Phenylalkyloxirane carboxylic acids, a new class of hypoglycaemic substances: hypoglycaemic and hypoketonaemic effects of sodium 2-[5-(4-chlorophenyl)-pentyl]-oxirane-2-carboxylate (B 807-27) in fasted animals.

Phenylalkyloxirane carboxylic acids and esters are a new class of potent hypoglycaemic substances. Sodium 2-[5-(4-chlorophenyl)-pentyl]-oxirane-2-carboxylate (B 807-27) produces a dose-dependent hypoglycaemic effect when administered orally or intravenously to several fasted laboratory animals, i.e. rats (with and without adrenalectomy), guinea pigs, dogs, streptozotocin-treated diabetic pigs and db/db-mice. In addition, the substances have a more pronounced lowering effect on ketone bodies in the blood than any other known substance. The minimal dose for lowering blood glucose significantly in rats is 15 mumol/kg. The corresponding dose for a significant lowering of ketone bodies in the blood is less than 2.4 mumol/kg. Hence, the substance B 807-27 is approximately five times more potent than tolbutamide and 30 times more potent than the biguanide buformin with respect to lowering blood glucose levels. B 807-27 differs from the sulphonylureas in that it fails to stimulate insulin secretion. In contrast to the biguanides, the substance decreases rather than increases blood lactate concentration and blocks both fatty acid oxidation and gluconeogenesis. The therapeutic usefulness of these compounds in diabetic man remains to be elucidated.

Transcortin: a corticosteroid-binding protein of plasma-XIV. Effects of time of equilibration of serum with cortisol on transcortin determinations by gel filtration.

The results of determination of plasma transcortin levels by gel filtration are subject to assay conditions such as duration of incubation and the amount of cortisol added. We investigated the cause for the dependency of results on assay conditions by incubating samples over prolonged periods of time. Transcortin concentration and the rate of dissociation of the cortisol-transcortin complex remained constant over many weeks of storage at 4 degree C, while irreversible binding to unidentified macromolecules increased continuously. Irreversible binding occurs faster at 37 degrees C than at 4 degrees C. In addition, with time, an ether-extractable, less polar metabolite of cortisol appears in the serum. The data suggest that a highly reactive metabolite of cortisol enters into covalent binding with macromolecules during prolonged incubations.

Nucleotide sequence and exact localization of the neomycin phosphotransferase gene from transposon Tn5.

The nucleotide sequence of 1200 bp from the unique region of transposon Tn5 containing the neomycin phosphotransferase gene (neo) was determined, and the location of the neo gene was identified by deletion mutants in a translational reading frame of 792 bp. The derived gene product, an aminoglycoside 3'-phosphotransferase (APH) II, consists of 264 amino acid residues and has a calculated Mr of 29053. Its amino acid sequence shows sequence homologies to the APH type I enzyme coded for by transposon Tn903 (Oka et al., 1981).

[Scanning electron microscopic examinations of the inner surfaces of central vessel catheters used in neonatology].

Inner surfaces of PVC central vessel catheters used in the treatment of mature and premature newborns for periods between 6 h and 12 days were examined by scanning electron microscopy. Fibrin in form of plain facets, polypous, lamellar of fibrillar-reticular agglomerations were seen at the surfaces of all examined catheters. Red blood cells and platelets attached to the fibrin or interspersed into the fibrin network were discovered. One catheter was completely occluded and a dense mycelium of candida albicans was found within the network. Fibrin depositions at the surfaces of central vessel catheters can probably be the cause of numerous complications e.g. embolic events and catheter-induced infections.

Structural analysis of Tn5.

Nucleotide sequences have been determined for the 1.5-kb inverted repeats of Tn5 and for their junctions with the central unique region and with host DNA. The primary findings stemming from this analysis are: 1. Integration of Tn5 is accompanied by the duplication of 9 bp of host DNA. 2. Loss of Tn5 occurs by crossover between short, homologous nucleotide sequences near the junction between Tn5 and host DNA. 3. The IR sequences contain long, open translational reading frames that may code for transposase proteins. 4. The two IR sequences differ by a single-base change. This alteration accounts for the two functional differences observed between IRL and IRR: It shortens the reading frame for the transposase gene in IRL, and it improves the efficiency of a promoter for the nearby Km-resistance gene. 5. The NH2 terminus of the structural gene for the Km-resistance gene maps at the very left border of the unique region, i.e., very close to the end of IRL. These results support the view that the inverted repeats of Tn5 stem from two identical copies of an originally independently moving DNA element. In the transposon, one of these, IRL, seems to have evolved toward a close physical and functional linkage with the antibiotic-resistance gene.

