[Mitral insufficiency in an adult, due to an abnormal left coronary artery arising from the pulmonary trunk: echocardiographic aspects. Apropos of a case].

The case of a 20 year old woman followed up since adolescence for a progressive congestive cardiomyopathy with mitral regurgitation is reported. She was reinvestigated after hospital admission for cardiac decompensation and supraventricular arrhythmias; 2D echocardiography showed signs of severe mitral regurgitation with thickened mitral leaflets prolapsing into a dilated left atrium and, above all, abnormal diastolic wall motion related to restriction of left ventricular filling with no apparent systolic dysfunction : pulsed Doppler studies confirmed the diagnosis of mitral regurgitation but also showed retrograde diastolic flow in the aortic arch (without aortic regurgitation) and in the main pulmonary artery. Angiography showed these disturbances to be due to an abnormal origin of the left coronary artery in the main pulmonary artery. Reimplantation of the left coronary in the aorta and mitral annuloplasty were justified by the poor spontaneous prognosis of this congenital abnormality which has rarely been described in adults.

[Cirrhosis induced by vitamin A].

The case of a 75-year-old woman who had been treated for 12 years by vitamin A (250,000 UI/day) for psoriasis is presented. During the hospitalisation, hepatic cirrhosis was detected and attributed to hypervitaminosis A based on disease history, clinical hypervitaminosis A features, increased vitamin A blood values and histological patterns, i. e. perisinusoidal fibrosis and spontaneous fluorescence of lipid vacuoles within fat-storing cells. Thus, although rare, vitamin A intoxication can be responsible for cirrhosis.

Atypical xanthoastrocytoma presenting as a meningioma.

A 15-year-old boy was admitted to excision of a large exophytic right parietal tumor, which, on computed tomography scans and angiograms, revealed marked contrast-medium enhancement in infusion studies. Angiographic studies showed meningeal feeders, which suggested a meningioma. Computed tomography scans demonstrated extensive erosion of the skull, which indicated a slowly growing process. Pathologically it shared many features with pleomorphic xanthoastrocytomas, suggesting that neoplasia of the glia limitans may have a wide range of clinicopathological manifestations, including a favorable long-term prognosis.

Stereotactic systems and procedures for depth electrode placement: technical aspects.

The authors describe the conversion of the Leksell spherical system to that of the orthogonal approach for depth electrode placement, by the addition of relatively simple devices to the main frame. These modifications have created the need for new instrumentation such as teleradiology with laser beam centering and the use of an all-purpose stereotactic chair. The main device used for the orthogonal approach is an instrument and electrode carrier moving on sliding side bars. This system can also take advantage of the newer imaging techniques, such as CT scanning, digital angiography and NMR, by the addition of computer markers which are interchangeable with the frame side bars.

Discovery of the first human stereotactic instrument.

It has been uncovered that the first human stereotactic instrument was designed around 1918, but was never put to human use. It was designed after the Horsley-Clarke animal apparatus by an engineer, Aubrey Mussen. The history surrounding that historical event is revealed in a series of letters between the engineer and his son some years later.

The role of computed tomographic and digital radiographic techniques in stereotactic procedures for electrode implantation and mapping, and lesion localization.

This paper describes a computer-based system for analyzing stereotactic CT scans and angiograms. Simple Plexiglas frames containing metallic marker pellets and rods are affixed to the sides of the frame during CT scanning and angiography. The images of these markers are recognized by the computer program and used to compute the frame coordinate system. Coordinates of a lesion on CT and angiogram images may be readily computed and coordinate sets specified by the operator may be displayed on the imager, along with positions of implanted electrodes and a numerical indication of recorded activity for each site.

A new head clamp for stereotactic and intracranial procedures. Technical note.

The authors have devised a clamp to fix the head to the surgical table during stereotactic procedures for placement of depth electrodes. The main advantages of this device derive from the rigid immobilization of the head. The clamp facilitates fixation of the stereotactic frame to the head, percutaneous twist drill trephination of any site of the skull, X-ray centering with a laser beam, mobilization of contrast material during ventriculography or pneumoencephalography, and placement of epidural screws. This head clamp can also be used during craniotomies for fixation of the head and support for brain retractors.

The first human stereotaxic apparatus. The contribution of Aubrey Mussen to the field of stereotaxis.

The first human stereotaxic apparatus was probably built in London, England, around 1918 to the specifications of Aubrey Mussen, a neuroanatomist and neurophysiologist who had worked with Clarke. The recent acquisition by the Montreal Neurological Institute of Mussen's original apparatus prompts this report on the salient points of his career, and summary of his early contributions to stereotaxy.

[Breast carcinoma with multinucleated reactive stromal giant cells (author's transl)].

Breast carcinoma with multinucleated reactive stromal giant cells is a rare histological type. The number of published cases, to which we add two new cases, is too small for it to be possible to reach a conclusion as to whether this type of complaint, for which the authors put forward morphological criteria, should or should not be considered as an anatomoclinical entity. The diagnosis can be made as soon as cytological examination has shown a large number of multinucleated giant cells associated with carcinomatous cells. Ultrastructural study confirms the macrophagic origin of the giant cells but does not show any macrophagic activity.

Stereotaxic device for percutaneous twist-drill insertion of depth electrodes and for brain biopsy. Technical note.

A stereotaxic device is described for percutaneous twist-drill insertion of depth electrodes for seizure recording and brain biopsy. This apparatus, which permits a lateral orthogonal approach, has been used in conjunction with a Leksell type of stereotaxic frame. Its main advantages are the ample working space it provides at the site of insertion of intracerebral electrodes, and the stable attachment it offers in fixing these electrodes on the skull.
