Vitreous inflammatory reaction. Vitreous inflammation in rabbits, induced by saline, air, or sodium salt of hyaluronic acid.

Inflammation of the vitreous body of rabbits after the injection of normal saline, sterile air, or SSHA was studied over a period of 30 days. The course of the inflammation follows a definite pattern: it shows an exacerbation 24 h after injection, an abrupt regression 48 h after injection, a new peak on the 8th day and subsequently a gradual regression until the 30th day. Although the course of the inflammation is approximately the same for the three vitreous substitutes, the reaction they provoke varies in intensity. The inflammation is most intense after the injection of SSHA and mildest after saline injection, while the intensity of the reaction caused by air lies between the two. The reaction was assessed by measuring the number and types of vitreous cells and the changes in the percentual proportions of globulin and albumin in the vitreous body, as well as by ascertaining the alterations of the vitreous structure.

The third component of complement (C'3) level in patients with uveitis.

In 23 cases of endogenous uveitis the third component of the complement (C'3) system was determined by the immunodiffusion method. No statistical difference was found in uveitis patients as a whole in comparison with the normal controls; although a rather high value of C'3 was found in six cases of Eales' disease and in three cases of Behçet's disease. The significance of these results is discussed.

[Irvine-Gass syndrome. Statistical and angiographic study].

This study is based on a detailed investigation of 12 cases of Irvine-Gass syndrome which was observed during the examination of 350 patients operated for pathological or senile cataract. The authors have found that: Males were affected more than females. Arteriol hypertension increases the frequency of the disease. The syndrome was found at an increase frequency in cases of nuclear cataract and in case where cataract was extracted by cryo-extraction. The bulge of the vitreous within the anterior chamber (without rupture of the anterior hyaloid membrane) causes a higher frequency of the disease. A similar observation was made when there was an incarceration of the vitreous to the surgical wound. The retreat of the edema was faster when the syndrome appeared earlier than the cataract operation. Whereas the restoration of good visual acuity did not depend upon the time of appearance of the disease. During the fluorescein study the following was observed. The possible participation of the chorio-capillaris layer in the pathogenicity of the disease. The diffusion of the fluorescein diminished with the improvement of the disease. Fluorescein angiography is unquestionably superior to simple ophthalmoscopy or the coloured photograph of the fundus of the eye, because the full extent of the existing damage can be revealed only through this method.

[A new argon-laser-approach for the management of aphakic pupillary block (author's transl)].

9 cases of aphakic pupillary block with high intraocular pressure were managed successfully by means of a new argon-laser-technique. This new approach consisted in the application at the iris border, 1.5 mm from the pupillary border, of a mobile argon-laser beam of variable intensity, ranging from 250 to 500 mw, of constant time (0.5 sec) and of fixed spot size (200 mu). In some instances the beam was moved parallel and in other cases vertically to the pupillary border in the direction towards the ciliary portion of the iris. This technique ensures maximum retraction of the pupillary border, corresponding to the line of running application, coupled with the least possible damage to the iris and a minimum of applications. Simultaneously with the retraction of the pupillary border the posterior synechiae of the iris are detached and the out-flow of the aqueous trapped in the posterior chamber is released.

[Experimental hypertony caused by obstruction of the anterior chamber angle ab interno (author's transl)].

In 30 rabbits, separated into 3 groups (A, B and C), the entire area (A), three quarters (B) half of the anterior chamber angle (C) was experimentally obstructed by means of a homologous scleral graft. In groups A and B a statistically significant change of the intraocular pressure and of the cornea diameter was shown as compared with the control eye and group C. This change is due to the obstructed area of the angle. The increased intraocular pressure lasted for some months. The graft was well tolerated by the tissue of the angle and the obstruction of the angle was total, as was confirmed by means of the histological preparations.

Extraocular observations in episcleral sponge implants.

The follow-up of 262 cases of retinal detachment operated on after the Custodis-Lincoff method in combination with cryopexy permitted the following observations: (1) The implant protruded in a number of cases under an intact conjunctiva and Tenon's capsule without discomfort to the patient. This protrusion depended mainly on the size and the manner of fixation of the implant, the distance of the point of fixation from the limbus and the condition of the conjunctiva. (2) Changes were observed in the conjunctiva in the fixation area, such as subconjunctival haemorrhage, cystoid degeneration of the conjunctiva and rupture with extrusion of the sponge. Subconjunctival haemorrhages were observed in 3.4% of the cases and were either absorbed or led to an inflammation. Cystoid degenerations occurred in 2.2% of our cases. These were limited to the edge of the sponge proximate to the limbus and involved only cases of radial fixation. Rupture of the conjuctiva and extrusion were noticed in 3.4% of the cases. (3) An inflammation due to the sponge was observed in 4.1% of the cases. Its time of onset varied between 3 days and 22 months. According to the time of onset the patients were divided into three groups. The gravity of the inflammation was proportionate to the earliness of the time of onset. Predisposing factors for the above complications were mentioned and discussed.

[Some clinical aspects concerning diplopia after retinal detachment surgery with cryopexy and episcleral silastic sponge (author's transl)].

The aetiology of diplopia following retinal detachment surgery after the Lincoff-Custodis method is examined. Of 140 patients operated, 16 (i.e. 11.4%) developed diplopia. The authors believe that this diplopia is due to a fixation of the eyeball at the point of the silastic sponge placement. As responsible factors, which depend on the manner of sponge fixation and the size of the sponge, they regard: Alteration of the bulb curvature, shortening of the Tenon's capsule placed over the sponge, and changes in the conditions of muscular action.

[A contribution to the study of the correlation between the depth of the anterior chamber and the coefficient of aqueous humor outflow (author's transl)].

The depth of the anterior chamber was measured and the coefficient of the aqueous humor outflow was ascertained by means of tonography in 88 normal eyes. The mean value of the depth of the anterior chamber established was 2095+/-0.42 and the median value of the outflow coefficient of the aqueous humor was found to be 0.27+/-0.07. Both values showed a regular distribution. A statistically significant correlation was established between the two values. Expressed through a correlation coefficient this would mean in practice that the coefficient of aqueous humor outflow is affected up to 10 percent by the value of the depth of the anterior chamber.
