Closed-loop glycemic control with a wearable artificial endocrine pancreas. Variations in daily insulin requirements to glycemic response.

We succeeded in miniaturizing a needle-type glucose monitoring system with characteristics suitable for application in a wearable, closed-loop control system. A wearable artificial endocrine pancreas (12 X 15 X 6 cm, 400 g) consisting of a sensor, a microcomputer system that calculates insulin and glucagon infusion rates, and two roller pumps was developed. Continuous glucose monitoring by a glucose sensor inserted in the subcutaneous tissue of the forearm or abdomen of healthy and diabetic volunteers revealed that glucose concentrations in subcutaneous tissue were 10% lower than, but were highly correlated with, blood glucose concentrations in the range of 49-388 mg/dl. Glycemic control was established in diabetic patients by intravenously infusing insulin in response to measured glucose concentrations on a moment-to-moment basis for a period of several days. By comparing the glycemic control obtained in each patient treated with multiple insulin injections or open-loop subcutaneous insulin infusion, the superiority of feedback control with the system was clearly demonstrated. During continuous glycemic regulation, day-to-day variations of insulin requirements were recognized in both basal insulin infusion and postprandial insulin infusion rates in response to identical meals and exercise. These data suggest the feasibility of long-term glycemic control in diabetic subjects with a wearable artificial endocrine pancreas, and indicate that to overcome changes in individual metabolic characteristics on a moment-to-moment basis, a closed-loop glycemic control system may be essential for ambulatory diabetic patients.

[Carcinoma of urachus: report of 3 cases and review of literature].

Three cases of urachal carcinoma are presented. Chief complaints were passage of mucous urine in a 58-year-old man (case 1), hematuria and frequency in a 63-year-old woman (case 2) and hematuria in a 45-year-old man (case 3). Urine cytology were negative for all cases and serum CEA level was elevated in case 1. Mass in the area of the bladder dome were revealed by cystoscopic examination in all cases. CT scanning, TUR biopsy and cystogram were valuable diagnostic procedure. En bloc segmental resection were performed on all cases, and case 1 and case 2 have been well without disease for 36 and 40 months, respectively. Case 3 died 65 months after operation with disseminated carcinoma. Histologically mucin-producing adenocarcinoma were found in all cases. Statistic examination and discussion are made of 237 cases of urachal carcinoma reported in Japan.

Antitumor activity of deoxyribonucleic acid fraction from Mycobacterium bovis BCG. I. Isolation, physicochemical characterization, and antitumor activity.

A fraction extracted from Mycobacterium bovis strain BCG, which was composed of 70.0% DNA, 28.0% RNA, 1.3% protein, 0.20% glucose, and 0.1% lipid and of no detectable amounts of cell wall components such as alpha, epsilon-diaminopimelic acid and hexosamine, was found to possess strong antitumor activity. Repeated intralesional injection of this fraction, designated MY-1, without attachment to oil or a single intralesional injection of MY-1 emulsified in mineral oil caused the IMC carcinoma of CDF1 mice and line 10 tumor of strain 2 guinea pigs to regress and/or prevented metastasis very effectively. MY-1 after digestion with RNase, which contained 97.0% single-stranded DNA with a guanine-cytosine content of 69.8%, was more effective than undigested MY-1 against IMC and line 10 tumor, while MY-1 digested with DNase, which contained 97.0% RNA, had reduced activity, suggesting that the DNA from BCG possessed strong antitumor activity under certain conditions. Details of the extraction procedures and physicochemical characterization of MY-1 were also described.

[A case of renal carcinoma with bone metastasis, producing alpha-fetoprotein].

A 54-year-old man was admitted with the complaint of severe lumbago, pain in the right shoulder and difficulty in walking. Laboratory tests showed markedly increased alpha-fetoprotein (AFP) level (600 ng/ml) and tomography demonstrated osteolytic lesion of the 5th lumbar vertebra and right scapula. Needle biopsy of the 5th lumbar vertebra revealed only necrotic tissue pathologically. Liver scan and whole body CT did not show any abnormality except for a right renal mass. Hypervascular tumor at the lower pole of the right kidney was noted by IVP and selective renal angiography. No pulmonary metastasis was found. The patient underwent transabdominal right nephrectomy. He had a lowered level of AFP (300 ng/ml) on the 17th post operative day. AFP level dropped further to 100 ng/ml after administration of vinblastine and ifosfamide. Lumbago continued without improving, and the patient died at home 20 weeks after surgery due to general weakness. Although we did not perform radiological examination of gastro-intestinal tract or autopsy, we concluded that renal cancer and/or a metastatic lesion to bone produced AFP because of the following reasons. No other tumor was detected by liver scan, whole body CT, laboratory examinations, operation or clinical symptoms. The AFP level was lowered after nephrectomy, and further dropped with chemotherapy. A case of renal cancer with bone metastasis and without liver metastasis, producing AFP was reported in the Japanese literature in 1983.

Platelet accumulation in carotid atherosclerotic lesions: semiquantitative analysis with indium-111 platelets and technetium-99m human serum albumin.

