[Werner syndrome-like disease in 3 brothers. Differential diagnosis of progeria syndrome].

A report is given on a progeroid disease affecting three brothers, 11, 13, and 14 years old. The difficult differential diagnosis of such progeroid syndromes is discussed. The clinical signs were mostly consistent with Werner's syndrome. Since some of the symptoms of this disease were not present, probably because of the young age of the patients, this progeroid syndrome was classified as Werner syndrome-like. Electronmicroscopic findings of the patients skin showed changes of the blood vessels and nerves, which were until now not described in Werner's syndrome or other progerias. The latter findings may give some hints with regard to the pathogenesis of this disease.

[Familial multiple naevogene melanomas. Electronmicroscopical findings in pigmented moles of the B-K-mole syndrome (author's transl)].

Report of a patient with familial melanocytic moles and melanomas as designated by Clark et al. as B-K-mole-syndrome. Beside the typical clinical and histological features, the ultrastructural changes of B-K-moles are described. Nevus cells in these moles showed lobed nuclei, active Golgi complexes and swollen mitochondriae. Membrane rich melanosomes and melanosome complexes were found. The B-K-moles had no distinctive ultrastructure. An ultrastructural differentiation between tumor cells of the B-K-moles and melanoma cells was not possible.

[Atypically localized persistent skin changes in erythropoietic protoporphyria (EPP)--histologic and electron microscopic findings (author's transl)].

The case of a 29 year old man is presented, who showed 'sunburn like' reactions following short exposures to light since early childhood. Demonstration of fluorocytes and analysis of porphyrins confirmed the diagnosis of erythropoietic protoporphyria (EPP). It is noteworthy that the patient failed to show clinical signs, characteristic for EPP at the nasal area or the extensor aspects of the hands, whereas indurated plaques with firm skin coloured papules were observed in the shoulder regions. Biopsies taken from these areas revealed PAS-positive material especially around the capillaries of the dermal papillae, a finding typically for EPP. Electronmicroscopic investigation revealed reduplication of the vascular basal lamina and perivascular deposition of a fine fibrillar material. Therefore the cutaneous changes, yet clinically atypical, seem to be correlated with EPP.

[Electronmicroscopic investigations of lichen sclerosus et atrophicus of the vulva after cryosurgery (author's transl)].

A female with lichen sclerosus et atrophicus of the vulva, which had been treated by intralesional corticosteroid injection and vulvectomy showed again a recurrence of the disease. This time the lesion was treated by cryosurgery. The diseased tissue was investigated by electronmicroscopy during the consecutive phases of the procedure of cryosurgery.

[Histologic and electron microscopical investigations in a patient with benign verrucosis generalisata (author's transl)].

A report is given on a 43-year-old man who has been suffering from pointed condylomata in the region of scrotum and anus since 1975. Repeated relapses. Since 1977 verrucous lesions spreading from thorax to both arms and the throat. Histologic examination revealed intraepidermal foci of balloon cells in the stratum spinosum and stratum granulosum, partly with intranuclear basophilic inclusions. Electron microscopy showed an intact basal lamina and also partly intact epidermis. Only in the focal area the epidermopoiesis is disturbed already from the basal layer, with vacuolated destruction of the basal keratinocytes and condensation of the cytoplasm, as well as formation of myelin figures. This degenerative process together with enlarged intercellular spaces and partly conserved desmosomes continues into the stratum granulosum. From the upper stratum spinosum balloon cells characteristic for warts are found. In the stratum granulosum there are abundant atypic keratohyalin granules, and in the stratum granulosum and stratum corneum characteristic virus-containing kerationcytes are found. Furthermore, there are lots of released virus elementary bodies deposited in bizarre shapes at the horny lamellae: crystalloid or garland-, chain-, letter-, or wreath-shaped.

[Development and functional morphology of the Weibel-Palade-bodies in endothelial cells (author's transl)].

Our results presented indicate a transformation of multivesicular bodies from the Golgi-apparatus into Weibel-Palade-Bodies (WPB). A morphological series of different developmental phases of the WPB can be demonstrated by an increase of a certain intervesicular electron dense material. There is a striking coincidence with early developmental phases of myocardial Z-band material. The WPB are directly related to the microfilament system of the endothelial cells. The capillary endothelium of diseases and tumors of the human skin show a drastic reduction of the WPB-microfilament system towards the central parts of the tissue changes; this is accompanied with a simultaneous reduction of the pinocytotic activities. The endothelial cells are transformed into a conical shape with nearly an occlusion of the capillary lumen and an opening of the interendothelial junctions. This is interpreted as a loss of tone rather than a contraction of endothelial cells.

[Significance of multivesicular bodies in synthetic cell processes].

Phosphatase-positive and -negative multivesicular bodies (mvb) are discernable by ultrastructural and ultracytochemical findings. The phosphatase-negative mvb are actually not considered to belong to the group of lysosomes. Because, they are obviously involved in synthesizing the cell-specific lamellated bodies (i.e. multilamellated bodies of type II alveolarepithelium cells, keratinosomes, praemelanosomes). By studying the literature about ultrastructural findings in tumors we found that mvb are generally increased in number in tumor cells. The changes in structure of the tumor cell membrane might be interpreted in a way that the synthesizing ability of mvb would generally provide phospholipoproteins to adjust a structural steady state of cell membranes. Our findings suggest that the synthetical activities of the mvb are coupled with multi-enzyme complexes of the vesicle membranes. Obviously, these membranes originate fro the Golgi-apparatus being closely related to the smooth endoplasmic reticulum.

