[Value of early diagnosis of urinary tract infections in pregnancy].

Clinical report of severe nephrological and urological complications in pregnancy leading to the importance of early diagnosis of urinary tract infections in pregnant women. In this respect the central point is looking for asymptomatic significant bacteriuria. Microscopical methods and chemical practices are to be abandoned. The method of choice is quantitative urine culture. Early diagnosis and treatment of clinically symptomless infections in pregnancy is one of the fundamental criteria of quality in prenatal care.

Motor performance achievements in rats of different ages.

Motor performance of rats of different ages was determined in a cross sectional study. The design includes a test battery of seven motor tasks of graduated complexity. The results show a hierarchical order of impairments of motor functions in aging rats; that is, the more complex the task requirement, the earlier and more pronounced is the susceptibility to deterioration of motor coordination. In spontaneous activity and swimming no difference could be observed between young and old rats. With increasing requirements for the tilting-plane, horizontal wire, climbing and chimney tests as well as the rotarod test, the older rats show a differentiated decrease in performance. The findings are discussed in respect to the theory that aging is a reverse process of early development.

DBH inhibitory factors of brain tissue.

In vivo turnover of NA in the brain is considerably high, but in vitro only a low activity of the enzyme DBH is detectable. It is supposed that this effect is brought about by endogenous enzyme inhibitors. It was demonstrated that the addition of rat brain homogenate, as well as different subcellular fractions from rat brain, inhibit the human serum DBH. Inhibitory activity is resistant to heat and acid but does not seem to be uniformly distributed within the cell. Pretreatment of brain homogenate with pronase reduced its inhibitory activity. In conclusion, pronase sensitive peptides are partially responsible for the inhibitory potency of tissue homogenates.

Nitrate in drinking water in the West German wine-growing areas of Baden and Württemberg.

Nitrate is regarded as a substance whose presence in drinking water is not considered as desirable. If reduced to nitrite it may cause methomoglobinemia in infants. Onward reaction with amines capable of coupling will result in the formatin of nitrosamins. Excessive use of fertilizers in agriculture is the most frequent cause of elevated nitrate contents of drinking water. To quantify this problem, drinking water was sampled at taps at 200 sites arranged on a grid covering the wine-growing areas of Baden and Württember. These samples were examined for their content of nitrate, chloride, and sulphate by means of ion chromatography. Clearly elevated nitrate contents were revealed by the resuls of measurements: 13% of samples showed values above the maximal concentration admissible under the EC Drinking Water Directive of 50 mg NO-3/l and 42% above the EC guide level. The excessive levels were mostly found in the regions of Kaiserstuhl-Tuniberg-Markgräflerland and Württembergisch Unterland. Many samples with an elevated nitrateacontent exhibited also elevated sulphate contents. The high nitrate levels are attributed to the use of nitrogen fertilizers in the vineyards.

Sonography of vesical and perivesical abnormalities in children.

Pelvic sonograms of 40 children who had abnormalities related to the bladder and perivesical region were reviewed. The lesions detected were categorized into three groups, intramural, intravesical, and perivesical; in each category, conditions not related to vesical or perivesical anomalies that could mimic such anomalies were encountered. Real-time ultrasonography was most helpful in identifying ectopic ureteroceles. Aperistaltic loops of bowel were found to mimic perivesical cysts.

[Technical and clinical aspects of gynaecological laparoscopy].

Described in this paper is experience so far gained from the use of Menken's technique for diagnostic laparoscopy. Particular reference is made to certain peculiarities of the technique, such as puncturing direction parallel to the vertebral column. At the Gynaecological Hospital of Cottbus Region, Diagnostic laparoscopy had been applied to 193 patients, between May 1st, 1978 and December 31st, 1980. Sterility (25,38 per cent) and suspicion of extra-uterine gravidity (17.62 per cent) had been the most common indications. - The laparoscopic findings are reported. Methodical problems and complications are mentioned and discussed. They accounted for something up to 4.66 per cent among the author's patients.

Operant thermoregulation of rats with anterior hypothalamic lesions.

