Quantum-chemical and physico-chemical properties of hydrochlorothiazide.

The electronic states of hydrochlorothiazide and its related molecules were obtained by CNDO/2 (CNDO = complete neglect of differential overlap), and van der Waals volume and hydrophobic parameters of the substituent of the 6th position in the benzothiadiazine were estimated. The results were discussed from the viewpoint of the structure-activity relationship analysis. Rather lower LUMO (lowest unoccupied molecular orbital level) of hydrochlorothiazide which had been predicted by the iterated Hückel's Molecular Orbital Method was confirmed by CNDO/2 calculation. The introduction of the sulfamoyl group to the 7th position in the benzothiadiazine ring brought out a negative formal charge for the 7th position. The diuretic effect of the substituent of the 6th position in the benzothiadiazine ring was analysed with respect to its van der Waals volume and its hydrophobic parameter. Van der Waals volume seemed to have a close relationship to diuretic activity. The highest correlated coefficient of the regression equation for the structure-activity relationship was obtained using the formal charge of the 7th position in the benzothiadiazine ring, van der Waals volume and the hydrophobic parameter of the substituent of the 6th position. On the basis of result obtained, the model for the action site of hydrochlorothiazide was proposed, consisting of a rather larger, lipophilic hole and an electrostatic interaction site in the tubular membrane.

[Establishment and characterization of human ovarian endometrioid carcinoma cell line].

Ascitic fluid was obtained from the patient of ovarian endometrioid cancer. Collected cells were incubated with Eagle's MEM containing 15% fetal calf serum at 37 degrees C under humidified 5% CO2 and 95% air. The epithelial colonies grew rapidly and were released without fibroblast cells. After the first passage, the cells are growing without interruption for over one year and 35 transfer generations. This cell line has following charactors: 1) The monolayer cultured cells appeared epithelial, pavement like arrangement and piling up, without contact inhibition. 2) In the cytoplasma PAS positive substance can be seen. 3) Desmosome-like structure, gap junction, microvilli and well developed cell organelles can be found by electron microscopy. 4) Chromosomal number shows pseudodiploidy which mode is 47. A submetacentric chromosome was present in all karyotype and identified by G-banding. It consists of No. 11 and a part of No. 1. 5) By heterotransplantation to the nude athymic mouse, the tumor easily develops. 6) By estradiol and promegestone its growth was inhibited. 7) Estrogen and progesterone receptors were not detected in the cytosol.

[Treatment of carotid-cavernous fistula using a detachable balloon catheter--a case report and review].

A case of post-traumatic carotid cavernous fistula, successfully treated by a detachable balloon catheter, is reported. A 55-year-old housewife was admitted to the author's department on 25th August, 1980. Three months prior to admission, she complained of right tinnitus following a traffic accident and then developed right chemosis and exophthalmus. A selective right internal carotid angiography revealed rapid filling of the carotid cavernous fistula. Under general anesthesia, the patient was treated by the maneuver after Debrun. Since it appeared necessary to employ the second balloon, the first one was released gently being inflated only with the contrast medium. During the procedure, the neck of the balloon was incidentally snapped upward and, then, the fistula was eventually occluded preserving the carotid flow as was verified by a control angiography. The bruit was abolished immediately after the procedure and all ocular symptoms disappeared during the following a few weeks. A skull film showed a deflation of the balloon at 4 weeks postoperatively, but clinical and angiographic follow-up examinations at 9 months thereafter revealed no signs or symptoms of recurrence. This Debrun's procedure is being widely accepted for a carotid cavernous fistula as a relatively simpler, safer, and more effective one comparing with the conventional methods. Our experiment on dogs, however, indicated that rupture of the balloon is not always avoidable even with a smaller volume than its maximum capacity and some resistance was felt during introduction of the co-axial catheter. Thus, it appeared mandatory to improve and quality control the balloon and the catheter. In the case presented here, a follow-up angiography showed no recurrence of the fistula except for an asymptomatic pseud-aneurysmal pouch, even though the balloon was inflated only with the contrast medium and was deflated within 4 weeks. These findings suggest possibility of successful, and long-lasting outcome without employing silicone.

