[Heroin dependence as a rare cause of symmetrical globus pallidus necrosis].

Long-term survival after carbon monoxide intoxication has been accepted for a long time as pathognomonic for elective cerebral tissue damage, especially in cases of isolated symmetrical necrosis of the globus pallidus. The results of old bilateral necrosis in the globus pallidus are described in a case of an acute heroin death after long-term drug abuse. The evaluation of such cerebral changes is discussed as they have to be interpreted as hypoxic or oligemic tissue damage. The different causes of symmetrical necrosis of the globus pallidus that are relevant for legal medicine are compiled and discussed.

[Assessment of damage to the child in acute CO poisoning in pregnancy].

Diaplacental transition of CO is discussed in two cases of lethal CO intoxication of pregnant women. Foetal CO-Hb has a time lag of several hours against maternal CO-Hb. In highly acute CO intoxications noxious foetal CO intoxication may not occur in spite of high maternal CO concentrations. However, there is a possibility of foetal hypoxia based on maternal hypoxaemia. In cases of subacute CO exposure for several hours, higher CO concentrations may be found in the foetal blood than in the maternal blood. Oxygen therapy of such CO intoxication cases must continue for a certain time after the mother had been detoxicated, because foetal CO elimination lags behind that of the mother. Subacute CO intoxications with sublethal CO-Hb concentrations may cause foetal damage and intra-uterine death as a consequence of CO-Hb, cellular toxic CO-effects and the hypoxia caused by maternal hypoxaemia. Medical decision for interruption of pregnancy as a consequence of foetal indication should be taken after due consideration of the legal problems involved in such action.

[Absolute alcohol, an adjustable embolization material in kidney tumors].

Fourty-four patients with angiographically verified malignant renal tumors underwent a complete or selective embolisation of the affected kidney with absolute alcohol. The occlusion is effected by slow infusion of absolute alcohol. The technique is supported by animal experiments. The quantity of alcohol applied is measured by its intracorporal spread. The method of slow infusion is compared to other published embolisation techniques using absolute alcohol. This method allows an individual application of the vascular occlusion of the tumor supplying and the normal tissue supplying vessels, depending on size and localisation of the tumor, and further clinical data.

[Forensic medicine aspects of fatal coronary insufficiency with normal coronary arteries].

Clinical experiences give examples for the existence of various courses of anginal symptomatology even with cases of sudden heart death demonstrating angiographically normal coronary arteries. Pathogenetically may be considered spasms of regular or little changed coronary arteries, coronary muscle bridges and acute arrhythmias. In cases of recurrent myocardial ischemias an interstitial fibrosis and endocardial fibrosis can be proved histologically in the myocardial supply area. However an acute coronary insufficiency based on rheological and metabolic etiology cannot be found with morphological methods. The results are discussed, considering forensic aspects in cases of competitive causes of death.

[Acute and subacute fetal CO-poisoning].

The main principles of placental CO exchange in cases of CO intoxications of pregnant women are shown. Fetal COHb lags behind maternal COHb by several hours depending on the exposure pattern. During CO elimination the fetal COHb levels again lag behind the mother's. A case of a lethal CO intoxication of a pregnant woman, mens IX, with exposure time of 10-12 h and COHb of 75% for the mother and 46% for the fetus allows to make conclusions about process of intoxication and about different times of death for mother and fetus. In highly acute lethal intoxications of the mother COHb in fetal blood is low or negative. A vital danger of the fetus is not probable when mother survived a short CO exposure.

[Neurosurgical management of gunshot injuries to the head].

The article describes the pathophysiological mechanisms of injuries caused by shots in the head. In particular, the effect exercised on the cerebral tissue by the impact of rapid projectiles is explained. The review is based on 23 patients treated by the authors. These cases are discussed and critically assessed, using the results communicated in other papers as basis. The requisite surgical measures are considered; the need for effecting complete cleansing of the path of the bullet is viewed with some reserve.

[Quantitative morphological examinations of coronary muscle bridges (author's transl)].

The coronary muscle bridge which was found in non-selected autopsy material with a frequency of 24% meets with increasing interest in the discussion on the causes of sudden cardiac death and within the scope of coronary bypass-surgery. Twenty morphometrically examined coronary arteries have been used to demonstrate that the coronary muscle bridge provide local protection from atherosclerosis. Pathomorphological and functional effects of coronary muscle bridges are discussed.

[Quantitative morphology of the coronary arteries in acute coronary death].

93 hearts were examined angiographically and morphometrically. The quantitative findings in coronary arteries of 30 cases of sudden coronary death could be compared with 30 cases of non-cardiac death and coronary sclerosis. Applying discriminant analysis a linear function T has been found, considering the measures of intima plane, lumen plane, maximum grade of stenosis and weight of the heart. This method points toward a critical constellation of factors which speaks for an acute lethal coronary insufficiency and is helpful in solving problems of causality and legal questions. The different quantitative results of lumen, intima and media are shown in cases with normal coronary arteries, with coronary sclerosis and in cases of sudden coronary death.

[Different concentrations of drugs in respiratory and skeleton muscles in cases of lethal poisonings (author's transl)].

The quantitative distribution of drugs in seven cases of intoxication has been investigated especially in different muscles distinguished by their functions. The analyses have been verified in two respiratory muscles (m. intercostalis and diaphragma) and two skeletal muscles, being inactive during unconsciousness (m. psoas major and m. quadriceps). In all cases, higher concentrations of drugs have been found in the respiratory muscles than in the skeletal muscles. During the acute phase of intoxication an enhanced quantity of drugs is stored in the active respiratory muscles. The skeletal muscle shows reduced metabolism activities with lowered storage of drugs.
