levels.

A method is described to determine the 8-methoxypsoralen (8-MOP) concentration in vivo in the skin by means of high-performance liquid chromatography (HPLC). Skin and blood samples were taken from 80 rats at specific intervals after oral administration of [3H]8-MOP. The pharmacokinetic results obtained for the skin levels were compared to the blood levels. In addition, liquid scintillation counting (LSC) was done on all the samples to compare the concentrations of 8-MOP plus metabolites to the concentrations of 8-MOP alone. There was a good correlation between the 8-MOP skin and blood levels. The values obtained with LSC were higher in function of time than the corresponding values obtained by HPLC, which indicates the presence of metabolites in both the skin and the blood. No statistically significant difference in the time of peaking was noted for the skin and blood levels. The blood levels seem to be a good parameter for the 8-MOP skin concentration.

Contact allergy to petrolatums. (I). Sensitizing capacity of different brands of yellow and white petrolatums.

46 yellow and white petrolatums were screened for their skin sensitizing properties in 3 subjects with known histories of contact sensitivity to yellow petrolatum. Patch testing revealed that all the petrolatums examined contain sensitizing substances in varying amounts. The results were analyzed with regard to origin, color, purification procedures involved in the production of pharmaceutical grade petrolatum, and pharmacopoeial requirements satisfied. Allergenicity correlated positively with these parameters, but only among petrolatums of the same brand. Petrolatum sensitivity may sometimes account for false positive patch test reactions to substances diluted in it. A petrolatum which causes no reaction on healthy skin may cause eczematous reactions on damaged skin.

Contact allergy to petrolatums. (II). Attempts to identify the nature of the allergens.

Using several chromatographic and spectrometric procedures, it was established that the allergens present in pharmaceutical and cosmetic petrolatums are probably polycyclic aromatic hydrocarbons. These substances are impurities, and the quantity present is largely a function of the source of the petrolatum and the purification procedures. As the aromatic portion in petrolatum is a very complex and unstable mixture of chemically-related agents, only 2 out of the several possible allergens present could be identified: they were most probably phenanthrene derivatives with molecular weights of 230 and 244.

Allergic contact dermatitis to topical corticosteroids: clobetasol propionate and clobetasone butyrate.

2 case reports are given of patients with positive patch test reactions to clobetasol propionate. One of the patients also reacted to clobetasone butyrate. 30 other steroids that were chemically very closely related to these two 21-chloro-9-alpha-fluoro-corticosteroids, were patch test negative. The literature on contact dermatitis reactions to corticosteroids is reviewed.

Pseudo-allergen-free diet in chronic urticaria.

An elimination diet for additives and tyramine was prescribed to 67 patients with chronic urticaria. 55% of them reacted favorably. Success of dietary treatment was associated with atopy and normal levels of biochemical markers of inflammation.
