Heart rate (HR) slowing action and the possible negative inotropic effect of pindolol (P) (daily dose +/- standard error of the means (SEM): 14.7 +/- 1.6 mg) were studied in 12 patients with heart failure and with rapid atrial fibrillation (AF) resistant to effective beta-methyl dioxin (BMD) doses (mean plasma digoxin concentration +/- SEM: 2.05 +/- 0.17 ng/ml). Mild bicycle ergometer exercise test with the same work load was performed under both BMD and combined BMD--P therapy. Rest exercise and 5 minute recovery HR were significantly lower under combined BMD--P therapy than under BMD alone (80.8 +/- 3 vs. 101 +/- 4.5 beats/min, 123.8 +/- 3.9 vs. 155.8 +/- 7.6 beats/min and 84.9 +/- 2.6 vs. 105.1 +/- 4.5 beats min respectively, p less than 0.001, paired t test). Differences in cardiac volume were not significant (1.131 +/- 76 vs. 1.116 +/- 92 ml). Although all patients were clinically improved under combined BMD--P therapy the 13 to 14% increase in cardiac volume noted in 3 cases could be related to some negative inotropic effect of P. It is concluded that in patients with heart failure and with rapid AF resistant to digitalis, the association of P to digitalis is effective in rest- and mild-exercise HR control without a clinically expressed negative inotropic effect.
Seven patients with complete AV heart block and wide QRS complexes, atypical chest pain and no specific serum enzymes modifications for myocardial necrosis are reported. The patients showed no prior complete AV heart block although myocardial perfusion scintigraphy showed localized uptake demonstrating a small area of acute myocardial necrosis. These cases may be described as "true silent" acute myocardial infarction which could not be clinically recognised without Technetium pyrophosphate scintigraphy.

By a complex biochemical, immunologic, morphohistochemical and ultrastructural study, carried out on a group of 42 obese and 38 diabetic children, comparatively with 12 normal controls, the authors arrive at conclusions which prove that obesity and diabetes mellitus carry great atherogenic risk factors. Even though the intimate atherogenesis mechanism is not perfectly known, the abnormal values of insulinemia, lipidemia and cortisolemia are certainly involved in this process. In diabetes mellitus the immunologic factor is also involved. Microangiopathy in diabetes mellitus and capillary lesions in the adipose tissue of obese children may also represent atherogenic risk factors. An efficient prophylaxis in atherosclerosis must therefore begin in childhood.

A case with transient WPW syndrome during silent acute myocardial infarction is presented. Although the clinical picture and laboratory data were non contributory and the surface electrocardiogram showed delta and not Q waves, myocardial perfusion scintigraphy showed typical area of acute necrosis. Abnormal accessory connections probably occur or latent such pathways become functional during acute myocardial ischemia.

In 35 patients with moderate or severe essential hypertension (EH) four groups could be identified by plasma angiotensin II (AII) (formula: see text) and by exchangeable sodium (ENa) (formula: see text) determination. In 6 patients with high AII (135.9 pg/ml +/- 26.29) and normal ENa (98.83% +/- 1.40) propranolol significantly lowered both blood pressure (BP) and AII, suggesting that these forms of EH are renin dependent. In 15 patients with normal AII (36.43 pg/ml +/- 2.27) and high ENa (124.23 +/- 2.78) and in 6 patients with low AII (8.36 pg/ml +/- 3.39) and high ENa (125.16% +/- 5.71) the maximal hypotensive effect was achieved after ENa reduction with thiabutazid. These forms of EH appear to be volume-sodium dependent. In 8 patients with increase of both AII (76.53 pg/ml +/- 5.72) and ENa (112% +/- 1.72), propranolol associated with thiabutazid lowered the BP, AII and ENa suggesting that these cases are mixed forms. ENa determination appears to be a reliable index for renin profiling; such a renin-sodium profile allows to identify some pathophysiologic forms of arterial hypertension.
