[The morphological basis of vascular (multiinfarct) dementia (author's transl)].

204 patients with an ischaemic stroke were investigated by computertomography. 84 of these patients were demented and 120 had no signs of dementia. Atrophy was significantly more often seen in the computertomograms of the demented patients. A normal computer-tomogram was commoner in the non demented patients. The distribution of infarcts was similar in both groups but bilateral infarcts were significantly more often present in patients with dementia. The combination of a single infarct with atrophy was seen significantly more often in the patients with dementia. Bilateral infarcts, even without atrophy, were significantly more common in the dementia patients. From these findings it was concluded that the bilateral distribution of the lesions is an important factor in vascular dementia.

[The importance of aluminiun in dialyseencephalopathie (author's transl)].

64 patient on chronic haemodialysis were investigated. The duration of the dialysis was 44 months; the mean age of patients was 41 years. In 6 patients a diagnosis of Dementia was made in addition to apraxia, dysarthria and anarthria. 5 of the patients died. All the demented patients showed an abnormal EEG and 4 had atrophy in the computertomogram. The plasma aluminium levels were significantly raised (409 microgram/l) in the demented group as compared to the non-demented dialysis patients (189 microgram/l).

Depressive syndromes in middle age and organic brain disease.

41 patients with a depressive syndrome in middle age (mean age 55 years) were investigated. 19 of them had purely monopolar depressive syndromes without any organic features; 22 patients had a depressive syndrome associated with an organic brain disease (11 vascular, 11 primary degenerative disease). In all patients psychiatric and neurological investigations, a WAIS test, an EEG and a computer tomography were performed. The clinical findings were significantly different in both groups of depression but there was still a considerable overlap of symptoms. The EEG was inconclusive, the psychological test (WAIS) and the computer tomography showed the clearest separation between the organic and the nonorganic depressions leaving only 10% of inconclusive findings. Therefore, it may be concluded that the differential diagnosis of organic or nonorganic depressive syndrome in middle age cannot be made on the basis of one investigation on its own but on the combined results of clinical, functional, and morphological examinations.

Diagnosis and treatment of middle fossa arachnoid cysts and subdural hematomas.

Nine cases with temporal fossa arachnoid cysts were diagnosed by computerized tomography (CT). Five patients also had subdural hematomas, three of them following head trauma. When the hematoma was chronic and of equal hypodensity with the cyst, a clear-cut differentiation was not possible from the CT scan. The presence of a subdural hematoma could only be suggested by thickened arachnoid structures crossing the hypodense area, indicating the wall between cyst and hematoma. The cyst could often be diagnosed by bulging of the skull bone and a temporal lobe defect. Differences in density between cyst and hematoma, such as in subacute subdural hematoma, delineated both entities. Typical examples are demonstrated. Treatment consisted of evacuation of the hematoma and excision of the cyst in all cases.

[The value of the EEG in patients with transient global amnesia (author's transl)].

The EEG findings from 30 patients with transient global amnesia (TGA) of vascular origin are described. A pathological result was seen in 18 cases, from which 17 had predominantly focal changes. 12 EEG's were normal. With increasing time interval between the TGA and the EEG investigation a normal record was slightly commoner. 20 patients had CT results, of which 8 cases were pathological. In 12 cases the EEG and CT results were correspondingly pathological or normal. The EEG shows functional disturbances even after the neurological deficits have resolved, which are related to transient metabolic or diminished blood-flow disturbances, but do not result in structural lesions to bae seen in the CT.

[Valproic acid -- clinical efficiency and serum concentration in the treatment of petit mal epilepsy in childhood (author's transl)].

28 children with Petit mal epilepsy were treated with valproic acid either as drug of primary choice or because of resistance to other antiepileptic drugs. Efficacy of treatment was measured by reduction of seizure frequency and EEG-recordings. Serum concentration of valproic acid was measured in all patients. The best clinical efficacy was found in children with absence seizures, who also required the lowest dosages. Side effects of therapy were (in most cases) transitory.

[Natural history and operative treatment of intracranial aneurysms after subarachnoid haemorrhage (author's transl)].

