[Spontaneous rupture of an uterine vein during pregnancy. A case report (author's transl)].

The rare case of a spontaneous rupture of the uterine vein is reported. The diagnosis and the etiology are discussed from a review of the literature and the report of a new case. The case is reported to alert to the possibility of rupture of an uterine vein when acute abdominal symptoms and signs are found during pregnancy and present diagnostic problems of an unclear nature. Early laparotomy is necessary to improve the treatment of the haemorrhagic shock and the maternal mortality. In most cases of antepartum of intrapartum rupture of the uterine vein a Caesarean section and suture ligation of the ruptured vein is adequate treatment. Without prior knowledge of this syndrome a Caesarean hysterectomy was performed in our case.

Effects of drugs influencing the cAMP level on hippocampal seizure activity.

Changes of hippocampal EEF seizure activity elicited by electrical stimulation of the hippocampus or penicillin injection into the hippocampus were investigated under the effect of locally applied drugs influencing the cAMP level in the brain. It was observed that some drugs which elevate the cAMP level such as papaverine, histamine +K+ and dibutyryl cAMP elevated the electric seizure threshold while in penicillin-induced epilepsy they reduced the occurrence of ictal activity and the interictal spike frequency. These drugs when applied before penicillin prolonged the time necessary for development of the epileptic focus. The effect of imidazole was the opposite in every respect. On the basis of these data the possible role of cAMP in the pathomechanism of epilepsy is discussed.

[Sulindac in the treatment of soft tissue rheumatism (periarthropathy of the hip and shoulder (author's transl)].

Sulindac (Clinoril) was tested in an open clinical-therapeutical study with regard to its analgesic effect and the improvement of function in 20 patients of both sexes suffering from periarthropathy of the hip or shoulder. The treatment lasted 4 weeks. The therapeutic effect was judged on the basis of the influence on the spontaneous pain and on the pain by pressure, on the functional improvement and on the daily habits of the patients. It was noted, that there was a lasting improvement due to the analgesic effect already in the first week of therapy and an improvement of function in the second week of therapy in the majority of the cases. Sulindac was well tolerated. Laboratory findings in the beginning and at the end of the four weeks treatment period revealed no toxic effects on organs, particularly on hematopoietic system.

[Is the use of large amounts of glucose in parenteral feeding a problem?].

Intravenous feeding with glucose as the sole carbohydrate was analyzed in a retrospective study of 30 patients. Very good assimilation of intravenous glucose without dangerous blood sugar variations could be demonstrated. As the administration of glucose was increased stepwise in the initial phase, the incidence of glucose intolerance was minimal. The good correlation of blood and urinary sugar shows that there is no problem in the control of carbohydrate metabolism during intravenous alimentation with glucose, and that urine samples provide adequate control after the initial phase.

[Hepatitis following acupuncture].

Three patients with hepatitis B are presented whose common source of infection was attributed to a physician practising acupuncture. The contamination appears to be due to repetitive use of the same needles for insertion. Among the different side effects of acupuncture, hepatitis B may thus be a serious complication if the needles used are not properly autoclaved.

Diurnal variations of estriol and cortisol in serum and urine in late pregnancy with and without fenoterol-therapy.

The serum concentration of unconjugated estriol, total estriol, cortisol in one hour intervals and the urinary excretion of total estrogens and cortisol in three hour intervals was determined in eleven apparently healthy women between 26 and 40 weeks of gestation. They were divided into two groups, in order to study the influence of Fenoterol treatment on the daily variation of the measured parameters. Six patients were treated with Fenoterol and five patients received no medication. Between the two groups no statistical differences could be detected for the measured parameters. Regular and marked diurnal variations in cortisol have been observed, whereas total and unconjugated estriol showed small and irregular daily changes. These daily variations of "estriol" were of the same magnitude as the two-fold methodical standard deviation. For this reason it is not necessary to standardize the sampling time.

[Quality and efficiency of surgery in a district-hospital (author's transl)].

Control of quality and efficiency of surgical procedures performed is of utmost importance to every surgeon. This voluntary self-control has regularly been made by us for ten years. It allows accurate statements to be made on both the level of proficiency in surgery and the effectiveness of the surgical work done.

Coding of stable 4S RNA molecules by the resistance factor R1.

With the aid of DNA-RNA hybridization experiments it was shown that an E. coli strain harbouring the resistance factor R1 contains small, stable RNA species which are capable of annealing with the DNA of this R-factor. At a higher hybridization temperature RNA molecules form the isogenic, sensitive strain could also hybridize to the R1-DNA. These RNA-species were again detected if the RNA was extracted from P. mirabilis carrying this plasmid. The fact that sulfur-labelled RNA also hybridized to the plasmid DNA seems to indicate that some of these molecules are tRNAs.

[Weight changes and risk factors during a 10-year period. Observations on subjects in the Basel study].

The change in body mass index (MI) over a 10 year period was compared to different risk factors for cardiovascular disease in 1663 men (aged 15-64) of the Basle Study. Blood pressure and betalipoproteins correlated significantly (alpha less than 0,01) with changes in MI as well as triglycerides (alpha less than 0,05). No correlation was observed between MI and cholesterol or smoking habits. Our results confirme that the age dependent increase in blood pressure, beta-lipoproteins and triglycerides is largely due to the increment of obesity in the adult.
