[The localization of parvovirus B19 in cardiac biopsy tissue of congenital heart disease].

OBJECTIVE: To explore the relationship between parvovirus B19 infection and congenital heart disease (CHD), and distribution of parvovirus B 19 gene in cardiac tissue. METHODS: We conducted a case controls study investigating the presence of B19 gene in cardiac biopsy tissue of 37 cases of CHD, 8 cases of rheumatic heart disease (RHD) and 20 cases of non-CHD with nested polymerase chain reaction (Nested PCR) and in situ hybridization (ISH). RESULTS: Among 37 CHD patients, 6 were B19 DNA positive in cardiac tissue (18.92%), while in control group, including 8 RHD patients (autopsy specimens) and 20 non-CHD patients, the B19 gene were all negative, there was a significant difference of B19 gene presence between CHD group and control group (P = 0.0421). ISH of B19 gene in cardiac tissue were positive in 4 CHD patients and all were negative in control group. The B19 gene was found locating in the nucleus of cardiac cell by ISH. CONCLUSIONS: Parvovirus B19 infection correlated with CHD. Since B19 gene was mainly found in the cardiac cell nucleus, maybe it influenced the regulation of gene expression and affected the development of heart with the result of CHD.

[Repair of soft tissue defect of the hand with a fascia lata flap by microvascular anastomosis].

OBJECTIVE: This is to report the clinical experience in repair of the soft tissue defects of the hand using a free fascia lata flap and skin graft. METHODS: Eight cases of soft tissue defects of the hand were repaired with a fascia lata flap by means of microvascular anastomosis. The size of the fascia lata flap ranged from 2.5 cm x 7 cm to 6 cm x 16 cm. RESULTS: All the fascia lata flaps survived completely with satisfactory results. CONCLUSION: The flap is nourished by the descending branch of the lateral femoral circumflex artery and seems to have a constant vascular supply and a long pedicle. The dissection of the flap is easy. The flap is thin and very suitable for repair of soft tissue defects of the hand.

Detection of Chlamydia pneumonia DNA in nasopharyngolaryngeal swab samples from patients with rhinitis and pharyngolaryngitis with polymerase chain reaction.

OBJECTIVE: To assess the prevalence of Chlamydia pneumomia DNA in patients with otolaryngic disease. METHODS: PCR assay was used to detect Chlamydia pneumonia specific Pst I 474 fragment DNA in swabs from patients with acute or subacute pharyngolaryngitis or rhinitis and sinusitis. C. pneumonia specific antibodies in sera were also assayed with microimmuno-fluoresence (MIF). RESULTS: About 28% (49/175) of the patients were PCR positive and 25.7% (45/175) were MIF antibodies positive. The accordance rate of the two methods was 91.8%. CONCLUSION: It is suggested that the C. pneumonia infection was common in this group of patients and the C. pneumonia Pst I 474 specific PCR was sensitive and specific for detecting C. pneumonia in pharyngolaryngitis or rhinitis and sinusitis.

Interventional treatment of huge hepatic cavernous hemangioma.

OBJECTIVE: To study the methods of interventional treatment of huge hepatic cavernous hemangioma (HCH). METHOD: A total of 14 patients with HCH were treated with lipiodol-ultrafluid (10-15 ml), bleomycin A (PYM 16-32 mg), and gelatin-sponge particles. RESULTS: DSA hepatic arteriography showed multiple vascular lakes in the early arterial phase, so-called "to hang the fruits on the branches", which persisted for a long time. CT scan showed a significant reduction in tumor size in 8 of the 14 patients after the treatment. CONCLUSION: Embolization with lipiodol-ultrafluid, PYM and gelatin sponge particles is one of the best methods for the treatment of HCH.

Basic and clinical study on the antithrombotic mechanism of glycosaminoglycan extracted from sea cucumber.

