[Transcallosal approach to para-ventricular tumors].

Our experience with the transcallosal approach to para-ventricular tumors is reviewed and our operative technique is described. A group of 7 patients with a variety of tumors affecting the lateral ventricle, 3rd ventricle and basal ganglia have been treated using an anterior transcallosal approach. Using a posterior transcallosal approach, we operated upon 2 patients with thalamic tumors. The anterior part of the body of the corpus callosum is divided in the anterior transcallosal approach and the posterior part of the body of the corpus callosum is divided with preservation of the splenium in the posterior transcallosal approach. The division amounting to approximately 2-3 cm is sufficient exposure. With the exception of tumors affecting the basal ganglia, thalamus and anterior 3rd ventricle, complete excision of each lesion was effected. There were no operative deaths. The complications in this group of patients were primarily related to the nature and location of the primary tumor, but four complications were directly related to the transcallosal approach. Venous infarctions occurred in 2 patients and subdural fluid collections occurred in 2 patients. Therefore, planning of the flap placement and the extent of brain retraction must be based on preoperative angiographic assessment of parasagittal venous tributaries. We performed some detailed studies of the interhemispheric transfer of somesthetic and perceptual motor tasks, as well as psychometric testing post-operatively in 2 patients. No significant clinical deficit subsequent to dividing the anterior part of the body of the corpus callosum could be demonstrated. The results and clinical material indicate that transcallosal approach is a safe, feasible alternative in the management of the tumors in these regions.

[Direct spinal cord and root injury caused by acupuncture--report of 2 cases].

Two cases of the direct spinal cord and root injury caused by acupuncture were presented with a review of eight cases reported previously. Case 1 was 56 years old female who had been developed progressive lumbar pain, which occasionally radiating to the right lateral femoral region, since she had been treated with acupuncture for lumbago 13 months before. Examination revealed the mild sensory impairment on the right lateral femoral area and straight-leg-raising was limited on the both sides. Plain films and myelogram showed the needle between L4/L5 vertebrae and the tip of it stuck in the lumbar root. She recovered after the needle had been removed. Case 2 was 17 years old male who had initially noticed numbness over his left foot after acupuncture for easy fatiguability, which had been progressed stepwise over 2 years, and then examination revealed Brown-Séquard syndrome. Radiological examination showed the needle stuck in the spinal cord between C1/C2 vertebrae and metrizamide CT myelogram disclosed the posterior end of it was in the subarachnoid space. Operative finding revealed a small cavity around the needle in the spinal cord. The needle was removed uneventfully. There was a good recovery of weakness but not of sensation. Although the direct spinal cord and root injury as a complication of acupuncture seems uncommon, our cases show one of a serious complication of acupuncture.

[Effect of PS-K on experimental neurogenic tumors in rats induced by ethylnitrosourea].

The effect of PS-K as immunomodulator was studied in Fischer rats bearing neurogenic tumors induced transplacentally by ENU. 114 rats were divided into control and PS-K treated groups. all rats were subjected to an autopsy at death and all of central nervous system were examined macroscopically. Final neurogenic tumor incidence in PS-K treated groups was significantly lower than that in the control group, but distribution of these neurogenic tumors and number of neurogenic tumors in a rat remained unchanged between the control and the each PS-K treated groups. Survival time of PS-K treated groups did not increase than the control group. From these experiments, it might be that induction of neurogenic tumors may be suppressed by administration of PS-K.

[The analysis of left ventricular filling dynamics by radionuclide ventriculography using ECG's R wave and the second heart sound gating techniques].

To assess the impairment of early diastolic left ventricular (LV) filling and the effect of atrial contraction on total LV filling in patients (pts) with coronary artery disease (CAD), LV volume (LVV) changes during rapid filling (RF) and atrial contraction (AC) phases were studied by equilibrium radionuclide ventriculography in 10 normals (N) and 17 pts with CAD including eight without (CAD-1) and nine with (CAD-2) previous myocardial infarction (MI). The data were acquired in a list-mode fashion as a series of X, Y coordinates, time markers and an ECG's R wave (R) plus the second heart sound (S2) markers. LVV curves were obtained from three types of multi-gated images by (1) R-synchronized forward reformatting for the analysis of systolic phase (ejection fraction; EF and peak ejection rate; PER), (2) S2-synchronized forward reformatting for the analysis of RF phase (peak filling rate; PFR-RF and filling fraction; FF) and (3) R-synchronized backward reformatting for the analysis of AC phase (peak filling rate; PFR-AC and LVV increment with atrial contraction/stroke volume; AC/SV). EF and PER were significantly lower (p less than 0.001) in CAD-2 (36.0 +/- 8.1% and -1.5 +/- 0.4 EDV/sec) than in N (58.2 +/- 5.8% and -2.4 +/- 0.4 EDV/sec), but those in CAD-1 (52.7 +/- 6.4% and -2.2 +/- 0.3 EDV/sec) were almost the same as N. However PFR-RF and FF were reduced both in CAD-1 (1.5 +/- 0.4 EDV/sec and 23.0 +/- 7.0%) and CAD-2 (1.3 +/- 0.4 EDV/sec and 19.0 +/- 8.0%) compared with N (2.3 +/- 0.4 EDV/sec and 37.0 +/- 8.3%). PFR-AC and AC/SV were significantly greater (p less than 0.01) in CAD-1 (1.3 +/- 0.3 EDV/sec and 0.30 +/- 0.06) than N (0.8 +/- 0.3 EDV/sec and 0.13 +/- 0.07), but those in CAD-2 (0.6 +/- 0.4 EDV/sec and 0.18 +/- 0.09) showed no increase like in CAD-1. These results indicate that 1) the impairment of early diastolic filling precedes the evidence of systolic dysfunction in pts with CAD and 2) AC could compensate the impaired early filling in pts without prior MI, whereas in pts with prior MI this compensatory mechanism is absent presumably because of elevated LV filling pressure.

