Cell-mediated immunity in alopecia areata.

A study of cell-mediated immunity was carried out in total of 69 subjects with alopecia areata (AA) of the scalp in various phases of its evolution. The blastic responses to mitogens PHA, Con A, and PWM proved significantly reduced, quite independently of the phase of the disease. The total E-rosettes test demonstrated a significant reduction in T-lymphocytes in patients with active AA. On the other hand, no significant differences could be demonstrated between the patients and the controls by means of active E rosette test. The mean values for Tgamma and for theophylline-sensitive T-lymphocytes were reduced in patients with active AA. The significance of the results is discussed.

Contact allergy from propylene glycol.

Propylene glycol (PG) 20% in water was tested in a group of 400 subjects with eczematous contact dermatitis. In six inpatients (1.5%) reactions were recorded as considered positive according to the following criteria: 1. the relevant history of prolonged contact with PG-containing topical pharmaceutical and cosmetic preparations; 2. the reaction extended in time outside the area of application of the test substance, and persisted locally for several day; 3. a similar reaction was obtained on repetition of the test with a lower concentration of PG; 4. the prompt improvement in clinical manifestations of eczema on withdrawal of PG-containing topical preparations. With the PG concentration employed no irritant reactions were observed.

Effect of graded doses of PGE2 on pancreatic exocrine secretion of bicarbonate, chymotrypsin and cyclic nucleotides during i.v. infusion of secretin in man.

We have previously reported that PGE2 inhibits pancreatic enzyme secretion during Secretin-Caerulein infusion, but differing results have also been reported. In this study 10 tests were performed in 5 healthy volunteers as follows: each subject received secretin (GIH) 2 CU/Kg-h by continuous infusion for 2 hours and, several days later, Secretin at the same dose plus PGE2 (Upjohn) in step-wise doses (0.04-0.08-0.12 gamma/Kg min) increasing every thirty min form 30' to 120' min. Duodenal juice was collected in 10 min samples. The volume of each sample was recorded and bicarbonate, chymotrypsin, cAMP and cGMP contents were determined. Statistical evaluation was performed by Student's "t" test for paired data and the regression test. PGE2 significantly decreased chymotrypsin concentration under Secretin. Both cyclic AMP and GMP Secretin-induced secretions were significantly increased by the highest dose of PGE2. No correlation was found in the behaviour of bicarbonate, chymotrypsin and cyclic nucleotides, but it must be noted that our data concern duodenal contents and not pure pancreatic secretion.

[Clinical and psychological aspects of sexuality in cardiac patients].

It's a general opinion that the sexual activity represent a stressing experience, both under the physical and emotional aspect; that's why many patients, after suffering from infarction concentrate their preoccupations on the risk related to their renewal of sexual intercourse. The aim of our research is to evaluate the risk factors through a literature review, but also to consider the personal attitudes of the practitioner and of the patient's family, which could cast on the patient their own anxieties, fears and cultural conventions, resulting in a delay of his psychosexual recovery. After a clinical approach to the problem of the sexual recovery of cardiopaths, two short clinical records are given, confirming the importance of both a physical and psychological counselling in order to obtain a satisfactory recovery of the cardiopathic patient.

The active E-rosette test in allergic contact dermatitis.

An active subpopulation of peripheral blood T-lymphocytes, characterized by rapid (5 min) rosette formation with sheep red blood cells (A-RFC), was measured in patients with acute allergic contact dermatitis. A significant rise in A-RFC occurred in these subjects. The number of total T cells was also increased in the same group of patients, but the percentage of increase was smaller than that found in A-RFC. The significance of A-RFC and their relationship to patient immunological status are discussed.

Shoe contact dermatitis.

The incidence of contact allergy was studied in a series of 165 patients with eczematous dermatitis of the feet correlated clinically with shoe contact. Positive reactions to one or more substances were recorded in 108 patients (65.4%). Among the relevant sensitizers were chromium, paraphenylenediamine, paratertiary butylphenolformaldehyde resin and nickel, while the other allergens were benzocaine, neomycin, balsam of Peru, ethylenediamine and parabens. Allergic contact dermatitis of the feet can be prevented by recognition of the allergens responsible, control of hyperhidrosis and avoidance of topical allergens.

Contact and microbial allergy in pompholyx.

Contact sensitivity to medicaments and other chemical allergens has been observed in about 30% of 364 patients with pompholyx. Pompholyx is a recurrent vesicular eruption which increases the incidence of contact sensitivity. The majority of patients tested by means of intradermal and cultural tests did not show any relevant finding on the microbial nature, allergic or not, of pompholyx.

Occupational dermatitis artefacta.

Nine cases of disease malingering with skin lesions attributed by the patients to occupational factors are reported. History and morphology of the lesions provided sufficient grounds for the diagnosis of occupational dermatitis arterfacta. Four out of nine patients admitted to having produced these artefacts.

Intradermal test in contact allergy to metals.

Variations of intradermal test results were studied in a group of patients with clinically evident contact dermatitis and with the history suggestive of contact with metals and with negative patch tests reactions to the latter. The positive reactions obtained in several cases indicate that intradermal tests may reveal specific sensitivities, which patch tests using standard concentrations are not always capable of demonstrating.