OBJECTIVE: To investigate the antithrombotic mechanism of glycosaminoglycan (GAG) extracted from sea cucumber. METHODS: We studied the effects of GAG on the coagulant pathway by measuring cloting time. The antithrombin mechanism of GAG was checked by assaying its effects on the thrombin activity in normal human pooled plasma, purified human heparin cofactor II system and antithrombin III system. The effects of GAG on the assembly, dispersion, and structure of fibrin gels as well as on the activity of plasmin were studied by means of turbidimetry, electron microscopy, and chromogenic substrate assay. We studied the effect of GAG on the expression and transcription of tissue factor (TF) and thrombomodulin (TM) in LPS (lipopolysaccharide)-stimulated human umbilical vein endothelial cells (HUVECs), and used heparin as a control. HUVECs were treated with different concentrations of GAG (1 microgram/ml, 5 micrograms/ml, and 10 micrograms/ml respectively) and 5 micrograms/ml heparin as a control together with LPS (1 microgram/ml). After incubation for 6 hours, TF and TM were investigated by ELISA and the mRNA study was carried out by RT-PCR. In a clinical trail, a series of variables were observed before and after treatment with GAG in patients recovering from cerebral ischemic stroke or suffering from ischemic heart disease. RESULTS: The TT and APTT were significantly prolonged by GAG (0.1 microgram/ml). GAG inhibited thrombin activity in the presence of HCII with a second order rate constant of 1.14 x 10(7) m-1.min-1, which was 4.6 times higher than that of ATIII. GAG significantly inhibited the polymerization of fibrin monomer and enhanced the activity of plasmin in a concentration dependent manner. GAG could impair TF mRNA expression and up-regulate TM mRNA expression. The result of clinical trail showed that the fat metabolism was enhanced in addition to the anticoagulant and the blood viscosity reducing effects. No side-effect was found. CONCLUSIONS: GAG mainly affected on the intrinsic pathway of blood coagulation. GAG was similar to dermatan sulfate both in the efficiency and in the mechanism of antithrombin. The acceleration of colt lysis by GAG depended on its ability to increase the activity of plasmin, to inhibit the polymerizing of fibrin monomer, and consequently, to alter the architecture of the fibrin net work. This effect on HUVECs appears to be at a transcriptional level and might be relevant for the antithrombotic action of GAG. GAG possess anticoagulant activity in vivo and it is a promising drug for antithrombotic therapy.

[The effects of the enameloplasty on the penetration and adaptation of sealant].

OBJECTIVE: To study the mechanism of enameloplasty technique by observing penetration and adaptation of sealant. METHODS: 60 extracted molars were selected and divided into three groups: pits and fissures of the samples in group A were prepared with bur(EST); those in group B were cleaned with cup-shaped brush(CST); group C, as control. RESULTS: The penetration of EST was (83.75 +/- 13.11)%, CST was (55.30 +/- 11.98)%. The presence of defects between sealants and inclines of cusp of EST and CST were 7.32% and 19.05% respectively, and between sealants and lateral walls of fissures were 16.07% and 38.93%. CONCLUSION: The penetration and adaptation of EST was superior to CST.

[Effect of kintop on gastrointestinal movement and its significance].

OBJECTIVE: To study the effect of obesity-reducing drug Kintop on gastrointestinal movement in rats. METHODS: Eighteen Wistar rats were divided into high dose (200 mg/100 g) Kintop group, low dose (125 mg/100 g) Kintop group, and control group. After anesthesia, stomach emptying time was directly observed using methylene blue as indicator. The frequency and amplitude of intestinal movement, and movement speed of intestinal content were detected. The serum tumor necrosis factora (TNF alpha) level was measured by ELISA. RESULTS: After administration of Kintop, the stomach emptying time was prolonged markedly, movement speed of intestinal content accelerated, time of dejection increased, the intestinal movement frequency and amplitude increased. There was a negative correlation between intestinal movement frequency and serum TNF alpha level (r = -0.91, P < 0.05). CONCLUSIONS: Kintop could significantly prolong the stomach emptying time, accelerate intestinal movement. These effects are correlated with not only the increased time of dejection but also the lowered serum TNF alpha level.

[Experimental study on effect of bushen yijing recipe in delaying senility of bone and brain of aged male rats].

OBJECTIVE: To evaluate the effect of Bushen Yijing recipe (BSYJR) in delaying senility of bone and brain of aged male rats and infer its mechanism in delaying systemic senility. METHODS: Forty male SD rats, 24 months old were randomly divided into 4 groups, the baseline control group, the aged control group (30 months old), the BSYJR high dose group and the BSYJR low dose group. The latter two groups received BSYJR treatment from 24 months old to 30 months old. Bone indexes (bone density of the proximal, middle and distal segments of left femur and break bending load of right femur) and brain indexes (binding capacity of M receptor and cholinesterase activity of brain) were measured after responding treatment. RESULTS: In bone, BSYJR could not only increase the bone mineral density in various segments of femur, but also raise the bending break load of femur dose-effect dependently. In brain, BSYJR could both up-regulate the binding capacity of M receptor and inhibit the activity of cholinesterase. CONCLUSION: BSYJR could delay the senility of bone and brain in male rats, inferring that it might regulate integrally the abnormality of aging in Kidney Asthenia and Essence Deficiency through mediation of nerve-endocrine-immunity network.

