[Monstrous hemangiopericytoma of the lung].

Report on a monstrous haemangiopericytoma of the left lung in a 76-year-old woman. On the basis of the observation the authors enter the frequency, localization, morphology, cellular and clinical differential diagnosis, prognosis and therapy of these tumours.

[Experiences with endoscopic sclerosing of esophageal varices].

In this paper is reported on endoscopic sclerotherapy in 35 patients, which have been treated from March 1980 to March 1983. The efficacy of submucosal (paravascular) wall sclerosis wasn't sufficient. We achieved better results in the prevention of esophageal variceal bleeding with combined para- and intravascular injections of sclerosant.

[Clinical pathology of prolactin-forming hypophyseal adenomas].

On the basis of own material and data of the literature problems of the classification of pituitary adenomas are discussed. Special emphasis is laid on typical light and electron microscopical as well as immune histochemical criteria of prolactinomas. Clinical and morphological aspecte of microadenomas and adenomatous hyperplasias are briefly described.

[Endocarditis parietalis fibroplastica Löffler].

Report on an endocarditis parietalis fibroplastica Löffler in a 54-year-old male. Discussion of the localisation, pathologico-morphologic findings as well as of the differential diagnosis and the therapy. Particular reference to the ten-year course of the disease, which possibly is the success of the treatment.

[Hyperplasias and adenomas of the pituitary gland in an unselected autopsy material].

5,100 formalin-fixed pituitaries of an unselected autopsy material of the Institute of Pathology of the Ernst-Moritz-Arndt-University of Greifswald were analysed with regard to the occurrence of adenomas and adenomatous hyperplasias. Beside the histological classification the size was determined. For some adenomas of the pituitary gland (ACTH-cell-adenomas, TSH-cell-adenomas and evidently also prolactinomas) the development from a hyperplasia which is a consequence of a central regulation disorder is supposed. The pathogenesis of the other types of adenomas remains still unclear. Difficulties in differential diagnosis between adenomatous hyperplasias and small adenomas are discussed.

[Topochemical and electron microscopical analysis of the pancreas of dogs after pancreatic denervation].

Histochemical and electronmicroscopic findings on chronically and selectively denervated pancreata of 8 dogs are described. Most nerve fibers showed marked degeneration and perineuritis, and there was an inflammation of intrapancreatic ganglia. There was no change in the number of islets or in the A-B-cell ratio. Insulitis frequently has been observed. Both immunohistochemically demonstrable insulin in the B-cells, and glucagon in the A-cells were reduced in relation to that in non-denervated controls. There were single A- and B-cells within the acini in an apparently normal frequency. Many pancreata showed a chronic infiltration and fibroblastic inflammation within the acini but their extent varied considerably between the animals. Intrapancreatic blood vessels were generally dilated, hyperemic, and perivascular fibrosis was seen in some cases; the vessels contained many leucocytes. Acinar cells were often subjected to vacuolic degeneration. Another part of them showed, however, increased appearance of ergastoplasm, mitochondria, and of Golgi apparatus as a sign of increased functional stimulation. There were fibrosis and mononuclear infiltration in the medium-sized and large pancreatic ducts.

[Cerebral mycotic aneurysms after abortion endometritis].

The genesis of abortion endometritis appears to be sufficiently clarified when the aneurysm was successfully demonstrated and the vascular bulgings were located at the typical sites of predilection of Forbus' malformation aneurysms. The demonstrated case shows that this conception is not generally valid and that surprising aspects can be derived from pathological-anatomic findings. A 43-year-old female patient had her third subarachnoid haemorrhage on the second day of admission before a neurosurgical operation could be performed and died of the consequences of this haemorrhage. The histological examination spoke for an embolic-mycotic aneurysm genesis in the presence of abortion endometritis. In the literature, the majority of cerebral mycotic aneurysms are described as a consequence of endometritis, only 20 per cent are due to other inflammatory basic diseases. If cerebral vessels are invaded by bacteria, wall structure, tissue reaction and clinical manifestation are dependent upon the virulence of the pathogens an the defense power of the patient. In case of clinically manifest infectious processes and subarachnoid haemorrhage are coinciding, an inflammatory affection of cerebral vessels should be included in the scope of differential-diagnostic considerations. The establishment of correlations becomes more difficult in case of an apparently inconspicuous interval of several months. That neglected symptoms of an (extracerebral) infection can be in direct connection with cerebral aneurysms haemorrhages is shown by the description of a case with cerebral mycotic aneurysms after endometritis not having been described in the literature until now.

[Lipomatous hamartoma of the brain. Contribution to the problemacy of intracranial lipomas (author's transl)].

On the basis of an own observation of polyposis coli combined with lipomatous hamartoma of the corpus callosum problems of the genesis of lipomas of the central nervous system and its coverings are discussed. Colectomy was performed in a 37-year-old man because of polyposis coli. His sister suffers from the same disease. The patient died 25 d after the operation as a result of diffuse peritonitis. As an incidental finding, the autopsy revealed a lipomatous hamartoma with a diameter of approximately 1 cm. Histologically the growth showed the typical structure of a lipoma and contained a remarkably high number of blood vessels with proliferations of the intima and fibrosis of the walls. At the periphery small calcifications were demonstrated. In the adjacent brain tissue angiomatous proliferations of blood vessels were observed. Furthermore, disturbances of the gross structure of the gyri cinguli and other gyri of the median surface of the brain were found. The histological examination discovered marked irregularities of the single layers of the cerebral cortex. It is suggested that developmental and genetic factors might play a significant role in the genesis of lipomatous tumors of the CNS.

[Neuropathological findings in Lesch-Nyhan syndrome (author's transl)].

The Lesch-Nyhan syndrome is commonly regarded as a metabolic disorder of the purine metabolism without specific morphological changes. In the present paper a report is given on a 13-year-old boy. The neuropathological investigation revealed PAS positive deposits in the ganglion cells of the nucleus olivaris. In HE preparations, the storage material appears as light foamy accumulations displacing nuclei to the cell border. The deposits are sudan-III-negative and there is no UV fluorescence. Electron microscopically the primarily formol fixed tissue of the nucleus olivaris shows circumscribed accumulations of a relatively homogeneous substance with medium density in the dilated smooth endoplasmic reticulum. The diameter ranges from 476 to 850 nm. In this connection, it seems to be possible that the lack of cGMP might lead to disturbances of the protein metabolism of postsynaptic structures, the significance of which is discussed. It is suggested that further investigations of the CNS in Lesch-Nyhan patients should be focussed on the nucleus olivaris.