Relative cardiac efficiency and ST depression during progressive exercise test.

The ratio of calculated myocardial oxygen consumption to estimated oxygen uptake of the body of (MVO2/VO2) provides insight into relative cardiac efficiency. The authors investigated the relation of ST depression to the calculated MVO2/VO2 ratio during a progressive bicycle exercise test in 23 patients with good chronotropic capacity after acute myocardial infarction. ST depression of 0.2 mV or more was required at the highest exercise level. In each case, the lowest MVO2/VO2 value was, on an average, at 60.5% (SD 12.7) of the maximally tolerated work load. The magnitude (mV) and the velocity (micro V/min of ST depression measured at the exercise level after the lowest MVO2/VO2 value were significantly higher than the same parameters at the exercise level corresponding to the lowest MVO2/VO/ value. The authors believe that the lowest value of the calculated MVO2/V.O2 ratio can be regarded as a critical point which is followed by the acceleration of ST depression.

Determination of selected pyrimidines, purines and their metabolites in serum and urine by reversed-phase ion-pair chromatography.

The qualitative and quantitative determination of selected pyrimidines, purines and azapurines and their metabolites by reversed-phase ion-pair high-performance liquid chromatography, using tetrabutylammonium hydroxide as pairing ion and isocratic chromatrographic conditions, is described. The method provides a selective and sensitive assay for these classes of compounds examined in complex biological fluids.

[Calibration of the urethra and video-cystomanometry. Indication and technic].

Recurrent urinary tract infections, vesicoureteral reflux and enuresis in children want a detailed urologic evalution which often makes - in addition to obvious urinealysis, urine culture, excretory urography and voiding cystourethrography-extended diagnostics necessary including an urethral calibration to definitely exclude a distal urethral stenosis, and a cystoscopy under general anesthesia in order to reveal abnormalities of shape and placement of the ureteral orifice (e.g. lateral placement, "stadium", "horseshoe" or "golf-hole" shape). Then micturition urodynamic flow studies using a suprapubic cystocath are done to recognize an infravesical obstruction, a sphincter dyssynergia, an unstable bladder, an uninhibited bladder or a neurogenic bladder. All these examinations are imperative concerning the decision on either conservative (medical) or surgical treatment or both.

[Fertility after experimental torsion of the spermatic cord (author's transl)].

In groups of 10 specimens the testicles of 100 Wistar rats were half unilaterally half bilaterally rotated 720 degrees and retorted 1, 2, 4, 6 and 8 hours after torsion. The fertility rate was investigated by copulation experiments following experimental torsion of the spermatic cord. Fertility was demonstrated by pregancy. Independently of torsion period the fertility rate of unilateral contorted specinens was 60% -- like in both control groups. The bilaterally contorted animals were fertile in 40% from the 1 h -- group and in 30% from the 2 hours -- group. If the torsion period was longer than 2 hrs. bilaterally conterted animals were infertile in all cases. There is no influence of unilateral conterted testicle on contralateral uncontorted testicle.

[Urologic complications of Crohn disease].

A report on urologic complications of Crohn's disease observed in 9 out of 51 operated patients. Primary resection of the diseased bowel segment is of decisive importance in the treatment of these vesicoenteric fistulae or ureteric stenoses. Additional ureterolysis is required only in case of extensive retroperitoneal fibrosis.

Apurinic acid endonuclease activity from mouse epidermal cells.

