[Trends and new possibilities in the treatment of urinary tract infections].

The immune therapy is an alternative to the classical chemotherapy of chronic urinary tract infections. A number of investigations confirm the efficiency of vaccination in animal experiments. It was shown that an antigen application did not influence negatively chronic pyelonephritis in animals. The immune therapy is international established and each one drug for oral and parenteral use in pyelonephritis are put on the market.

[Excretion of antibody-coated bacteria in chronic pyelonephritis].

Of 168 urine sediments, which were obtained from 55 patients with chronic pyelonephritis in the course of 3 years when a significant bacteriuria with E. coli was present, we demonstrated antibody-coated bacteria in 81 cases (48.21%). In the active stage of the disease with 54.10% were found significantly more than in the inactive with 32.61%. In obstructive chronic pyelonephritis the positive rate was 54.79% in contrast to 43.16% in non-obstructive chronic pyelonephritis. While in the active stage of the obstructive chronic pyelonephritis with 57.41% more antibody-coated bacteria were excreted only ACB+-, 34.29% only ACB-- and 40% ACB+- and ACB--germs in the course of the disease. The ACB-positive quote was in rough forms with 62.50% statistically significantly increased in contrast to 45.54% in O-typable and 42.86% in non-O-typable strains. In the ACB+-group the immunofluorescence titres to the homologous strain and the C3-activator in the serum as well as the urine lysozyme were significantly higher than in the ACB--group.

[Rapid typing of Escherichia coli K antigens using bacteriophages].

E. coli capsular (K) antigens are important virulence factors contributing to the development of urinary tract infections (UTI). Serotyping of these antigens is laborious and depends on the availability of respective antisera which are difficult to prepare because of the low immunogenicity of these polysaccharide antigens. The application of specific K phages are a big advantage. Two collections of E. coli strains (500 from Rostock, 214 from Erfurt) isolated from patients with UTI have been investigated with a set of ten K specific bacteriophages including phi 1, phi 2, phi 5, phi 7, phi 9, phi 12, phi 13, phi 20, phi 30 and phi 36. The K1 antigen has been found to be the most frequent capsular antigen (16.4% in Rostock; 12.2% in Erfurt) followed by K13/K20 in Erfurt (8.9%) and K5 in Rostock (8%) and Erfurt (7.5%). The collection of phages proved to be suitable for the detection of UTI associated K antigens covering the most common O serogroups found in UTI. The method appears to be simple, non-laborious and applicable for routine use.

[Differentiation of coagulase negative staphylococci from significant bacteriuria].

According to our experience in identifying coagulase negative staphylococci (CNS) a modified Kloos-Schleifer-scheme proved useful in species identification. Of 595 urine isolates from cases with significant bacteriuria tested 89.5% could be identified. For routine identification on the one hand the Kloos-Schleifer-scheme is only practical by using commercial identification systems, on the other hand applying an abbreviated scheme species identification is unsatisfactory. As an alternative for identification of CNS in clinical microbiology the Phosphatase-Novobiocin-Mannose-Inhibition-Test (PNMI-Test) is presented. This test in combination with a coagulase test proved suitable to identify the three most important staphylococcal species S. aureus, S epidermidis, and S. saprophyticus and seems to be an acceptable compromise in the bacteriological routine laboratory. For extended species identification of CNS the Kloos-Schleifer-scheme should not be reduced but this identification should be reserved to special laboratories.

Detection of different fimbriae-like structures on the surface of Staphylococcus saprophyticus.

In relation to the ability of adhesiveness to HEp-2-cells the sizes of capsules on three strains of Staphylococcus saprophyticus were examined using an electron microscopic India ink technique. There was a good correlation between small amounts of capsular material and good adherence to the tissue culture cells. In contrast to this a very large capsule could be demonstrated on a strain which adhered only in a poor manner. Some indications of fimbriae-like structures could be confirmed by negative staining. Three different appendages: thick, short sticks, long, branched fimbriae and fine, thin filaments on the surface of S. saprophyticus were detected. The possible functions of these surface structures in the adherence process represented in a hypothetical cell model is discussed.

[O- and H-antigens of Escherichia coli K5 isolates in the urine].

88 E. coli-K5-strains identified by a K5-phage were serotyped with regard to O- and H-antigens and 19 different O:K5:H-serotypes were registered. Further 11 isolates were rough strains and 2 other strains could not be O-typed. The following frequency of O:K:H-serotypes (number) was determined: O75:K5:H- (21), O18:K5:H- (11), O6:K5:H1 (9), and O18:K5:H1 (6). Seven other O:K:H-serotypes were detected once only. The results of typing are different in dependence of the geographic area and the time of isolation, respectively.

[Detection of fibria-like surface structures in Staphylococcus saprophyticus as a pathogen in chronic pyelonephritis].

