[Epidemiologic aspects of seronegative spondylarthritis].

In accordance with the prevailing trends of the development of science in rheumatology Wright, Moll and Haslock inaugurated the integrative conception of the seronegative spondarthritides in the middle of the seventies. The diseases which belong to the seronegative spondarthritides are the ankylosing spondylitis, the psoriasis-arthritis, the Reiter syndrome, the Behçet syndrome, the ulcerous colitis, Crohn's disease and Whipple's disease. Object of the representation is the determination of the notion of seronegative spondarthritis and its presence among the dwelling population. Furthermore, the authors deal more in detail with the distribution according to the demographic and ethnographic characteristics, the frequency of the presence of HLA B 27 as well as with the results of familial investigations (familial aggregation and association). The heuristic value of the classification scheme of the seronegative spondarthritides developed by clinics appears worth of discussion, since from the epidemiological point of view aggravating critical objections concerning the method must be raised. Clinico-epidemiological statements of investigation, prospective and multicentric longitudinal investigations concerning the more profound study of the "natural history" of the seronegative spondarthritides are hopeful and necessary within the future research strategy.

[Relation between atmospheric events and internal medicine emergency situations].

Over a period of 2,541 days (7 years) patients with the diagnosis groups acute oesophageal and gastrointestinal haemorrhage, respectively (n = 395), cerebrovascular insufficiency (n = 689) and acute myocardial infarction (n = 701) were registered. From the individual meteorological data registered in the same period the temperature the evaporation warmth of the water, the specific warmth capacity of the air, the air pressure and a constant calculation factor were used for the calculation of the equivalent temperature. Thus by its means a measure for the warmth content of the air was at our disposal which is determined by a series of factors. Between the emergency situations mentioned and the equivalent temperature established a correlation test for significance was performed. In these cases was the result that there are significant relations between the cerebrovascular insufficiency. Though already a larger number of meteorological parameters entered into our investigations, the result supposes that further important meteorological data should not be neglected, such as phases of weather and front transits. This shall be taken into consideration in future investigations.

[Peritoneal dialysis treatment in psoriasis vulgaris].

In 8 patients (4 males, 4 females, at the age from 23 to 64 years) with a since 2 to 32 years existing generalised psoriasis vulgaris of pure plaque type a treatment with peritoneal dialysis was performed over a period of 1-6 months with on an average 18 treatment days. A full remission of the changes did not appear in any case, only in one patient a transitory involution of the skin changes and of the arthropathy could be established. In 5 patients the picture of the disease remained uninfluenced, 2 showed a deterioration. Wether this failure of the treatment is conditioned by an inadequate, too little intensive treatment, must, in comparison to the reports of literature, be regarded as little probable. By reason of our results the treatment with peritoneal dialysis according to the scheme mentioned cannot be recommended.

[New pathogenetic aspects of rheumatoid arthritis].

Inflammatory changes in the epiphyseal bones in patients with rheumatoid arthritis are demonstrated which correspond to changes in the bone organ far from pannus. They may be the cause for the demineralisation near to joint (first x-ray sign to rheumatoid arthritis). Further from the analysis of immune complexes of patients with rheumatoid arthritis apart from rheumatoid and antinuclear factors and IgM-antibody against plasma membranes was the result. These two findings are new in the description of the pathogenesis of rheumatoid arthritis. Their possible importance for the development of the arthrosis is discussed.

[Infusion therapy and parenteral feeding in acute pancreatitis].

The acute pancreatitis can be a life-threatening disease and can demand the performance of intensive-therapeutic measures. The central position there in occupy the infusion therapy and the parenteral nutrition. On account of the increased catabolism already in the initial phase of the acute pancreatitis the supply of suitable carriers of calories in adequate quantity is necessary. The indication of glucose and the differential indication of fructose and Alvesin "Neu" are discussed. In the treatment of shock human albumin, plasma and plasma substitutes as well as blood take the precedence. Apart from the diagnostic and therapeutic value of the peritoneal lavage in the acute pancreatitis is possesses the additional effect of the dialysis in acute renal failure.

[The unusual case: Wegener's granulomatosis].

In a casuistic report of a case of Wegener's granulomatosis the diagnostic and therapeutic problems of a relatively rare clinical picture are described. Special emphasis is laid on the variety of organic manifestations and on the fact that bioptic investigations have not resulted in typical findings leading to diagnosis. Despite immunosuppressive therapy the lethal exitus cannot be influenced.

[Nature and significance of osteopenia].

New realisations on the regulation of bone and calcium metabolism by hormones and vitamins, modern methods of the radiologic and morphologico-morphometric diagnostics considerably increased our knowledge about the deminerlising diseases of the skeletal system. The possibilities for diagnostics and therapy resulting from this are restricted in practice by a non-adequate nomenclature. The introduction of the disease group notion "osteopenia" for all demineralising processes and its definition as well as the representation of the differential diagnostics of osteolamacia, fibrosteoclasia and syndrome of osteoporosis as clinical main representatives of osteopenia shall help to prevent diagnostic and therapeutic errors.

[Intra-articular therapy with radionuclides].

The 12-year experiences of a working team with the intraarticular radiogold therapy in persisting synovialitides of chronic rheumatic diseases and resistant irritation phases of arthroses are supplementarily described and a critical valuation is performed. Finally practical conclusions for the local joint therapy are made, taking into consideration also the experiences of other investigators.

[Undesirable drug interactions. Notes on general principles].

Increasing knowledge about undersirable drug interactions make necessary new differential-therapeutic considerations. It is attempted to meet the problem in its general importance and - without any over-refinements which have nothing to do with practice - to draw conclusions for pharmacotherapy.
