Platelet kinetics in Crohn's disease.

The pathogenesis of thrombocytosis in Crohn's disease was analized in ten non-treated patients using the 51Cr labelling method. The data obtained were compared to those taken from a control group. Besides platelet survival time, platelet turnover, surface radioactivity, platelet adhesiveness and morphological aspects of bone marrow were investigated. Platelet survival time in patients with Crohn's disease proved to be significantly shortened (p less than 0.001), whereas platelet turnover appeared augmented. No additional radioactivity is encountered from the organs scanned. The relative platelet adhesiveness is diminished while thrombocytopoesis in bone marrow is augmented.

[Clinical aspects of alcohol induced liver injury (author's transl)].

Chronic alcohol consumption results in early biochemical and ultrastructural alterations of the hepatocyte which in turn may lead to alcoholic fatty liver as well as alcoholic hepatitis and via the central hyaline sclerosis to fibrosis and cirrhosis of the liver. Already at the stage of the alcoholic fatty liver an isolated increase of serum gamma-glutamyltransferase activity can often be observed; it results from hepatic microsomal enzyme induction and may facilitate early recognition of alcoholic liver injury. To establish the diagnosis, however, a histological examination of the liver is necessary. The therapy of alcohol-induced liver injury is based upon an absolute alcohol abstinence since alcohol itself or one of its metabolites are hepatotoxic.

[Functional disorders of the gastrointestinal tract (author's transl)].

Functional disroders are the most important cause for complaints in the gastrointestinal tract. Dysfunction may concern one or more physiologic properties like tonus, motility, secretion, sometimes also resorption and digestion, or their interaction. Functional disorders of the esophagus (esophagospasm and achalasia) become manifest as dysphagia. Halitosis, bad taste, burning tongue, and flatulent abdomen are frequent symptoms of functional disorders of the gastrointestinal tract. Irritable bowel syndrome is probably the functional disorder most freqently found in the gastrointestinal tract. Characteristic symptoms are pain in the lower and upper middle abdominal region, obstipation and/or diarrhea, flatulent abdomen, mucous discharge with the stools and urgent defecation with cramps relieved after discharge. Prognosis quoad vitam is good, the course, however, is subject to many changes. Therapie is symptomatic. Diagnostic and psychotherapeutic measures are intended to help remove carcinophobia and to overcome conflicts and fears.

[The effect of acute liver damage on inactivation of renin and aldosterone (author's transl)].

Plasma renin activity was measured in 5 surgical patients with hydatoid disease of the liver before and after partial hepatectomy. Plasma renin activity rose statistically significantly after surgical operation and remained virtually constant thereafter. This rise was accompanied by a statistically significant rise of plasma aldosterone while urinary excretion of aldo-18oxo-glucuronid remained unchanged. No change of urinary excretion of electrolytes was observed. It is concluded that besides stress after surgical operation decreased hepatic inactivation of renin and aldosterone were the main causes which led to elevation of plasma aldosterone concentration.

Treatment of acute viral hepatitis with (+)-cyanidanol-3.

A double-blind trial of (+)-cyanidanol-3 (2 g/day) versus placebo tablets was carried out in 100 patients with acute viral hepatitis. 51 received the drug and 49 placebo. (+)-Cyanidanol-3 accelerated the disappearance of HBsAg from the blood, lowered serum-bilirubin, and relieved symptoms such as anorexia, nausea, and pruritus. The drug was well tolerated. None of the patients had a relapse of acute hepatitis. Chronic active hepatitis developed in 1 of the placebo-treated patients. Thus, (+)-cyanidanol-3 seems to be of benefit in acute viral hepatitis.

[Treatment of peptic ulcer in the Zollinger-Ellison syndrome with histamine H2-receptor antagonists (author's transl)].

Histamine H2-receptor antagonists metiamide and cimetidine were used in the treatment of severe peptic ulceration in Zollinger-Ellison syndrome. The ulcerations were completely healed in all four patients after treatment lasting from six weeks to four-and-a-half-months. Two patients developed recurrent ulcer after the treatment had stopped, but responded to a second course. One patient developed hepatitis B during cimetidine treatment and it is possible that the course of the hepatitis was unfavourable affected by cimetidine. But no other side effects were noted nor was there a significant change in basal serum-gastrin concentration or an increase in H+ secretion. Total gastrectomy remains the treatment of choice in Zollinger-Ellison syndrome, but cimetidine should be considered if the patient refuses operation or operation is not feasible because of a poor general state.
