[Method for studying spinal evoked potentials].

The authors describe a new method of examing evoked spinal potentials. Derivation is by means of unipolar needle electrodes which are ventrolaterally introduced into the cervical disk space as far as the posterior longitudinal ligament. Detection of a spinal potential following the stimulation of brachial and crural nerves allows to exclude the possibility of complete transverse lesion of the spinal cord or severing of the peripheral nerve or plexus.

[Uncoforaminectomy of Jung in the management of osteoarthritis of the cervical spine and the post-traumatic cervical syndrome (author's transl)].

An anterolateral approach to the cervical spine with uncoforaminectomy is used to free the intra transverse portion of the vertebral artery (from C7 to C3) and the nerve roots. Four surgical indications are discussed; the cervicocephalic syndrome due to osteoarthritis of the joints of Luschka, the cervicobrachialgic syndrome which is often combined wirth the former, cervical myelopathy associated with osteoarthritis and the post-traumatic cervical syndrome. Clincal and neuro-radiological details are given. The results of operation are described in 149 cases and they have been successful in 82%.

[Radionuclide angiography in the functional assessment of surgical extra- and intracranial vascular anastomoses].

Extra-intercranial arterial vascular anastomosing is a surgical form of treatment in selected patients with ischemic cerebral diseases. For the demarkation of the indication for an operation and for the objectification of the functional results cerebrovascular radionuclide angiography was carried out in seven patients with cerebrovascular insufficiency before and after the bypass operation. The non-invasive method only requires an intravenous bolar radionuclide application. It permits a well reproducible appraisal of the regional cerebral blood flow. The best functional results of the shunt operation were observed in the radionuclide angiogram of patients with pronounced focal perfusion reductions. Functionally unsatisfactory results were obtained with low-degree diffuse perfusion reductions of one hemisphere. The results permit to derive criteria for the surgical indication.

[Arthroplasty of the hip by cemented coupled cups. A new technique (author's transl)].

A presentation of a technique of arthroplasty using cemented coupled cups which has been used in 50 cas over a period of more than a year. The hip is approached via an anterolateral incision with the patient in a dorsal horizontal position. This cemented cup arthroplasty has all the advantages of a classical total prosthesis: solid fixation, minimal rubbing by the use of a metal head and an acetabulum in high density polyethylene. However, it has none of the disadvantages since little cement is used, the medullary cavity is not opened and, above all, the neck of the femur is preserved, thereby rendering possible any later operation which may be necessary. The technique is described in detail and the indications rapidly reviewed.

[Cylindrical osteotomy of the upper end of the tibia [author's transl)].

The authors describe a modification of the original technique of Blaimont and Maquet. Cylindrical osteotomy allows correction of deformity in the frontal plane (genu varum or valgum) with forward displacement of the tibial tuberosity. The technique and instruments are described. The osteotomy is fixed by screws and joint mobilisation is begun after two weeks. Thirty patients have been operated on. The results are described as satisfactory.

[Spontaneous ventriculocisternostomies].

A pantopaque ventriculogram is used to demonstrate spontaneous ventriculocisternostomy over the r. suprapinealis in a patient with benign occlusion of the aqueduct. Clinical remission corresponds to sufficient spontaneous liquor drainage. Thus, a total of thirteen real perforations of the third ventricle have so far been described in the literature in addition to nine complete lateral ventricular ruptures. The differences between incomplete ventricular wall ruptures, ventricular diverticula, and ventriculocisternostomies are discussed.

[Functional scintigraphy in the diagnosis of gastroesophageal reflux (author's transl)].

The accepted diagnostical procedures to prove a gastroesophageal reflux by direct or indirect methods do not allow any quantification and are of limited sensitivity. A quantification is possible by use of a gamma-camera with a computer-system after application of a small amount of 99m Tc-DPTA (reflux scintiscan). The high accuracy of this method could be proven by evaluation of 51 patients with suspected gastroesophageal reflux esophagitis or after total gastrectomy. The results were compared with the histological findings. With this method we found after total gastrectomy the expected high incidence of relux. The long-term results after semifundoplication were not as good as after fundoplication. The reflux scintiscan is a less invasive, but reliable method to detect the gastroesophageal reflux.

Rat gastric secretion and mucosal blood flow during restraint stress--effect of a low dose metiamide.

In rats under either mild stress (= controls) or severe stress (= restraint) gastric secretion (acid; pepsin, volume), mucosal blood flow, serum gastrin and ulcer formation were evaluated without and with additional infusion of a low dose (0.25 mg/kg.h) metiamide over an 8 h experimental period. Calculated constants of dose-response characteristics for acid output in this species are: Km = 1.41 +/- 0.36 mg/kg.h; Vmax = 94.4 +/- 11.0 per cent. Restraint depresses markedly secretion and microcirculation, but stimulates ulcer formation and raises serum gastrin. In controls, metiamide inhibits secretion and ulcer index significantly, but leaves mucosal blood flow and gastrin unaltered. In contrast, with restraint and metiamide, ulcer index was not improved, whereas mucosal blood flow was restored to almost control levels. Therefore, H2-receptor blockers, in addition to inhibition of secretion, may interfere with microcirculation either directly or by secondary release of yet unknown mediator substance(s), depending on the prevailing tissue environment.
