The synthesis of two protected large peptides 1--45, and 46--86 covering the entire amino acid sequence of human proinsulin is described. Peptide 1--45, was synthesized from two intermediate fragments 1--23 and 24--45 and purified by countercurrent distribution in dimethylformamide system (K = 0.5). Peptide 46--86 was synthesized from two intermediate fragments 46--70 and 71--86 and purified by countercurrent distribution in two solvent systems, the dimethylformamide system (K = 0.06), and the toluene system (K = 0.16).
A randomized clinical trial on adjuvant treatment of operable breast cancer in female aged between 35 and 70 years was performed from 1962 to 1972. Criteria for admission into the trial were: unequivocal clinical evidence of breast cancer stage T1-3 NC-3; no general or technical contraindications against standard radical mastectomy type Rotter-Halsted; no contraindications against radiation or cytostatic treatment: 435 patients entered the trial and 372 patients were evaluable. Of these 126 were treated by surgery, 126 by surgery followed by cyclophosphamide (100 mg/kg BW in daily intravenous doses of 200 mg), and 120 by preoperative radiation therapy (about 5500 rad tumour dose by telecobalt) and surgery. The crude 5-year survival rate was 58.7 percent, 57.1 percent, and 57.5 percent respectively. The number of local recurrences and local node metastases was 29.4% after surgery, 23.0% after surgery combined with chemotherapy, and 12,5% after surgery combined with preoperative radiation therapy. This significant reduction in locoregional recurrences was most impressive in the supraclavicular node region. In the irradiated group, the mean diameter of tumours in the surgical specimen was obviously smaller and in 11 cases the tumour in the breast was completely destroyed.

By means of Eysenck's Maudsley Personality Inventory (MPI), the personality dimensions neuroticism and extraversion were estimated in 1148 control probands (m. 195; f. 953), in 375 patients with x-ray negative dyspepsia (m. 233; f. 142), in 77 males with duodenal ulcer, 42 patients with gastric ulcer (m. 25; f. 17), and 27 patients with gastric cancer (m. 18; f. 9). In x-ray negative dyspepsia and in duodenal ulcer, slightly higher scores for neuroticism and lower scores for extraversion than in controls were found whereas patients with stomach cancer and gastric ulcer are characterized by high emotional stability. It is assumed that knowledge of the patient's personality structure may improve the physician-patient relationship.
Bladder cancer and age. A study on the morbidity and on the state of detection and treatment with respect to age in the G.D.R. Malignant neoplasms of the urinary bladder account for about 1,400 registered new cancer cases and 1,100 deaths annually in the G.D.R. As compared with other developed industrialized countries, incidence and mortality from bladder cancer is relatively low in the G.D.R. Analysis of age-standardized incidence and mortality from 1956 to 1974 gives no clearcut evidence of an increase of bladder cancer morbidity. A regional analysis of the state of detection and treatment of bladder cancer was performed in the capital Berlin (1955-1974) and the city of Dresden (1969/70). It can be shown that there is a decrease of localized stages, of radically operated tumours and of survival rates with increasing age at time of diagnosis. In Berlin more than 30 per cent of new cases are 75 years and older. Localized stages I and II amount to nearly 50 per cent of newly registered bladder cancer cases. While mean age of patients increased in the observation period and stage distribution remained about equal, rate of radical interventions showed a slight increase reflecting a more aggressive approach to treatment. Higher survival rates can be expected in the future.
In 1148 control persons and 498 cancer patients, the Maudsley Personality Inventory (MPI), developed by H. J. Eysenck, was used to determine the personality dimensions neuroticism (N) and extraversion (E). The results in the control group were similar to studies in other countries showing that the MPI is a useful instrument for psychometry in the G.D.R., too. Personality scores depend upon sex, age and social group (defined by education and occupation). Cancer patients generally do not differ from non-cancerous patients (so-called "controls"). In patients with breast cancer (female N = 231) or lung cancer (male N = 123), it was found that patients consulting the doctor without delay after onset of symptoms (breast cancer < 4 weeks; lung cancer < 8 weeks) are characterized by low scores for neuroticism (N) and high scores for extraversion (E). The combination of low N (--12) and high E (13 +) is significantly more prevalent in patients seeking medical advice without delay. It is suggested that these observations which are in agreement with reports from other countries and observers should be used in planning health education. Obviously it is not enough to disseminate factual information on cancer but it seems necessary to develop means and methods to reach people with low education, high scores for neuroticism and for introversion in order to influence their health behavior by personal discussion.

Based on data from the cancer register of the German Democratic Republic established in 1952 and on the official mortality statistics, incidence of and mortality from malignant lymphomas (ICD 200-203) in the GDR are analysed. Age-specific incidence and mortality of Hodgkin's disease show a peak in the age group of 25-30 years and rise steadily from 45 years on up to the highest age. Lymphosarcoma and reticulosarcoma increase slowly from infancy to old age, whereas multiple myeloma is a disease of the elderly and extremely rare before the age of 40. The apparent increase of malignant lymphoma may be due to underregistration at the beginning of the cancer register. In the past years mortality from Hodgkin's disease is slowly decreasing, thus reflecting progress in methods of treatment and results.
Concerning the stomach biopsy without optical control has only historical importance, concerning the small intestine it serves for the recognition of diffuse changes--particularly for the the differential diagnosis of the malabsorption and in the large intestine for the diagnosis and course control of inflammation. The aimed biopsy is a constituent of every endoscopy investigation, with the exception of completely normal or fully clear pathological findings. Without an aimed biopsy the diagnosis of the early cancer in ulcers, "polyps" and "ulcer scars" is not possible. Its gain depends on the number of biopsy particles; only the positive findings are proving.

About 1% of all malignant neoplasms of the mammary gland affect the male sex. The diagnosis is simple if a general physical examination is carried out independent of the reason for the consultation. The occurrence of mammary carcinoma is more frequent in males with primary and secondary hypogonadism and after estrogen therapy. The results of operative treatment are the same as for mammary carcinoma in women, i. e. about 45% of the patients survive the five-year limit. For metastasizing mammary carcinoma orchidectomy is recommended, for a fresh relapse, energetic estrogen therapy.
Guinea pigs were exposed to pure tone noise (2.7 kHz, 130 dB, 1 h) and cochlear microphonic potentials were measured 24 h after exposure. There is the possibility to modify the resulting noise-induced cochlea damage by regulating the function of the thyroid gland to alter the rate of metabolism. A hypofunction of the thyroid gland during sound exposure lessens, an over-function aggravates the damage. After gradual adaptation of the animals to a simulated 10,000 m altitude, the electrophysiologically demonstratable noise-induced damage was reduced. This might be explained by the greater hypoxia tolerance and perhaps additional better oxygen supply to the receptor cells.

The influence of mammography upon stage classification according to the TNM system is evaluated in 590 breast cancer patients. By using tumour size measured on the radiograph instead of the tumour diameter determined by palpation it was possible to reclassify a significant proportion of cases in stage III. As much as 23% (52/228) of cases of stage III could correctly be grouped into stage II or I. The prognostic significance of this change is evident from the 5-year survival rates. The cases, removed from stage III by correction of the tumour size, show the same 5-year survival rates (65%) as cases primarily classified into stage I (64%) and II (63%). In view of these results we do recommend the use of mammography for stage classification, because it allows correction with prognostic significance.
