[Rupture of the rotator tendon].

The authors report 25 cases of rupture of the rotator tendons operated on at the Stephanie Hospital in Strasburg, and discuss the surgical indications. Debeyre's technic was used with satisfactory overall results both on pain and function.

Mössbauer investigation of deoxymyoglobin in a high magnetic field. Orientation of the electric field gradient and magnetic tensors.

We have examined the Mössbauer spectra of deosymyoglobin in a 6 T magnetic field in the temperature range 4.2-195 K. Spectra were fitted by the least-squares method using a phenomenological model in which the internal magnetic hyperfine field was assumed to be related to the applied field by a temperature dependent tensor õmega. The results indicate that õmega has axial symmetry and a principal axis system aligned with that of the electric field gradient (efg). The principal component of the latter is negative and lies on the symmetry axis of õmega. Our fits indicate that of efg asymmetry parameter is eta = 0.7, with no appreciable temperature dependence. Both axial and transverse components of õmega have the expected 1/T temperature dependence for T greater than 20 K. Our experimentally determined value of eta, combined with published single crystal zero-field measurements, constrains the efg-heme relative orientation to two possibilities. In neither of these is an efg principal axis near to the heme normal.

[Occurence, significance and clinical consequences of lipid A antibody titers in patients with urinary tract infection (author's transl)].

Lipid A antibody titers were measured by the passive hemolysis test in 349 humans. In two out of 20 healthy adults and 16 out of 18 children with recurrent urinary tract infection (UTI) in the presence of anomalies anti-lipid A antibodies were present. In contrast, no titers were found in 23 newborn babies. In a group of 156 patients with acute UTI, 28% revealed positive titers, whereas in a group of 132 patients with recurrent UTI titers occurred in 81%. In a selected group of 132 patients with recurrent infections of the upper tract 59 (=96%) showed definite titers. There was no difference in the development of anti-lipid A antibodies between men and women and the height of the titers did not correlate with the clinical picture of the disease (acute or chronic). The combination of proteinuria and anti-lipid A antibodies indicates the presence of recurrent UTI or chronic pyelonephritis with about 90% accuracy. The titers are caused by immunogenically active lipid A in the body. Since lipid A has the ability to remain in the renal tissue for a long period of time and thereby to maintain the inflamatory response, long-term antimicrobial prophylaxis (six months) should be given to patients with a high risk of recurrent UTI.

[Measurement of muscle reflex time in radicular disorders of the lumbosacral region].

The results of studies performed by the authors show that time measurements of proprioceptive muscular reflexes (triceps surae and tibialis posterior reflexes) may be considered a valuable addition to methods of topical diagnosis of radicular disturbances in the lumbosacral region. The reflexes were tested in twenty normal subjects. There were found an intraindividual constancy of the respective reflex time with a maximum deviation of 2 ms as well as a lateral variability up to 1.3 ms. Radicular compressions resulted in an increase in reflex time by 2 to 66 ms or failure of the reflex potential to be recorded. The pathological cases were compared with clinical and neurological studies, positive myelographies using radiopaque substances, and results of surgical operations. Compared with the results of surgical operations, changes in reflex time indicated damage to the reflex arc in ninety percent of the cases.

[Current status of diagnosis of the cerebrospinal-fluid space using Amipaque (metrizamide)].

A report is given on 73 examinations using Metrizamide (8 thoracolumbar myelographies, 18 ventriculographies, 47 suboccipital myelographies) under consideration of the latest literature. Subsequent systematic EEG-controls were carried out in 52 patients. Exact observance of the indication, adequate dosage, optimum preparation and after-treatment of the patients as well as a appropriate operation technique can limitate the vegetativ side-effects and avoid rarely occurring serious complications. Publications in the literature as well as our own experiences stress the excellent representation of the details of the ventricular system and the spinal subarachnoid space.

[Evaluation of H2 receptor blockade with metiamide in stress ulcers in rats].

Restrain stress in rats inhibits gastric secretion and microcirculation. Secretion is further inhibited by metiamide, but mucosal blood flow is restored to normal. During stress the ulcer incidence is high, lower with prophylactic administration of metiamide and least with the latter and preserved gastroduodenal acid transit. No therapeutic metiamide effect can be detected.

Anti-stress ulcer and anti-secretory effect of somatostatin in rats -- failure to suppress serum gastrin.

