[Target cimetidine treatment of peptic ulcer. Considerations and assessment of a new dosage].

32 patients suffering from peptic ulcer have been selected. Of these 12 formed part of a previous series treated with cimetidine and subsequently relapsed; the other 20 were selected on the basis of endoscopic and functional examinations and were treated for the first time with cimetidine. The drug doses used were 1.2 g for 30 days and 0.3 g for a further 60 days. The purpose of the study was to check for possible recurrences by comparing the results obtained and their maintenance in the long term using these doses. 2 months after suspension of maintenance treatment there has not been any recurrence of the acute pain and dyspeptic symptomatology.

Partial trisomy 4q: two cases with a familial translocation t(4;18)(q27;q23).

Clinical and cytogenetic data of two related patients, both trisomic for the segment 4q27 to qter, are reported. Familial studies determined that the mothers of the two probands were carriers of the same balanced translocation between chromosomes 4 and 18. Altogether, two partial trisomies 4q, five balanced karyotypes, and one 45,X0 karyotype were found in the family. The 18 cases reported to date are reviewed with respect to the karyotype-phenotype correlation.

Impotence caused by pudendal arteriovenous fistula.

Partial impotence in a twenty-year-old was found, on angiography, to be the result of an arteriovenous fistula of the internal pudendal artery. This condition was corrected by corpus cavernosum revascularization.

Reimplantation of the mandibular condyle in cases of intraoral resection and reconstruction of the mandible.

A new technique for the reconstruction of the disarticulated ramus is presented. From an intra-oral approach the mandible is resected, and if the posterior part of the ramus and the neck of the condyle are not involved by the tumour, this part is preserved. It is then fixed to the bone graft from the iliac crest bridging the remaining defect. The procedure is outlined, based on the experience of 4 cases which have been successfully operated on according to this technique. The earliest of them has been followed-up for seven years.

Paraparesis in hereditary multiple exostoses: case report.

The authors report a case of hereditary multiple exostoses (HME) with neurologic complications, and review the literature. A 23-year-old man exhibited a worsening spastic paraparesis with sphincter dysfunction. The cranial nerves and the exteroceptive and deep sensations were apparently undamaged. The family history, the physical examination, and the systemic radiologic examination revealed all the characteristics of HME. The neurologic complication was caused by an exostosis, arising from the C2 right hemilamina, compressing the spinal cord. The patient quickly improved after a laminectomy.

[Assay of antithrombin III and soluble fibrin complexes in the study of blood thrombophilia (author's transl)].

The authors conducted a series of experiments concerning the condition of hypercoagulability of patients with a history of thrombosis, in view of determining whether or not the risk of thrombosis could be assessed by laboratory tests. To that end they assayed antithrombin iii activity and soluble fibrin complexes in their test subjects. Antithrombin activity was assayed by a chromogenic method; soluble fibrin complexes by the hemoagglutination test. The activity of antithrombon III was significantly reduced in patients with venous thrombosis, not so in arterial thrombosis. Testing for soluble fibrin complexes was invariably unrewarding, probably because the thrombosis episodes under investigation were of too long standing.

A case of acute myelomonocytic leukaemia associated with therapy: Feulgen-DNA microdensitometric determinations.

A cytochemical study of a case of acute myelomonocytic leukaemia, which arose after radiation therapy and polychemotherapy for Hodgkin's disease, is presented. The distribution of the DNA content of the circulating blood cells appears to be different from the unimodal diploid distribution reported in the literature for acute myelomonocytic leukamias not associated with therapy. This finding seems to strengthen the hypothesis, already put forward by Marinone et al. (1979), that a particular pattern of DNA content distribution could be characteristic of leukaemias linked to chemotherapy and radiotherapy.

Polymorphism of A, B and H substances in human urine.

The gel-chromatographic behavior of A, B and H substances from urine was examined and compared with that of previously described AHB salivary fractions. Urinary fractions 1 and 2 exhibited a molecular size smaller than those of salivary fractions 1 and 2. In the urine of 52 subjects a polymorphism of A, B and H substances, with four main types, was observed which is independent of the salivary one and of the so-called secretor and nonsecretor status.

Classification and radiographic features of uterine malformations: hysterosalpingographic study.

A series of 13470 hysterosalpingographic investigations demonstrated congenital uterine malformations in 1160 cases (8.6%). Uterus arcuatus and uterine hypoplasia, the latter ranking as a malformation on embryological grounds, together accounted for two-thirds of the cases, uterus unicornis unicollis and uterus bicornis unicollis together for over a quarter and the others, uterine didelphys, uterus bicornis bicollis and uterus septus/subseptus, for much smaller percentages. Each malformation is described and illustrated by typical hysterosalpingograms. In addition to its decisive importance in diagnosis, hysterosalpingography it also valuable in planning surgical correction in many cases.

[Parasites of Salmo trutta L. from the Tirino River. Part IV. Intermediate hosts of Cyathocephalus truncatus (Pallas, 1781) and Dentitruncus truttae Sinzar, 1955].

During studies on helminth parasites of brown trout (Salmo trutta L.) from the river Tirino (L'Aquila - Italy), the intermediate hosts of Cyathocephalus truncatus (Pallas, 1781) and Dentitruncus truttae Sinzar, 1955 have been searched for. A total of 15,444 amphipoda belonging to the following 3 species: Echinogammarus roco Karman, 1973, Echinogammarus tibaldii Pinkster and Stock, 1970, and Gammarus italicus Goedmakers and Pinkster, 1977, have been dissected. E. roco, E. tibaldii and G. italicus were found infected with procercoid of Cyathocephalus truncatus (new hosts record and first procercoid record in Italy). E. roco, E. tibaldii and G. italicus were found infected with acanthella and cystacanth of Dentitruncus truttae (first record of intermediate hosts). Accordingly the ecology of the cestod C. truncatus and of the acanthocephalan D. truttae in their intermediate hosts has been studied considering principally the incidence and the seasonal occurence of the parasites.

Vascular trauma observed at an urban hospital center.

The records of 50 patients with 67 major civilian type vascular injuries treated in a large urban center from 1967 to 1975 were reviewed in terms of nature of the injury, form of surgical management and early and late results. Civilian injuries differ in many aspects from those of their military counterparts and often require different treatment. In both, however, treatment protocols should be periodically re-evaluated for a number of reasons, chiefly among which are changing patterns of delivery of the injured patient to the medical center and advances in technical skills. Notable in the presently reported series were the low mortality and the absence of a need for amputation, with a limb salvage rate of 100 per cent. Despite modern advances in technical proficiency in vascular reconstruction, there is still a place for careful vascular ligation in the definitive surgical management of major vascular trauma.