[Exploration of weight-reducing effect of Kintop in obese rats and its mechanism].

OBJECTIVE: To investigate the weight-reducing effect of Kintop and its probable mechanism in obese rats. METHODS: Body weight, Lee's index, total weight of celiac fat tissue, adipocyte size, blood glucose, blood lipid, expression of leptin in adipocyte (by ABC method), level of serum leptin (by RIA) of obese rats were chosen as the indexes to compare the weight-reducing effect of different dosages of Kintop. The administration of Kintop lasted 1 month. RESULTS: Good effect could be obtained by administration of medium dose Kintop (40 mg/100 g body weight per day) in simple obese rats. At the same time of reducing body weight, blood cholesterol and sugar levels also decreased in a certain degree. The adipocyte leptin expression reduced and serum leptin elevated in those rats with body weight reduced effectively by Kintop. CONCLUSION: Medium dose of Kintop administration for 1 month shows evident weight-reducing effect in simple obese rats, the effect might be related to raising the serum leptin level. Kintop also has the effect in lowering blood cholesterol level, and it could lower the blood sugar in obesity complicated with hyperglycemia by increasing the dosage appropriately.

[Endoluminal stent-graft repair of aortic aneurysms].

OBJECTIVE: To study the clinical value of the treatment of aortic aneurysms with endovascular stent-graft prosthesis. METHODS: After a unilateral surgical arteriotomy, Talent, Vanguard and Chinese stent-graft were advanced through the femoral arteries and placed in the proper position of the aneurysm sac under X-ray fluoroscopic guidance. RESULTS: Four patients with descending aneurysm, 1 patient with abdominal aortic aneurysm (AAA) involving the renal artery, superior mesenteric artery and celiac artery, and 1 patient with right common iliac artery aneurysm received straight stent-grafts. Five patients with infrarenal AAA received bifurcated stent-grafts. CT and MRA were performed during a follow-up of 3 - 19 months. Aortic aneurysms were completely excluded from the circulation. Five patients prolonged fever was noted and 1 patient was found to have a leakage 3 months later, and hemiplegia in 1 after procedure. CONCLUSIONS: Endovascular treatment of aortic aneurysm is technically feasible and can effectively exclude aortic aneurysms from the circulation. Endoluminal repair may serve as an interventional strategy to treat aneurysm, especially in patients at high surgical risk, but long term effect needs further study.

[Surgery for lung cancer complicated by bronchial asthma].

OBJECTIVE: To assess surgery for lung cancer complicated by bronchial asthma. METHODS: Lung hilum denervation was performed for asthma during radical resection of lung cancer. One patient with peripheral bronchogenic carcinoma in the right upper lobe underwent upper lobectomy and extensive dissection of lymph-nodes. Simultaneously, lung hilum denervation was done by complete amputation of the sympathetic and parasympathetic nerves around the lung hilum. Another patient with central bronchogenic carcinoma in the left upper lobe underwent lung autotransplantation. Pneumonectomy was performed, and the tumor parts of the lung were resected subsequently and the preservable parts of the lung were replanted into the bed of the upper lobe. RESULTS: Follow-up showed that the two patients had been alive with tumor free for 80 months and 17 months respectively and the time of postoperative mechanical ventilation was 4 and 18 hours. The frequency of asthma paroxysm decreased and the severity attenuated. Parameters of blood gas and lung function were improved. CONCLUSIONS: The poor lung function caused by asthma should not be regarded as a contradiction for the removal of lung cancer. Complete denervation around the lung hilum during the radical resection of lung cancer may contribute to the better treatment of lung cancer and asthma.

Lung autotransplantation technique for treating central lung cancer of upper lobe.