The relative importance of different effector mechanisms of thermoregulation may change depending on their availability. Intact rats make only limited use of a learned response on a cold ambient temperature stimulus, and rely almost entirely on autonomic regulatory functions. After destruction of the anterior hypothalamus, rats exhibit a reduced thermoregulatory capacity; i.e. body temperature drops in the cold and rises in the heat. Under this situation a conditioned operant behavior (lever pressing for increasing or decreasing ambient temperature) becomes an important factor to keep body temperature almost constant. Receptor blockers of some putative transmitters in central thermoregulatory pathways influence thermoregulation. Phentolamine induces hypothermia in intact rats in the cold. Hypothalamic lesions are additive in effect with with the drug. Pimozide has no effect neither in the cold nor in the heat intact and lesioned rats. Biperiden in the heat reinforces hyperthermia in intact and lesioned rats as well; in the cold the drug is ineffective. Performance of lesioned rats in an operant pain titration procedure does not differ from intact rats.

Stimulation of gallbladder fluid and electrolyte absorption by butyrate.

Gallbladder fluid and electrolyte transport was investigated in vitro. In guinea pig gallbladder, equimolar substitution of acetate, propionate, butyrate or valerate for HCO3 was increasingly effective in stimulating fluid absorption. The stimulatory potency of these compounds was a function of their chloroform water partition coefficients. The stimulatory effects of the isomers isobutyrate and isovalerate were less than predicted from their partition coefficients. Acidification of the gallbladder lumen, however, was strictly dependent on the partition coefficients for all of the above fatty acids. Unidirectional 22Na fluxes were measured in rabbit and guinea pig gallbladders under short-circuit conditions. In the presence of butyrate stimulation of net Na flux was due entirely to an increase in the mucosal-to-serosal Na flux. Stimulation by butyrate was abolished by its omission from the mucosal bathing solution. The transepithelial electrical potential difference in both rabbit and guinea pig gallbladder became more lumen positive following mucosal but not serosal addition of butyrate. Net 14C-butyrate fluxes were too small to account for stimulation of Na absorption in either species. Butyrate stimulation of Na absorption by guinea pig gallbladder was abolished by increasing the bathing pH from 7.4 to 8.1. Tris buffer (25 mM) partially inhibited butyrate-dependent gallbladder fluid absorption by rabbit and guinea pig at pH 6.4 and 7.0, respectively, and completely at pH 8.4. These results reveal a marked similarity between butyrate and HCO stimulation of gallbladder NaCl and fluid absorption. The results are best explained by a double ion-exchange model, in which butyrate (HCO3) in the mucosal solution acts to maintain the intracellular supply of H+ and butyrate (HCO3) for countertransport of Na and Cl, respectively.

[Indication for caesarean section for multiple pregnancy (author's transl)].

Indications for caesarean section in 213 cases of multiple pregnancy were retrospectively analysed for a period between 1969 and 1980. The average rate of caesarean sections was 6.1 per cent, almost identical with that of monocyesis (5.9 per cent). Perinatal mortality, in particular that of the second twin, may be reduced by expanded indication for caesarean section to cope with malpresentation in cases of multiple pregnancy. Caesarean section is recommended as the primary approach to multiple pregnancy beyond the 34th week with three or more newborns.

[Laboratory animal models to test the efficacy of geriatrics (author's transl)].

The main effect of drugs called geriatrics should be improvement of brain functioning in the aged. Testing chemical substances in laboratory animals is a necessary prerequisite in the development of new drugs. These findings are becoming especially useful when they are interpreted in terms of a valid model. It is believed that hypoxia is often the cause of diminished brain function in the aged. Taking hypoxia as an example it is attempted to find a basis for establishing an animal model which could serve to test geriatrics.

[Prophylactic antibiotics in Cesarean section (author's transl)].

Report of maternal infections by 498 abdominal deliveries in the years 1968--1975. Three groups of patients were analyzed retrospectively: 1. group: patients without antibiotics; 2. group: patients with prophylactic antibiotics; 3. group: patients with secondary dose from antibiotics. - The maternal infections was lower significant in the second group. Although prophylactic antibiotics by cesarean section non indicated because of an ascent from hospital infections.

[Indications for caesarean section in modern obstetrics (author's transl)].

A retrospective analysis was made of deliveries over three decades (1946 to 1977). The rate of caesarean sections went up from 1.9 per cent in 1946 to 8.1 per cent in 1977. Such higher frequency was attributable to various neonatal indications. For example, breech presentation was responsible for an upswing from zero to 26.9 per cent. Maternal morbidity accounted for 31.2 per cent in 1977.
