Late complications in branch retinal vein occlusion.

Neovascularization in branch retinal vein occlusion usually develops 6-12 months after the onset of the disease, although some cases have been reported in which neovascularization and subsequent vitreous haemorrhage developed 2-3 years later. This is a report of nine cases of late appearance of vitreous haemorrhage due to branch retinal vein occlusion, which occurred 3-6 years after the initial onset of the disease. In two of these nine cases the vitreous haemorrhage was very profound and had to be managed by vitrectomy. We have studied the remaining seven cases, which had retinal ischaemia and optic disc or retinal neovascularization documented by fluorescein angiographic examination. Laser coagulation was applied in these seven cases, which resulted in considerable regression of the neovascularization and absorption of the vitreous haemorrhage. In one out of the seven cases recurrent vitreous haemorrhages appeared and it was finally treated by vitrectomy. Visual acuity improved in six of the seven cases. The follow-up period ranged from 12 to 48 months. The late appearance of the ischaemic type of branch retinal vein occlusion with neovascularization can be attributed to the change in character of the initially mild oedematous form of the disease. Partial posterior vitreous detachment and traction exerted at some later stage upon the neovascularization could be additional factors of the late appearance of vitreous haemorrhage. Patients with branch retinal vein occlusion should be followed up regularly over a long period of time in order to avoid late complications of the disease, such as vitreous haemorrhage following optic disc or retinal neovascularization.

The effect of low-dose interferon treatment on visual evoked potentials.

Patients suffering from chronic viral hepatitis, treated with interferon alpha-2b in a dose of 3 million units trice weekly subcutaneously, were studied. The influence of interferon on the optic nerve was evaluated by measuring the P100 implicit time of the visual evoked potentials. The results before the treatment were compared with those after 6 and 12 months of therapy, as well as with those of normal subjects. There were statistically significant differences between the groups. Furthermore, in 10 of 56 eyes (17.8%) after 6 months of treatment, and 8 of 22 eyes (36.3%) after 12 months of treatment, the P100 was abnormally delayed. The long term neurovisual effects of low-dose interferon are currently under investigation.

Treatment of inferior oblique muscle overaction with myectomy or with anterior transposition.

This paper presents a comparative study of the effectiveness of myectomy and anterior transposition in the treatment of inferior oblique muscle overaction. We operated 160 patients with overaction of the inferior oblique muscle. Eighty patients (148 eyes) were operated by myectomy at the insertion and 80 patients (151 eyes) by anterior transposition of the insertion of the inferior oblique near the temporal side of the insertion of the inferior rectus muscle. Comparison of the two methods, using the chi-squared test, showed that: 1) both surgical procedures were equally effective (chi 2 = 0.26) for correcting overaction of the inferior oblique muscle and V-phenomenon; 2) weakening of the inferior oblique muscle of both eyes was almost always required (in 115 out of 116 cases) in cases with V-phenomenon and often (24 out of 44 cases) in cases of congenital paresis of the superior oblique muscle. We conclude that both procedures are equally effective and equally easy to perform.

Risk factors in ocular hypertension.

The aim of the study was to determine whether certain factors are related to an increased risk of developing open-angle glaucoma. A total of 345 untreated glaucoma suspects with intraocular pressure (IOP) > or = 21 mmHg, cup to disc ratio 0.4 or less and no visual field defects, were followed up for 6 to 8 years (mean 7.3). During the follow-up 71 patients developed established glaucoma and were compared to the remaining 274 patients. The following factors were analysed: age, family history of glaucoma, IOP, Humphrey 30-2 visual fields, optic disc appearance, myopia, exfoliation, arterial hypertension and diabetes. Analysis yielded statistically significant results regarding a number of these factors in the patients who subsequently developed open-angle glaucoma. A significant association with the subsequent development of field loss in ocular hypertension (OHT) included: heredity (p < 0.001), age > or = 60 years (p = 0.013), axial myopia (0.001 < p < 0.01) and arterial hypertension (p = 0.05). About 20% of patients with ocular hypertension developed glaucoma over a period of seven years. Risk factors such as heredity, age, myopia and arterial hypertension, among others, must be considered in the follow-up of glaucoma suspects.

Multifocal ERG in retinal detachment surgery.

PURPOSE: The main purpose of this study is to evaluate the visual function of eyes operated on for rhegmatogenous retinal detachment. METHODS: Fifteen patients were studied suffering from peripheral retinal detachment treated with the Lincoff-Custodis method. Visual field was tested with the Humphrey Field Analyzer and the analyzer program 30-2, which scanned a 0-30 degrees field. The multifocal ERG was recorded with the VERIS III (Visual Evoked Imaging System, Tomey, Nagoya, Japan). RESULTS: There was great loss of retinal response densities in the area of retinal detachment (1.3 +/- 0.7 nV/deg2). However, a decrease in retinal response densities was also recorded in the reattached retina (6.90 +/- 0.7 nV/deg2). CONCLUSIONS: MFERG is an objective tool for monitoring the functional abnormalities affecting the vision of eyes operated for retinal detachment.

[Intraocular complications following Custodis-Lincoff operation].

This investigation is based on 262 cases of retinal detachment treated with episcleral silastic sponge implants and cryopexy. Anatomical restoration of the retina after one operation or more was successful in 89 percent. Drainage of subretinal fluid was carried out in 52 percent of the cases. Supplementary light-coagulation 3 or 4 weeks after the operation was applied in 43 cases (17 percent) in order to seal off retinal holes on the buckle due to inadequate chorioretinal adhesion following cryopexy. Insufficient adhesion was directly related to the retinal elevation in the area of the tear. Chorioretinal haemorrhages in the group without release of subretinal fluid were caused by the following factors: Repeated cryo-application in the same place, application of the probe on the open part of the tear instead of around the tear, sponge-fixation in the immediate area to vortex veins, pressure exerted by the sponge on choroidal and retinal vessels, particularly in persons of advanced age. Local haze of the vitreous corresponding to the location of the cryp-application was directly related to the number of applications and the position of the tear. Vitreous haze was more frequent where the hole was situated towards the ciliary body. Pigment migration was observed in 9 percent of the cases. It should be noted that this complication was also encountered preoperatively in a group amounting to 4 percent of the total number of cases after padding of the eyes and absorption of subretinal fluid. Such cases should be distinguished from those of postoperative occurrence of pigment migration. Detachment of the choroid happened more often in the group in which subretinal fluid was released. In this group expulsive haemorrhage also occurred, which, apart from the choroidal detachment, resulted, immediately upon drainage of subretinal fluid, in high intracular pressure. Macular puckering was noticed in 5 cases (2,7 percent). In 4 out of the 5 cases with this complication, the tear was located towards the posterior pole and sponge fixation was radial.
