[Occurrence of Escherichia coli K1 and K5 strains in patients with urinary tract infections].

1499 E. coli isolates from patients with urinary tract infections were investigated for the presence of K1 and K5 strains using specific phages. K1 antigen was detected in 10,5% and K5 in 6,1% of the strains examined. The strains were isolated in two laboratories with different hospitals and outpatient departments. With respect to the isolation area and isolation time the frequency of K1 and K5 strains was found to be different.

Effect of autovaccination on the course of urinary tract infection in animal experiment.

Autovaccination of rats with chronic pyelonephritis carried out approximately two months after the onset of infection does not result in an improved histological picture in the infected but can prevent destructive processes in the controlateral kidney. Cyclophosphamide administered in three doses of 30 mg/kg simultaneously with autovaccination slightly modulates the immune response to the infectious strain. The temporal relationship between immunization and cyclophosphamide administration determines the mode of action of cyclophosphamide. In the present experiment, the concept of Miller has not proved to be applicable. Cyclophosphamide administration causes a distinct increase in inflammatory processes in the kidney. Should an enhancement phenomenon be involved in the bacterial infection of the kidney, of which we have found no proof, cyclophosphamide therapy as it was used in the present study would not result in its removal and thus in improved elimination of the infectious organism. Additional experiments are required to determine whether animals subjected to autovaccination are protected against a new episode of urinary tract infection.

[Serum resistance of Escherichia coli in chronic pyelonephritis. 1. Serum resistance in the human serum pool].

123 patients of the kidney department of the Clinic for Inner Medicine of Rostock University suffering from chronic pyelonephritis were taken into microbiological observation for between one and four years. 170 E. coli strains were bred from 59 patients with significant bacteriuria in the course of the disease and their serum resistence was determined with pooled human serum using Taylor's method. 78.24% of the strains examined were serum-sensitive, 11.18% intermediate and 10.59% serum-resistent. All strains were O-, K- and H-typed. 57.06% were successfully O-typed and were distributed over 40 O-serogroups. 24.12% were not typable and 18.82% were rough colonies. 86.50% of the resistent and intermediate strains strains were O-typable, 13.50% could not be typed. The significance of E. coli antigens (O, K, H) and serum resistence for the maintenance of a chronic infection is discussed.

[Preliminary results of the vaccine treatment of chronic pyelonephritis in animal experiments].

The immunization of rats with a formaldehyde-inactivated vaccine of the homologous infection strain does not lead to an improved tendency for an existing infection of the urinary tract to heal in the primarily infected kidney, but it can prevent it spreading to the other one. The dose is probably significant. In previously published investigations this effect did not occur after six vaccinations (11). In existing infection cyclophosphamide treatment does not prevent any possible enhancement and thus does not improve the elimination of germs.

[Serum resistance of Escherichia coli and serum bactericidal activity in chronic pyelonephritis].

123 patients with chronic pyelonephritis of the renal dispensary of the clinic of internal medicine of the Department of Medicine Wilhelm Pieck University Rostock were controlled microbiologically for one to four years. Of 59 patients of this group with significant bacteriuria we cultivated 170 strains of E. coli, which were standardised O, K, and H. Of all causative organisms the serum resistance was determined in a human serum pool. All sera of patients were examined for their bactericidal activity to the homologous E. coli urine strain as well as to a serum-sensitive E. coli reference strain (E. coli O 22:K-:H+). In 11.8% we found in the sera investigated at the time of the excretion of causative organisms disturbances of the serum bactericidal action, which rendered possible the survival of the sensitive in the human serum pool causative organism. In 8.82% of the sera of patients at the time of bacteriuria there was a decreased bactericidal action against the E. coli reference strain, only two of these sera did also not kill the urine strain of the patients. In individual cases sera of patients were examined for bactericidal activity with the homologous germ after absorption. By this absorption the antibactericidal activity of the sera could be removed. The proof of a disturbance of a bactericidal action may be an important reference for the course of the chronic pyelonephritis.

Urinary tract infectivity or R strains of Escherichia coli carrying various virulence factors.

The virulence factors of Escherichia coli supposed to act in urinary tract infections were studied on R strains in a suckling mouse model. The production of alpha-(diffusible-) haemolysin or the possession of antigen K1 enhanced the virulence significantly, while the type 1 (common) fimbriae failed to do so. An isogenic motile and non-motile pair of E. coli did not show any difference in infectivity in the model. The adhesins, the diffusible haemolysin, and the acidic polysaccharide K antigens (K1) are definitely additive virulence factors in the model. This is in good agreement with the experience of clinical bacteriology.

[The significance of laparoscopic examination in acute adnexitis for bacteriological diagnosis].

