Concomitant incidence of fertility chance reducing varicocele and chromosome aberration.

Authors describe the case of a male patient with varicocele, oligozoospermatism and subfertility, a 46 XY t (8; 10) (q22; p15) balanced chromosome carrier. Following varicectomy, successful pregnancy has developed even with balanced translocation meaning only 50% fertility chance. Prenatal diagnostics by chorion biopsy showed a fetus of normal chromosome constellation. Authors call attention to the fact that even minor, step-by-step improvements in fertility chance may lead to success in multiple aberrations.

Vacuum therapy in the treatment of erectal impotence.

The authors were the first in Hungary to use vacuum-induced erection in the treatment of erectal dysfunction. Negative pressure was induced by OSBON ERECAID SYSTEM. Erectal impotence was successfully treated with this non-invasive and complications-free method in patients with diabetes and psychic problems. The method was applied first for ameliorating sexual complaints due to virile climacteric and--based on the results--its application is recommended.

Betaloc therapy in pregnancy-associated mild and moderately severe hypertension.

In a prospective study the effect of Betaloc therapy on pregnancy-associated mild and moderately severe hypertension has been examined. Besides the effects on the mother the changes of foetal heart rate were also observed. The antihypertensive therapy was performed with daily 3 x 100 mg (3 x 1 tab) oral doses of Betaloc in 30 pregnant women with 140-170 mmHg systolic and 90-110 mmHg diastolic blood pressure values at admittance. In the course of the examinations definite and statistically significant fall of systolic flood pressure could be observed following the onset of Betaloc therapy as early as on the first day of drug administration which could be maintained during therapy. Somewhat less definite but the same change was observed in the diastolic blood pressure. Maternal pulse rate was significantly higher before therapy than after finishing Betaloc therapy. Notable foetal bradycardia was not observed, in the course of Betaloc administration the decrease of foetal heart rate did not surpass the normal limit values. During the therapy no side-effect attributable to the drug was observed.

[Incidence over 10% of premature deliveries in Hungary (based on the experience in Borsod-Abaúj-Zemplń County)].

The incidence over 10% of premature delivery in Hungary was studied. The cause which motivated this work was the fact that despite of several preventive and therapeutic measures the incidence e.g. in BAZ county surpassed even the average of the country. Professors of clinics in different countries of Europe as well as medical societies dealing with this question have been consulted about the matter of whether it was sufficient to consider a neonate a premature infant and the delivery a premature one solely on the basis of birth weight (under 2.500 g). Data of BAZ county proved that this determination was misleading and proposals are made concerning the modification on the basis of the examinations of the authors.

Testicular volume variations from 0 to 28 years of age.

To determine the standard testicular volume growth, 1985 males aged 0 to 28 years were subjected to measurement by means of Hynie's testometer and the results were related to the body size (testicular volume cm3/Livi index). Data for comparison were obtained from a gypsy and a non-gypsy sample. While at the ages of 0 to 9 years the volume did not show any notable variation, there was a conspicuous and rapid (approx. 10-fold) increase between 10 and 15 years. The maximum size was attained among the non-gypsies at 17-18, among the gypsies at 21-22 years. The average volume for every age group above 19-20 years was higher in the gypsy than in the non-gypsy sample. In each group, from the start of puberty the right testis was bigger on the average than the left.

Results on postcoital and penetration test.

Since adequate transcervical passage of sperm is indispensable to the natural fertilization, the sperm-cervical mucus studies provide a new possibility for investigating the marriages infertile due to unknown causes. The behaviour of sperm in the cervical mucus is also important from the point of view of selecting the mode of insemination. Consequently, the comparative evaluation of postcoital, slide penetration and capillary tests were made by the authors.

Operative andrology.

Surgical corrections of male fertility disturbances are discussed. Following a short historical review, operative treatment of the undescended testicle, the indication and date of surgery and postoperative andrological results are dealt with. Modern principles of varicocoele surgery are discussed with special attention to fertility correction. Surgical treatment of vas deferens obstruction, results and microsurgical interventions concerning vaso-epididymal anastomoses and vasovasostomies are also described together with the ultimate possibilities for surgical therapy of sexual disturbances.

Bone scanning and radiography in the evaluation of patients with malignant lymphoma.

The diagnostic value of bone scintigraphy and radiography in the detection of lymphomatous bone involvement, and the role of bone scintigraphy in the evaluation of lymphomatous bone marrow involvement, were investigated in 41 patients with malignant lymphoma. 10 patients had lymphomatous bone involvement. Whereas scintigraphy detected all the 10 cases, radiography was false negative in 2 cases. The lytic bone lesions on radiography were in most cases not detected by scintigraphy. Scintigraphy is insensitive for the detection of early bone marrow metastases. The simultaneous use of bone scanning and x-ray, however, seems to be helpful in the detection of lymphomatous bone involvement and consequently in the clinical management of patients with malignant lymphoma.

Uroandrology II. Andrological significance of the diseases of the seminal vesicle.

The andrological implications of the diseases of the seminal vesicle are analysed on the basis of the author's clinical experience, with special reference to the symptomatology of circumscribed seminal vesiculitis and to the therapeutic possibilities. A diagnostic scheme for recognizing the disease is recommended.

Uroandrology. I. Andrological importance of epididymal functions and diseases.

The evaluation, from the andrological point of view, of epididymal functions is made with an emphasis on the necessity of incorporating new knowledge into practical andrology. The andrological importance of the most common epididymal diseases and the more important modes of their correction are reviewed.

Diagnosis and therapy of varicocele. Fertility aspects.

Calling attention to the andrological importance of subclinical varicocele, the authors' own operative material is analysed from the point of view of fertility. Problems of surgical indications, with particular regard to the azoospermic and normozoospermic groups, are discussed. Based on their own results, they express their views on the choice of surgical intervention.
