[The effect of formula diets with a modified fat content on human plasma phosphatide fractions].

Variation of human plasma phospholipids by formula diets with different composition of fat are caused by changes of the lecithin fraction, which amounts to 70 to 80% of total phospholipids. The changes of lysolecithin correspond to that of lecithin. The absolute amounts of sphingomyelin are unchanged by the different formula diets, while the relative amounts are negatively correlated to the lecithin fraction.

Factors influencing essential fatty acid requirement in total parenteral nutrition (TPN).

Twenty patients, after major injuries or elective abdominal surgery, were studied with respect to essential fatty acids in total serum and lipoprotein fractions. The regimen of total parenteral nutrition was altered in its content of energy, carbohydrates or fat. In all patients the linoleic acid levels decreased and the 20:3/20:4 ratio increased. Serum low density lipoproteins developed remarkable alterations of the biochemical composition and of morphology by electronmicroscopy. The degree of these alterations depended on the kind of illness, the amount of carbohydrates infused, special hormonal situation due to surgery and linoleic acid supply. As a consequence of these results, the supply of a sufficient amount of linoleic acid should be considered in every case of total parenteral nutrition.

The early phase of insulin release in man--a new method for quantitative analysis.

A method is presented for the quantitative analysis of early insulin release in man. There were measured arterial insulin levels after glibornuride administered intravenously. The mathematical procedure has been modified: Modification I is based on the assumption that early insulin release represents a wave like insulin delivery, modification II is based on the assumption that this insulin bolus is immediately followed by a slower insulin release which must be distinguished from the second phase of insulin release. For the calculations there was used a "primary insulin space" derived from experiments with exogenous insulin. The results of calculations were varying up to 1.5 units of early insulin release in healthy volunteers receiving glibornuride with dosages varying up to 50 mg. The value of the presented method for examinations of insulin release for theoretical and clinical purposes is discussed.

[Treatment of primary hyperlipoproteinemia type IIb and IV. Comparison of the lipid lowering effect of phenformin, clofibrate, and a combination of both (author's transl)].

22 outpatients with primary hyperlipoproteinemia type IIb and IV were treated in periods of eight weeks as follows: placebo; 0,15 g phenformin/day; 0,15 g phenformin + 1,5 g clofibrate/day; 1,5 g clofibrate/day; placebo. Compared to the first placeboperiode the serumtriglycerides were significantly lowered by phenformin (about 26%), by the combined treatment with phenformin + clofibrate (60%) and by clofibrate (51%) after eight weeks of treatment. The serumcholesterol was significantly lowered by phenformin (10%) and by the combined treatment with phenformin and clofibrate (14%), but not significantly by clofibrate (8%). After eight weeks of treatment with phenformin alone or in combination with clofibrate the body weight decreased significantly (1,9% or 1,4%). These changes in body weight were not related to changes in blood lipids. In conclusion, the combined treatment with 0,15 g clofibrate/day was more effective in lowering increased serum lipids than the treatment with phenformin or clofibrate alone.

The treatment of hypercholesterolemia with beta-pyridylcarbinol. Part 5. Report on 16 cases with severe hypercholesterolemia treated for 12 years.

A group of 78 patients with severe hypercholesterolemia (-X = 464 mg/dl) and symptoms of vascular disease of the heart, the extremities, or the brain, started a beta-pyridylcarbinol treatment with an average daily dosage of 1.2 g in 1964. In 1976 we could re-examine 12 patients, still on the same therapy. No myocardial infarction has occurred in this group since 1973, only 2 patients have had more attacks of angina pectoris that 1964. In contrast patients discontinuing therapy or replacing beta-pyridylcarbinol by other hypolipidemic drugs had a higher mortality.

Metabolism of maltose in perfused rat liver and in isolated hepatocytes.

The conversion of 14C-maltose into glucose, lactate and 14 CO2 was studied in perfused livers from fed and fasted rats and in isolated hepatocytes. Maximal glucose production was 30 mM x g-1 x h-1; half-maximal rates were found with 3 mM maltose. About 0.01 % of the radioactivity infused was recovered as 14CO2. The addition of maltose had no effect on rates of oxygen consumption, lactate production or ketogenesis. The data suggest that maltose did not serve as a major substrate for biosynthetic or energy producing processes under the conditions of the perfused rat liver.

[Treatment of hypercholesterolemia with beta-pyridylcarbinol. Experiences after long term treatment over 8 years (author's transl)].

78 patients with hypercholesterolemia were treated with beta-pyridylcarbinol from 1964 to 1966. In 1972 42 patients could be contacted again, 37 of them were reexamined. 14 of these 42 patients were still taking beta-pyridylcarbinol in 1972. Those who had taken more than 0.9 gm per day had a lower serum cholesterol level than in the beginning of the treatment in 1964. Clinical signs of atherosclerosis (angina pectoris and peripheral arterial disease) were significantly less in this group of patients. Besides a few flush reactions or heartburn no side effects to treatment were noted. The two groups of patients, with whom the 14 were compared, included 9 who had been taking clofibrate since 1966 and 19 patients without drug treatment for hypercholesterolemia. Both groups had higher serum cholesterol levels than the group of 14 with beta-pyridylcarbinol treatment over 8 years.