An endonuclease activity making single-strand breaks into depurinated and alkylated DNA has been purified 500-fold from carcinogen-transformed mouse epidermal cells. The enzyme was active only at apurinic/apyrimidinic sites, regardless of whether they were produced by heating at an acidic pH or by alkylation with the ultimate carcinogen MeSO2OMe. The enzyme did not act on native DNA nor on ultraviolet-induced pyrimidine-dimers nor on steric distortions caused by modification of DNA with the carcinogen (Ac)2ONFln. The enzyme was active in the presence of 1 mM EDTA; however, at pH 7.4 optimal conditions were: 6mM MgCl2 and 40--120 mM KCl or 10--40 mM potassium phosphate. The enzyme eluted from hydroxyapatite, phosphocellulose and heparin-cellulose between 100--250 mM potassium phosphate but did not bind to DEAE-cellulose. Using four chromatographic steps the endonuclease was obtained free of exonuclease, demethylase and DNA glycosylase activity specific for DNA bases methylated with MeSO2OMe or MeNOUr. The molecular weight was 31 000 +/- 3000 as calculated from the diffusion coefficient (8.2 x 10-7 cm2/s) and the sedimentation value (2.7 S).

[Antibiotic therapy after prostatectomy? (author's transl)].

This prospective study shows, that, neither an antibiotic prophylaxis of abacteriuric patients nor postoperative treatment of an asymptomatic urinary tract infection after transurethral prostatectomy have a significant influence on the clinical complications or the microbiologic results. After suprapubic prostatectomy complications in the control group were not significantly more frequent than in the group with antimicrobial prophylaxis. Patients with asymptomatic urinary tract infections before suprapubic prostatectomy had better clinical results, if they were treated with antibiotics as compared to a control group, although the difference control group was not significant. The correlation between consumption of antibiotics and development of resistant in pathogenic organisms forced us to use a more critical approach in the use of antimicrobial prophylaxis and therapy. This study shows clearly, that aseptic operations do not need an antibiotic protection and that faulty of asepsis cannot be corrected by antibiotics.

[Late changes of the testicular tissue in experimental torsion of the spermatic cord (author's transl)].

In three experimental groups and a control group unilateral or bilateral torsion of the testicle were carried out in 195 mature rats. The testicles were retorted at various intervals up to 24 hours. Four weeks and three months following the torsion the retorted as well as the contralateral normal testicles were examined morphologically and histologically. A total testicular torsion is fully reversable after one hour in all cases and after 2 hours in 40% of the cases. The Sertoli cells are destroyed after 6 hours and the Leydig cells after 10 hours. The time causing irreversable tissue damage therefore appears to be shorter than generally assumed, and the germinative epithelium is reacting faster than the testosterone producing Leydig cells. No changes were noted in the contralateral normal testis.

[Hemorrhagic calico papillitis. Clinical picture of pyelitic hematuria with special reference to cytodiagnostic problems].

A case is reported of hemorrhagic calico papillitis showing unilateral hematuria. There was no evidence of pathological changes either in i.v.P. or in retrograde pyelogram, angiogram, or angiogram with epinephrine. Large were found in the urine papillary cellgroups, suspicious for papilloma of the renal pelvis. It is pointed on the difficulty of differentiating such cellgroups from real papillomas of the renal pelvis. Possible are discussed causes of this mucosa proliferation and dysplasia. If there is found the following trias: unilateral hematuria, pseudopapillomatous cellgroups in urine, missing pathological changes in urological X-ray examinations (or only minimal blurs at the fornices) the diagnosis of calico papillitis has to be considered carefully.

[Spontaneous regression of pulmonary metastases from hypernephroma: report of a case and review of literature (author's transl)].

Until now there are reported some 60 cases of spontaneous regression of pulmonary metastases from hypernephroma. An own case is shown up: There was taken nephrectomy and following local radiation of hypernephroma in a 59 years old male patient in spite of pulmonary metastases. 8 months after operation these metastases had disappeared. There was no indication for any metastases in radiologic and scintigrafic examination even 4 years following up neither in lungs nor in other organ. The causality of hormones and autoantibodies - perphaps additionally stimulated by wound infection-in the phenomenon of spontaneous regression of pulmonary hypernephroma-metastases is discussed. According to the literature and to experience in an own case nephrectomy and following local radiation - possibly additional progesterone-therapy for one year - is recommended in hypernephroma even in case of pulmonary metastases.
