[Sialic acid in the serum--an indicator for malignant growth?].

Changes of the sialic acid content of patients sera reflect growth processes of benign and malignant character. Using an enzymatic test the value of sialic acid as a primary diagnostic parameter has been investigated. We found a poor diagnostic sensitivity with exception of lung and ovarian carcinomas where the sensitivity attained nearly 90%. In monitoring of patients with pre-therapeutically elevated sialic acid content in serum dropping values indicate a successful therapeutic intervention. An increase of post-therapeutic values demonstrates recurrent processes but the reaction of the serum level may be delayed up to 2 weeks. The outcome of the studies supports the notion that sialic acid is a tumor marker of very limited value.

[Cesarean section and perinatal mortality].

Based on a multicentric prospective study 727 cases of caesarean section (C.S.) in 13 departments of gynecology and obstetrics in 1987 will be examined by means of computer aided analysis. The rate of C.S. was between 6.5% and 13.4% in the obstetrical centres, and between 2.6% and 6.7% in the general district hospitals. There isn't a defined correlation between the rate of C.S. and perinatal mortality in one department. The increased C.S. rate of the centres is based on more neonatological indications like prematurity, perinatal infection risk and traumatic hazard. Perinatal mortality and C.S. rate are only comparable under consideration with all cases of deliveries especially with frequency of obstetrical high risk cases in a department. The high C.S. rate of first obstetrical centre in comparison with the second one is connected with reducing of perinatal mortality of newborn delivered by C.S. to about the half. Therefore we suggest the caesarean section-index of perinetal mortality. It is the relation between perinatal mortality of newborn delivered by C.S. and the total perinatal mortality.

Experimental infection of rhesus monkeys with SIV isolated from African green monkeys.

To establish an animal model for AIDS, 6 juvenile rhesus monkeys were infected intravenously with cell-free SIVagmTYO-1, 5, or 7. One animal was infected with SIV of known pathogenicity isolated from a sooty mangabey (SIVsmm). The 2 animals infected with TYO-1, 1 of 2 infected with TYO-7 and the 1 infected with SIVsmm seroconverted within 4-8 weeks after infection and infectious virus could be recovered 8-10 or 44 weeks after infection. Three of the 4 seroconverted monkeys developed a persistent lymphadenopathy 12 weeks after infection. Rhesus monkeys infected with SIVagm could serve as a model to study HIV infection and for vaccine trials.

[Results of work schedule studies of hospital physicians of the Cottbus district hospital].

The accomplishment of a scientific organisation of work is not an end in itself, but it is an objective necessity in Public Health, too. That is why studies of the working hours are an important instrument of management. At the district hospital Cottbus these studies were carried out not only in nurses, but also in doctors on wards of the clinic for internal diseases, of the clinic for surgical diseases, of the clinic for pediatric diseases and of the clinic for gynecological diseases. The effective action on the patient of ward physicians and interns in these departments amounts to the very balanced daily portion of about 70 to 85% on weekdays. The portion of only 2 to 5% of management of senior physicians in 2 of the 4 inspected departments seems to be too small. The portion of social and scientific work of doctors from several departments can also not be evaluated as sufficiently. The analysis of this pilot study shows a range of important results and conclusions concerning the effective utilization of the working hours fund and the opening of working hours reserves.

HIV-II infection with initial neurological manifestation.

A patient with positive serological HIV-II reactions is presented, who lived for many years in North and West Africa and on the Arabian peninsula, and who developed asthenica, incontinence, gait disturbances, impaired mental function, personality changes and finally spastic paraplegia and peripheral neuropathy. He was shown to have a chronic inflammatory process of the central nervous system with cerebral atrophy.

Solid and papillary epithelial pancreatic neoplasm: an unusual tumor.

Solid and papillary epithelial neoplasms of the pancreas are uncommon. These grow to a large size, are often palpable, and occur most often in young black women. The tumors have a characteristic histologic appearance and a low malignant potential. Heavy tumor calcification is an unusual finding. Two cases with radiologic-pathologic correlation are presented herein.

Diuresis-dependent excretion of multiple forms of renal brush-border enzymes in urine.

