Data on the clinical aspects and therapy of primary gastrointestinal lymphomas.

Fifteen cases of primary gastrointestinal lymphoma diagnosed over 8 years are reviewed. In the period 1980 to 1982 there was a cumulative appearance of GI lymphomas, nine out of 15 cases were diagnosed in that period. According to its localization, lymphoma occurred in 12 cases in the stomach, and in 3 in the small intestines and the colon. One case of gastric lymphoma was Hodgkin type, the others were non-Hodgkin types. The clinical symptoms were not characteristic of lymphoma. The age of the patients was, on the average, ten years lower than the mean age of carcinoma patients. Preoperative diagnosis by gastric biopsy was successful in four cases. In patients with lymphoma of the colon not subjects to surgery, colonoscopy verified the origin of lymphoma. Exact clinical classification in the majority of cases was made intraoperatively. In the non-operated cases, sonography and lymphography were performed. In general, operation was attempted, but patients in stage II, in very poor condition, were possibly not operated. The possibility and indications of the "second look" operation are discussed. Histological typing was made according to the Kiel classification. In the literature, in the most controversial question of therapy, individual consideration of the cases is recommended. Based on our experience, in devising therapy or therapeutic strategy as well as concerning prognosis, the degree of malignancy according to histological type, clinical stage and anatomical localization seem to be the most decisive factors. In exceptionally malignant cases a protocol with doxorubicine + bleomycin + teniposide and prednisolone was applied.

Andrologic aspects of varicocele.

The changes in the spermatogram subsequent to surgery for varicocele because of infertility were examined, consideration being given to the various sperm parameters. Improvement of one parameter or more was demonstrable in 30 out of 40 surgical cases. The parameters indicative of motility showed the most marked improvement. The andrologic significance of early diagnosis is emphasized.

The role of basic amino acids of the seminal plasma in fertility.

The role of basic amino acids in male infertility is discussed. The changes in the arginine, ornithine and total amino acid concentrations of the seminal plasma in various types of pathospermia are commented upon in the light of published evidence and of the authors' own observations. A recent procedure (CV-technique) introduced by the authors into the routine investigation scheme of infertility is reported. The method detects the arginine-deficient type of pathospermia, thus contributing to the possibility of its selective correction.

A rapid, routinely applicable method for the determination of the amino acid content of the ejaculate.

Examination of the amino acid content of the ejaculate was carried out from the point of view of fertility. Following a brief survey of the known examination techniques, the authors report on the experiences gained by the Van Pielsum-Sakagutchi and a thin-layer ion exchange chromatographic methods. The chromatographic videodensitometry is proposed as a suitable routine andrologic examination since it is rapid, simple and requires small sample size.

[Orchidopexy and fertility].

The authors report on experiences which they could prove during the last ten years (1970-1979) in disturbances of a descent of the testis. Particular consideration is given to the orchidopexy in later childhood or adolescence and to fertility. The importance of the right time of operation and the technique of the intervention protecting the tissue are emphasized.

Follow-up examination of patients with undescended testicles.

A detailed follow-up examination was performed in 20 patients who had been operated upon 10 to 20 years ago because of uni- or bilateral cryptorchidism. The youngest patient was 18 years old at the time of the follow-up. The following values were analysed: primary and secondary sexual characteristics, the possible sexual disturbances, penile volume and the diameter of the testes, the serum testosterone, HCG, FSH, and LH level, sperm and immune peroxidase examination, and histologic study of both testes. The absence of sexual disturbances and the normal primary and secondary sexual characteristics were explained by the results of hormonal assays and these also showed that in adults there is no possibility to improve fertility by hormones. Early operation at 2 years of age at the latest is the only possibility to prevent in- or subfertility. The intervention prevents secondary damage to the retained and to the contralateral testicle. Immune peroxidase tests pointed to the possibility of therapy by immunosuppressive drugs.

Modification of mouse immune responses against allogeneic tumor by pretreatment of antigen with mitomycin C or X-irradiation.

Treatment of P815 mastocytoma cells with mitomycin C or X-irradiation prior to their inoculation into an allogeneic host resulted in a shift in the appearance of the peak of the T-killer cell population from day 10 to day 6. This finding is discussed in relation to the caution that must be exerted in extrapolating data obtained in studies in vitro using one-way mixed lymphocyte cultures to in vivo model systems involving proliferating tumor cells as immunogens.