In rats the influence of arresting free motions (= controls; mild stress) and of restraint (= severe stress) without and with additional somatostatin (SRIF) on gastric secretion, mucosal microcirculation and stress ulcer formation was studied. Severe stress alone reduces volume, acid and pepsin secretion, acid concentration, aminopyrine clearance and ratio. The ulcer index is elevated. Under both conditions SRIF inhibits in a dose-dependent manner volume and acid secretion, and ulcer incidence is lower. The interaction between severe stress and the antiulcer component of SRIF is characterized as non-competitive. Under these conditions and a continuous infusion of SRIF (10.0 microgram/kg.h over 8 h) acid concentration, acid and pepsin secretion fall subtotally, ulcer index to 50 percent of vehicle (saline) treated rats, if gastric fistula drains juice outside stomach. Microcirculation and serum gastrin are unchanged. In animals with closed fistula ulcer index is reduced by 85 percent, average gastrin again is unchanged. It is concluded that stress ulcers develop at a low secretory and microcirculatory state of gastric mucosa. The prophylactic effect of SRIF results from direct inhibition of parietal and chief cells and additional yet unknown factors.

[Activities of lactate dehydrogenase, acetylcholinesterase and cholinesterase in the cerebrospinal fluid and blood of patients with craniocerebral injuries].

LDH, ACHE, and CHE are determined in liquor and serum of 31 patients with commontio cerebri and 20 patients with contusio cerebri and the values are compared with a reference range found in 25 test persons. Functional cerebral lesion causes an increase in LDH in the cerebrospinal fluid. After cerebral contusions, the behaviour of LDH is uncharacteristic whereas ACHE and CHE show a distinct decrease in activity. When considering the individual values, a considerable scattering is found, so that the clinical applicability of the method for a differentiation of functional and primary-morphological brain lesions remains doubtful.

[Manometric study on the effect of proximal vagotomy on the lower esophageal sphincter].

Selective proximal vagotomy (SPV) decreases the LES pressure (resting pressure and pentagastrin-stimulated pressure), significantly 6 months postoperatively without achieving LES insufficiency by clinical means. Immediately after stimulation LES pressure is identical, compared to preoperative measurements. The postoperative significantly increased concentration of basal gastrin seems not to be a major determinant of LES tone in humans.

Mössbauer spectroscopic study of compound ES of cytochrome c peroxidase.

Mössbauer spectra of Compound ES of cytochrome c peroxidase have been observed over a range of temperature and applied magnetic field. These have been interpreted in terms of a crystal field model of the iron site in which the iron is assumed to be in the Fe(IV) state with unpaired spin S = 1. Detailed least-squares fitting of the spectra fitting of the spectra indicates that both the electric field gradient choice of a single parameter, the axial crystal field, the magnetic properties are well reproduced. The model also provides the observed positive sign for the electric field gradient interaction, but overestimates its magnitude. This apparent discrepnancy may be caused by the presence of significant electronic charge in filled bonding orbitals, a feature which is in keeping with expected covalent charge compensation of the extreme oxidation state. There is no evidence in the Mössbauer spectra of interaction between the iron and the ESR-visible free radical. This suggests they are well separated.

[Uncusectomy and uncoforaminectomy according to Jung. Technique, indications and results (author's transl)].

The authors described the technique of uncusectomy and uncoforaminectomia according to A. Jung. The Jung-operation can be defined as the opening of the intervertebral foramina through the anterior approach. The aims of this operation are the liberation of a displaced or narrowed vertebral artery and to free compressed or irritated nerve roots by means of opening and widening the intervertebral foramina from the front. This report is based on 180 such operations performed by A. Jung and colleagues. The operation is indicated whenever cervical uncarthrosis leads to a vertebrobasilar insufficiency syndrome or a cervical-brachial irritation syndrome. Both syndromes commonly occur together. The results of operative treatment are analysed and the success rate lies around 80%.

Low temperature photodissociation studies of ferrous hemoglobin and myoglobin complexes by Mössbauer spectroscopy.

57Fe-enriched complexes of hemoglobin and myoglobin with CO and O2 were photodissociated at 4.2 degrees K, and the resulting spectra were compared with those of the deoxy forms. Differences in both quadrupole splitting and isomer shift were noted for each protein, the photoproducts having smaller isomer shift and larger quadrupole splitting than the deoxy forms. The photoproducts of HbCO and HbO2 had narrow absorption lines, indicating a well-defined iron environment. The corresponding myoglobin species had broader absorption lines, as did both deoxy forms. The weak absorption lines of photodissociated NO complexes appeared to be wide, possibly indicating magnetic interaction with the unpaired electron of the nearby NO.

[Possible error in interpretation of liquor enzyme activities as demonstrated on the example of lactate dehydrogenase and cholinesterase].

If examinations of the liquor are to give an insight into the metabolic condition of the brain, then it is necessary for all sources of error to be removed. The use fo exact methods requires that the liquor be free of major pleocytosis. In vitro examinations showed that additions of blood caused the activity of LDH to be doubled within thirty minutes, while that of cholinesterase even increased sixfold. It is especially after craniocerebral injuries that extra care must be taken to insure that the liquor used is as clear as water.