[Synthesis of elastoidic fibers in the organ culture of human dermis (author's transl)].

Organ cultures of human dermal excisions placed on the chorioallantoic membrane (CAM) of incubated chick eggs showed that a elastoidic fiber type could be achieved under experimental conditions within 5 h. The stages leading up to fiber formation could very well be produced by fibrocytes as well as by the splitting of collagen fibrils. The microfibrils (lacking a periodicity and with a diameter of 60-100 A) are characterized by a strong capacity to bind carbohydrate-proteinrich complexes (proteoglycans and glycoproteins). These then aggregate to elastoidic fibers. The amorphous fiber center remains strongly attractive for binding cations. This fiber type is then compared to the (elastoidic) fibers present in senile Elastosis and Pseudoxanthoma elasticum. Finally, the possibility of using CAM-cultures as experimental means for simulating such diseases is discussed.

[Information regulation in the vegetative periphery].

The importance of the proteoglycans (PG) of the extracellular matrix in regulating the information reaching the vegetative periphery is demonstrated. Information brought to the vegetative periphery by autonomic nerves and the microcirculatory system are coded by the PG, thereby changing their biochemical structure. These changes (i.e. degree of polymerisation, capacity of ion-exchange and water-binding, electrostatical tonus) stimulate via the specific cellglycocalyx the cAMP-adenylcyclase-system of the cells in the vegetative periphery. In this way the information becomes decoded and is followed by the enzymatically steered answer of the cell. The center for all of these relations is the fibrocyte. The fibrocytes form the metabolic active center in the vegetative periphery and answer to "informative" changes in the extracellular matrix with a situation-dependent synthesis of PG. The possibilities of modulating the intercellular transfer of informations --- various cell contacts, temporary intercytoplasmic connections (fusomes), the role of mastcells.

Origin and function of 'rod-like structures' in mitochondria.

Both organ culture (choriallantoic membrane) of striped muscle from mouse diaphragm and freeze-drying of myocardial cells presented very characteristic structures in mitochondria. The organ culture evidence 'rod-like structures' in the intracristal spaces of the cristae mitochondriales. It is believed that these structures are the central apparatus responsible for mitochondrial division. On the other hand, the freeze-dried mitochondria presented neither intracristal spaces nor a space in the outer mitochondrial membrane. Because these spaces appeared only after fixation and dehydration, it is discussed if they are really artifacts. The mitochondrial cristae appearing as united structures after the freeze-drying process have been compared to the rod-like structures appearing intracristally after organ culture and conventional preparation techniques.

[Cancer en cuirasse, induced by a malignant mixed salivary gland tumor].

A case of cancer en cuirasse is reported, deriving from a metastasizing malignant mixed tumor of the lacrimal gland. Based on histological criteria, the primary benigne prognosis of mixed tumors of lacrimal glands is discussed. According to sensitivity testing to cytostatic agents treatment with 375 mg of bleomycin achieved a recurrence-free interval of eight months. Pathogenesis of secondary effects are discussed (so-drug-induced called bleomycin-lung).

[Viral acanthomas and specialized forms of keratinosome "membrane coating granules" (author's transl)].

In the case of viral acanthomas, the stratum spinosum and granulosum presents ballooned cells which contain all transitional stages from multivesicular bodies (MVB) to keratinosomes. A particularity in condylomata acuminata are the "wagon-wheel" bodies. These structures are typical for the non keratinazed squamous epithelium. The participation of intercellular extruded "wagon-wheel" bodies, MVB and atypical keratinosomes on an irregular baso-apical diffusion-barrier in the epidermis of cases with viral acanthomas has been discussed. On the basis of the relation seen between MVB and the Golgi-apparatus, their transition to partially atypical keratinosomes in cases of viral acanthomas and their "expulsion" into the intercellular space could indicate that in keratinozytes the enzymatically regulated feed-back between the cellular surface and the capability to synthesize is changed by viral agents. The interference appears to manifest itself in the Golgi-apparatus and also appears to be "specified" by the terrain present.

[Experimental investigation of thoracic aorta myocytes in mice. Contribution to the problem of the cause of arteriosclerosis].

The reactivity of myocytes has been ultrastructurally investigated. Pieces of aorta thoracic from mice were organ-cultured on the chorioallantoic membrane of chicken eggs incubated for 14 days. The outstanding finding were individual myocytes of the media--probably in a particular cytoenergetic stage--undergoing necrosis. They were enclosed from almost unaffected myocytes. Transferred to in vivo conditions, these findings may serve as a model for a participation of myocytes in the devleopment of disseminated multifocal arteriosclerotic plaques. The reactivity of the myocytes as to their ability of proteoglycan and lipid synthesis was opposed to Benditt's hypothesis of the genesis of an arteriosclerotic plaque caused by a monoclonal cell line originating from a mutated myocyte of the artery wall near the site of the plaque. The hypothesis of Kaunitz has been discussed, according to which the high serum cholesterin level with severe arteriosclerosis may be rather a consequence of cholesterin synthesis by the 'reparative' tissue of the arteriosclerotic lesion than its reason.