To evaluate platelet accumulation in carotid atherosclerotic lesions semiquantitatively, a dual-tracer technique was applied, using In-111 platelets and Tc-99m human serum albumin. With this approach, we investigated the ratio of radioactivity in In-111 platelets deposited on the vascular wall to those circulating in the blood pool, platelet accumulation index ( PAI ). This study included 12 normal subjects and 25 patients with ischemic cerebrovascular disease (CVD). Angiographic abnormalities were observed at 34 of 50 carotid bifurcations in the CVD patients. The mean PAI value was significantly higher at the carotid bifurcations with angiographic abnormality than at the normal ones (p less than 0.001). Furthermore, elevations of mean PAI were prominent at the lesions with severe stenosis or ulceration. The degree of platelet accumulation was well demonstrated by this technique, which can also yield information on thrombogenicity and efficiency of antiplatelet therapy in carotid atherosclerotic disease.

[Case of acoustic neurinoma with intratumoral hemorrhage extending to cerebellar hemisphere].

A case of acoustic neurinoma which developed intratumoral hemorrhage extending to the right cerebellar hemisphere, is reported. A 54-year-old man was diagnosed as cerebello-pontine angle tumor, possibly acoustic neurinoma, from typical clinical course, neurological signs and findings of skull X-P and CT scan at the first visit to our out-patient clinic on July 28, 1981. On August 6, 1981, he was transferred to our in-patient ward in emergency because of sudden onset of headache and impairment of consciousness. At emergency admission, he was found to be comatous and CT scan revealed round-shaped high density in the right cerebellar hemisphere, continuing from the right cerebello-pontine angle tumor with enlarged lateral ventricles, which indicated occurrence of intratumoral hemorrhage. At emergency operation, hematoma in the right cerebellar hemisphere was evacuated and the tumor in the right cerebello-pontine angle was partially resected. Microscopically, the tumor was composed of compact, finely fibrillated cells of Antoni A tissues, and loose patterns of Antoni B tissue, being diagnosed as schwannoma, and also abnormal vascular proliferations were identified just adjacent to foci of hemorrhage noted within tumor tissues. The pattern of abnormal vascular proliferations within this tumor were almost identical with telangiectasia. The authors concluded that intratumoral hemorrhage in this case occurred most likely due to disruption of histologically verified abnormal vascular proliferations.

[A case of a congenital unilateral multicystic kidney and a review of 126 cases in Japan].

A 3-year-old boy was admitted to our hospital with complaints of left flank mass and growth retardation. Examinations with IVP (Fig. 1), retrograde pyelography (Fig. 4) and cystoscopic examination (Fig. 3) showed a left nonopacified kidney, and right hydroureteronephrosis due to stenosis of the midureter. Two large cystic patterns were found in the left kidney by ultrasonic scanning tomography (Fig. 2); and, the left kidney was found to be occupied by a round and homogeneously low density mass by CT scanning (Fig. 6). The left renal artery was not demonstrated on the aortogram (Fig. 5). The patient was diagnosed to have a left congenital unilateral multicystic kidney, and laparotomy was performed. The left kidney was easily removed, and the part with stenosis in the right ureter was removed, then end-to-end anastomosis was performed. Grossly, two large cysts (upper and lower part) and connective tissues were found in the left kidney (Fig. 7), and the vessels of the renal pedicle were filiform. No luminal formation of the renal pelvis or ureter was found. The epithelium of the cyst wall (Fig. 8) was deciduated leaving the connective tissue and smooth muscle, as shown by the histological examination. Histological examination of the parenchymal tissue (Fig. 9) between the two cysts showed primitive glomeruli and renal tubules among abundant connective tissue, but no cartilage tissue. Statistic examination and discussion have been made of 126 cases of congenital multicystic kidney reported in Japan.

The development of wearable-type artificial endocrine pancreas and its usefulness in glycaemic control of human diabetes mellitus.

We have succeeded in miniaturizing a glucose monitoring system into a needle-type which preserves the characteristics suitable for application in a wearable closed-loop control system. The wearable artificial endocrine pancreas (12 X 15 X 6 cm, 400 g) consists of the sensor, a microcomputer which calculates the insulin and glucagon infusion rates and of a 2-syringe driving system. The device succeeded in controlling glycaemia perfectly in depancreatized dogs for a period of 3 days, and with replacement of subcutaneously inserted sensor for up to 7 days. When glucose monitoring by a needle-type glucose sensor inserted into subcutaneous tissue of the forearm or abdomen of healthy and diabetic volunteers was attempted, the readings were about 10% lower than blood glucose concentrations, but good correlation between them was observed in the range of 60 to 400 mg/dl. Perfect glycaemic control was established by infusing insulin either intravenously or subcutaneously in response to measured glucose concentrations on a moment-to-moment basis in diabetics for a period of several days. These results might indicate the feasibility of long-term glycaemic control in ambulatory diabetic patients with a wearable closed-loop glycaemic control system.