It is reported on a 25-year-old female patient suffering from chronic pyelonephritis. Six times coagulase-negative staphylococci were bacteriological proved in a significant number. Four strains were identified as Staphylococcus saprophyticus. The in each case in the autumn occurring complaints could be correlated well with the seasonal fluctuations of urinary tract infections by S. saprophyticus as described in the literature. Chemotherapeutic measures be determined by the clinical symptoms and subjective complaints of the patient. By an antibiotic therapy only a temporary loss of complaint and normalization of urinary findings, respectively, were obtained. In the presence of a marked leukocyturia a capsula-fashioned variant of S. saprophyticus was established. By scanning electron microscopy investigations fimbria-like surface structures were detected for the first time in this germ species. Three morphological different appendage structures are described. By a hypothetical model possible functions of these structures in the adherence process are discussed.

[Effect of immunization with Escherichia coli J5 on the course of experimental pyelonephritis in rats].

Among the germs of urinary tract infections is a great heterogenicity. Therefore, in search of vaccine candidates of pyelonephritis such germs must be checked the antigens of which to be occurred as much in other pyelonephritis germs as possible. E. coli J5 as a re-mutante of E. coli O111 has a core-antigen which is found in the most enterobacteria. We have examined whether a pyelonephritis in rats was prevented by prophylactic use of a E. coli J5 vaccine and if the course of the disease was influenced by vaccine treatment of animals with pyelonephritis, respectively. Neither a pyelonephritis was prevented nor an existing infection was influenced favourable by the vaccine of E. coli J5.

Effect of immunization with Escherichia coli K1 antigen on the course of experimental infection of the urinary tract of the rat.

The influence of vaccination with an immunogenic K1 antigen of Escherichia coli on the course of pyelonephritis induced experimentally in rats by infection with Escherichia coli O2:K1:H4 was investigated. The immunogenic properties of K1 antigen are poor. A conjugate of K1 antigen with bovine serum albumin (BSA) was therefore tested. Preliminary experiments showed that the K1-BSA conjugate is immunogenic. Renal inflammatory activity in rats after retrograde infection was significantly reduced after vaccination of the animals with K1-BSA conjugate within six weeks of infection.

[Modification by cyclophosphamide of the effect of autovaccination in chronic pyelonephritis in an animal experiment].

In an existing chronic pyelonephritis the influence of cyclophosphamide on the efficacy of an immunisation with autopathogenic antigens and the surface antigen of an E. coli rough strain was tested in an animal experiment. After an existence of the infection of about five weeks a decisive decrease of the antibody production by cyclophosphamide was not to be observed. The animals treated with cyclophosphamide did not show any improved elimination of germs. The histological findings of the kidneys documented the cytotoxical effect of the cyclophosphamide on the renal tissues and the epithelium of the urinary tract. The immunisation of animals with chronic pyelonephritis with various autopathogenic antigens altogether led to a statistically significant decrease of inflammatorily changed kidneys.

[Incidence of B streptococci in gynecologic patients, pregnant patients and newborn infants].

821 patients have been examined with regard to the presence of B streptococci. We isolated B streptococci in 5.4 per cent in gynaecologic outpatients, in 0.4 per cent in 236 pregnant women and in 5.8 per cent in 156 newborns. From 78 to these newborns we were able to examine the mothers during their stay in the delivery room, too. Only in one parturient and her newborn we could detect B streptococci. In three B streptococci positive newborns is was not possible to prove this germ in the vaginal content of their mothers. Clinical symptoms were not present in no case. Antibiograms were not peculiar.

Stimulation of particle-induced chemiluminescence in human granulocytes by various Escherichia coli strains.

The patterns of neutrophil chemiluminescence stimulated by groups of E. coli strains opsonized with pooled normal human serum were compared. All strains of E. coli were obtained from patients with chronic pyelonephritis. 'O' serovars which lacked capsular (K) antigen produced significantly higher chemiluminescence than strains possessing a variety of K antigens. Chemiluminescence produced by a range 'O' serovars bearing the K1 antigen did not differ significantly from those containing other K antigens. The lowest chemiluminescence values were obtained with a group of O2:K1 strains. Rough strains containing K1 antigen were also poor stimulators of chemiluminescence.

[Effect of immunization with K1-antigen of Escherichia coli on the course of experimental urinary tract infection in the rat].

The influence of the vaccination with an immunogenic K1-antigen of Escherichia coli on the course of an experimental pyelonephritis in rats due to Escherichia coli O2:K1:H3 was investigated. K1-antigen is a poor immunogen. Therefore a conjugate of K1-antigen with bovine serum albumin (BSA) was used. This K1-BSA-conjugate was immunogenic in preliminary experiments. The influence of a vaccination on the course of pyelonephritis may be dependent on the route of infection. Rats which are infected by a retrograde route shows significant less inflammated kidneys when they received a vaccination by K1-BSA-conjugate within the first six weeks of infection.