The urinary brush-border enzymes alanine aminopeptidase (EC 3.4.11.2), alkaline phosphatase (EC 3.1.3.1) and gamma-glutamyltransferase (EC 2.3.2.2) and their activity pattern of soluble and particulate forms separated by ultracentrifugation were investigated over 8 h after a high intake of fluid (22 ml/kg body wt.). The activity pattern of the two multiple forms was markedly influenced by the urine flow rate. The time-related excretion of soluble forms of these three enzymes increased with the extent of urine output whereas the excretion of particulate form was independent on diuresis. Diuresis-dependent excretion of total activities of brush-border enzymes can be exclusively considered to be a consequence of changed excretion of soluble forms.

Different diuresis-dependent excretions of urinary enzymes: N-acetyl-beta-D-glucosaminidase, alanine aminopeptidase, alkaline phosphatase, and gamma-glutamyltransferase.

We studied how much of the lysosomal enzyme N-acetyl-beta-D-glucosaminidase (EC 3.2.1.30) and of the brush-border enzymes alanine aminopeptidase (EC 3.4.11.2), alkaline phosphatase (EC 3.1.3.1), and gamma-glutamyltransferase (EC 2.3.2.2) was excreted in urine over 8 h after a high intake of fluid (22 mL per kilogram of body weight). The hourly excretion of all four enzymes increased with the increasing urine flow rate. The excretion rate of the brush-border enzymes was more markedly influenced than that of N-acetyl-beta-D-glucosaminidase. By relating the enzyme excretion to urinary creatinine we could reduce the variability of brush-border enzyme output and could completely compensate for the effect of diuresis on the excretion of N-acetyl-beta-D-glucosaminidase.

[Tasks of family-oriented obstetrics in the maternity ward].

Psychological drawbacks of intensive obstetrics on the one hand and the results from psychology in the perinatal period on the other hand demand a new mental hygiene in the delivery room and on maternity wards. The "family-oriented obstetrics" met these problems best. Interviews were made with 650 mothers, just after delivery to find on the attitude for "family-oriented obstetrics". Presence of husband in the delivery room is wanted from 47,4% of the married couple. 98,15% from the mothers wanted nursing and 83,69% rooming-in.

[Increase in serum aldolase caused by bronchial carcinoma].

13 patients were undergone a resection of their lung tumours. Under operation aldolase were determined in V. pulmonalis, A. pulmonalis and V. basilica. First time it is demonstrated: Human bronchial carcinomas give aldolase in circulating blood. Aldolase-level in V. pulmonalis of carcinoma increases with size and dedifferentiation of tumours.

[Complications in pregnancy following abortion induced with PGF2 alpha ].

The influence of induced abortion on the subsequent pregnancy by intermittent intrauterine extraamniotic application of PG F2 alpha was studied in 159 secundigravidae. The results are compared with courses of pregnancy after conventional interruption and after induced abortion by extraamniotic application of glucose (Kovács). The investigations demonstrate the efficiency of cervical dilatation with PG F2 alpha for interruption in primigravidae.

Prostaglandin production in patients with pulmonary embolism.

Release of prostaglandins (PG's) after experimental pulmonary embolism has been reported. Therefore, prostaglandin E2 (PGE2), prostaglandin F2 alpha (PGF2 alpha), thromboxane B2 (TXB2) and 6-keto-prostaglandin F1 alpha (6-keto-PGF1 alpha) were determined in venous plasma of 21 patients with acute pulmonary embolism. Venous plasma levels were followed up for one week after admission. Arterial and mixed-venous PG levels were additionally determined in 6 patients with acute pulmonary embolism prior to pulmonary angiography. Venous levels were substantially elevated (PGE2 16-1300, PGF2 alpha 48-592, TXB2 1-247, 6-ketoPGF1 alpha 1-248 pg/ml), differing significantly from normal controls (p less than 0.001). PG's remained elevated throughout the 7-day postadmission study period. No significant arterial-venous PG differences were detected, though 6-keto-PGF1 alpha and TXB2 levels were somewhat higher in arterial blood. There was no correlation between clinical data (blood pressure, mean pulmonary artery pressure, etc.) and PG levels. These data suggest that elevated prostaglandin levels are probably not, or only in part, responsible for the cardiopulmonary changes that occur in patients with acute pulmonary embolism.