OBJECTIVE: To assess the feasibility of applying lung autotransplantation technique in the treatment of central lung cancer of the upper lobe. METHODS: Two patients underwent double-sleeve right upper and middle bilobectomy. Because the length of resected bronchus or pulmonary artery involved by tumor was too long to perform tension-free anastomosis, we had to transplant the lower lobar vein into the proximal stump of the upper lobar vein. Another two patients underwent pneumonectomy firstly, because we could not perform sleeve lobectomy in suit as the tumor extended through the oblique fissure to the margin of the lower lobe. Subsequently, we resected the tumor parts at a separate table, and replanted the preservable part of the lung into the chest. RESULTS: Being followed up to December 1999, the patient 1, 3 and 4 had been alive with tumor free for 31, 18 and 13 months. The patient 1 and 3 had a good living quality but the patient 4 had a very poor activity. The patient 4 received resection of the replanted lung for pulmonary artery bronchus pleural fistula on the 42nd day after operation. The second patient died of tension pneumothorax associated with bronchopleural fistula on the 19th day after operation. CONCLUSION: Lung autotransplantation is an alternative technique for pulmonary preservation in patients with stage III central lung cancer of the upper lobe whose cardiopulmonary function is too poor to undergo pneumonectomy.

[Abdominal aortic aneurysm treated by endovascular stent-graft and conventional surgical repair: a comparison].

OBJECTIVE: To evaluate the feasibility of abdominal aortic aneurysm (AAA) treated by endovascular stent-graft. METHODS: The clinical data of 52 patients with AAA treated by endovascular stent-graft (n = 20) and conventional surgical repair (n = 32) were analysed retrospectively. Patients conditions, operative hours, blood loss, function recovery and complications were compared. RESULTS: No significant difference was observed in sex, age, other disease, anesthesia risk category, aneurysm type, aneurysm diameter, technical success rate, and mortality rate between the groups (P > 0.05). The patients who underwent intraluminal treatment had significant reductions in operative time, blood loss, intensive care unit and hospitalization. But there was a high complication rate in the endovascular stent-graft group. All complications were connected with the interventional technique; endo-leak was the chief complication after operation. CONCLUSIONS: Endovascular treatment has marked merits such as reduced trauma, short hospitalization stays, and early functional recovery. It is suitable for the patients who can't undergo open surgical repair. But the complications caused by this technique needs further study.

[The survey of HIV prevalence among drug users in Guangxi, China].

OBJECTIVE: To understand the distribution and related risk factors of HIV infection among drug users in Guangxi. METHOD: Three hundred and twenty five drug users were tested for serum HIV antibody, with enzyme linked immunosorbent assay. RESULTS: One hundred and sixteen drug users (35.7%) were found to be HIV positive. The main risk factors associated with HIV infection were intravenous drug users and needle sharing among drug users. The HIV prevalence was 40.9% (114/279) among IDUs, and only 4.3% (2/46) among oral drug users. The phenomenon of sharing injecting equipment (76.7%) among intravenous drug users in Guangxi represents a very serious problem. CONCLUSION: There is a high risk of HIV infection among drug users in Guangxi. Prevention programs urgently need to be implemented.

[The epidemiological study of HIV infection among paid blood donors in one county of China].

OBJECTIVE: To observe the prevalence of HIV infection among paid blood donors and risk factors. METHODS: Selected the residents between 18 to 50 years old from two villages in one county of China in 1997. We observed the prevalence of HIV infection and keep following them to 2000. RESULTS: Nine hundred sixty-three residents were investigated and the prevalence of HIV was 1.8% (17/963). There were 88 paid blood donors among 963 interviewed subjects, the HIV prevalence was significantly higher (17.0% or 15/88) than those who didn't have the experiences of paid blood donations (0.2% or 2/875). Only one person out of 668 who were followed from 1997 to 2000 has converted to sero-positive, the incidence of HIV infection is 0.06 per 100 person-years. Nearly half of sero-negative and sero-positive people have the experiences of muscle injection. Among those with HIV-positive spouse, we found no one converted to sero-positive. The CD(4) count of those sero-positive people declined every year. HIV subtype and C2-V3 region sequence analysis were made for five HIV-positive people in 1998, the result indicated that all of them were infected with HIV-1 subtype B' and the genetic divergence rate was 2.4%. CONCLUSION: There was a high prevalence of HIV among paid blood donors in some regions, and paid blood (plasma) donations were the main risk factor for HIV infection. Although no evidence suggests that during the follow-up period the epidemic of HIV infection has spread locally, it still shall take measurements to prevent further transmission of HIV through paid blood donors.