Cultures of abdominal bioptic material from 102 cases of acute pelvic inflammatory disease diagnosed laparoscopically revealed that anaerobes were involved in 42.2% of the cases; 18.6% of the cultures were sterile. There were no signs that anaerobes predominate in cases, in which an abscess has already formed. The proportion of predominantly polymicrobial multiple infections was 54.2%, the remaining 45.8% of the cases involving mono-microbial infection. Scrutiny of the abdominal and cervical distribution of the germs in 42 of our female patients confirmed that cervical smears are inadequate for detecting the causal germ involved in a case of acute pelvic laparoscopy it seems advisable to use abdominal cultures of tissue taken from the locality of the inflammation in order to identify exactly the causal germ and to test these cultures also for anaerobes and gonococci.

[The detection of the agent of Legionnaires' disease--a confirmation of Koch's postulates].

The genus Legionella, belonging to the family of Legionellaceae, comprises nowadays seven species, among which Legionella pneumophila might be the most important. The identification of the agent is difficult because L. pneumophila is very pretentious requiring peculiar conditions concerning culture medium, temperature, and time. The initial cultivation will succeed the best in an atmosphere enriched with CO2. The demonstration of serum antibodies will succeed by means of indirect immunofluorescence; recently the micro-agglutination test is vastly applied. The clinical picture of Legionellosis is characterized by an atypical pneumonia with a serious course in most cases. The Pontiac Fever is an illness with milder course than the classical form of Legionellosis. Erythromycin and Rifampicin are the chemotherapeutics of choice.

[Detection of antibody-coated bacteria in the urine of adults with chronic pyelonephritis].

Urine samples of 59 patients suffering from chronic pyelonephritis were tested repeatedly for antibody-coated bacteria (ACB). We demonstrated that the number of positive results increased with the frequency of the tests performed. It was observed, however, that antibody-coated bacteria were not found consistently in every sample during a longer period of observation.

[Identification of the site of block in high degree AV conduction disorders on the surface ECG].

The possibility of identifying the site of block by analysing the surface ECG was studied in 124 patients with 2nd or 3rd degree AV-block. In all cases His-bundle-electrography was performed. With regard to the QRS-duration (both in conducted and in escape beats) and to functional peculiarities of the AV-conduction or of the automatic centres in patients with narrow QRS complexes nearly all cases with the prognostically less favourable His-Purkinje-block are discernible.

[Infections with non-sporing anaerobic bacteria. Diagnostics, therapy and clinical importance].

The increasing significance of infections with non-sporing anaerobic bacteria is a very important problem for clinicians and microbiologists too. The author deals with the properties of anaerobic bacteria and gives a review of our present knowledge in the fields of clinical symptoms, the mechanisms of pathogenicity. Especially, emphasis is laid on the collection and transportation of specimens for anaerobic cultures. Possibilities of rapid laboratory diagnosis are described as well as the exact bacteriological diagnosis and the in-vitro testing in case of antimicrobial susceptibility. Remarks on the clinical significance and chemotherapy of anaerobic infections complete this review.

[The effect of plasmids of various incompatibility groups on the serum sensitivity of Escherichia coli K 12].

Using the method of Henkel and Taylor (11, 18) 60 Escherichia coli K 12 J 53 strains carrying R-plasmids of 15 incompatibility groups (FI, FIme, FII, FIV, FV, FVI, I alpha, I gamma, I omega, I5, B, C, K, M, S) have been tested with respect to their serum sensitivity in pooled human serum. We found for 41 plasmid carrying strains a grade of resistance to serum bactericidal activity of 0, for 19 strains a grade of 1, 2 or 3 according to the method of Henkel. Applying the method the method of Taylor the results of the bactericidal tests of the same 60 strains could all graded in category 1. The results obtained show that there was not any change of sensitivity to serum bactericidal activity after the uptake of a plasmid. 29 strains carrying different plasmids of the same 15 incompatibility groups were selected for studies in a low concentration system of serum. Survivors' curves in 1, 2, 3, 5, 10 and 20% of serum after different times of incubation (10, 20, 30, 60, 90, 120 and 180 min) were established. In relation to the control strain E. coli K 12 J 53 R- a difference in the percentage of survivors of 18 strains carrying plasmids of 10 incompatibility groups (FI, FIme, FIV, FVI, I alpha, I omega, I5, C, K, and M) could be demonstrated. However, this could only be observed in concentrations of serum from 1 to 5% and most clearly after an incubation time of 10 min. The results were graded into 3 categories in dependence of the difference of the percentage of survivors to the control strain. The plasmid pIE 307 and pIE 403 (Inc FI and C) showed the strongest activity in increasing the serum resistance. We found the method used to be suitable for the estimation of small alterations of serum sensitivity. The results obtained in E. coli K 12 made us control their influence in other E. coli strains carrying a complete O-antigen.