[Efficacy of nidus embolization of large arterio-venous malformation prior to surgical excision].

Although it is undoubted that surgical excision is the ideal treatment for arterio-venous malformation, its large nidus and deep localization make it difficult in some instances. To make surgical procedure easier, efficacy of nidus embolization or of obstruction of feeding arteries by detachable balloon technique is reported. A case presented here is such a case with large arterio-venous malformation which was successfully removed surgically following artificial nidus embolization with silastic spheres. On May 12th, 1983, a 27-year-old housewife was admitted to our department with chief complaints of episodes of alexia, apraxia, and aphasia lasting for a few minutes during 4 years prior to admission. Neurological examination on admission showed no abnormalities. Left internal carotid angiogram taken on admission showed a large arterio-venous malformation in the left parieto-temporo-occipital region fed by three branches of the left middle cerebral artery and two branches of the left posterior cerebral artery (Fig. 2a, 3a). To enable successful excision without neurological deficits, nidus embolization with silastic spheres was performed on June 2, 1983. Forty silastic spheres of 1.5 mm in diameter were injected via a catheter introduced in the left internal carotid artery and a second injection of 48 silastic spheres of 2.0 mm in diameter was performed after confirmation of no stray of silastic spheres in the arteries unrelated to the nidus. Embolization procedure was interrupted after the second injection, because of stray of silastic sphere into the posterior internal frontal artery of the left anterior cerebral artery.(ABSTRACT TRUNCATED AT 250 WORDS)

[Ectopic ureterocele: a case report].

A case of ectopic ureterocele is reported. A 2-year-old girl was admitted for recurrent episode of urinary tract infection associated with fever. IVP showed bilateral duplex kidney, although no excretion of dye was noted from the right upper kidney. IVP also revealed a large filling defect at the bladder neck which was diagnosed as ureterocele by cystoscopy. Cystography demonstrated VUR to the right lower kidney. A complete duplication of ureter with ectopic ureterocele on the right, and incomplete duplication of ureter on the left were found at operation. She underwent complete removal of the ectopic ureterocele and reimplantation of the right two ureters. Her postoperative course was uneventful, post-operative IVP revealed improvement of pyelography of the right lower kidney and cystography revealed no VUR.

[A case of normal pressure hydrocephalus associated with cerebrovascular disorders].

There has been much argument in recent years about the diagnostic and pathogenetic differences between normal pressure hydrocephalus (NPH) and vascular dementia (Multi-infarct dementia) or Binswanger's disease. We experienced an interesting patient, a 54-year-old male, who had left hemiparesis due to two past attacks of cerebral infarction and possible hydrocephalus had been pointed out by CT findings in 1981. He was seen on May 22, 1982, with the chief complaints of dementia, gait disturbance and urinary incontinence. He had the clinical and pathogenetic features, common to NPH and MID or Binswanger's disease. The followings were observed. These symptoms (dementia, gait disturbance, urinary incontinence) and computed tomographic findings (bilateral ventricular dilatation, periventricular lucency), as is often the case in NPH, are at times similar to those of MID or Binswanger's disease and it is difficult to distinguish NPH from MID or Binswanger's disease. We consider that there are probably many patients who are on the borderline of these three diseases. For treatment, the V-P shunt was very effective in combination with the preceding bilateral STA-MCA shunt but it is not clear whether the preceding STA-MCA shunt was required before the V-P shunt. We believe that the clinical symptoms will not appear until the ventricle is dilated to a critical degree. There was a correlation between the degree of dementia and Huckman number, one of the assessment methods for ventricular dilatation.

Effects of 3,4-dihydro-6-[4-(3,4-dimethoxybenzoyl)-1-piperazinyl] -2(1H)-quinolinone (OPC-8212), a new inotropic agent, on cardiotonic activity and coronary hemodynamics.