Gastric mucosal blood distribution and its changes in the healing process of gastric ulcer.

Changes in gastric mucosal blood volume in the healing process of ulcers were investigated by organ reflectance spectrophotometry performed during endoscopy. Spectra were taken from 24 regions in the stomach in 42 patients with gastric ulcers located at the angle of the stomach. Fifty-one patients with no gastric lesions were used as normal controls. In patients with active gastric ulcers, mucosal blood volume decreased significantly in most regions in the stomach. During the healing process, mucosal blood volume returned to the same levels as those of the controls. On the other hand, mucosal blood volume around the gastric ulcer increased significantly as compared with that of surrounding mucosal at the healing stage. The rise in mucosal blood volume of the ulcer margin at the healing stage (H2 stage) was 33% in cases where gastric ulcers healed within 3 mo, and 2% in cases where the gastric ulcer failed to heal after 3 mo of therapy. It was concluded that the mucosal blood volume changes during the healing process of gastric ulcer and that an increased blood supply to the ulcer margin at the healing stage has a strong effect on the healing periods of ulcer. The reduction of mucosal blood volume at the active stage of the ulcer may also be implicated in the pathogenesis of gastric ulcers.

Wearable artificial endocrine pancrease with needle-type glucose sensor.

Miniaturisation of the bedside artificial endocrine pancreas in necessary to provide a means of restoring physiological glycaemic excursions in diabetic patients in the long term. One of the remaining problems in producing such a sophisticated device is the difficulty in developing a sufficiently small glucose-monitoring system. A needle-type glucose sensor has been developed which is suitable for use in a closed-loop glycaemic control system. The wearable artificial endocrine pancreas, incorporating the needle-type glucose sensor, a computer calculating infusion rates of insulin, glucagon, or both, and infusion pumps, was tested in pancreatectomised dogs: the device produced perfect control of blood glucose for up to 7 days.

Platelet hyperaggregability in ischemic cerebrovascular disease and effects of aspirin.

Platelet aggregation was studied in 24 patients in the chronic stage of ischemic cerebrovascular disease (CVD), with cerebral affluent and effluent blood, i.e., carotid arterial and internal jugular venous blood, and also with peripheral venous blood. Aggregation tests were performed at various final concentrations of sodium arachidonate (A.A.) and ADP. In 17 patients, not taking aspirin, platelet aggregability in jugular venous blood was significantly accentuated compared with that in arterial and peripheral venous blood. This tendency was more marked in the patients with cerebral artery stenosis and/or occlusion than in those with normal cerebral angiogram. In 7 patients taking 500 mg or more oral aspirin, aggregation differences across the brain were not observed and A.A. aggregation and the second phase of ADP aggregation were completely suppressed. These results suggest that a prophylactic administration of aspirin may be beneficial for patients in chronic stage of CVD.

Protective effect of saikosaponin-d isolated from Bupleurum falcatum L. on CCl4-induced liver injury in the rat.

The effects of saikosaponin-d extracted from the roots of Bupleurum falcatum L. on carbon tetrachloride-induced hepatic injury were studied in rats. Pretreatment with saikosaponin-d produced a remarkable inhibitory action on acute hepatic injury by CCl4. A significant inhibition of lipid peroxidation induced by an acute dose of CCl4 in the liver of rats pre-treated with saikosaponin-d was also noted. Continuous injection of CCl4 caused liver cirrhosis in rats but the severity of cirrhosis was reduced in rats treated simultaneously with CCl4 and saikosaponin-d.

Substance P decreases membrane conductance in neurons of the guinea pig hypothalamus in vitro.

Substance P (SP) depolarized the membrane and increased the spontaneous firing rate of neurons in slices of the guinea pig hypothalamus. These excitatory actions of SP were accompanied by a decrease in membrane conductance, and this reduction was observed even during a perfusion of the medium which was Ca2+ free and in which the concentration of Mg/+ had been increased to 12 mM. Therefore, the contribution of possible presynaptic or indirect effects of SP to the decreased membrane conductance can be ruled out. These observations suggest that SP exerts an excitatory action on neurons in the guinea pig hypothalamus by decreasing the membrane ionic permeability of these neurons.
