[Ependyma cyst of the 3d ventricle, causing sudden death].

A case of ependymal cyst of the third ventricle in a 18 years old female causing sudden death is reported. The patient had no typical clinical symptoms. Literary data dealing with the origin and clinical symptoms of the ependymal cysts are reviewed. It is assumed, that ependymal cyst is a congenital fissular tumour.

[Changes in the lecithin-sphingomyelin relationship in the amniotic fluid following the effect of dexamethasone].

Amniotic fluid samples were taken by transabdominal amniocentesis from 22 pregnant women in the 32-37th gestation weeks. Determination of lecithin/sphingomyelin ratio showed that in 20 cases the L/S ratio was 2 or less than 2. The patients were treated with i.m. Dexamethason in a total dose of 0,3 mg/kg body weight. Subsequently the L/S determination was repeated. Dexamethason treatment resulted in a significant increase of L/S ratio. In 3 cases out of 22 the L/S ratio was below 2. No neonate developed RDS from this patient material. The authors are convinced that the risk of RDS is an indication for amniocentesis. If the L/S ratio equals 2 or is lower than this value, steroid prophylaxis is recommended.

[Relationship of time interval between steroid therapy and labor to the incidence of respiratory distress syndrome].

The authors used Dexamethasone before delivery in 577 cases of pregnancy. There were 460 prematurities. Occurrence of RDS was significantly lower in cases, where delivery took place more than 48 hours after the steroid treatment. Where the steroid effect was not optimum (48 hours), the RDS was similar to that of the control group, although mild in form. In their studies they found no connection between the tocolysis (Partusisten) and the occurence of RDS. They emphasize that in the cases where respiratory insufficiencies are predisponated (elective caesarean section, diabetes mellitus etc.) the steroids significantly reduce the frequency of the disease. Neither the number of apoplexy nor that of death due to infections increase in dexamethasone treatment.

The use of ion exchange chromatography for demonstration of rubella-specific IgM antibodies.

IgM and IgG immunoglobulins of human sera were separated by stepwise column chromatography in QAE-Sephadex A-25 ion exchanger gel bed. The procedure resulted within 30 min in a fraction suitable for direct titration of rubella-specific IgM antibodies by haemagglutination inhibition test. The method proved to be a useful diagnostic tool for primary rubella. Serum samples of 13 individuals with previously acquired immunity, 152 patients with a recent rubella-like illness, and 194 pregnant women exposed to rubella infection were tested for the presence of rubella-specific IgM antibodies. Sera of individuals with previous immunity proved to be negative for specific IgM antibodies. Specific IgM titre was demonstrated in the blood of all the 25 patients with significant titre-rise tested because of rubella-like illness, and also in the sera of additional 8 patients whose serum samples were taken too late for demonstration of a rise in titre. Significant titre-rises were found in 5 women exposed to rubella infection, but only two of them exhibited rubella-specific IgM antibodies. The absence of specific IgM antibodies refers presumably to subclinical reinfection in the other three cases.

[Prevention of respiratory distress syndrome in pregnant diabetic women during labor].

In the period between 1973 and 1976, in order to prevent neonatal RDS, 24 insulin-dependent diabetic mothers were given Dexamethason before delivery. The treated group was compared with a control group comprising 26 women of the same gesational age, severity of diabetes and mode of delivery. Neonatal care was similar in both groups. In the group where steroid prophylaxis was applied, decrease of RDS morbidity and mortality was observed. The authors accentuate that Dexamethason given to the mother may have an unfavourable effect on carbohydrate metabolism. Therefore, it is recommended to perform 4-hour determinations of blood sugar, urinary sugar and acetone and accordingly, modification of therapy.

Secondary polycythaemia.

The secondary polycythaemias are classified on an aetio-pathological basis and the main pathological and clinical features of the individual types are presented in the light of data in the literature and own observations in 138 cases seen during 15 years. The general lines and possibilities of therapy are discussed.

Effects of respiratory alkalosis and acidosis on myocardial excitation.

In anesthetized dogs electrocardiogram and monophasic action potentials (MAPs) were recorded from the right atrium and the right ventricle by intracardiac suction electrode technique. The animals were subjected, by means of ventilation with CO2 and hyperventilation, to periods of respiratory acidosis and respiratory alkalosis, respectively. Pronounced respiratory acidosis induced an increased sympathetic activity followed by a decrease in heart rate and prolongation of the A-V conduction time whereas the shape and duration of the atrial and ventricular MAPs remained unaltered. Arterial hypoxia in combination with pronounced respiratory acidosis did not influence the MAP durations. Respiratory alkalosis resulted in an increased sympathetic influence on the heart activity whereas the shape and duration of the atrial and the ventricular MAPs remained unaffected. During pronounced hyperventilation with increasing central venous pressure an increased parasympathetic influence on the heart activity with decrease in the heart rate, prolongation of the A-V conduction time and shortening of the atrial MAP duration was recorded.

[On the appearance of the eye fundus in polycythaemia vera (author's transl)].

The authors discuss the ophthalmological and physical symptoms of polycythaemia vera. They established that the severity of the changes in the fundus depend in the same way on the red cell count and the duration of the illness. In an attempt to find the cause of the complications communicated in the literature in arterial spasm and venous stasis, the authors compared statistically the less severe cases and those cases with arterial stenosis which can also be found in uncomplicated cases, and the cases with venous stasis. In the group with arterial stenosis they showed that its occurrence was not associated with the hypertension often found in polycythaemia vera.

Caseous lymphadenitis in sheep -- methods of infection.

Depending on the route of infection, experimental exposure of 30 sheep to Corynebacterium pseudotuberculosis resulted in either mucopurulent vaginitis, prosthitis, conjunctivitis, rhinitis, or in subcutaneous abscessation. Caseous lymphadenitis, from which the organism could be recovered in 52 out of 56 instances, occurred in every instance with the exception of animals infected by the preputial route.