Cardiotonic activity and chronotropic effect of a new positive inotropic agent, 3,4-dihydro-6-[4-(3,4- dimethoxybenzoyl )-1-piperazinyl]-2(1H)- qu inolinone ( OPC -8212), were studied in isolated canine hearts eliminating the secondary effect due to neurohumoral alterations. Positive inotropic action of OPC -8212 was dose-dependent and its potency was equivalent to about 1/10(5) of isoprenaline (isoproterenol). However, the duration of action was two to three times longer than that of isoprenaline, and arrhythmogenic activity was not observed at any dose studied. An index of inotropic state, Emax, was increased by this agent even under the blockade of beta-adrenoceptors, indicating that the positive inotropic effect of this agent is not due to a stimulation of beta-adrenoceptors. the ventricular relaxation rate was enhanced dose-dependently in similar extent to the equivalent dose of isoprenaline. Effects of this agent on coronary blood flow (CBF) and myocardial oxygen consumption (MVO2) were also assessed in open chest dogs, eliminating secondary increases in these parameters due to an increase in mechanical load or heart rate. OPC -8212 increased MVO2 significantly even without significant alterations in heart rate and LV pressure. These increases both in CBF and MVO2 were similar to those of isoprenaline while arterio-venous difference in oxygen content was not affected by OPC -8212. Thus, we conclude that the efficiency of myocardial oxygenation of OPC -8212 is similar to that of isoprenaline and a direct coronary vasodilatory effect of this agent is minimal.

[Treatment of advanced prostatic carcinoma with cis-diamminedichloroplatinum].

Chemotherapeutic effects of CDDP used as the main drug were studied in 20 patients with progressive prostatic cancer in stage C or D. On the average 208 mg of CDDP was given to the patients receiving chemotherapy without antiandrogen therapy (13 patients who showed resistance to hormone and an untreated new patient) and both ADM and IFM were also given to 3 of them. According to the criterion proposed by Shida and his coworkers, the chemotherapy without antiandrogen therapy was effective in 2 cases, relatively effective in 7 cases, and ineffective in 5 cases. The chemotherapy was effective for metastatic tumors of the lung in 2 out of 2 cases, but had no effect on tumors of the lymph node (1 case) and primary lesion of the tumors (14 cases). The chemotherapy improved acid phosphatase values in 5 out of 10 cases, alkaline phosphatase values in 3 out of 10 cases, dysuria in 4 out of 8 cases, nocturia in 1 out of 12 cases, residual urine in 5 out of 6 cases, lumbago in 6 out of 8 cases, and constitutional symptom in 6 out of 12 cases. The effect of the chemotherapy in combination with antiandrogen therapy was excellent in 4 and good in 2 of the 6 patients treated with castration + diethylstilbestrol diphosphate + CDDP + ADM +/- IFM. The chemotherapy with antiandrogen therapy had no effect on metastatic tumors of the bone (2 cases), but decreased the hardness and size of primary lesion in 6 out of 6 cases. Urethrography showed better changes in 6 out of 6 cases.(ABSTRACT TRUNCATED AT 250 WORDS)

[The age incidence of spontaneous abortion].

A series of studies on the age incidence of spontaneous abortion was conducted. The results of the studies is summarized as follows: A total of 16,179 deliveries accompanied by 1,537 cases of spontaneous abortion in 5 hospitals was investigated. The ratio (spontaneous abortion/deliveries) was 0.095 (1,537/16,179) in all age groups, 0.197 (15/76) in the under 20 years age group, 0.089 (215/2,422) in 20-24 years age group, 0.083 (705/8,478) in 25-29 years age group, 0.090 (404/4,496) in 30-34 years age group, 0.232 (149/641) in 35-39 years age group, 0.650 (39/60) in 40-44 years age group and 1.169 (7/6) in the over 45 years age group. The ratio in the under 20 years age group was higher than the ratio in 20-34 years age group (P less than 0.05). The ratio in the over 35 years age group was higher than the ratio in 20-24 years age group (P less than 0.001). This results indicated that spontaneous abortions depended on the maternal age. Deliveries in 20-34 years old occupied 95.2% of all 16,179 deliveries and the incidence of spontaneous abortions in 20-34 age group was stable. It is suggested, that the spontaneous abortion is natural selection in the mother's body and nature protects the older mother's body from deliveries and child-rearing.

[The diagnosis and pathogenetic study of syringomyelia].

Thirteen cases of syringomyelia which was confirmed by metrizamide CT myelography (MCTM) were reported. Nine cases were associated with Chiari malformation, and four were idiopathic type. Delayed CTM which were performed in several hours after the metrizamide myelography disclosed not only the existence of the syrinx but its shape and location. From the investigation of their clinical findings together with the appearance of their syrinx on CT scan, the following results were obtained. The syrinx was not always situated centrally within the cord, but had laterality at some spinal level in nine out of thirteen cases. The main laterality of the syrinx seemed to be located in the posterior horn. In the cases which had unilateral dissociated sensory disturbance, the laterality of sensory disturbance corresponded with the side of syrinx. These results suggested the possibility that the intrathecal injected contrast materials entered into the syrinx via the dorsolateral sulcus of the cord rather than via IV ventricle in some cases.
