Statistical comparison of the fibrometer and the Electra 600 for prothrombin time determination.

Determinations of prothrombin time with the semiautomatic fibrometer and with a more automated machine (Electra 600) were compared in regard to reproducibility, accuracy, and speed. Prothrombin times determined for replicate samples with the two machines showed a correlation coefficient of 0.98, but the Electra 600 sensed the clot 0.5 seconds before the fibrometer. The overall coefficients of variation for multiple tests in the normal therapeutic and elevated ranges of prothrombin time were 3.5% for the fibrometer and 2.0% for the Electra 600. The average technician working time was 60% shorter with the fully automated machine than with fibrometer. It is concluded that the automated machine was more accurate and more rapid in determining prothrombin time.

The diagnostic and prognostic significance of delayed hypersensitivity skin testing in patients with urological cancer.

The responses to several skin test antigens of patients with bladder or prostatic cancer has been compared with responses of normal controls. All of the controls and all of the patients with prostatic cancer (irrespective of the stage of the tumour or the method of treatment) showed responses to dinitrochlorbenzene. 80% of all patients with bladder cancer responded to DNCB, but a highly significant number of patients with advanced disease showed no response. This test appears especially useful in predicting the prognosis in patients with bladder tumours and its routine use is recommended. Response to candida extract, streptokinase/streptodornase (SKSD) and purified protein derivative (PPD) were also studied. No conclusion could be drawn as to their value as a prognostic index.

Stone screening and follow-up are necessary?

In a series of 120 patients with renal stones 60% were found to have a treatable metabolic abnormality. Idiopathic hypercalciuria and the incomplete form of renal tubular acidosis were the two most common abnormalities. Both conditions in the untreated form were associated with an unacceptably high stone recurrence rate and incidence of change. In view of this, follow-up is considered essential and if medical treatment is to be offered this should be in the form of a controlled clinical trial.

Cryoglobulins. III. Further studies on the nature, incidence, clinical, diagnostic, prognostic, and immunopathologic significance of cryoproteins in renal disease.

Serial serum samples from a large number of patients with immunologic renal disease, normal healthy controls, acute infections as well as non-immunological renal disease were studied for the presence, nature and properties of cryoproteins, and these correlated with serial renal functional, morphologic, immunohistologic and clinical fingings as well as serologic observations. A high incidence of cryoproteins were found in renal disease thought to be mediated by immune complexes. Cryoprecipitates were not detected in the other patients. The presence of fibrinogen in a serum cryoprecipitate was always associated with rapidly progressive disease and poor prognosis. An association between the detection of cryoproteins and the clinical and morphological activity of disease was observed. Persistence of cryoproteinemia was associated with progression and apparent disappearance with resolution or progression to end stage renal disease. In patients with hematuria or proteinuria of questionable significance cryoprotein detection was always associated with immune complex nephritis. Renal transplantation in the presence of cryoproteinemia was associated with recurrent nephritis in the graft. Cryoproteins were found to have biologic properties attributable to antigen-antibody complexes, to contain immune complexes of antigen and antibody and to have serologic factors concentrated. The detection of serum cryoglobulins was found to be a better index of clinical and morphologic activity of immune complex renal disease than was serum complement. As in our previous studies, these proteins appear to be of diagnostic and prognostic value in renal disease and provide a method of antigen identification in these disorders.

A randomized controlled trial of acetyl salicylic acid in the secondary prevention of mortality from myocardial infarction.

The results of a randomized controlled trial of a single daily dose of acetyl salicylic acid (aspirin) in the prevention of reinfarction in 1,239 men who had had a recent myocardial infarct were statistically inconclusive. Nevertheless, they showed a reduction in total mortality of 12% at six months and 25% at twelve months after admission to the trial. Further trials are urgently required to establish whether or not this effect is real.

Nature of hyperacute (accelerated second set) rejection in dog renal allografts and effects of heparin on rejection process.

Renal allografts were exchanged between unrelated mongrel dogs after previous sensitization with skin and kidney grafts from the same donors. Rapid rejection of the renal allografts was associated with the accumulation of platelets and leucocytes in the peritubular and glomerular capillaries but fibrin deposition was not demonstrated.Heparin infusion delayed but did not prevent the rejection process.