This report presents an evaluation of prognosis in 227 patients with subarachnoid haemorrhage caused by intracranial aneurysm, comparing operative treatment in 118 patients with conservative treatment in 109. In all operable cases, the prognosis following early surgery was better than that of conservative treatment. Statistical comparison of patient groups according to the clinical condition allowed the formation of guidelines for the optimal timing of surgery: patients in good clinical condition should be operated on at the earliest possible moment. The overall mortality with such a policy is lower than with a waiting policy, since the majority of recurrences occurred within 2 weeks. Patients with disturbed consciousness and neurological deficit are best operated on either within 24 hours or following stabilization or improvement of the clinical picture -- mostly after 2 weeks. The risk of rebleeding is much lower in these cases, which justifies awaiting optimum clinical conditions. Surgery on comatose patients is contraindicated.

[Morphologically assessable changes in transitory ischaemic attacks in the computer tomogram (author's transl)].

83 patients with a transient ischemic attack have been investigated by computerized tomography. 71 patients had symptoms in the region of the middle cerebral artery, 12 in the basilar region. In 14 patients a hypodense lesion in the CT was found. Two of those patients showed multiple lesions. Two of those patients showed multiple lesions. The lesion affected always the region of the middle cerebral artery. Atrophy mostly of minimal degree was diagnosed in 31 patients.

Medical versus surgical treatment of patients with cerebrovascular insufficiency. A retrospective comparative study.

Report on a series of patients suffering from cerebrovascular insufficiency caused by stenosis and/or occlusion of one or several major cerebral vessels. The patients were divided in two groups, one being medically treated with anticoagulants, the other operated on either a carotid artery thrombosis by thromboendarterectomy or occlusion treated with an extracranial-intracranial arterial bypass. Results after a follow-up period of 1-3 years are compared and discussed in view of the recent literature.

Computer tomography in children with epilepsy.

Computer tomography investigations in 72 children with epilepsy showed 42 to be normal and 30 abnormal. An abnormal scan was more common children with partial seizures and in the presence of neurological deficits and/or mental retardation.

[Results of computerized tomography of the skull in hemodialysis patients].

Psychiatric and neurological symptoms as well as the clinical diagnosis of 22 patients on chronic hemodialysis were set in contrast with the findings of the cranial computerized tomography. The cranial computerized tomography turns out to be a valuable diagnostic aid to recognise or exclude morphologic lesions in the central nervous system, particularly because the complex overlap of the various pathogenetic mechanisms in hemodialysed patients points out frequently the little value of the clinical symptoms.

[Psychosocial aspects of epilepsy (author's transl)].

We tried to show the attitude of the environment towards the epileptic patient with special consideration of teachers and the mother of the epileptic child. specific problems of the epileptic patient like frequency of convulsions, psychiatric manifestations and the disease process itself were viewed in relationship to the ability to work and the position in school. We looked in addition into legal problems like fitness to drive, delinquency and insurance problems.

[The natural history of dialysis encephalopathy (author's transl)].

18 data with etiopathogenetic relations to the dialysis encephalopathy, such as plasma aluminum levels, parameters of azotemia and parathyroid function, blood pressure, IQ, and EEG-findings, were compared between 5 patients suffering with encephalopathy and 54 dialysis-patients without neurological symptoms. The investigations, including statistical computations (variance analysis, multiple linear regression, multivariate analysis) elicited the following results: 1. All patients suffering from encephalopathy showed higher plasma aluminum concentrations (505 +/- 58 microgram/L : 228.9 +/- 213 microgram/L, p = greater than 0.01), higher serum calcium levels (2.48 +/- 0.15 mmol/L : 2.35 +/- 0.11 mmol/L, p = less than 0.001), higher rates of osteopathy and abnormal EEG's (5 of 5 : 7 in 41 patients). 2. Age, duration of dialysis treatment, quality of dialysis treatment, blood pressure, did not show any pathogenetic influence. 3. Hyperaluminemia was caused by the oral administration of aluminum hydroxide. After reduction of the daily dose from originally 6.18 +/- 3.24 gm to 1.52 +/- 0.45 gm, the plasma aluminum levels decreased to 54.05 +/- 34.59 microgram/L without any adverse effects on the serum phosphate levels. The results suggest that dialysis-patients need less AI-OH than is usually indicated. 4. Encephalopathy did not occur after normalisation of plasma aluminum levels. 5. With respect to the aluminum toxicity in dialysis-patients, dialysis encephalopathy should not be classified as a complication of multifactorial etiology. Only hyperparathyroidism seems to be an additional risk factor.
